2.Diagnosis and treatment of extra-adrenal pheochromocytoma: a report of 33 cases

Objective To study the diagnosis and management of extra-adrenal pheochromocytoma.Methods A retrospective clinical study was done on 33 patients of extra-adrenal pheochromocytoma treated at our department.Results Extra-adrenal pheochromocytoma accounted for 16.2% (33/204) of all pheochromocytoma patients admitted between Jan 2005 and Dec 2012.There were 13 males and 20 females.The mean age at diagnosis was 45 years,with a range from 17 to 81 years.14 patients were hospitalized for abdominal discomfort,8 patients suffered from typical hypertension symptoms including headache and dizziness and blurred vision,one of bladder pheochromocytoma was found paroxysmal hypertension and 10 patients had no obvious clinical symptoms.All the patients underwent CT scan,and 8 extra-adrenal pheochromocytoma were discovered.11 patients were given circulation expansion therapy.Most of the tumors were located at bladder(3 case),renal hilus (7 case) and para-aorta(23 case).There were two patients with multiple tumors.In 26 cases,blood pressure fluctuated when touching tumor during operation.All the patients were treated with surgical operation.Except for 1 biopsy,32 patients underwent total resection of tumor including a case combined resection with left kidney and a case with the spleen.There were no perioperative mortality.Postoperative follow up showed very good prognosis,with no death.Conclusions Compared with imaging examination,clinical manifestations and laboratory tests were less evaluable for the diagnosis of extra-adrenal pheochromocytoma.Proper preoperative preparation and precise operation is essential for the prognosis.
3.Biological Control of Alternaria solani by Bacillus subtilis NJ-18

Strong inhibition of Bacillus subtilis strain NJ-18 on mycelia growth of Alternaria solani was observed in the antagonistic tests by cylinder plate methods, and the inhibition width was 21.5 mm. Observation under microscope found that the supernatant of fermentation from NJ-18 could make the pathogen hyphae cells malformed and swelled, and consequently the growth of the pathogen was inhibited. Determining of the colonization in potato plants by the signed rifampicin-resistance in NJ-18 showed that it could colonize well in the plants, the colonization quantity of NJ-18 in the root and stem of the potato detected 30 days after fermentation irrigation was 103 CFU/g plant’s fresh weight. In pot experiment, we inoculated the tomato plants with the spore suspensions of Alternaria solani after spraying the fermentation of NJ-18, the results were investigated in 14 days and the efficacy in controlling the disease was 72.9%, which was significantly higher than 45.7%, the efficacy resulted from spray treatment of 2000 fold dilution of 50% iprodione wetable powder.

A? plays a crucial role in Alzheimer's Disease and the soluble A? oligomers have been recognized as the emerging neurotoxins,which ultimately cause memory impairment and neuronal loss through different mechanisms.The development of novel drugs targeting A? oligomers indicates new and promising therapy approaches for AD.The pathological roles as the proximal toxins in AD and the compounds,targeting soluble A? oligomers,which are currently in preclinical and early clinical development,are reviewed.
5.The analysis of the dimensional measurement in high myopia by MRI

Objective To discuss the expansion model of the eyeball and investigate the morphologic characteristics of high-my- opic eyeball through the dimensional measurement in high-myopia and emmetropia by MRI. Design Case controll study. Participants Thirty-two emmetropes (60 eyes) and 33 high myopes (60 eyes) were enrolled, without eye diseases and history of ocular surgery or in- jury. Methods 60 high-myopic eyes and 60 emmetropic eyes were measured with MRI (I.5T,PHILIPS) to get the data of three inner ocular dimensions, intraocular volume and the volume of different parts. Main Outcome Measures Three dimensions and volumes of eyeballs. Results The average value of axial (28.16?2.80 mm), horizontal (22.87+1.23 mm) and vertical length (23.40?0.99 ram) of high-myopic eyes were much bigger than those of emmetropic eyes(P=0.000), especially the axial length( with difference of 5.38 mm); The axial length was correlated with refractive error (0.36 mm/D,r~2=0.88, P=0.000). The average value of the whole ocular volume (7. 46?0.89 ml) and vitreous volume(6.90?0.8 ml) of the high myopic eyes were bigger than those of emmetropic eyes(P=0.000), while ante- rior segment volume and lens volume were about the same as that of emmetropic eyes (P=0.220, P=0.630). Conclusions The three di- mensions of high-myopic eyes were significantly longer than that of emmetropic eyes. In high myopes, the increased vitreous volume lead to the increase of the whole ocular volume. There may be two models in the ocular expansion of high myopia: global expansion and axial elongation expansion. More serious refractive error cause more obvious expansion in axial elongation.

Objectives To discuss the CT features of primary tuberculosis in adults Methods CT images of 39 adult patients with primary tuberculosis were retrospectively analyzed and 30 cases were also examined after injection of contrast materials Results 39 patients had both mediastinal tuberculous lymphadenitis and parenchymal infiltration There were 25 cases (64 1%) with primary lesions involving the right lung and 14 cases (35 9%) in left side 28 cases(71 8%) appeared as patchy shadows,nodules,and lobular consolidation; 11 cases (28 2%) were with segmental or lobar consolidation In 30 cases with contrast enhanced CT,the mediastinal tuberculous lymphadenitis appeared as homogenous enhancement in 10 cases, irregular enhancement in 18 cases,central low attenuation and peripheral rim enhancement in 20 cases 14 cases showed that the nodes were confluenced with inhomogenous enhancement There were 26 cases with acute bronchogenic spread tuberculosis Conclusion The parenchymal infiltration and mediastinal tuberculous lymphadenitis are the basic features in adult primary tuberculosis Acute bronchogenic spread into other fields and the features of node enhancement after injection of contrast materials are very important findings in the diagnosis of primary tuberculosis
8.Comparison of detecting heat-sensitive condition of Neiguan (PC 6) in heart-qi deficiency patients with moxibustion sensation and with infrared.

OBJECTIVETo compare the method of detecting heat-sensitive (HS) condition of Neiguan (PC 6) with moxibustion sensation and with infrared in heart-qi deficiency patients (HQD), and to find an objective detection method for HS condition of acupoints.
METHODSThe study was performed on 74 HQD patients. Take infrared thermograph of the tested skin area in natural state. Then thermosensitive test was taken after infrared thermograph acquisition (suspended moxibustion for 10 min). After moxibustion infrared radiometry was implemented for the second time. The infrared radiation information of the forearms was collected and the major axis of the moxibustion reaction area was recorded. The effect of moxibustion sensation and infrared detection on HS Neiguan (PC 6) in HQD patients was compared.
RESULTSWhen the Neiguan (PC 6) in HQD patients was on HS condition, most of the infrared intensity was high temperature and compared with moxibustion sensation method, the sensitivity was 66.7%, the specificity 76.9%, and the accuracy 70.3%. After moxibustion on the HS Neiguan (PC 6) area, the infrared intensity diffused longitudinally along inner side of forearms. Compared with the result of the moxibustion sensation method, its sensitivity was 87.5%, the specificity 92.3%, and the accuracy 89.2%.
CONCLUSION(1) The state of HS condition of Neiguan (PC 6) in HQD patients could be detected objectively by infrared radiation. (2) After moxibustion, the HS condition of acupoints produced thermal phenomena as thermal expansion, heat transfer, which could be demonstated objectively by the infrared radiation image as well as the sensation of subjects.

9.Dose-respones relationship between daily total fluoride intake and prevalence of osteofluorosis

Objective To investigate the daily total fluoride intake in relating to the prevalence of skeletal fluorosis in two villages in Jiangsu Province,in order to provide the scientific evidences for the control and prevention of endemic fluorosis.Methods Adults sampled from a high-fluoride Village,Wamiao,and a low-fluoride Village,Xinhuai,were surveyed in this study according to the fluoride concentration in their household shallow well.The average daily total fluoride intake from difierent sources and the skeletal fluorosis were investigated in each subject.Then the subjects from two villages were allocated into five subgroups(<2.00,2.00~,3.00~,4.00~,≥5.00 mg/d),the relation fluoride intake and prevalence of osteofluorosis was analyzed.Results The prevalence of skeletal fluorosis in Wamiao Village was 31.06%(41/132),but no skeletal fluorosis case(0/35)was found in Xinhuai Village.According to the daily total fluoride intake,subjects with higher daily total fluoride intake tended to associated with a higher prevalence of skeletal fluorosis in a significant dose-response relationship(regression equation:y=2.624-6.855x+3.424x2:r=0.997).The benchmark dose lower limitation of daily total fluoride intake with 95% confidencewas 2.50 mg/d calculated according to this dose-response relationship,the reference dose(RfD)was 2.50 mg/d.In Wamiao Village a significant difference was also found between daily total fluoride intake in 41 subjects[(5.09±1.20)mg/d]with X-ray detectable skeletal fluorosis and in 91 subjects[(3.08±1.12)mg/d]without X-ray detectable skeletal fluorosis(t=-9.32,P<0.01).Conclusions These findings indicate that the daily total fluoride intake has a significant dose-response relationship with the prevalence of skeletal fluorosis in an endemic fluorosis area associated with high-fluoride drinking water;and the RfD in this study was lower than that in the national standard of"Chinese hygienic standard for daily total fluoride intake(WS/T 87-1996)"(3.50 mg/d).
