1.Application of Healthcare Failure Mode and Effects Analysis system to reduce occupational exposure to HIV for surgical medical staff

Objective To investigate the application of the Healthcare Failure Mode and Effects Analysis ( HFMEA) system to reduce occupational exposure of HIV for surgical staff.Methods Based on the method of HFMEA system utilization, the occupational exposure prevention team for surgical medical staff had been set up to analyze the failure mode and the causes of potential risks,and to calculate the Risk Priority Number ( RPN) .In this study some improvement measures had been carried out, aiming at the failure mode and priority problems.We counted and implemented circumstances of exposure before and after the HFMEA implementation, and evaluated the appraisement of surgical staffs for the implementation of HFMEA.Results Before administration of HFMEA,twenty two HIV-infected patients were admitted in our institution,of which six patients caused occupational exposure, five cases of blood-borne exposure, and one cases of mucous membrane exposure.After administration of HFMEA,thirty HIV-infected patients were admitted, of which only one patient caused occupational exposure, for blood-borne exposed type.The exposure rate before and after HFMEA administration had statistical significance (χ2 =4.36, P <0.05 ).Surgical staffs rendered a positive appraisement for HFMEA administration and 32 (100) staffs recommended it to enhance the recognition of HIV patients contact safety protection and minimize HIV occupational exposure.Conclusions Could reduce occupational exposure rate for surgical medical staff by usage of HFMEA to evaluate and reduce occupational exposure of HIV patients, safe injection, personal protective equipment and medical waste management.
2.Application of the healthcare failure mode and effects analysis system in the improvement of functional exercise effects for post-operation breast cancer patients

Objective To improve the functional exercise effects with healthcare failure mode and effects analysis system for post-operation breast cancer patients .Methods Based on the method of healthcare failure mode and effects analysis ( HFMEA) system,the post-operation breast cancer functional exercise team was set up to determine the functional exercise process , to analyze the failure mode and the causes of potential risks, to calculate the Risk Priority Number ( RPN) and to find out the reason of failure .Results After the application of the HFMEA , the degrees of breast cancer patients'lateral humeral joint proneness , outreach, adduction and abduction were (118.4 ±22.1) °, (39.8 ±5.6) °, (36.2 ±5.8)°and (100.2 ±14.5) °, respectively, which were significantly improved than (88.7 ±17.9) °, (34.9 ±5.4) °, (21.7 ±6.3)°and (69.8 ±23.1) °before (t=2.87,3.65,3.48,3.12, respectively;P<0.01).Positive assessment rate of the HFMEA was 76.19%given by the nurses.Conclusions The usage of HFMEA can improve the effects of the post-operation exercise process .
3.Analysis of common pathogenic gene mutations of arrhythmogenic right ventricular cardiomyopathy in Yunnan unexplained sudden death cases

Objective:To analyze common pathogenic gene mutations of arrhythmogenic right ventricular cardiomyopathy (ARVC) in Yunnan unexplained sudden death (hereinafter referred to as Yunnan sudden death) cases, and explore the etiological relationship between Yunnan sudden death and ARVC.Methods:Four typical Yunnan sudden death affected counties (cities) were selected as investigation sites. Cryopreserved autopsy cardiac cavity blood samples were collected from Yunnan sudden death cases ( n = 3), and peripheral venous blood samples were harvested from their relatives (first, second, third and immediate degree of kinship, n = 67) and control population ( n = 49). The DNA of blood samples was extracted for amplification and sequencing of 97 exons of 5 common ARVC desmosomal protein [desmoplakin (DSP), desmocollin-2 (DSC2), desmoglein-2 (DSG2), plakophilin-2 (PKP2) and junction plakoglobin (JUP)] genes, and genetic lineage of Yunnan sudden death cases was investigated. Results:A total of 17 gene mutation sites were discovered in Yunnan sudden death cases and their relatives, with 6, 5, 4, 1 and 1 in the DSP, DSC2, DSG2, PKP2 and JUP genes, which were not found in the control population. Among them, 9 were newly discovered mutation sites and 8 were reported mutation sites. The DSP gene exon 24 c.8472 G>C, a pure contractual sense mutation, was common in the relatives of 4 cases in the same family surveyed; and one immediate relative carried a deletion mutation at c.2368 - 2370 of exon 15 of DSC2 gene.Conclusion:Yunnan sudden death cases and their relatives carry mutations in the ARVC desmosomal protein DSP, DSC2, DSG2, PKP2, and JUP genes, and the onset of some Yunnan sudden death may be associated with mutations in the ARVC desmosomal protein genes.
