1.Levels and values of serum uric acid in patients with multiple system atrophy

Objective To investigate the level and value of serum uric acid (UA) in patients with multiple system atrophy (MSA).Methods One hundred and one MSA patients were selected as the MSA group and 101 age and sex-matched healthy controls as control group.The UA concentrations and other biochemical indexes were measured by an enzymatic method and carefully analyzed.By comparing the UA concentrations for different genders, subtypes, and different levels of certainty of the MSA group with their counterparts in the control group, we obtained the differences between these two groups and also discussed possible reasons and factors.Results The serum UA levels of MSA patients ((283.74 ± 87.76) μmol/L) were significantly lower than the controls ((317.86 ± 76.95) μmol/L;t =-2.94, P < 0.05).In addition, when the male and female groups were investigated separately, the decrease of serum UA was more significant in the male group compared to the female group (t =-3.88, P < 0.01).Furthermore, by comparing different subtypes of the MSA patients with the control group, we found that the serum UA levels in both P-type or C-type MSA patients were significantly lower than in controls (t =2.92, 2.02;all P < 0.05).By comparing different levels of MSA with the control group, we obtained that UA levels in both probable MSA patients and possible MSA patients were significantly lower than in controls (t =3.13, 2.09;all P < 0.05).By adjusting for the confounding factors, such as age, alanine aminotransferase, aspartate aminotransferase, urea nitrogen, creatinine, high density lipoprotein-cholesterol and low density lipoprotein-cholesterol, the multiple Logistic regression analysis showed that the numbers of MSA patients in the lowest level UA group (UA < 263.0 μmol/L) were higher than the high level UA group (UA > 370.5 μmol/L), though the difference did not reach statistical significance (P > 0.05).Nevertheless, the numbers of male MSA patients in the lowest level and the lower level UA groups (UA =263.0-309.0 μmol/L) were 11.5 and 10.8 times that in the high level UA group (P =0.001, P =0.003).No obviously linear relation between the level of UA and disease progression was found.Conclusions The level of serum UA is significantly decreased in MSA patients, especially in the male patients.The UA, as an antioxidant, may be beneficial to the neurodegenerative diseases.

3.Meta analysis of comparison between atropine and cyciopentolate in cycloplegia

Background Medical refraction after cycloplegia is the preferable choice for precise measurement of degree of refractive error.Drugs used in China for cycloplegia include atropine and tropicamide,and the use of cyclopentolate is an alternative for ophthalmologist.However,the data for the evaluation and comparison of efficacy of the available drugs in cycloplegia is still lacking.Objective This system analysis was to evaluate the difference between atropine and cyclopentolate in cycloplegia in children.Methods A systematic literature retrieval was conducted in MEDLINE,EMbase,Google residual accommodation after cycloplegia by atropine and cyclopentolate were compared.Statistical analysis was performed using the RevMan 5.1.0 software.Results A total of 7 studies were included in this meta analysis,including 6 cohort study design and 1 randomized,doubleblinded clinical trial and 1232 eyes.For retinoscopic evaluation after cycloplegia,no significant differences were found between cyclopentolate and atropine in children with hyperopia and myopia (WMD =-0.21,95% CI:-0.47-0.06,P=0.13 ; WMD =-0.10,95% CI:-0.36-0.15,P =0.43).For residual accommodation after cycloplegia,no significant difference was seen between cyclopentolate and atropine in ammetropic children (WMD =0.30,95% CI:-0.10-0.71,P =0.15).Conclusions Cyclopentolate shows the same effect on the cycloplegia as atropine in children,and it can take the place of atropine in cycloplegia in childhood.

Objective To investigate the infection of Helicobacter pylori infection on the serum concentration of homocysteine (Hcy) and its relationship with coronary heart disease (CHD) .Methods 159 cases of patients with CHD were selected as the re‐search subjects .They were divided into two groups :infection group and non‐infection group ,according to the results of 14 C‐urea breathe test .And they were also divided into three groups :negative group ,mild infection group and severe infestation group ,accord‐ing to the severity of infection .The CHD patients infected Helicobacter pylori were divided into three groups:low risk group ,mod‐erate risk group and high risk group ,according to SYNTAX scores .The serum Hcy concentration was determined by cyclic enzy‐matic method .Results Comparing with non‐infection group ,the serum Hcy concentration significantly increased in infection group (P<0 .01) .With the aggravation of Helicobacter pylori infection ,the serum Hcy concentration increased .There were significant difference among negative group ,mild infection group and severe infestation group (P<0 .01) .In the CHD patients infected Heli‐cobacter pylori ,the serum Hcy concentration also increased with the aggravation of the severity of coronary lesion .And there were significant difference among low risk group ,moderate risk group and high risk group (P<0 .01) .Conclusion Helicobacter pylori plays a role in the incidence and development of CHD through increasing the serum Hcy concentration .
7.Cemented versus cementless hemiarthroplasty for femoral neck fractures in the elderly

BACKGROUND: Hemiarthroplasty is widely recommended for the treatment of femoral neck fractures in elderly patients, but it is controversial in choosing cemented or cementless fixation types. OBJECTIVE: To compare the clinical effects of cemented hemiarthroplasty with cementless hemiarthroplasty for the treatment of femoral neck fractures in elderly and to provide a scientific basis for the selection of clinical prosthesis. METHODS: 167 elderly patients with femoral neck fractures undergoing hemiarthroplasty in the Foshan Hospital of Traditional Chinese Medicine from March 2013 to March 2015 were enrolled in this study. According to the type of prosthesis, patients were divided into cementless group (n=81) and cemented group (n=86). The operation time, intraoperative blood loss, hemoglobin levels 1 day after surgery, reoperation rate, mortality and complications at 1 year after surgery were compared. After surgery, clinical function of hip joint was assessed by Harris scoring system. The pain was evaluated by Visual Analogue Scale. RESULTS AND CONCLUSION: (1) All patients were followed up for ≥ 12 months. (2) The operation time in the cementless group was significantly shorter than that in the cemented group (P < 0.01). (3) There was no significant difference in the intraoperative blood loss and hemoglobin levels at postoperative 1 day (P > 0.05). (4) There were no significant differences in Harris hip scores and Visual Analogue Scale score at various time points between both groups (P > 0.05). (5) The incidence of thigh pain in cementless group was slightly higher than that in cemented group at 1 week, 1 and 6 months after operation, but the differences were not statistically significant (P > 0.05). Two patients died from myocardial infarct in the cementless group, and one patient died from pulmonary infection in the cemented group. The mortality was not significantly different at postoperative 1 year between the two groups (P > 0.05). No complications, such as prosthesis loosening and sinking, periprosthetic fracture, and periprosthetic infection, which needed to be refurbished, occurred in both groups. (6) Our findings suggest that both cemented and cementless hemiarthroplasties can obtain satisfactory effect in the treatment of femoral neck fractures in the elderly patients. Cementless hemiarthroplasty can significantly shorten operation time.
8.Effects of alkaloids from Coptidis Rhizoma on mouse peritoneal macrophages in vitro.

This work was mainly studied the effects of the four alkaloids from Coptidis Rhizoma on the mouse peritoneal macrophages in vitro and preliminarily discussed the regulating mechanisms. The effect of alkaloids from Coptidis Rhizoma on the vitality of macrophages was measured by the MTT assay. The effect of alkaloids on the phagocytosis of macrophages was determined by neutral red trial and respiratory burst activity was tested by NBT. The expressions of respiratory-burst-associated genes influenced by alkaloids were detected by qRT-PCR. The conformation change of membrane protein in macrophages by the impact of alkaloids was studied by fluorospectro-photometer. Results showed that the four alkaloids from Coptidis Rhizoma could increase the phagocytosis of macrophages in different level and berberine had the best effect. Berberine, coptisine and palmatine had up-regulation effects on respiratory burst activity of mouse peritoneal macrophages stimulated by PMA and regulatory activity on the mRNA expression of PKC, p40phox or p47phox, whereas the epiberberine had no significant influence on respiratory burst. Moreover, alkaloids from Coptidis Rhizoma could change the conformation of membrane protein and the berberine showed the strongest activity. The results suggested that the four alkaloids from Coptidis Rhizoma might activate macrophages through changing the conformation of membrane protein of macrophages and then enhanced the phagocytosis and respiratory burst activity of macrophages. Furthermore, the regulatory mechanism of alkaloids on the respiratory burst activity of macrophages may be also related to the expression level of PKC, p40phox and p47phox.

9.Role of acid-sensing ion channel 1a in global cerebral ischemia-reperfusion injury in rats

Objective To investigate the role of acid-sensing ion channel 1a(ASIC1a) in global cerebral ischemia-reperfusion injury in rats.Methods Forty male SD rats weighing 250-300 g were randomly divided into 4 groups (n =10 each): sham operation group (group S),cerebral ischemia-reperfusion group (group I/R),solvent control group (group SC) and group PcTX1 (a ASIC1 a blocker,group P).Global cerebral ischemia-reperfusion was induced by four-vessel occlusion.PcTX1(500 ng/ml)6 μl or solvent 6 μl was injected into the crerbral ventricular at the begining of reperfusion in groups P and SC respectively.The rats were sacrificed at 24 h of reperfusion,and then the hippocampi were removed for determination of Caspase-3,Bcl-2 and Bax protein expression and microscopic examination.Results Compared with group S,the expression of Caspase-3,Bcl-2 and Bax protein was up-regulated in groups I/R,SC and P (P < 0.05).Compared with group I/R,the expression of Caspase-3 and Bax was down-regulated,and the expression of Bcl-2 was up-regulated in group P ( P < 0.05).There was no significant difference in Caspase-3,Bcl-2 and Bax protein expression between groups I/R and SC (P > 0.05).The histopathologic damage was ameliorated in group P as compared with group I/R.Conclusion ASIC1a can induce global cerebral ischemia-reperfusion injury in rats by up-regulating Caspase-3 and Bax expression,and down-regulating Bcl-2 expression and inducing apoptosis.
10.Analysis and treatment of postoperative complications using one-stage posterior spinal osteotomy in the treatment of severe spinal deformity

Objective To analyze the cause and treatment of the postoperative complications using one-stage posterior spinal osteotomy in the treatment of severe spinal deformity.Methods From September 2006 to May 2013,17 patients with severe spinal deformity (congenital scoliosis in 11 cases,congenital kyphosis in 4 cases,and congenital kyphoscoliosis in 2 cases) underwent one-stage posterior spinal osteotomy,including 5 males and 12 females with an average age of 22.6 (14-51) years.The preoperative mean coronal Cobb angle was 109° (85°-160°) while the mean sagittal Cobb angle was 104° (65°-152°).Two patients had neurological symptoms preoperatively whose spinal cord function was D,evaluated by ASIA classification.All patients were treated with pedicle subtration osteotomy and pedicle screw internal fixation,which SPO osteotomy 2 cases,PSO osteotomy 11 cases,VCR osteotomy 4 cases.Results There were 17 cases of complications in 147 patients,the complication rate was 11.6%(17/147).The causes were as followed,screw malposition in 2 cases,compromised by close of resected areas in 2 case,residual bone compression in 1 case,acute spinal cord injury in 2 cases,infection in 2 cases,broken stick or loosen hat in 3 cases,and superior mesenteric artery syndrome in 5 cases.Postoperative neurological complications occurred in 7 cases.Two cases with preoperative ASIA D became ASIA C,5 cases with normal nerve function became ASIA C in 2 cases and ASIA D in 3 cases.After surgical exploration,given Methylprednisolone and neurotrophic drugs,removal or changing of the internal fixation,anti-infection and symptomatic treatment,15 cases recovered completely and 2 cases improved partially.Conclusion One-stage posterior spinal osteotomy for severe spinal deformity is technical demanding and risky,and the postoperative complications are common.Appropriate operative procedure,close observation of sensation and motor function,timely surgical exploration and nerve decompression,and early brace wear are all required.
