Objective To observe the expression of bag-1 and heat shock protein (HSP) 70 in clinical malignant tumors and the correlation between them. Methods RT-PCR and Immunohistochemistry were employed to examine the expression of bag-1 and HSP70 in 69 cases with gastroenteric cancer (54 cancer tissues and 15 non-cancer tissues). Results The expression of Bag-1 was detected in 96.3% of the cases, much higher than that in the control group (P0.05), and no correlation was observed between histological grade and HSP 70 expression. Conclusion There exists definite relationship between the gastroenteric cancer and the expression of bag-1 and HSP70, and there are close correlation between the tumor progress and the expression of bag-1.

Objective To evaluate the efficacy of oxycodone for painless gastroscopy in pediatric patients when combined with propofol.Methods Sixty pediatric patients of either sex,aged 2-7 yr,weighing 10-28 kg,of American Society of Anesthesiologists physical status Ⅰ or Ⅱ,scheduled for elective painless gastroscopy,were divided into 2 groups (n=30 each) using a random number table:oxycodone group (O group) and sufentanil group (F group).The pediatric patients held by their parents entered the gastroscopy room,and propofol 1.5-2.0 mg/kg and penehyclidine hydrochloride 0.3 mg were intravenously injected.Oxycodone 0.1 mg/kg and sufentanil 0.1 μg/kg were intravenously injected in O and F groups,respectively.When body movement occurred during operation,additional propofol 1.0 mg/kg was intravenously injected.The occurrence of adverse reactions such as intraoperative body movement,laryngospasm and respiratory depression was recorded.The occurrence of postoperative vomiting and pain (FLACC pain scale scores ＞3) was also recorded.Results The incidence of intraoperative body movement,laryngospasm and respiratory depression and postoperative pain was significantly lower in O group than in F group (P＜0.05).Conclusion When combined with propofol,oxycodone produces better efficacy than sufentanil for painless gastroscopy in pediatric patients.

Objective To investigate the effect of lobaplatin combined with irradiation on human nasopharyngeal cancer cell line CNE2,and to illuminate its mechanism of radiosensitization.Methods MTT assay was used to detect the outcome of lobaplatin and irradiation on CNE2 cell proliferation.Clonogenic assay was applied to testify the radiosensitization effect of lobaplatin on the cells.Flow cytometry was used to check the cell cycle distribution and cell apoptosis.Western was used to detect the expression of Bcl-2,Bax and cleaved Caspase-3.Results The proliferation of CNE2 cells was reduced by lobaplatin in a dose-dependent manner.50IC of lobaplatin on CNE2 cells and lobaplatin combined with 4 Gy irradiation was 1.610 μmol/L and 0.077 μmol/L,respectively.The radiosensitization ratio of the combination group was over 3.Within 24 h of drug treatment,the percent of cells in G2/M phase increased with the concentration of lobaplatin.When the concentration of lobaplatin increased to 6 μmol/L,the cells of combination group were arrested at S phase.The apoptosis rate of lobaplatin (5 μmol/L) group,radiotherapy(4 Gy)group and combination group was 15.6％,11.3％ and 61.8％,respectively.Western blot showed that the expressions of Bax and cleaved Caspase-3 increased but Bcl-2 decreased in the combination group.Conclusion Lobaplatin could increase radiosensitization of human nasopharyngeal cancer cell line CNE2,probably by depressing Bcl-2 but enhancing Bax expression and hence activating Bcl-2/Bax-Caspase signaling pathway.

BACKGROUND:Because insulin-like growth factor I has the ability to induce mesenchymal stem cells into chondrocytes, we hypothesized that the chondrogenic diferentiation of adipose-derived stem cels can be improvedvia insulin-like growth factor I transfection. OBJECTIVE:To investigate the influence of insulin-like growth factor I transfection on chondrogenic potential of adipose-derived stem cels in vitroand tumor necrosis factor-like weak inducer of apoptosis (TWEAK)/fibroblast growth factor-inducible 14 (Fn14) signaling pathway. METHODS:Recombinant lentivirus plasmid pLVX-IGF-I-IRES-ZsGreenl was constructed and transferred into passage 3 adipose-derived stem cels, and then these stem cels were induced to diferentiate into chondrocytes (experimental group 1). Meanwhile, the cels transfected with pLVX-IRES-ZsGreenl were taken as green fluorescent protein/adipose mesenchymal stem cel group (experimental group 2), and those with no transfection acted as control group. RESULTS AND CONCLUSION:The mRNA expression of TWEAK was reduced in the experimental group 1 as compared with the other two groups, but the mRNA expressions of insulin-like growth factor I, Col2a1 and Sox9 were up-regulated in the experimental group 1. At the same time, the protein expression of matrix metaloproteinase-3 and TWEAK were down-regulated, while the protein expression of Col2a1 was increased in the insulin-like growth factor I-transfected cels in contrast to the cels modified with pLVX-IRES-ZsGreenl or with no transfection. These findings indicate that pLVX-IGF-I-IRES-ZsGreenl transfection of adipose-derived stem cels results in a higher expression of insulin-like growth factor I, and down-regulates the expression of TWEAK mRNA and protein, which improves the diferentiation of adipose-derived mesenchymal stem cels into chondrocytes.

Objective To explore the neutrophil to lymphocyte ratio (NLR) and its relationship with the effect of chemotherapy and prognosis in patients with diffuse large B-cell lymphoma (DLBCL).Methods The clinicopathological characteristics and outcome of 51 patients with DLBCL diagnosed by pathological biopsy and immunohistochemistry who received CHOP or R-CHOP regimen were collected and reviewed.According to the median of NLR,the patients were divided into low NLR group (NLR≤2.32) and high NLR group (NLR＞2.32).The prognostic influence of the NLR on overall survival (OS) was studied by Kaplan-Meier method and Log-rank test.To evaluate the independent prognostic relevance of NLR,univariate and multivariate Cox regression models were applied.Results The complete response (CR) rates of the low and high NLR groups were 71.4 ％ (20/28) and 39.1％ (9/23),respectively (P =0.02).The OS in the low NLR group was significantly better than that in the high NLR group (1,2 and 3-year OSs were 96.4 ％,90.4 ％ and 72.3 ％ vs 63.9 ％,52.7 ％ and 42.2 ％,respectively,P =0.009).Univariate and multivariate Cox regression models analysis showed that NLR ＞ 2.32 was an independent prognostic factor (P =0.016).Conclusion An elevated NLR before treatment indicates the poor effect of chemotherapy and prognosis of patients.NLR is an independent prognostic factor for DLBCL.

BACKGROUND:Infrapatelar fat pad is often partialy resected in the knee surgery, which can be used as an important source of adipose-derived mesenchymal stem cels. OBJECTIVE: To explore the strategies of isolation, culture, and identification of adipose-derived mesenchymal stem cels from the infrapatelar fat pad and to detect the expression of cel surface markers of human adipose-derived stem cels. METHODS: Infrapatelar fat pad was obtained from patients undergoing knee arthroscopy surgery, and attached cels were obtained from adipose tissue by using colagenase I. Cels were cultured in 10% low-sugar DMEM. Stem cels proliferation was detected by means of MTT and then, cel growth curve was made. The obtained cels were induced and differentiated into adipocytes and osteocytes. Expressions of cel surface markers CD29 and CD44 were detected. RESULTS AND CONCLUSION:A few of attached cels were observed after cultured 24 hours. Cels proliferated faster and exhibited spindle shape after 1 week. Cel adherence and proliferation were speeded up after subculture. Growth curve of cels exhibited that the passages 5 and 2 cels had higher reproductive activity than passage 8 cels. The obtained cels can be induced and differentiated into adipocytes and osteocytes. Results from flow cytometry showed that 96.8% passage 5 cels expressed CD29 and 97.6% expressed CD44. These findings indicate that high-purity adipose-derived mesenchymal stem cels with high reproductive ability are easy to be isolated from the infrapatelar fat pad, which may be a kind of ideal seed cels for cartilage tissue engineering.

We report the management and maternal and neonatal outcomes of four cases of delayed interval delivery after cervical cerclage in twin pregnancies, who hospitalized in Shanghai First Maternity and Infant Hospital from year 2017 to 2018. Four patients with dichorionic diamniotic twin pregnancies all presented with gradually weakened uterine contractions after spontaneous vaginal delivery of twin 1. After informed consent, the patients received cervical cerclage at 2 to 3 days after delivery when the maternal and fetal conditions were stable, to delay delivery of twin 2. The gestational weeks at delivery of twin 1 were 25 +4, 23 +1, 27 +1, and 23 weeks, which were noted for 30 +1, 32 +6, 31 +4, and 24 weeks for twin 2, respectively. Delayed delivery intervals were 32, 69, 31 and 7 d. Three of the four twin 2 survived, with Apgar score ≥ 8 at 1 minute and birth weight of 1 390, 2 290 and 1 620 g. During a 2-year follow-up, no developmental abnormalities were observed in the children, and no serious maternal complications were found, including severe infection, bleeding or cervical laceration.
