1.Influence of diabetic self-management education on blood glucose control and inflammatory factor levels in inpatients with diabetes mellitus

Objective:To explore the influence of diabetic self-management education on blood glucose control and inflammatory factor levels in inpatients with diabetes mellitus (DM).Methods:A total of 168 DM patients,who hospitalized in department of endocrinology in 163rd hospital of Chinese PLA from Jun 2012 to Jun 2013,were ran-domly divided into self-management education group (self-management group,n=88)and routine health education group (routine management group,n = 80).Self-management group received self-management education course namely AADE7TM ,provided by full-time DM health education staff,including healthy eating,being active,monito-ring,taking medication,problem solving,healthy coping and reducing risks,while routine management group re-ceived routine DM health education.Blood glucose control,changes of levels of myeloperoxidase (MPO),tumor necrosis factor-α(TNF-α)and intercellular adhesion molecule-1 (ICAM-1)were examined before and after interven-tion.Results:Compared with before intervention,there were significant reductions in levels of fasting blood glucose (FBG),postprandial blood glucose (PBG)and glycosylated hemoglobin (HbA1c)in both groups after interventions (P <0.05 all).Compared with routine management group,there were significant reductions in levels of FBG [(7.1 ±1.3)mmol/L vs.(6.0±1.1)mmol/L],PBG [(9.5±1.6)mmol/L vs.(8.8±1.4)mmol/L]and HbA1c [(7.9 ±1.5)% vs.(7.2±1.1)%]in self-management group,P <0.05 all.Before discharge,there were significant de-crease in inflammatory factor levels in both groups compared with before intervention (P < 0.05 all).Compared with routine management group after intervention,there were significant decrease in inflammatory factor levels [TNF-α:(201.0±57.3)pg/ml vs.(100.7±49.2)pg/ml,ICAM-1:(25.2±3.6)ng/ml vs.(18.2±2.9)ng/ml] in self-management group,P < 0.05 both.Conclusion:Self-management education provided by full-time diabetic health education staff is superior to routine diabetic health education method in controlling blood glucose and decrea-sing inflammatory factor levels,indicating it can improveβcell function and relieve insulin resistance.
2.Effect of Pulse Microwave Combined with Ofloxoacin on Chronic Prostatitis

Objective To observe the effect of pulse microwave combined with antibiotics on chronic prostatitis.Methods100 outpatients with chronic prostatitis were randomly divided into the trail group and control group with 50 cases in each group. The patients of the trail group were treated with pulse microwave combined with Ofloxoacin, and those of the control group were treated only with Ofloxoacin. The treatment course was 5 weeks. The evaluation with National Institutes of Health-Chronic Prostatitis Symptom Index (NIH-CPSI) and test of white blood cell (WBC) count in prostate secretion were performed before and after treatment for all patients of two groups.ResultsIn the trail group, the mean symptom degree score of NIH-CPSI was 9.3±2.6 and living quality score was 3.4±1.2, the total score was 12.4±4.1, and WBC count was 13.7±5.4/HP. In the control group, these indexes were 13.3±3.5, 4.5±1.2, 17.5±4.6 and 17.1±5.2 respectively. The NIH-CPSI score and WBC count of the trail group were lower than that of the control group ( P<0.05), and cure rate and total effective rate were higher than that of the control group ( P<0.05).ConclusionThe treatment of pulse microwave combined with Ofloxoacin for chronic prostatitis can improve the cure ratio and efficiency.
3.Effects of Kangguzengsheng Capsules-containing serum on the proliferation and osteogenic differentiation of bone marrow mesenchymal stem cells

BACKGROUND: Our previous studies have found that the Kangguzengsheng Capsules can promote fracture healing.OBJECTIVE: To explore the influence of Kangguzengsheng Capsules-containing serum on the proliferation andosteogenic differentiation of bone marrow mesenchymal stem cells.METHODS: Forty Sprague-Dawley rats were randomly divided into four groups. The capsule powder was resolved into1 mL natural saline and intragastrically administered into rats according to 1.16 g/100 g, 3.48 g/100 g, 10.44 g/100 g in low,medium and high dose groups. Rats in control group were given equal volume of natural saline. After consecutiveadministration for 12 days, blood samples from the abdominal aorta were collected and serum samples were isolated andpreserved until use. Bone marrow mesenchymal stem cells were isolated and purified by the whole marrow adhesion method,and identified by flow cytometry. Harvested cells were divided into four groups and cultured in the osteogenic culture mediumcontaining different kinds of serum samples as described above. MTT method was adopted to test the cell proliferation at 24,48 and 72 hours of culture. Alkaline phosphatase activity in cells was detected at 7 and 14 days of culture. Alkalinephosphatase staining was performed at 7 days of culture, and alizarin red staining performed at 14 days of culture.RESULTS AND CONCLUSION: Compared with the blank control group, bone marrow mesenchymal stem cells showedno significant changes in the proliferative ability and alkaline phosphatase activity in the low dose group, but these twoindices were significantly increased in the high and medium dose groups at 48 and 72 hours of culture or at 7 and 14days of culture, respectively (P < 0.05). Cells positive for alkaline phosphatase and alizarin red staining were observed inthe low, medium and high dose groups, and the cell staining was most remarkable in the high dose group. To conclude,Kangguzengsheng Capsules-containing serum can promote the proliferation and osteogenic differentiation of bonemarrow mesenchymal stem cells.
4.Effect of selective laser trabecularplasty on early chronic primary angle-closure glaucoma

21 mmHg.The side effects,IOP and application of anti-glaucoma drugs were examined 1 h,1 d,1 week,1 month,3 months and 6 months after SLT. Results The IOP was significantly decreased 1 week,1 month,3 months and 6 months after SLT compared with that before treatment(P21 mmHg using two anti-glaucoma drugs,respectively. Conclusion SLT is a safe and effective method for IOP control in early CPACG after treatment with laser peripheral iridoplasty and laser iridectomy.
5.Needling revision with mitomycin C subconjuctival injection on early failed filtering blebs

Objective To evaluate the therapeutic effects of needling revision with mitomycin C(MMC) subconjuctival injection on early failed filtering blebs after trabeculectomy for glaucoma. Methods Needling revision with MMC 0.2 mL(0.04 mg) subconjuctival injection was performed on 86 eyes of 76 patients with failed filtering blebs 2 to 6 weeks after trabeculectomy for glaucoma.An average of 1.88 times of treatment was performed.The intraocular pressure(IOP),blebs and side effects were observed,and follow-up was conducted for 6 months. Results Two to six weeks after trabeculectomy,there were 50 eyes with thickened and focalized blebs,32 eyes with encapsulated blebs and 4 eyes with no bleb.Six months after needling revision with MMC subconjuctival injection,blebs of 61 eyes turned into functional ones.The blebs were thinned and multicysted in 24 eyes,diffused and elevated in 37 eyes,thickened and focalized in 6 eyes,encapsulated in 13 eyes and disappeared in 6 eyes.Three months after treatment,the mean IOP was(15.2?6.1) mmHg,and there were 57 eyes with IOP
6.Comparison of effects of different pterygium surgeries

AIM: To compare the clinical outcomes of pterygium excision followed by translocation of adjacent conjunctival autograft with stem cells or transplantation of conjunctival autograft with stem cells to treat and prevent the recurrence of pterygium.METHODS: Fifty patients (60 eyes) were divided into 2groups, and pterygium excision followed by translocation of adjacent conjunctival autograft with stem cells (group A) and pterygium excision followed by transplantation of conjunctiva autograft harvested from counter-side limbus with stem cells (group B) were performed respectively.The surgical time of each case was recorded.RESULTS: All the 50 cases (60 eyes) of pterygium were recruited. The epithelization of cornea and conjunctiva in all cases finished in 2-5d, During the follow-up of 6-12mo, pterygium reoccurred in 2 cases (1 in each group), with no difference in recurrence rate. The mean surgical time was 25± 8min for group A,43± 11min for group B, the difference being statistically significant (P<0.05, t-test).CONCLUSION: Both methods, pterygium excision followed by translocation of adjacent conjunctival autograft with stem cells or transplantation of conjunctival autograft with stem cells are effective on treating and reducing the recurrence of pterygium. Compared with the latter, pterygium excision followed by translocation of adjacent conjunctival autograft with stem cells is simpler and easier to perform, and is preferred by the patients too.
7.Mental health and the related factors of Zangzu undergraduates in inland

Objective To explore the state of the mental health and related factors of Zangzu undergraduates in inland.Methods Two inland typical universities were chosen,according to random whole-class sampling principle from different grades,and 370 Zangzu undergraduates in inland were tested with Symptom Checklist 90(SCL-90),Self-Esteem Scale(SES),Simplified Coping Style Questionnaire (SCSQ) and social support scales,and compared with norm and Hanzu undergraduates.Results The scores of the factors of compulsion,interpersonal relation sensitiveness,depression,anxiety,hostility of Zangzu undergraduates in inland in SCL-90 were higher than 2.The rate of their mental problems(the total average score was higher than 2) was 45.3%,among them the rate of mild mental problems was 42.0%,the rate of secondary mental problems was 3.3%,without undergraduates of serious mental problems.Scores of the 9 factors were higher than that of the norm,and differences were significant (P< 0.01).Scores of the 9 factors were higher than that of Hanzu undergraduates,and scores of somatization,compulsion,anxiety,hostility,terror were significantly higher than that of Hanzu undergraduates (P <0.01).Rate of their mild mental problems was significantly higher than that of Hanzu undergraduates(P<0.01).Scores of their self-esteem and positive coping styles were lower than that of Hanzu undergraduates,and differences were significant (P =0.000).The negative coping styles and self-esteem could predict the state of mental health of Zangzu undergraduates in a great extent.Conclusion Zangzu undergraduates in inland have more mild mental problems,which are correlated with their self-esteem and coping styles.
8.Clinical application of anterior transpediclar screw reconstruction in treating lower cervical spine fracture and dislocation.

OBJECTIVETo investigate the clinical effects of anterior transpediclar screw (ATPS) fixation in treating lower cervical spine fracture and dislocation.
METHODSFrom January 2009 to December 2011, 18 patients with lower cervical spine fracture and dislocation were treated with ATPS technique, including 12 males and 6 females, aged from 17 to 47 years old with an average of 38.2 years. Severity score of lower cervical spine injuries (SLIC) ranged from 6 to 9 points with an average of 7.5 points. According to ASIA grade of spinal cord injury, 2 cases were classified in grade A, 8 cases in grade B, 6 cases in grade C and 2 cases in grade D. X-ray and CT scan were done after surgery in order to evaluate the safety of ATPS and observe the stability and fusion of injured segment. Spinal cord function was evaluated according to ASIA grade at 3 months after operation and last follow-up.
RESULTSAll patients were followed up for 6 to 15 months with an average of 9.5 months. Three months after operation, in aspect of spinal cord function, 8 cases improved 1 grade, 2 cases improved 2 grades; and at final follow-up, 7 cases improved 1 grade, 4 cases improved 2 grades. All patients obtained bony fusion 6 to 8 months after operation with an average of 6.5 months. After operation, 1 case had transient hoarseness and recovered 2 months later;2 cases felt swallowing discomfort, but the symptoms disappeared after about 3 weeks by inhalation. No internal fixation breakage and loosening as well as nerve, blood vessel and esophageal injuries were found.
CONCLUSIONAs for three columns injury caused by lower cervical spine fracture and dislocation, treatment with anterior transpediclar screw reconstruction can achieve the effect of decompression thoroughly and restore the cervical spine height and physiological curvature. Moreover, this kind of treatment has good stability and can create the favorable conditions for the recovery of spinal cord function.

9.The effect of cannabinoid receptor 2 agonist JWH015 on the hyperalgesia induced by remifentanil

ObjectiveTo investigate the effects of cannabinoid receptor 2 (cannabinoid receptor 2,CB2R) agonist JWHO15 on the hyperalgesia induced by remifentanil in a rat model of postoperative pain.Methods Sixty SD rats were randomly divided into 10 groups ( n =6 each ):control groups ( C1 and C2 ),incisional pain groups (I1 and I2),incisional pain plus JWHO15 groups (QI and FI),remifentanil groups (R1 and R2),and JWHO15 plus remifentanil groups ( QR and FR).Rats in group QL/QR and FI/FR were intrathecal injection with 10μg JWHO15 ( diluted in 10μl 20% DMSO solution) and intraperitoneal administration with 100μg JWHO15 ( diluted in 10μl 4% DMSO solution) respectively 30 min before plantar incision while rats in group C,I and R were received with the same volume of DMSO solution.Plantar incision surgery was operated in rats of group I,R,QI/FI,and QR/FR.In group R and QR/FR,remifentanil (0.04 mg/kg) was infused subcutaneously to rats with a pump for 30 min at the moment of surgical incision.The paw withdrawal mechanical threshold (PWMT) and paw withdrawal thermal latency (PWTL) at 24 h before incision and at 2 h,6 h,24 h and 48 h after incision were tested to evaluate the behavioral changes.ResultsCompared with group C and baseline,the level of PWMT and PWTL decreased at 2 h,6 h,24 h and 48h after incision in group Ⅰ (P< 0.01 ) ;Compared with group Ⅰ,the significant decrease of PWMT and PWTL were observed after incision in group R (P < 0.05 ) ; Compared with group R,the significant increase of PWMT (7.78 ± 1.09) and PWTL ( 17.28 ± 1.58) were observed from 6 h after incision in group QR(P<0.05).And the increase of PWMT (7.79 ±0.72,9.50 ± 1.17,7.86 ± 1.16) and PWTL ( 16.23 ± 1.50,19.53 ± 1.63,18.10 ± 0.93) were observed at 6 h,24 h and 48 h after incision in group FR(P<0.05).ConclusionIntrathecal and intraperitoneal administration of JWHO15 in this investigation dose could relief remifentanil-induced hyperalgesia in a rat model of postoperative pain.
