1.Clinical analysis of the CD_(117)-weak and CD_(117)-negative gastrointestinal stromal tumors:a report of 13 cases

Objective To explore the CT features in CD_(117)-weak and CD_(117)-negative gastrointestinal stromal tumors(GIST).Methods The CD_(117)-weak and CD_(117)-negative Gist proved by pathologically and sur- gically in 13 cases were retrospectively analyzed.Results The tumor was located in the stomach in 6 cases, small intestine in 4 cases,colorectum in 2 cases,greater omentum in one case.The most common clinical manifestations were abdominal pain or discomfort in 6 cases,hematemesis or melena in 3 cases.Malignant tu- mors occurred in 8 cases.Benign tumors occurred in 5 cases.A soft tissue was identified in all 13 cases in CT examination.The CT examinations revealed cystic regions and bleeding in 5 cases.Calcification was not noted within the masses in all cases.The mean diameter of benign tumors was 3.4 cm?2.8 cm while that of malig- nant tumor was 7.5 cm?6.2 cm.All the cases underwent operation,the resection rate was 100%.After follow up from 6 months to 4 years in 10 cases,1 case died of tumor recurrence and metastasis,the others survived. Conclusion CD_(117)-weak and CD_(117)-negative Gist occurred more common in the stomach.CT is useful in lo- cation and differentiation of CD_(117)-weak and CD_(117)-negative gastric stromal tumors with regard to benign or malignant.Surgery is the main method for final diagnosis and treatment.
2.Effect of Neoadjuvant Chemotherapy Treatment on Prognosis of Patients with Advanced Gastric Cancer: a Retrospective Study.

OBJECTIVETo evaluate the prognostic effects of neoadjuvant chemotherapy (NAC) in patients with local advanced gastric cancer.
METHODSWe retrospectively analyzed prognosis in 191 patients with advanced gastric cancer, of whom 71 were treated with NAC and 120 received surgery only between February 2007 and July 2013. Postoperative complication rate was recorded. Survival by clinicopathological features, pathological T and N stages, and histopathological tumor regression was retrospectively compared between the two groups.
RESULTSAccording to Response Evaluation Criteria in Solid Tumors, none of the 71 patients in the NAC followed by surgery group showed complete response, 36 showed partial response, 25 had stable disease, and 10 had progressive disease. The chemotherapy response rate was 50.7%; the disease control rate was 85.9%. Grade 3/4 adverse events were seen in less than 20% patients, with acceptable toxicities. No difference was found in the overall postoperative complication rates between the two groups (7 versus 22 cases, P=0.18). Median survival time was significantly different, at 54 months in the NAC combined with surgery group and 25 months in the surgery-only group (P=0.025).
CONCLUSIONIn patients with operable gastric adenocarcinomas, NAC can significantly improve overall survival without increasing surgical complications.

3.Outcome assessment of health education on endemic diseases in Henan province in 2010

Objective To evaluate the effect of health education on endemic diseases in Henan province in 2010.Methods According to the requirements of The Education Program for Endemic Diseases Control in 2009 Henan Province,50 counties (districts) of 18 cities with endemic diseases were selected.Referring to the health education questionnaire in the program,knowledge of endemic diseases were asked of pupils and housewives.Intervention effects were evaluated two months after teaching pupils and housewives in counties,towns,villages and schools with the knowledge of endemic diseases and the questionnaire was used.Results Among 50 counties investigated,5523 pupils and 3206 housewives were surveyed in the baseline according to the requirement,and 5417 pupils and 2891 housewives were surveyed for outcome evaluation.The results showed that after this education,pupils' knowledge about endemic diseases increased from 66.31%(10 987/16 569) in the baseline survey to 93.84%(15 250/16 251),the difference have statistical significant(x2 =3877.78,P < 0.01) ; housewives from 67.56%(6133/9078)to 92.67%(8037/8673),the difference have statistical significant(x2 =1736.33,P < 0.01).After the education,the pupils' knowledge about the iodine deficiency disorders,drinking-water-borne fluorosis and arsenic poisning,coal-burning-borne fluorosis increased from 70.15% (6263/8928),62.29%(4423/7101),42.96% (185/270),68.52% (116/270) to 94.19% (8344/8859),93.05% (6376/6852),99.63% (261/270),96.67% (269/270),the difference have statistical significant (x2 =1749.85,1939.26,211.83,74.43,all P < 0.01),housewives' knowledge about the iodine shortage diseases,drinking-water-borne fluorosis,the endemic fluorine,the arsenic poisoning increased from 73.27% (3330/4545),62.79% (2677/4263,30.37% (85/135),62.96%(41/135) to 93.96%(4228/4500),91.08%(3555/3903),97.04%(123/135),91.11%(131/135),the difference have statistical significant (x2 =751.03,924.65,129.75,30.23,all P < 0.01).Conclusions Health education intervention has a marked effect in increasing the peoples' knowledge of endemic diseases.Therefore health education should be enhanced in the future.
4.Surgical therapy of thyroid cancer.

The prevalence of thyroid cancer has shown an upward trend in China in recent years. Advances in thyroid ultrasound and fine needle puncture cytology have improved the accuracy of the preoperative diagnosis of thyroid cancer. Also,the application of endoscopy-assisted techniques and intraoperative nerve monitoring technology and the further understanding of thyroid lymph node metastasis have made the thyroid surgeries safer and less invasive. This article summarizes the recent advances in the surgical therapy of thyroid cancer.

5.Analysis of allergens characteristic in 4637 patients with allergic rhinitis.

OBJECTIVETo determine the distribution of allergens in patients with allergic rhinitis during the past 5 years in Shanghai area, and provide clinical reference for management strategies with regional character.
METHODSPatients with symptoms of allergic rhinitis were given skin prick test (SPT) from June 2005 to May 2010. Among the patients, the distribution of all allergens was analyzed. The positive rate was compared with age, gender, season, and so on.
RESULTSAmong 4637 individuals, the total positive rate of SPT was 62.84%. Dust mites were identified as the mostly responsible allergens. There was significant difference of variation with seasons (χ2=93.581, P<0.01), the highest rate was found in autumn and the lowest was found in spring. There was significant difference of the positive rate in different months (χ2=112.001, P<0.01), the most in October. There was a negative correlation between the positive rate and age (r=-0.792, P<0.01). The response intensity to dust mite allergy was decreased with age increased (P<0.01).
CONCLUSIONSDust mites were identified as the most common allergens among patients with allergic rhinitis during the past 5 years in Shanghai area. The younger group had the tendency of higher positive rate and response intensity than the elder. The positive rates of dust mites varied with different seasons.

6.Radiosensitivity of nasopharyngeal carcinoma in relation to cell cycle synchronization effect of tumor necrosis factor alpha.

OBJECTIVETo investigate the effect of tumor necrosis factor alpha (TNFalpha) on radiosensitivity of nasopharyngeal carcinoma (NPC) in relation to TNFalpha-induced cell cycle synchronization.
METHODSThe radio-resistance of a NPC cell line subclone CNE-2Z-S1 was verified by in vivo experiments and flow cytometry was performed to evaluate cell cycle synchronization in TNFalpha-treated CNE-2Z-S1 cells. The radiosensitivity of the cell synchronized CNE-2Z-S1 cells was determined by clone formation in vitro and in vivo experiment in nude mice.
RESULTSTNFalpha was capable of inducing cell cycle arrest and synchronization of CNE-2Z-S1 cells. Pretreatment with TNFalpha remarkably enhanced the radiosensitivity of CNE-2Z-S1 in vitro, and in vivo experiments with nude mice also suggested the role of TNFalpha in enhancing the radiosensitivity of NPC.
CONCLUSIONTNFalpha can enhance the radiosensitivity of NPC cells by inducing cell cycle synchronization.

AIMTo investigate antimalarial mechanism of Qinghaosu ( QHS) and its derivatives.
METHODSThe electronic structure of QHS and its derivatives were completely optimized and calculated at B3LYP/6-31G * level, while the route was at HF/STO-3G level.
RESULTSThe peroxide bridge is the active center of QHS and induced by ferrous iron to produce cyclic product.
CONCLUSIONHeme can link with QHS derivatives.

8.Study on the effect of concentrations of glucose on initial adherence of Streptococcus mutans.

OBJECTIVETo study the effect of concentrations of glucose on the initial adherence of Streptococcus mutans (S. mutans) to saliva-coated hydroxyapatite (SHA), and to compare the initial adherence of S. mutans from caries-active group with that of S. mutans from caries-free group.
METHODSEach 10 clinical isolates of S. mutans from caries-active and caries-free subjects were used in this study. And S. mutans UA159 was also included in this experiment. SHA was used to simulate tooth surface in oral cavity. S. mutans clinical isolates and strain UA159 were cultured in TPY liquid medium containing 3H-TdR in the same radioactive concentration and glucose in 0.2%, 1.0%, 5.0% concentration. Then grown cells were harvested to produce a suspension. SHA and radiolabelled bacterial suspension (A550(nm) = 0.52) were mixed for 90 minutes, samples were assayed by using liquid scintillation counter, and binding abilities of strains were evaluated by the count per minute (CPM).
RESULTSThe initial adherence ability of S. mutans from caries-active group was higher than that of S. mutans from caries-free group (P < 0.05). And the initial adherence ability of S. mutans cultured in different concentration of glucose was also significantly different (P < 0.05), 5.0% glucose group had the highest adherence ability, and 0.2% glucose group had the lowest adherenceability.
CONCLUSION(1)Difference of the initial adherence of S. mutans might relate to difference of carious experiences; (2) Glucose may play an important role in S. mutans initial adherence, to some extent, S. mutans cultured in the higher concentration of glucose has the higher initial adherence property.

9.Clinical observation and mechanism study on treatment of senile dementia with Naohuandan.

OBJECTIVETo observe the therapeutic effect and mechanism of Naohuandan (NHD) in treating senile dementia (SD).
METHODSClinical study: Fifty-eight patients with SD, whose diagnosis conforms to the Diagnostic Standard of DSM-IV issued by American Association of Psychiatry, were enrolled and randomly assigned into two groups. The 30 patients in the treated group were treated with NHD, 4 capsules each time, 3 times daily. The 28 patients in the control group were treated with Piracetam, 1.6 g each time, 3 times daily. The therapeutic course for both groups was 3 months. The therapeutic efficacy was estimated and compared by comprehensive scores of memory and cognition, scores of Mini-mental State Examination (MMSE) and Activities of Daily Living (ADL). Experimental study: Rats were divided into the control group, the model group and the high-dosage and low-dosage NHD treated groups. The protective effect of NHD on the per-oxidative damage of hippocampal neurons in beta-amyloid protein induced SD model was observed and the related criteria were determined.
RESULTSClinical study showed that both NHD and Piracetam could improve the clinical symptoms of patients, the two medicines showing insignificant difference in total effective rate. But NHD was better in elevating MMSE score and lowering ADL score in patients than Piracetam (P < 0.05 and P < 0.01). Experimental study showed that (1) 24 and 72 hrs after modeling, the activity of SOD and GSH were lower and the level of MDA higher in the model group than those in the control group (P < 0.05 or P < 0.01). Compared with the model group at the corresponding time points, in the high-dosage NHD group, SOD and GSH were higher, MDA was lower (P < 0.05 or P < 0.01); but in the low-dosage NHD group, SOD at the 72nd hr was higher (P < 0.05) and MDA at 24th and 72nd hrs was lower (P < 0.01). And most of the criteria in the high-dosage NHD group was improved better than that in the low-dosage NHD group. (2) The survival rates of neurons in various groups were not different significantly (P > 0.05) 24 hrs after modeling, but that in the high-dosage NHD group was significantly higher than that in the model group (P < 0.01) and in the low-dosage NHD group 72 hrs after modeling (P < 0.05).
CONCLUSIONNHD is an effective Chinese herbal preparation for treatment of SD, and its mechanism is related with its inhibition on peroxidative injury and protection on neurons.

10.Research advances in molecular pathogenesis of thyroid cancer.

Thyroid cancer is the one of the most common endocrine tumors. The biological behaviors and prognoses of the thyroid cancer of different histological types remarkably differ. The highly invasive thyroid cancer responds poorly to traditional therapies. Recent research advances in the molecular mechanisms of the pathogenesis of thyroid cancer have revealed the roles of many genetic and epigenetic variations such as gene mutation, abnormal gene amplification, and abnormal gene methylation in the development of thyroid cancer, which provides new insights in the molecular diagnosis, prognosis, and target therapy of the thyroid cancer.
