Objective To investigate regional systolic function in left ventricle(LV) and postsystolic shortening(PSS) in patients with essential hypertension with different remodeling patterns of LV by strain and strain rate imaging. Methods According to the level of LV mass index and relative wall thickness, forty patients with hypertension were divided into LV normal geometry(LVN) group and LV remodeling(LVR) group, with thirty healthy subjects as control. The strain and strain rate were compared among the basal,middle and apex segments. Results Compared with control, systolic peak strain and strain rate in both groups of LVN and LVR were decreased significantly(P<0.05). Prevalence of PSS in LVR group (55%) was higher than that in LVN group(39%) and control(22%) with significant differences(P<0.01).Conclusions High prevalence of pathologic PSS in patients with hypertension could implied that unbalance of segmental motion in regional myocardial ischemia. However,presence of PSS in healthy subjects might be the result of synchronization and balance of normal myocardium under physical condition.
4.Clinical significance and changes on serum levels of α-MSH, TNF-α, PCT in patients with acute pancreatitis

Objective To investigate the dynamic changes in peripheral blood of patients with acute pancreatitis α-MSH,TNF-α,PCTS and the value of the patient assessment of disease severity.Methods Emergency surgery in the Second Affiliated Hospital of Anhui Medical University from March 2011 to March 2014 were treated 68 cases of acute pancreatitis patients,36 patients with mild,32 cases of severe group.All patients were extracted 1,3,5,7 days after admission blood,and α-MSH was measured by enzymelinked immunosorbent assay,the content of TNF-αand PCT.Results Compared with the mild group,the serum of patients with severe group of α-MSH concentrations were lower (P < 0.05),while serum TNF-αand PCT levels higher than the mild group (P < 0.05);in disease severity assessment,α-MSH sensitivity of 82.35% and a specificity of 88.24%,the accuracy was 87.5%,TNF-α sensitivity of 76.47% and a specificity of 82.35%,the accuracy was 81.25%,PCT sensitivity of 70.59% and a specificity of 76.47%,75.00% accuracy,PCT + TNF-α + α-MSH sensitivity of 88.24% and a specificity of 94.12%,93.75% accuracy;area under the PCT,TNF-α,α-MSH and the combination of the three curves 0.690,0.696,0.768 and 0.885,respectively.Conclusions Serum α-MSH,monitoring TNF-α,PCT levels have some clinical value in patients with acute pancreatitis disease severity assessment,including α-MSH in the condition assessment of the sensitivity,specificity and positive predictive value were higher than the latter two,and peripheral α-MSH in patients with a low level indicates a serious condition,poor prognosis;addition,combined detection with higher clinical value in the assessment of disease severity.
5.Effects of anti-somatisation therapy on the quality of life in patients with functional dyspepsia

Objective To explore somatic symptoms of patients with functional dyspepsia (FD) and effects of combined anti somatisation therapy on the quality of life in patients with FD.Methods Two hundred and nineteen patients with FD were enrolled.Patient Health Questionnaire-15 (PHQ-15) and Nepean Dyspepsia Index (NDI) were completed,and the relation between them was analyzed.Lansoprazole,mosapride and flupentixol melitracen (the anti-somatisation medication) were taken for four weeks by 131 FD patients who had poor response to four-week conventional treatment with proton pump inhibitors (PPI) and/or prokinetic medication and who got more than 4 scores in PHQ 15.And then therapeutic effects were observed and the changes of NDI before and after treatment were analyzed.The t test,chi-square test,Spearman's correlation and linear regression were performed for statistical analysis.Results The total PHQ-15 and NDI score of 219 patients with FD were 5.7±3.4 and 42.0± 10.4,and there was positive correlation between them (r=0.493,P<0.05).Stomach pain,headache,chest pain,dizziness,shortness of breath,fatigue,sleeping disorder,thirst and so on were positively correlated with totalNDIscore (r=0.262,0.230,0.241,0.243,0.352,0.385,0.266,0.281,all P<0.05).Chest pain,stomach pain,shortness of breath,dizziness and fatigue were the influence factors of NDI (t=2.340,2.488,5.278,2.167,2.443,all P<0.05).Among 131 patients with FD who received anti somatisation combination therapy,six patients did not complete the therapy because of the side effects,and finally 125 patients were enrolled and analyzed.Before and four weeks after the treatment,the total PHQ-15 scores of 125 patients with FD were 7.3±3.2 and 2.9±2.2,respectively; and the difference was statistically significant (t =12.653,P<0.01).There were significant differences in the influence of stomach pain,headache,chest pain,dizziness,shortness of breath,fatigue,sleeping disorder and thirst before and after the treatment (not bothered at all,bothered a little,bothered a lot) (x2=39.231,6.796,6.693,15.520,6.698,51.572,44.390 and 16.506,all P<0.05).The total NDI score before and after the treatment were 44.3 ± 11.7 and 29.2 ± 6.9,and the difference was statistically significant (t=12.503,P<0.01).Conclusions The quality of life in patients with FD is affected by somatic symptoms such as chest pain,stomach pain,shortness of breath,dizziness,fatigue and so on.For FD patients with poor response to conventional treatment,the quality of life could be improved by the combination of anti-somatisation therapy and the conventional therapy.
6.Diagnosis and treatment of hilar cholangiocarcinoma

Objective To improve the level of diagnosis and treatment of hilar cholangiocarcinoma by investgating it′s clinical characteristics.Methods The clinical data of 98 patients with hilar cholangiocarcinoma were analyzed retrospectively.Results According to the Bismuth-Corlitte grouping: typeⅠ,8 cases;typeⅡ,19 cases;type Ⅲ,17 cases;type Ⅲb,14 cases;and type Ⅳ,40 cases.Radical resection(49),palliative operation(12),endoscopic retrograde biliary drainage(16),and PTCD(5) were performed The median survival time of radical resection group was 28.6 months and the survival rate at 1,2,3,and 5 years was 74.2%,65.3%,37.5%,and 3.9% respectively,while the median survival time of palliative operation group was 17.6 months and the survival rate at 1,2,3,and 5 years was 71.7%,26.8%,8.9%,and 0% respectively.The median survival time of endoscopic retrograde biliary drainage group was 4.5 months and the survival rate at 1,2,3,and 5 years was 21%,13%,and 0% respectively.Conclusions Early diagnosis by adjunctive examinations is vital to treatment of hilar cholangiocarcinoma.Radical resection is the sole procedure to improve the outcome and prognosis of hilar cholangiocarcinoma.Multiple therapeutic methods should be adopted and should be individualized.
7.Effect of pretreatment with danazol on expressions of EGFR and VEGF in endometrium

Objective To study the expressions of EGFR and VEGF in endometrium after danazol pretreatment and the pathological changes of endometrium. Methods A total of 60 patients with anovulartory functional bleeding were randomly divided into two groups: Danazol group and control group with 30 cases in each group. Danazol group were given transcervical resection of endometrium with danazol pretreatment, and control group without any pretreatment. The endometrium resected from uterus was sent for histological assessment, observation of the grandular quantity and stromal loose degree, and of the expressions of EGFR and VEGF in endometrium. Results The endometrium of patients who took Danazol were almost in proliferative phase(similar to early proliferative phase).The numbers of endometrial glands near the basal layer with Danazol pretreatment were lower than that in the control group; the stromal components (+~) were more compact than the contral group (~). The expression intensity and rates of EGFR and VEGF in glands were higher than those of stromal components. Danazol decreased the expression of EGFR and VEGF in endometrium compared to the control group. Conclusion Pretreatment with Danazol can suppress the prolifieration of endometrial cells, thin the endometrium,and disperse the glands, compact the stromal components, and it can also reduce the expressions of EGFR and VEGF in endometrial glands and stromal components compared with the control group.
8.Dendrobium Resources and Ecological Investigation of Chongqing

Through the investigation on Dendrobium Sw. resources of Chongqing, the species, distribution, ecological environment, and industry situations were basically clarified. The results showed that six counties including the Nanchuan district and Shizhu county had about 4 species, which mainly grew on trunks of forest plants such as Quercus phillyreoides, Ficus virens, walnut tree, and Fagus longipetiolata, as well as cliff valley. Its growth of the main conditions and illuminations has a great influence on ecological factors in Dendrobium Sw. stem growth. The cultivated variety of Dendrobium Sw. was a single. It was still in the early-stage trial of industry development.
9.Iatrogenic bile duct injury: experience and management

Objective To investigate the etiology of iatrogenic bile duct injury and experience in its diagnosis and treatment. Methods A retrospective study was conducted on the clinical data of 86 patients with iatrogenic trauma in the bile duct. Results These 86 cases have received a total of 156 sessions of surgical procedures, including 2 explorations in 66 cases,3 explorations in 6 cases. Lateral bile duct injury in 22 cases was treated by suture repair and T tube stent. Eight cases suffering from iatrogenic transection of the common bile duct were retrieved by cholangiojejunostomy and T tube stent. Nine cases with inadvertent CBD ligature were managed by a lysis and T tube stent. Roux en Y cholangiojejunostomy was adopted in 77 cases. Sixty cases were followed up for 1~10 years, with good result in 90% of the cases. Conclusions Most CBD iatrogenic injury cases were encountered as a result of cholecystectomy.Roux en Y cholangiojejunostomy is the choice of therapy.
10.Potential pathogenicity of Bacillus amyloliquefaciens to infants

Bacillus amyloliquefaciensin can cause infections and the building related ill-health syndrome among newborns, infants and other immunocompromised populations by polluting hospital disinfectants, such as alcohol 75%, moisture housing and food. In this article, we reviewed the prevalence of Bacillus amyloliquefaciens in environment, food and intrahospital infection, the toxicology by describing its toxicity in mammalian cells, the physicoehemical characteristics by analyzing its special structure of bacterial spores.
