Objective To evaluate the effect of procaine on prevention of skin slap necrosis after modified radical mastectomy in breast cancer.Methods 106 breast cancer patients were randomly divided into treatment and control groups.In the treatment group,procaine(1% 100ml diluted in 300ml mormal saline,42℃)were compressed under the skin flap in operation,the control group received same amount of normal salin instead.The rates and sizes of skin flap necrosis were obeserved and compared.Results There were 13 cases suffured from skin flap necrosis among 106 breast cancer cases who received surgery operation.The rate of the flap necrosis in treatment group was5.76%(3/52),and the control group was 18.51%(10/54),there was significant difference(P

50 years).The values of ADC,FA,?_1,?_2 and ?_3 in putamen and globus pallidus were calculated after diffusion tensor scanning,and then analyzed the differences between the two tissues.The differences of ADC,FA,?_1,?_2 and ?_3 values among age groups were analyzed by using analysis of variance(ANOVA),and their relationship with age were accessed by using Pearson correlation.Results Between the correspondent age groups,the ADC value was higher in the putamen[(6.68?0.40)?10~(-4),(6.47?0.36)?10~(-4),(6.44?0.34)?10~(-4)mm~2/s]than that in the golbus pallidus[(6.13?0.50)?10~(-4),(6.05?0.33?10~(-4),(6.05?0.52)?10~(-4)mm~2/s],and the value of FA in the putamen(0.20?0.03,0.23?0.03,0.25?0.03)was lower than that in the globus pallidus(0.35?0.03,0.36?0.03,0.37?0.04).ANOVA analysis of the FA value among age groups revealed there were significant differences in the putamen(F=10.082,P=0.000),and the same analysis of eigenvalue ?_3 in the putamen also showed significant differences(F=5.675,P=0.005).The FA value in the putamen was significantly elevated with aging(r=0.555,P

OBJECTIVE: To analyze the case characteristics of poisoning by exhaust gas of the imperfect combustion of natural gas and provide references for forensic identification and prevention of such accidents. METHODS: Twenty-two cases of poisoning by exhaust gas of the imperfect combustion of natural gas in Minhang District during 2004 to 2013 were collected. Some aspects such as general conditions of deaths, incidence time, weather, field investigation, and autopsy were retrospectively analyzed. RESULTS: In the 22 cases, there were 15 males and 16 females. The age range was between 2 and 82 years old. The major occurring time was in January or February (8 cases in each) and the cases almost occurred in small area room (21 cases). There was wide crack next to the exhaust port when the gas water heater was been used in all cases. CONCLUSION There are more prone to occurrence of exhaust gas poisoning of imperfect combustion of natural gas in small area room with a ventilation window near the exhaust port of gas water heated. It shows that the scene of combustion exhaust gas poisoning should be more concerned in the cold season.

Objective To observe the relationship of awake bispectral index (BISawake) and the postoperative cognitive dysfunction (POCD). Methods 100 patients aged 60~75 years with ASA status Ⅰ~Ⅱ, underwent elective lower abdominal surgery with intravenous anesthesia (propofol, remifentanyl and rocuronium). The BIS was adjusted in 40~60 during the surgery. Their cognitive function was assessed with the neuropsychological test battery commended by International study of postoperative cognitive dysfunction (ISPOCD). Results 8 patients withdrew for unwilling or serious complications. The BISawake was more than 65 in 68 patients (group A), and less than 65 in 24 patients (group B). The difference of the verbal learning immediate and delayed of Hopkins Verbal Learning Test-Revised was more in the group B than in the group A. Conclusion Lower BISawake may be associated with the decline of postoperative cognitive function.

ObjectiveTo observe the effect of megace (MA) on the survival condition of cancer patients in chemotherapy (CT) periods.Methods92 patients with cancer were divided into the MA group (treated with MA+CT) and control group (treated only with CT). The changing of the appetite, normal food amount, weight, gastrointestinal reaction and whole body conditions of two groups were evaluated.ResultsIn the MA group, 52.2% patients had appetite improvement, 47.8% had more food amount, 45.7% gained more weight, 50% had no obvious gastrointestinal reaction such as vomiting and nausea, and 50% had improve the survival condition according to the Karnofsky performance status (KPS) scores (increment >10). In the control group, only 6.5% had appetite improvement, 4.3% had more food amount, 13% gained more weight, 28.3% had no vomiting and nausea, and 15.2% had improved the survival condition. There was a significant difference between two groups (P<0.01).ConclusionMegace is able to reduce nausea and vomiting caused by CT, improve appetite, increase patients weight, protect bone marrow from the inhibition of CT, improve the life quality of cancer patients, and has no evident side-effects.

Evaluation of the permeability mainly focuses on intestinal absorption in biopharmaceutics classification system (BCS). It is more complicated that the absorption and metabolism under multicomponent environment in biopharmaceutics classification system of Chinese materia medica (CMMBCS) compared with single component environment, which needs suitable mathematical models to be described. Therefore, with full consideration of existing single component mathematical algorithm combining with the characteristics of intestinal absorption and metabolism, we explored and designed a new mathematical algorithm of intestinal absorption and metabolism of multicomponent drug. Then we put forward a new coefficient, P (influence), the relative change rate of the single component's intestinal absorption and metabolism under multicomponent environment compared with single component environment, which described the influences of intestinal absorption and metabolism of the component under multicomponent environment. Moreover, P (influence) highlights the distinctive characteristics of multicomponent drug's intestinal absorption and metabolism, and lays the foundation for the construction of CMMBCS.

Based on the characteristics of multicomponent of traditional Chinese medicine and drawing lessons from the concepts, methods and techniques of biopharmaceutics classification system (BCS) in chemical field, this study comes up with the science framework of biopharmaceutics classification system of Chinese materia medica (CMMBCS). Using the different comparison method of multicomponent level and the CMMBCS method of overall traditional Chinese medicine, the study constructs the method process while setting forth academic thoughts and analyzing theory. The basic role of this system is clear to reveal the interaction and the related absorption mechanism of multicomponent in traditional Chinese medicine. It also provides new ideas and methods for improving the quality of Chinese materia medica and the development of new drug research.

Objective To explore the clinical value of image gradings of T2 weighted imaging (T2WI) and diffusion weighted imaging (DWI) for prostate cancer diagnosis in patients with prostate specific antigen (PSA) less than 20 μg/L.Methods A total of 133 patients with PSA＜20 μg/L who received prostate biopsies in our hospital from December 2011 to January 2013 were recruited.The prostate was divided into six regions in MRI images.T2WI and DWI image gradings on each region in peripheral zone were evaluated with reference to histopathological evaluation of prostate biopsies.The significance of grading system for the diagnosis of prostate cancer in the two scanning methods alone or in combination were compared.Results 60 patients were diagnosed as prostate cancer.The positive biopsy rates were 2.8％,13.5％,20.4％,34.6％,52.6％ and 88.9％,respectively in grade 0-5 based on T2WI imaging grading.The positive biopsy rates were 11.1％,16.1％,48.5％,71.0％ and 86.1％,respectively in grade 1-5 based on T2WI+ DWI imaging grading.The positive biopsy rate was significantly increased with the increments of pre-established T2WI and T2WI+DWI image gradings (Z=-9.8552,-13.4148,both P＜0.0001).Receiver operating characteristic analysis showed that the diagnostic capability was better in T2WI combined with DWI than in T2W1 alone in prostate cancer detection (AUC:0.743 vs.0.715,P=0.0072).Conclusions T2WI image grading can predict the risk of prostate cancer in patients with PSA＜20 μg/L,and DWI can enhance the ability to predict prostate cancer.

Objective To evaluate the effects of adeno-associated virus-enhanced green fluorescent protein (AAV-EGFP)on the biologic behavior of rabbit's bone marrow stromal cells(BMSCs)by means of a simple method of culturing and osteogenic induction in vitro so as to find an ideal viral vector and cell tracing mark for tissue en- gineering.Methods Total bone marrow culture was conducted to obtain rabbit BMSCs which were then induced in the osteogenic direction.The morphology of the cells was observed continuously,and their surface antigen and ossification were detected by alkali phosphatase stain and Von Kossa stain.On the basis of the above results, AAV-EGFP was transfected into the induced cells.The morphologic changes of the cells,the expression time and intensity of fluorescent light were observed.The transfection efficiency was detected to find the best multiplicity of infection(MOI)value.The cell growth curves were drawn to evaluate the biologic effects of AAV-EGFP on the cyto-activity.Results The morphology and purity of the rabbit BMSCs obtained were good.The ossification of the cells was significant after osteogenic induction.The best MOI value was found to be 1?10~5.The expression intensity of fluorescent light was strong with the expression time more than eight weeks so that the fluorescent light could be observed after cell generations.The transfection efficiency of AAV was high without significant biologic effects on the cyto-activity.Conclusions The total bone marrow culture and in vitro cell induction can satisfy the requirements for seeding cells in tissue engineering.AAV is an ideal viral vector for tissue engineering.Transfection of AAV-EGFP to cells could be an ideal method for cell tracing mark.
