Objective: To investigate the influence of unilateral internal carotid artery (ICA) stenosis (70-99%) or occlusion evaluated by color Doppler ultrasound (CDU) and transcranial Doppler (TCD) on ICA hemodynamics of contralateral carotid artery (50-69% stenosis) in patients with bilateral infernal carotid artery stenosis. Methods: A total of 102 consecutive patients with bilateral ICA stenosis (unilateral stenosis 50-69%, contralateral stenosis 70-99% or occlusion) were enrolled from June 2005 to June 2011 and they were used as study group (group A). The patients were screened by color Doppler flow imaging (CDFI) and TCD, and they were confirmed as bilateral ICA stenosis by DSA. At the same time, 89 patients with unilateral ICA stenosis 50-69% and normal contralateral ICA were used as control group (group B). The differences of ICA stenosis 50-69% in the two groups and the peak systolic velocity (PSV), end-diastolic flow velocity (EDV), resistance index (RI) and pulsatility index (PI) of anterior cerebral artery (ACA) in both groups were compared. Group A was redivided into an anterior communicating antery patent group (A1 group, n = 54) and an anterior communicating artery occluded group (A2 group, n = 48) according to the results of TCD and DSA examinations. The differences of the hemodynamic parameters of the 50-69% stenotic ICA and ACA stenosis, and the correlations of the hemodynamic changes in both groups were further compared. Results: Circled digit oneThe PSV and EDV of unilateral stenotic ICA stenosis and ACA with 50-69% stenosis in group A were significantly higher than those in group B (all P < 0.05), RI and PI were significantly lower (P = 0.001, P = 0.000). Circled digit twoPSV and EDV of unilateral ICA and ACA with 50-69% stenosis in group A1 were higher than those in group A2 (all P < 0.01), but RI of ICA and PI of ACA in group A1 were lower than those in group A2 (P = 0.000, P = 0.000), the differences were statistically significant. Circled digit threeThe velocity of ICA was positively correlated with that of ACA (r = 0.327, 0.422, P = 0.007, P = 0.000). Conclusion: When one side of internal carotid artery is severely stenotic, the measured value of velocity of the other side of ICA (50-69% stenosis) is significantly higher than the diagnostic criteria of the actual lesion. The anterior communicating artery patency is the basis of hemodynamic changes of the bilateral stenotic ICA. The identification of the degree of bilateral ICA stenosis should be evaluated comprehensively in combination with the extracranial and intracranial hemodynamic changes.

In this experiment, the HPLC specific chromatogram was adopted, with Agilent Extend-C18 (4.6 mm x 250 mm, 5 microm) as the chromatographic column, and 0.5 per thousand trifluoroacetic acid and acetonitrile as the mobile phase for gradient elution, so as to establish specific chromatograms for drug pair of Schizonepetae Herba and Saposhnikoviae Radix from different producing area, identify 12 common characteristic peaks, and obtain the comparison specific chromatography of drug pair of Schizonepetae Herba and Saposhnikoviae Radix. The method is simple, accurate and highly reproducible, and thus can be used as the basis for the quality control of the drug pair.

AIM: To observe the comparison of vitrectomy combined drug therapy ( Ranibizumab injection ) and single vitrectomy for proliferatived diabetic retinopathy ( PDR ) and the influence of the curative effect and prognosis of patients.
METHODS:In this study, 112 cases (125 eyes) with PDR were selected and randomly divided into experimental group and control group ( n= 56 ) . Fifty-six cases ( 61 eyes ) in experimental group were injected by drug therapy of 0. 5mg ranibizumab and received vitrectomy;In control group, 56 cases ( 64 eyes ) were received single vitrectomy. The intraoperative and postoperative differences of clinical indicators were analyzed in two groups.
RESULTS: The average operation time, intraoperative electric coagulation hemostasis rate and iatrogenic hiatal incidence of the experimental group were lower than that of the control group:(95. 00±13. 00) min vs (133. 00±14.5) min, 11% vs 34%, 5% vs 20%, respectively (P<0. 05). The anterior chamber and vitreous body cavity hemorrhage rate within postoperative 1mo in the experimental group were lower than that of the control group:7% vs 23%, 5% vs 12%, respectively (P<0. 05). The postoperative visual acuity of experimental group was better than that of the control group:0. 375±0. 210 vs 0. 261±0. 170, respectively (P<0. 05).
CONCLUSION: Patients with PDR injected with ranibizumab in vitreous cavity before vitrectomy can effectively reduce the operation time, less intraoperative blood loss, the incidence of iatrogenic hiatus, and intraoperative and postoperative complications. The postoperative visual acuity was better than before.

OBJECTIVETo systematically review the effectiveness of minimally invasive total hip arthroplasty (MIS-THA) versus traditional total hip arthroplasty (THA) in patients with hip diseases.

METHODSThrough a method of combining Free words and keywords,we searched databases including PubMed,The Cochrane Library, EMbase,Web of Science, CBM , CNKI and Wanfang Data for randomized controlled trials (RCTs) on the comparison between MIS-THA and THA for hip disease from inception to June, 2014. Two reviewers independently screened literatures according to the inclusion and exclusion criteria, extracted data and assessed the quality of the included studies according to the "bias risk assessment" tool recommended by Cochrane Handbook 5.0 for Systematic Reviews. Then, meta-analysis was performed using RevMan 5.3 software.

RESULTSThirteen RCTs involving 1 213 cases of surgeries and total 1 284 hips (MIS-THA: n = 631; THA: n = 653) were identified. The results of meta-analysis showed that statistically significant differences were found in Harris hip score on the 3rd month after operation [MD = 8.37, 95% CI (6.02,10.72)], Hematocrit [MD = 0.02, 95% CI (0.01, 0.03)] and Hemoglobin [MD = 0.50, 95% CI (0.16, 0.85)] at the 48th hour after operation, changed value of femoral offset [MD = 0.30, 95% CI (0.04, 0.56)] between two groups. In the change value of femoral offset, THA was better than MIS-THA; There were no statistically significant differences between two groups in Harris hip score at 1st year after operation [MD = 3.26, 95% CI (-3.25, 9.76)], WOMAC score [MD = -0.53, 95% CI (-3.67, 2.60)] and Oxford score [MD = 1.34, 95% CI (-3.46, 6.13)] at the 6th week after operation, Hematocrit at the 8th hour after operation [MD = -0.01, 95% CI (-0.02, 0.00)], the incidence of hip varus [RR = 0.82, 95% CI (0.45,1.52)] and dislocation [RR = 1.40, 95% CI (0.48, 4.12)].

CONCLUSIONTHA brings less trauma, less hemorrhage and better early clinical outcome compared with MIS-THA, but the difference of the complication rates between the two groups is similar.

ObjectiveTo investigate the clinical biocharacteristics,treatment,prognosis and the factors that influence prognosis of anaplastic thyroid carcinoma(ATC). MethodsThe clinical data of 108 ATC patients treated at our institution from January 1981 to April 2009 and follow-up results were retrospectively reviewed.ResultsThe median survival time of 108 patients was 6 months.The overall 1-year,2-year,5-year survival rate was 40.3％,30.9％ and 21.9％ respectively.Univariate analysis showed the factors influencing the prognosis of ATC patients were tumor size, distant metastases, stage,leukocytosis, radiotherapy, localresection, postoperativeradiotherapyandmultimodalitytherapy.Multivariate analysis indicated that tumor size,stage,leukocytosis and radiotherapy were independently associated with the prognosis.ConclusionsAnaplastic thyroid carcinoma is rare but highly aggressive malignancy with poor prognosis. ATC patients should receive multimodality therapy,especially local resection and postoperative radiotherapy.Even inoperable patients could benefit from radiotherapy.

Objective To study the chemical constituents of Cynanchum auriculatum. Methods Two steroidal glycosides were purified by column chromatography and their structures were elucidated by spectroscopic analysis. Results and Conclusion Compound 1 is a new steroidal glycoside, named kidjoranin-3-O-β-D-cymaropyranoside. Compound 2 is isolated from this plant for the first time.
