1.Preparation and characteristics of 10-hydroxycamptothecin loaded folate-receptor targeted phase-change contrast agent

Objective To prepare a 10-hydroxycamptothecin (10-HCPT) loaded folate-receptor targeted phase-change contrast agent (FR-HCPT-PNPCA),and to study the general characteristics including drug loading,phase changing and targeting capability in vitro.Methods Using a method of two-step emulsification,the phase-change nanoparticles loading anticancer drug (10-HCPT) with lipids shell and liquid pefluorocarbon core were prepared.The entrapment efficiency and the drug-loading amounts were studied by high performance liquid chromatography,and the phase transition of the nanoparticles after heating was observed.The targeting ability was evaluated on liver cancer cell line 7721 in vitro.Results The FR-HCPT-PNPCA,with a drug encapsulation rate of about 70.42 % and drug loading amounts of about 20.05 %,was prepared successfully.When being heated to 70℃,obvious phase changing and microbubbles generating could be observed under microscope.In addition,a large amount of FR-HCPT-PNPCA particles could adhere specifically around the 7721 cells.Conclusion The prepared FR-HCPT-PNPCA,which has a stable characteristic and high performance of drug loading and tumor targeting,is expected to become a promising multifunctional molecular ultrasound probe for diagnosis and treatment of tumor.

Nasopharyngeal carcinoma (NPC) is a polygenetic inheritance tumor. It has very high incidence in southern China and among immigrants and their offspring from south Chinese, and has done a great harm to the health of south Chinese. As a malignant tumor with obvious tendency of familial aggregation and regional difference, it is suggested that the etiology and pathogenesis of NPC are relevant to the coeffects of genetic and environmental factors. It is the main point to focus on the elucidating of the pathogenesis of PNC as a polygenetic inheritance tumor, primarily establishing of the concept of susceptibility gene groups in NPC and the carcinogenesis model of the multi-step Domino effect involved in the molecular mechanisms of NPC dominated by susceptibility gene groups in order to lay out a substantial foundation for finding the high-risk susceptibility factors of NPC, screening the high-risk population of NPC and exploring the targetic and individual measures in diagnosis and treatment of NPC.
3.The Effect of immunomodulatory therapy On CD4+CD25+ tregs in peripheral blood of patients with Sepsis

Objective To investigate the percentage of CD4 + C125 +Tregs in peripheral blood of patients with sepsis and its effect on cell immunity so as to unravel the effect of immunomodulatory therapy on it. Method Fourty patients with sepsis in ICU were randomly (random number) divided into experimental group and control group . The patients of experimental group were treated with Ulinastatin and immunoregulation agent (Thymosin αl) as well. The blood specimens were collected just before treatment, 3 days and 8 days after treatment. The percentages of CD4 + CD25 + Tregs and lymphocyte subsets were detected by using FCM (flow cytometry), and TNF-α, IL-6 and IL-10 assayed by using ELISA, and APACHE Ⅱ scores were calculated. Results Before treatment, the percentage of CD4 + CD25 + Tregs increased, and the number of lymphocytes and the percentage of T lymphocytes decreased, especially the CD4 + T lymphocytes and CD4+/CD8+ decreased more markedly, and the levels of IL-6 and TNF-α increased. After treatment,the percentage of CD4+ CD25 + Tregs was decreased, the number of lymphocytes and CD4 +/CD8 + increased, and the levels of APACHE Ⅱ score, IL-6 and TNF-α decreased especially in the experimental group decreased more significantly (P < 0. 05). Conclusions The percentage of CD4 + CD25+ Tregs in peripheral blood can reflect the immune status of patients with sepsis and become a novel indicator to estimate the progress of sepsis, and the immunity and prognosis of patients. Treating the patients with Thymosin αl and Ulinastatin can raise their immunity, decrease the levels of IL-6, TNF-α and APACHE Ⅱ score and improve their prognosis.
4.Nodular regenerative hyperplasia of the liver:a clinical study of 12 cases

Objective To review the clinical,histological and diagnostic aspects of 12 documented cases of nodular regenerative hyperplasia of the liver(NRHL),to make this condition be understood and dealt with better. Method Twelve NRHL cases were diagnosed based on liver biopsy from 300 portal hypertension patients who had been underwent splenectomy.Imaging studies were performed as part of the diagnostic evaluation.Clinical manifestation and biochemical tests were recorded at the time of diagnosis.Management and prognosis were also reviewed.Results Most patients were complicated with autoimmune disease,6 cases were diagnosed systemic lupus erythematosus and 1 was Crohn's disease and 1 suspected ulcerative colitis.Six cases were treated by prednisone and 3 cases by immunosuppressant.Eleven cases had suffered from portal hypertension.All cases had no history of viral hepatitis.Biochemical tests showed mild increase of liver enzyme and relative normal synthetic liver function.The histological finding was nodular in the hepatic parenehyma,with mild periportal fibrosis,intraportal lymphocytic infiltration,narrow and obstruction of branch of portal vein,and lack of hepatocyte necrosis.All cases were diagnosed liver cirrhosis and portal hypertension before operation.Management was directed to portal hypertension and varices bleeding with satisfactory results.Most of them keep a stable condition during the follow-up. Conclusion The NRHL was uncommon and its cause and pathogenesis was unclear,may be related with immune and hepatic blood circulation disorder.It should be considered in patients with unexplained portal hypertension and distinguished it from liver cirrhosis.Liver biopsy confirms the diagnosis.Management directed to portal hypertension may improve clinical condition.
5.Clinical Significance of N Terminal Pro-Brain Natriuretic Peptide in Children with Ventricular Septal Defects

Objective To assess the value of serum N terminal pro-brain natriuretic peptide(NT-Pro-BNP) in the diagnosis of congestive heart failure(CHF) and evaluation of cardiac function in children with ventricular septal defect (VSD).Methods Fifty one children were enrolled from March 2004 to March 2005.NT-Pro-BNP was measured by enzyme immunoassay technique.At the same time,left ventricular ejection fraction(LVEF) and left ventricular shortening fraction(LVFS) were detected with echocardiography.Results The data of NT-Pro-BNP were showed but the logarithms of which were normal distribution.The values of NT-Pro-BNP were developed successively along with the severity of cardiac function.But there was no difference between the group of no heart failure and mild heart failure.But the values of LVEF and LVFS had no differences in the control,the mild and the moderate heart failure and the same time all of which beyond the standard of diagnosing heart failure.NT-Pro-BNP could reflect the degree of heart failure or cardiac function (r=0.826).But LVEF and LVFS can not reflect the degree of the cardiac function.Conclusions NT-Pro-BNP can reflect the degree of the cardiac function in VSD,and the degree of the heart failure can be classed by the levels of serum NT-Pro-BNP.But the value of LVEF and LVFS can not reflect the degree of the heart failure in the same disease.
7.Preventive effect of alprostadil on contrast-induced nephropathy in patients undergoing coronary intervention

Objective To observe the preventive effect of alprostadil on contrast-induced nephropathy (CIN) in patients undergoing coronary intervention.Methods Two hundred and fifteen patients undergoing coronary intervention were divided into treatment group (112 cases) and control group (103 cases) by random digits table method.Patients in treatment group were treated routinely with addition of alprostadil (10 μ g intravenous injection every 12 h once for 7-10 days,given before surgery).Patients in control group were given routine therapy only.Complications of the two groups and CIN occurrence were observed.Results The occurrence rate of CIN in treatment group was lower than that in control group[7.1% (8/112) vs.18.4% (19/103),P < 0.05].The occurrence rate of CIN in patients with renal insufficiency was higher than that in patients with normal renal function [51.6% (16/31) vs.6.0% (11/184),P < 0.01].The occurrence rate of CIN in patients with diabetes was higher than that in patients without diabetes [22.1%(15/68) vs.8.2% (12/147),P < 0.01].Conclusions Alprostadil in coronary intervention before treatment can obviously reduce the incidence of CIN.CIN is more likely to occur in patients with renal insufficiency or with diabetes.
8.One-stage anterior radical debridement,titanium mesh bone fusion and internal fixation for the treatment of thoracolumbar spinal tuberculosis

[Objective]To evaluate the curative effect of the one-stage anterior radical debridement,titanium mesh cage bone fusion and inernal fixation for the treatment of thoracolumbar spinal tuberculosis.[Method]Twenty-two thoracolumbar spinal tuberculosis cases from January 2003 to February 2006 were analyzed retrospectively,among whom 12 were male and 10 were female,age ranging from 19 to 76 years with a mean of 36 years.T_9~L_5 vertebrae were involved in,in detail 1 was involved in single vertebra,17 were involved in 2 vertebrae,3 were involved in 3 vertebrae,1 was involved in the type of jumping style,M=2,Q=0.All cases received one-stage anerior radical debridement,titanium mesh cage bone fusion and internal fixation along with strict anti-tuberculosis chemical therapies.The improvement of symptoms and neurological function,angles reduced by the surgeries and bone fusion,angles lost as well as the subsidence of the titanium mesh during the long-term follow-up were observed and compared statistically.[Result]No recurrence was found in the follow-up period extending from 6 months to 24 months with an averaging of 12.5 months.All the incisions acquired primary healing.The systematic and local symptoms were released postoperatively.Neurological function recovered in different degrees(P
9.Protective effect of oxygen carrying liquid to brain tissue after hypertensive intracerebral hemorrhage in rats

Objective To study the protective effect of oxygen carrying liquid to brain tissue after hypertensive intracerebral hemorrhage in rats.Methods Collagenase and heparin were injected into the caudate nucleus of rats by stereotactic operation to induce a hemorrhage model,we observed the water content of brain,the morphology of the brain,the change of cerebral blood flow (CBF) and biochemical markers in each group.Results Group B without oxygen carrying liquid had more serious injury than group A with oxygen carrying liquid.Water content of brain tissue in group B (80.55?0.80%) was higher than group A (77.94?1 16%),group B had higher MBP levels(0.87?0.44 ng/ml) than group A (0.54?0.19 ng/ml), group B had higher NSE levels (1.74?0.68 ng/ml) than group A (1.19?0 49 ng/ml),There was remarkable difference in the two groups.Conclusion The oxygen carrying liquid had fine protective effect to the brain tissue around the hematoma after hypertensive intracerebral hemorrhage.
10.The biological characterization of Pseudomonas aeruginosa phage PaP3

Objective To identify some basic biological properties,optimal multiplicity of infection(MOI),one-step growth kinetics,serum neutralization rate constant of Pseudomonas aeruginosa phage PaP3 were determined, respectively. Methods The host bacteria were infected with PaP3 at six different MOI(0.0001,0.001,0.01,0.1,1 and 10,then lysated adequately, the phage titer of supernatant was determined.For one-step growth experiments,the host bacteria were infected with PaP3 at MOI=10, following centrifugation after 15 min adsorption,the pellet containing (partially) infected cells was resuspended in 3 ml of pre-warmed LB broth and incubated with 160rpm at 37℃.Samples(50?l) were taken at 10 min-intervals and immediately tittered by the double-layer agar plate method. A rabbit was immunized with purified PaP3 to prepare anti-PaP3 serum, reaction constants between antiserum and PaP1, PaP2, PaP3 were determined using cross neutralization test. Results 0.001 MOI-infected host bacteria gave the highest phage offsprings. One-step growth curve was determined according to the results of one-step growth experiments.Reaction constant was determined using neutralization test. Conclusion The optimal MOI of PaP3 was determined to be 0.001. One-step growth curve for PaP3 shows that the latent period is about 20 min, the rise period is 60 min, and the average burst size is about 31pfu/cell. Rate constant of reaction between antiserum and PaP3 is 262.
