3.A survey of iodine nutritional status in the populations of Guizhou Province after implementation of a new standard of salt iodine

Objective To investigate the iodine nutritional status in the populations of Guizhou Province after implementation of a new standard of salt iodine and to provide a basis in formulating strategy for control of iodine deficiency disorders.Methods In 2011 and 2014,30 units were sampled in Guizhou Province using the sampling method of Probability Proportional to Size.One primary school was chosen in each unit,by the randomized sampling method,40 or 50 students in each school were sampled for examining their urinary iodine level and for testing their household salt iodine level;12 and 50 children were chosen before and after the adjustment,respectively,for testing their urinary iodine level;near the location of the primary school,1 township was chosen,15-20 pregnant women in each township were sampled to test their urinary iodine level.Results Totally 1215 salt samples were collected before adjustment,the median of salt iodine was 32.2 mg/kg;consumption rate of qualified salt was 96.63%;totally 364 urine samples of 8-10 children were collected,the median of urinary iodine (MUI)of children was 308.9 μg/L;450 urine samples of pregnant women were sampled,and their MUI was 216.5 μg/L,among these,121 below 150 μg/L,the proportion was 26.9%.Totally 1 538 salt samples were collected after the adjustment,the median of salt iodine was 26.6 mg/kg;consumption rate of qualified salt was 96.40%;totally 1538urinary samples of 8-10 children were collected,the MUI of children was 218.3 μg/L;and 601 urinary samples of pregnant women were collected,the MUI was 175.1μg/L,among these,241 below 150 μg/L,the proportion was 40.1%.As can be seen from the results,the percentage of sample below 150 μg/L after the adjustment was increased to 13.2% compared to that before the adjustment,the difference was statistically significant (x2 =19.89,P <0.01).Conclusion After implementation of a new standard of salt iodine in Guizhou Province,the iodine nutrition is suitable in residents,but the iodine nutrition of pregnant women is in a trend of declining,which should be noticed.
4.Inflammatory response and immune regulation of high mobility group box-1 protein in treatment of sepsis

Sepsis is an infection induced systemic inflammatory response syndrome and is a major cause of morbidity as well as mortality in intensive care units. A growing body of evidence suggests that the activation of a proinflammatory cascade is responsible for the development of immune dysfunction, susceptibility to severe sepsis and septic shock. The present theories of sepsis as a dysregulated inflammatory response and immune function, as manifested by excessive release of inflammatory mediators such as high mobility group box 1 protein (HMGB1), are supported by increasing studies employing animal models and clinical observations of sepsis. HMGB1, originally described as a DNA-binding protein and released passively by necrotic cells and actively by macrophages/monocytes, has been discovered to be one of essential cytokines that mediates the response to infection, injury and inflammation. A growing number of studies still focus on the inflammation-regulatory function and its contribution to infectious and inflammatory disorders, recent data suggest that HMGB1 formation can also markedly influence the host cell-mediated immunity, including T lymphocytes and macrophages. Here we review emerging evidence that support extracellular HMGB1 as a late mediator of septic complications, and discuss the therapeutic potential of several HMGB1-targeting agents in experimental sepsis. In addition, with the development of traditional Chinese medicine in recent years, it has been proven that traditional Chinese herbal materials and their extracts have remarkable effective in treating severe sepsis. In this review, we therefore provide some new concepts of HMGB1-targeted Chinese herbal therapies in sepsis.
5.Role of TGF-β Expressed by Biliary Epithelial Cells in Pathogenesis of Cholestatic Liver Disease

[Objective] To investigate the change of liver histology, proliferation of BDEC, and expression of TGF-β in different stages of liver chelestusis. [Methods] ①Rat cholestatic livers were induced by common bile duct ligation (BDL) and separated into 3 groups, namely control group (DO), 7 days after BDL group (DT), and 18 days after BDL group (D18). ② Histological changes of livers in different groups were evaluated based on Knodell HAI score. ③ Real-time PCR was used to detect the expression of TGF-β in liver tissue and isolated BDEC in different groups. ④ Statistically analyzing the correlation between Knedell HAl score and the levels of TGF-β_1 mRNA. ⑤ In vitro study was performed to investigate the effect of TGF-β_1 on an immortalized mouse intrahepatic bililary epithelial cell line (mIBEC). [Results] (I) Knedell HAI score and the proliferation of intrahepatic bile ducts increased as the liver cholestasis aggravated. ② The levels of TGF-β_1, TGF-β_2, and TGF-β_3 mRNA were significantly up-regulated in liver tissues and BDEC as the liver cholestasis aggravated. ③ Positive correlation was found between Knedell fibrotic score and the levels of TGF-β_1 mRNA in liver tissues and BDEC(r=0.9376, P<0.05 and r=0.9682, P < 0.01). ④ In vitro study showed that TGF-β_1 inhibited the proliferation of mIBEC. [Conclusion] ① Liver injury, biliary proliferation, and the levels of TGF-β mRNA expression increased as liver cholestasis aggravated. ② The interaction of TGF-β_1 and BDEC plays an important role in the pathogenesis of BDL induced cholestatic liver disease. ④ Up-regulated expression of TGF-β_1 mRNA in the proliferated BDEC participates in the formation of BDL induced cholestatic liver fibrosis.
6.Clinical analysis of robotic mitral valve Replacement

Objective To determine the safety and efficacy of robotic mitral valvereplacement using da Vinci S system.Methods From August 2008 to April 2011, over 400 cases of robotic cardiac surgery have been completed in Chinese PLA general hospital, in which 20 patients with isolated mitral valve stenosis underwent robotic mitral replacement, including 7 male and 13 female patients with a mean age of (44.7 ±9.8) years (ranging from 32 to 65 years). 16 patients had a NYHA class Ⅰ~Ⅱ heart function and 4 patients were NYHA class Ⅲ. Fifteen patients were concomitant with atiral fibrillation. Surgery approach was achieved through 4 right chest ports with femoral perfusion and Chitwood aortic occlusion. Antegrade cold blood cardioplegia was administered directly via chest for myocardial protection. The transesophageal echocardiography was used intraoperatively to estimate the surgical results. Results All patients had successful valve replacement including mechanical and tissue valve replacement. There was no conversion to a median sternotomy. The mean cardiopulmonary bypass and arrested heart time were(137.1 ±21.9) minutes and (99.3 ±17.4) minutes. Echocardiographic follow-up in all patients revealed no complications. Conclusion Robotic mitral valve replacement is safe and efficacious in the patients with isolated mitral valve disease.
7.The observation of 40 cases of totally robotic myxoma resection

Objective Objective This study is to discuss a surgical approach for ideal and safe resection of atrial myxoma using da Vinci S surgical system. Methods Forty consecutive patients underwent resection of atrial myxoma with the da Vinci S Surgical System. Mean age of the patients was(48 ± 13) yeas. Mean tumor size was 2. 3 cm ×4. 6 cm ~4. 3 cm ×7. 4 cm. 36 tumors were in the left atrium, of which 31 tumors arose form the interatrial septum, 2 from the postercaudal wall, 2 from the root of the anterior leaflet of the mitral valve, and 1 from the left atrial roof. In 34 patients, exploration was conducted through a left atriotomy anterior to the pulmonary veins and excision was achieved by dissecting a plane through the atrial muscle at the point of attachment. In the first 2 patients, exploration and excision were conducted through an oblique right atriotomy.Four tumors were in the right atrium, all of which were resected from the beating heart. The da Vinci instrument arms were inserted through three 0. 8 cm trocar incision in the right side of the chest via 4 port incision and 2 cm working port, all the procedures were completed with 30 o angled endoscopic facing upward with da Vinci S robot. Results Resection were successful in all patients. There were no operative deaths, strokes or other complication. All the patients were discharged. No recurrences of tumor or septal learkag were found in the follow-up. Conclusion The excision of atrial myxomas with the da Vinci S Surgical System is feasible, efficacious, and safe. Surgical results are excellent.
8.Application of dextran sulfate pretreatment in animal model of umbilical cord blood transplantation

Objective: To better understand homing potentiality of hematopoietic stem cells (HSC) in human umbilial cord blood (UCB) and the mechanism of dextran sulfate (DS) mobilization. Methods: Sub lethally irradiated or DS pretreated severe combined immunodeficient (SCID) mice were transplanted with UCB, which was cryopreserved at -80℃.Human cells in recipient mice were detected by flow cytometry and CFU GM assay from each host organ. Results: In contrast with the controls, engraftment after irradiation or administration of DS resulted in a higher percentage of CD45 +,CD34 +,CD19 + cells produced in SCID mice. While comparison between the experimental groups, higher implantation level was obtained in the former if equivalent donor cells were used in both groups. Conclusion:DS is a safe and effective pretreatment, which can mobilize HSC, but also vocate niches for transplanted HSC homing. [
9.Action observation therapy improves upper extremity motor function after stroke

Objective To explore the effects of action observation therapy on upper-extremity motor function and ability in the activities of daily living after cerebral infarction.Methods Forty-one cerebral infarction survivors were randomly assigned to an observation group (n =21) or a control group (n =20).Both groups were given the conventional rehabilitation treatment,while the observation group additionally received action observation therapy 20 mins per day,6 times per week for 8 weeks.Before and after the 8 weeks of treatment,both groups were assessed using the Fugl-Meyer assessment (FMA),Wolf's motor function test (WMFT) and the modified Barthel index (MBI).Results Before the intervention there was no significant difference between the groups in any of the measurements.After the 8 weeks of treatment,all of the results in the treatment group were significantly better,on average,than those of the control group.Conclusions Action observation therapy can improve upper-extremity motor function and ability in the activities of daily living after stroke.
10.Effects of lumbar sympathetic nerve modulation guided by computed tomography for treating female pelvic limb hyperhidrosis

A total of 17 cases with pelvic limb hyperhidrosis were selected for lumbar sympathetic nerve modulation.The puncture of lumbar sympathetic nerve was guided by computed tomography (CT) through the gap of L2-3 until the needle tip reached the lateral of the second or third lumbar vertebra body and outside of psoas major.A mixture of 1% lidocaine 3 ml and 30% Iohexol injection 0.3 ml was injected.If the diffusion of lidocaine was observed between lumbar vertebra body and psoas major.After 15 min,5 ml of absolute alcohol was injected to modulate the lumbar sympathetic nerve.After 5 min,oximetry-derived perfusion index of toe and sole temperature increased (5.9 ± 1.4) folds and (4.16± 0.89) ℃ on average respectively.Pelvic limb hyperhidrosis was all cured within 6 months and one case recurred after a year.There were no occurrences of complications.Lumbar sympathetic nerve modulation guided by CT may be used for treating female pelvic limbs hyperhidrosis.
