1.Influence of Yishen-Huoxue-Huatan capsule on the level of serum lipids, plasma endothelin (ET) and nitric oxide(NO) in patients with carotid atherosclerosis

ObjectiveTo observe the effects of Yishen-Huoxue-Huatan capsule on carotid artery intima-media thickness (IMT),carotid atherosclerosis plaque area and the level of serum lipids,plasma endothelin (ET) and nitric oxide (NO) in patients with carotid atherosclerosis.Methods126 patients with carotid atherosclerotic were randomly assigned to treatment group and control group.Patients in treatment group was treated by yishen huoxue huatan capsule,and patients in control group was treated by zocor.Each patient was treated for 24 weeks.ResultsAfter the treatment,carotid IMT [respectively (0.81±0.11)mm,(1.10± 0.27)mm],atherosclerotic plaque area [respectively (11.21 ± 9.3)mm2,(16.53± 6.9)mm2] in both the treatment group and the control group were significantly lower compared with the same group before treatment (P<0.05),and the treatment group was better than the control group (P<0.05); serum cholesterol (TC),respectively [ (4.51 ± 0.37) mmol/L,(5.15 ± 0.20) mmol/L],triglyceride (TG) [respectively ( 1.77± 0.09) mmol/L,(1.79 ± 0.36) mmol/L],high-density lipoprotein cholesterol (HDL-C) [respectively (1.28 ± 0.26) mmol/L,(1.27 ±0.36) mmol/L],low-density lipoprotein cholesterol (LDL-C) [ (2.50± 0.30) mmol/L,(3.10± 0.39) mmol/L] in the treatment group and the control group were improved significantly (P<0.05) compared with the same group before treatment,and the TC and LDL-C levels in the treatment group were significantly lower than the control group (P<0.05); plasma ET [respectively (67.26± 14.13) ng/L,(79.38± 11.54)ng/L] NO [respectively (71.65± 111.64)μmol / L,(60.64±14.02)μmol/L] in the treatment group and the control group were higher than those in the same group before treatment (P<0.05),and the plasma ET levels in the treatment group was significantly lower than the control group (P<0.05),while the plasma NO level was significantly higher(P< 0.05).ConclusionYishen-Huoxue-Huatan capsule is effective in the treatment of carotid atheroselerosis.
3.Effects of different acupuncture and moxibustion methods on ultrastructure of gastrocnemius in rats.

OBJECTIVETo explore the efficacy differences in early intervention of different acupuncture and moxibustion methods on gastrocnemius fatigue in rats induced by electrical stimulation.
METHODSFifty male SD rats were randomly divided into a control group, a model group, a hand acupuncture group, an electroacupuncture group and a moxibustion group, 10 rats in each group. Electrical stimulation of the sciatic nerve was given in the control group and gastrocnemius fatigue test was induced by electrical stimulation of the sciatic nerve in the model group after anesthesia without other treatment, but just take 6 times interval stimulation in the control group. The hand acupuncture group, the electroacupuncture group and the moxibustion group were treated with the corresponding acupoints stimulation method respectively for 20 min before gastrocnemius fatigue test, and Dazhui (GV 14) and Zusanli (ST 36) were selected. Immediately after gastrocnemius fatigue test, three or four gastrocnemius tissues at the same site on the right side were quickly taken for making specimen for transmission electron microscope (TEM). The changes of skeletal muscle ultrastructure of myofibrils, mitochondria, sarcoplasmic reticulum, glycogen particles were observed under TEM.
RESULTS(1) Muscle fibers disorder, partial mitochondrial vacuolization and glycogen particles smaller were shown in the model group. (2) No abnormalities were shown in the hand acupuncture group and the moxibustion group with mitochondrial morphology and number, which better than that in the model group, and glycogen particles increased. (3) Abnormal changes in morphology were shown in the electroacupuncture group with part of the muscle fibers derangement, Z line malalignment and a few mitochondria vacuolization.
CONCLUSIONHand acupuncture, electroacupuncture and moxibustion have the different effects on ultrastructure of gastrocoemius in rats. Acupuncture and moxibustion have shown good effects on the prevention and treatment of exercise-induced skeletal muscle cell and organelle damage and delaying exercise-induced fatigue.

4.The difference in acknowledging the AAHRPP between China and America.

With the active encouragement of the Chinese government, all domestic clinical research institutes pay more attention to the human research protect program (HRPP) during the process of clinical trials, and actively follow the regulations of medical ethical practice. We could make fully preparation for the accreditation by the correlated international organizations only by further analyzing the Association for Accreditation of Human Research Protection Program (AAHRPP) from a whole and in each accreditation field at different levels, thus having a clear understanding the difference in acknowledging the difference between China's hospitals and America's hospitals.

5.The Mechanisms of Tcrolimus on the Migration of Langerhans' Cells

Objective To investigate the mechanisms of tacrolimus on the migration of Langerhans′ cells. Methods The concentration of MCP-1 in the cultured supernatants of HaCaT cell treated with tacrolimus was determined by ELISA. The expression level of MCP-1 mRNA in HaCaT cell treated with tacrolimus was studied by RT-PCR method. Results The concentration of MCP-1 in HaCaT cell treated with 625 ~ 5 000 ng/mL of tacrolimus was significantly decreased. The expression level of MCP-1 mRNA was also markedly decreased. Conclusions Tacrolimus may probably decrease the expression of MCP-1 in keratinocytes and suppress the chemotactic ability of Langerhans′ cells precursor and Langerhans′ cells, thus inhibiting or diminishing certain local immunoreaction.
6.Effect of training under high temperature environment on changes in IL-17, neutro-phils and lymphocytes in peripheral blood of soldiers

Objective To observe the effect of a high temperature environment on IL-17 concentration , neutrophil and lymphocyte in peripheral blood of soldiers on a training task .Methods Totally 160 health training soldiers were randomly and equally divided into two groups .The test group was trained and exposed to an environment of ( 32 ±1 )℃ vs control group at ( 22 ±1 )℃ for 2 hours before the IL-17 levels in venous blood were measured and peripheral neutrophils and lymphocytes were determined.Results The body temperatures were (38.74 ±0.26),(39.23 ±0.24),and (39.51 ± 0.34)℃after training for 40 min,50 min,and 60 min under a high temperature environment , respectively, and there was a significant increase of body temperature compared to the soldiers trained under normal conditions (P <0.05).Their perceived exertion rating was 17.62 ±0.66, 18.03 ±0.56, and 18.47 ±0.84, respectively, much unlike the control group (P<0.05).After 2 hours of training under high temperature , the concentration of IL-17 was (7.12 ±4.03) pg/ml, while the number of neutrophils and lymphocytes was (6.43 ±1.27) ×109/L and (9.84 ±1.36) ×109/L, respectively in peripheral blood, significantly higher than in the control group (P <0.05).Conclusion The body temperature and rating of perceived exertion of the training soldiers are significantly elevated under a high temperature environment, and the concentration of IL-17, and the neutrophils and lymphocytes counts in peripheral blood are increased .
7.Heterologous Expression of Zygosacharomyces rouxii ZrGpd 1 in Pichia farinosa

To examine the effects of heterologous expression of ZrGPD1 (encoding glycerol 3-phosphate dehydrogenase ) cloned from osmotolerant yeast Zygosacharomyces rouxii on glycerol production in wild Pichia farinosa,the URA3 gene was amplified from P. farinosa as the homology integrative region. A recombinant plasmid (pUR-ZG) was constructed then transformed into P. farinosa by electroporation. The transformant pfa-gu was obtained by the selectable marker Zeocin TM . Primary results showed that the biomass of pfa-gu was higher than the wild type in the flask and after 72h fermentation the concentration of glycerol of pfa-gu was 37g/L enhanced 30% in comparison with the wild type. It is concluded that heterologous expression of ZrGPD1 is useful for increasing glycerol production and the ability of osmoregulation in P. farinosa.
8.Construction Fermentation of Engineered Strain and Properties of Recombinant ?-glutamyltranspeptidase

The ?glutamyltranspeptidase encoding gene(ggt) from Bacillus subtilis SYU 20016 was amplified by PCR. The ggt gene was inserted in pBV220 to yield the recombinant expression vector pBV220ggt. Overexpression of ggt in E.coli JM109 was achieved with pBV220ggt. SDSPAGE analysis showed an overexpressed recombinant product at about 65kDa,consistent with the molecular weight predicted from gene sequence. The ferment conditions of r-glutamyltranspeptidase were also discussed. The optimum temperature and pH for the enzyme were determined as 30℃ and 7.2 respectively.The cultures were incubated at 42℃ for 4h with broth volume 20ml/250ml flask and the yield of 6U/ml was obtained, enzyme activity of B. subtilis NX2 was only 3.2 U/ml.
9.Spectrum-effect relationship of Moutan cortex against lipopolysaccharide-induced acute lung injury.

This research is to study the relationship between HPLC fingerprints of Moutan Cortex, Paeoniae Radix Rubra and Paeoniae Radix Alba and their activity on lipopolysaccharide-induced acute lung injury. HPLC fingerprints of each extract of Moutan Cortex,Paeoniae Radix Rubra and Paeoniae Radix Alba were established by an optimized HPLC-MS method. The activities of all samples against protein and tumor necrosis a factor were tested by the model of lipopolysaccharide-induced acute lung injury. The possible relationship between HPLC-MS fingerprints and the activitieswere deduced by the Partial least squares regression analysis method. Samples were analyzed by HPLC-MS/MS to identify the major peaks. The results showed that each sample had some effect on acute lung injury. Four components with a lager contribution rate of efficacy were calculated by the research of spectrum-effect relationship. Moutan Cortex exhibited good activity on acute lung injury, and gallic acid, paeoniflorin, galloylpaeoniflorin and paeonol were the main effective components.

10.Treatment of two-part proximal humeral fracture with closed or mini-open assisted reduction and percutaneous pinning.

OBJECTIVETo discuss the therapeutic effects of closed or mini-open assisted reduction and percutaneous pinning for the treatment of two-part proximal humeral fracture.
METHODSTwenty-eight patients with two-part proximal humeral fractures were treated with closed or mini-open assisted reduction and percutaneous pinning from November 2005 to November 2006. There were 12 males and 16 females. The mean age was 54.7 years, ranging from 31 to 73 years. The two-part proximal humeral fractures of 28 patients were all fresh fractures. According to Neer classification, there were 18 patients with two-part surgical neck fractures and 10 patients with impacted valgus two-part greater tuberosity fractures.
RESULTSThe average operation time was 42 minutes and the wound healed at the primary stage. The X-ray showed all fracture had good reduction with almost no anterior angulation. The average neck shaft angle was changed from 175 (ranged, 160 degrees to 200 degrees) preoperatively to 136 degrees (ranged, 128 degrees to 142 degrees) postoperatively, and the displacement of greater tuberosity was reduced to less than 3 mm postoperatively in impacted valgus two-part greater tuberosity fracture group. The average clinical follow-up time was 10.3 months (ranged, 6 to 13 months). All fractures healed and had no avascular necrosis of the humeral head. The wires were removed 8 to 10 weeks post-op. According to the shoulder score of Constant-Murley and ASES,the mean ASES score was 91.2 points (ranged, 63 to 100 points) and Constant-Murley score was 90.4 points (ranged, 67 to 100 points). After the treatment, 12 cases got an excellent result, 14 good and 2 moderate.
CONCLUSIONClosed or mini-open assisted reduction and percutaneous pinning is not as easy as it looks and must be applied with fluoroscopy. But it is the effective method for managing the certain types of proximal humeral fracture with the advantage of less injury and faster recovery. The bone condition of proximal humerus is the key factor to the stability of fixation.
