Objective:To understand the status and identify the determinants of self-treatment of rural floating population in Beijing. Methods: The data were derived from the Fourth National Health Service Investigation of China, part of which was about rural floating population in Beijing. According to Andersen health services utilization model, Binary Logistic Regression model was used to analyze the influencing fac-tors. Results:In the study, 51. 7% of the research objects were inclined to self-treatment when they ever felt uncomfortable or sick in Beijing. The Binary Logistic Regression model suggested that, some factors from predisposing, enabling and need, were statistically significant determinants of utilization of self-treat-ment. Conclusion:There exists a certain percentage of self-treatment behavior for rural floating population in Beijing, and we should take some measures to promote the efficacy and safety of self-treatment.

Selectins are a family of adhesion molecules,including P-,L-,E-selectins.The three adhesion molecules all participate in the inflammatory processes of ischemia-reperfusion injury of brain.P-selectin is expressed on activated platelets as well as on endothelial cells.E-selectin is only expressed on endothelial cells.P-and E-selectin mediate the adhesion of the leukocytes,platelets and endothelial cells.L-selectin is mainly expressed on leukocytes and mediates leukocytes rolling contact with microvascular endothelial cells.

Objective To evaluate the efficacy and tolerability of long-term application of cyclosporin A(CsA) in patients with severe aplastic anemia(SAA).Methods 39 patients with SAA were treated with CsA and androgen.The efficacy at 3-month,6-month,1-year,2-year treatment was evaluated.Main side effects of CsA were assessed.Results An overall response rate to CsA was 45.5% in the patients with SAA.19.1% of patients showed response to the treatment at 3 months,45.0% at 6 months,56.3% at 1 year,83.3% at 2 years,respectively.The early mortality rate in SAA Ⅰ patients was significantly higher than that in SAA Ⅱ patients(P

Objective To explore the clinical characteristics of pancytopenic acute leukemia(PAL) and hyperleukocytic acute leukemia(HAL). Methods 183 patients with primary AL were analyzed retrospectively and divided into three groups according to patients' hemogram. Results Among these patients, 38 cases(20 8%) were PAL(group A), 42 cases(23 0%) were HAL(group B) and 103 cases(56 3%) were generic type(group C). 23 0% cases of acute myeloid leukemia(AML), 56 3% acute promyelocytic leukemia and 16 9% acute lymphoblastic leukemia( ALL) appeared PAL. The manifestations of group A were mainly milder infiltration of extra-bone marrow, more severe cytopenias. The early mortality, short and long-term effectiveness were similar to those of group C. 23 8% AML, 51 2% acute monoblastic leukemia and 18 6% ALL presented HAL. There were higher tumor burden,obviously infiltration of extra-bone marrow, higher early mortality, more refractory treatment, poorer prognosis in group B as compared with group A or C. Conclusions Pancytopenia or hyperleukocytosis were common manifestations in primary AL. It is important to understand these features of patients' hemogram for diagnosis, treatment and prognosis evaluation of leukemia.

Objective To search for new tumor markers from urine of bladder transitional cell carcinomas. Methods Urine samples from 61 bladder cancer patients who were histologically diagnosed and 53 healthy volunteers and 42 cases with other urogenital diseases were analyzed using surface-enhanced laser desorption/ionization time of flight mass spectrometry (SELDI-TOF-MS) IMAC-Cu-3 ProteinChip technology,which can specifically bind the metal-combining-proteins.Proteomic spectra were generated by mass spectrometry.A blinded test set was used to determine the sensitivity and specificity of the differentially expressed markers. Results Four differentialy expressed potential markers were identified.Their corresponding molecular weights were 3445,3703,3896 and 5932, respectively.The 3445 protein was identified as ?-defensin,with its sensitivity for diagnosing bladder cancer being 80% and specificity being 75%.The sensitivity of 3703 protein was 32%,while its specificity was 94%.97% of the patients who were positive for 3703 protein were with high grade and invasive bladder cancer.The sensitivity for 3896 and 5932 proteins were 78% and 55%;the specificity for them were 40% and 35%,respectively. Conclusions SELDI-TOF-MS ProteinChip technology is a quick,easy and practical,high throughput analytic method.It can screen out several relatively specific,potential markers from urine of bladder cancer patients,so it has better clinical feasibility.

Objective To explore the prevalence of metabolic syndrome( MetS) and its components in patients with bipolar disorder and their lifestyle. Methods Collected 148 cases of bipolar disorder in patients with body mass index (BMI),blood glucose,blood pressure,triglyceride(TG),high-density lipoprotein( HDL-C) etc, and self-compiled scale was used to assess their lifestyle. All patients were followed 6 months. The patients werecompared with 65 normal controls. Results The patients group in the baseline period,treatment after 1 months,3 months and 6 months,the incidence of MetS was as followed:11.5% ,15.5% ,18.2% ,20.9%. The patients at all stages,the incidence of MetS,BMI≥25kg/m2 ,high TG levels,low HDL-C levels and hypertension were significantly higher than the control (P<0.01). After 6 months of treatment the incidence of MetS,hypertriglyceridemia were higher than baseline (P < 0.05). The patients group' s work intensity, frequency of exercise, eat more than 3 times per day compared with the control group were significantly different (P<0.01, P<0.05). Conclusion The incidence of MetS and hyperuricemia in patients with bipolar disorder after treatment is higher than the control group; as a continuation of the treatment,the incidence of metabolic abnormalities increased gradually. Unhealthy lifestyle, lack of exercise,unreasonable diet maybe increase the risk of metabolic disorders.

Objective To analyze the clinical and imaging features of 2 siblings with leukoencephalopathy due to NADH dehydrogenase (ubiquinone)flavoprotein 2 (NDUFV2) gene mutation,in order to better understand and diagnose it earlier.Methods Clinical and follow-up data of the proband and his brother were collected.Clinical features including symptoms,signs and cranial magnetic resonance imaging (MRI) were analyzed,and 2 patients were followed up for a long time.Sanger sequencing,targeted next generation sequencing,and whole exome sequencing were performed to identify potential genetic variations in the 2 patients and their parents.Results (1) Clinical characteristics and follow-up:ages of onset were 4 months and 1 year respectively.Both of the patients presented rapid motor regression hyperinyotonia,positive pathological character.During the follow-up the condition became stable,motor function and cognition improved gradually after cocktail therapy.(2) Brain MRI of the 2 patients showed prominent abnormalities in deep cerebral white matter,presenting T1 hypointense,T2 and fluid attenuated inversion recovery (FLAIR) hyperintense in the periventricular area.FLAIR images revealed that the abnormal white matter was partially rarefied and cavitated.Diffusion weighted images (DWI) showed high signals along the periphery of the involved areas.The follow-up MRI showed the cavitation still existed and even expanded,and DWI showed regional linear or spotty high signals around the original lesions.(3) Novel mutations in NDUFV2 gene,c.467T>A and c.404G>C,were identified in proband and his brother.The former inherited from his father,while the latter inherited from his mother,which was the new mutation not reported in the international.Conclusions The clinical features of the brothers presented subacute leukoencephalopathy with relatively stable or improved outcome.This was distinctive from the phenotypic features reported in 12 cases with hypertrophic cardiomyopathy or Leigh syndrome.The finding expanded the phenotypic spectrum of NDUFV2 mutations.Pathogenic gene of these patients which is the basis of genetic counseling for this family was determined.

To investigate the clinical improvement of limping gait in patients with ankylotic hips and limb length discrepancy.Methods:From 1996 to 2005,12 patients with ankylotic hips and limb length discrepancy were treated by distraction osteogenesis with a mono-lateral external fixator and an intramedullary nail.The limb length discrepancy was 6.20-12.50 (median 8.45) cm.Limping gait was classified according to the recommendations of the American Academy of Orthopedic Surgeons/Hip Society and scored according to Harris:no limping scored 11 points,mild limping scored 8 points,moderate limping scored 5 points,while severe limping scored 0 points.Limping gait was severe in all patients pre-operatively and the total score was 0.Results:All patients were followed up for 30.00-46.00 (median 38.55) months,and all reported improvement in limping gait.The gain in length was 6.00-12.50 (median 8.20) cm,and the mean residual limb length discrepancy was 0-0.50 (median 0.20) cm.The total treatment time was 41.00-82.00 (median 61.50) weeks,the lengthening time was 14.00-38.00 (median 29.55) weeks.At the last follow-up,10patients had mild limping gait and 2 had moderate limping gait; the total score was 90.00.The median score was 7.50 (P25 was 8.00,P75 was 8.00).According to Wilcoxon signed rank test,the post-operative limping gait scores were significantly higher than pre-operative (P=0.001 ).Conclusion:Femoral lengthening can improve the limping gait significantly in ankylotic hips and limb length discrepancy.

BACKGROUND:Neuron nitrogen monoxide(NO) is related to synaptic plasticity and is the key transmitter of normal learning and memory.Neuronal nitric oxide synthase(nNOS) is the biological synthetic enzyme of NO in neurons.The reports have been fewer yet on cerebral nNOS activity in Alzheimer disease(AD) patients,especially on changes in positive neural fibers of nNOS in temporal lobe cortex and hippocapmus CA1 region. OBJECTIVE:To observe cerebral nNOS activity in AD patients, especially on changes in positive neural fibers of nNOS in temporal lobe cortex and hippocapmus CA1 region and effects of Tiantai Ⅰ on nNOS activity. DESIGN:A randomized and controlled trial. SETTING:Institute of Collaboration of Chinese and Western Medicine, Shenzhen City. MATERIALS:The experiment was performed in No.2 Grade Animal Experimental Room of Institute of Collaboration of Chinese and Western Medicine,Shenzhen City.A total of 65 Kunming mice were employed in the experiment. METHODS:The experimental animals were bred till 21 months old and the mice with senile dementia were screened from the aged ones according to the international general standards and methods.The experimental animals were randomized into 4 groups,named as senile dementia group,western drug control,group with small dosage of Tiantai Ⅰ and group with large dosage of Tiantai Ⅰ .In addition,a group with normal learning and memory of the senile mice(simply called senile normal group) was designed.There were 13 mice in each group.In western drug control,hydergine 0.6 mg/kg was prescribed; in the groups of small and large dosage of Tiantai Ⅰ ,the dosages were 6.80 g/kg and 20.41 g/kg respectively,continuously for 60 days.In senile normal group and senile dementia group,the bi distilled water of equal dosage was applied for perfusion.The scores of learning and memory were determined by step down test. Slices of brain tissues were prepared with frozen,nNOS activity was displayed by NBT histochemistry method and the analysis was done systematically with auto micrography in quantity. RESULTS:Learning and memory were significantly decreased in mice with senile dementia(P< 0.01),the positive reaction of nNOS was remarkably weaker compared with normal aged mice.It was the first time to discover that the surface densities of nNOS positive neural fibers in temporal lobe cortex and hippocampus CA1 region [(9.48± 2.09),(5.11± 2.74) respectively]were remarkably lower than those of the normal aged mice [(17.75± 3.47),(11.95± 2.00) respectively](P< 0.01). Simultaneously, it was also the first time to discover that normal correlation was apparent between nNOS activity of cortex and hippocampus and the scores of learning and memory.Tiantai Ⅰ could significantly increase the surface densities of nNOS positive neural fibers in cortex and hippocampus in senile dementia mice, of which, the values of the small dosage were (11.57± 2.44) and (8.18± 2.92) respectively and of the large dosage were (12.59± 2.88) and (9.33± 2.18) respectively,(P< 0.01- 0.05). CONCLUSION:The hypo activity of central nNOS in senile dementia mice resulted in significantly decreased positive neural fibers of nNOS in cerebral cortex and hippocampus.The surface densities of nNOS positive neural fibers in temporal lobe cortex and hippocampus CA1 region were significantly related to the scores of learning and memory.Tiantai Ⅰ could remarkably improve the disturbance of learning and memory in spontaneous senile dementia model and enhance its nNOS activity in cerebral cortex and hippocampus.
