1.Quality assessment of Liuwei Wuling tablet by HPLC fingerprint and quantitative analysis.

Reversed phase high performance liquid chromatography with diode array detector was employed for simultaneous determination of six components and specific chromatograms analysis in Liuwei Wuling tablets with gradient elution of acetonitrile and water containing 0.1% phosphoric acid as mobile phase. The results showed that six components containing specnuezhenide, phillyrin, schisandrin, schisantherin A, schizandrin A and schizandrin B were separated well under the analytical condition. The average recoveries ranged from 98.96% to 100.5% with RSD less than 2. 0%. Twenty-five common peaks were selected as the specific chromatograms of Liuwei Wuling tablets with schisantherin A as the reference peak. Similarities calculated by cosine of angle, correlation coefficient and peak area ratio similarity (PAR) were all above 0. 95, indicating a good similarity between the reference and twenty batches of samples. Grubbs test and cluster analysis indicated that the established HPLC fingerprints and HPLC quantitative analysis can be used efficiently in the quality control of Liuwei Wuling tablets.

2.Relationship between genetic polymorphism of MTHFR C677T and nonsyndromic cleft lip with or without cleft palate in Shanxi Province of China.

OBJECTIVETo assess the association between polymorphism of methylenetetrahydrofolate reductase (MTHFR) gene C677T locus and nonsyndromic cleft lip with or without cleft palate (NSCL/P) in Shanxi Province of China.
METHODSBlood samples from 150 patients and their parents and 150 controls and their mothers were collected. The polymorphism of MTHFR gene C677T locus were analyzed by the methods of polymerase chain reaction and restriction fragment length polymorphism technique(PCR-RFLP). Case-control analysis, transmission-disequilibrium test(TDT) and haplotype-based haplotype relative risk analysis(HHRR) were used to study the correlation between the gene mutation and NSCL/P.
RESULTSHardy-Weinberg equilibrium test results showed that, the offspring and mother genotype in the case group and the control group was not deviated from the Hardy-Weinberg law of genetic equilibrium (P > 0.05). The distribution of genotype CC, CT and TT in offspring had significant difference between the two groups (P < 0.05). In the offspring and mother of case group and the control group, the carriers of the TT genotype compared to CC genotype, the OR are greater than 1 and 95% CI do not contain 1 (offspring OR: 2.692, 95% CI: 1.319-5.495; mother OR: 2.469, 95% CI: 1.136-5.363). The distribution of C allele and T mutation gene were significantly different in the offspring and mother between the two groups (P < 0.05). The TDT test showed: chi2 = 4.507, P < 0.05. The HHRR test showed: P < 0.05.
CONCLUSIONThe single nucleotide polymorphism of MTHFR C677T locus is associated with the development of NSCL/P in Shanxi Province.

3.The application of fluorescien sodium guided resection in glioma

Objecitive Under the fluorescent microscope ,we used fluorescein sodium fluorescence to determine glioma boundary ,thus gliomas removed through surgery more thoroughly .Mtehods We randomly se-lected 14 patients who were admitted in the First Hospital of JiLin University as the research objects ,patients with glioma were diagnosed as glioma according to the physical signs ,physical examination a,nd imaging findings before surgery.Diagnosed with glioma,intraoperative application of fluorescein sodium yellow fluorescence was deter -mined the tumor boundary ,and removal of the tumor ,according to the fluorescence intensity strength is different . The pathological diagnosis was to determine the boundary of fluorescent was accurate .Postoperative examined MRI was performed in order to make clear the excision of the tumor ,and the neurological condition of postoperative was observed.Results Glioma could be inspired by yellow fluorescence under fluorescent microscope .The normal brain tissue was not light .Postoperative pathological results showed that the fluorescent yellow area contained a lot of glioma cells,pale yellow fluorescence area found a small amount of glioma cells .Postoperative enhanced MRI scan had confirmed that application of fluorescein sodium could be more thoroughly resection of glioma ,postopera-tive dysfunction was reduced .Conclusion This method is prior to tumor boundary observasion without fluoresent staining and reducing the recurrence of the tumor and reducing the normal brain tissue damage ,and therefore,im-proving the quality of postoperative survival of patients .
4.Influence of isoflurane in neuronal protein damage and aggregation in APP transgenic mouse hippocampus

Objective:To observe the influence of inhaled anesthetic isoflurane in the neuronal protein damage and aggregation in the APP transgenic mouse hippocampus,and to investigate the intervention effect of trehalose. Methods:Sixty APP transgenic mice aged 12 months were divided into control group,isoflurane group (Iso group) and trehalose group (Tre group)(n=20).The rats in control group were not given any drugs and were put into the anesthetic box with continuonsly entering 2 L·min-1 oxygen for 2 h;the rats in Iso group and Tre group were respectively injected intraperitoneally with 2 mL saline or trehalose (400 μg·kg-1 )30 min before anesthesia,and then inhalated 1.4% isoflurane for 2 h.6 h after anesthesia,the hippocampus tissue of the mice was prepared,and DCFH-DA fluorescence was applied to detect the reactive oxygen species (ROS)level; 24 h after anesthesia, immunohistochemical method and Western blotting method were used to detect the contents of carbonyl compounds and nitrotyrosine and the Aβ1-42 protein expression level in hippocampus;TEM was applied to observe the formation of protein aggregates; TUNEL staining was performed to observe the apoptotic rate of hippocampal neurons. Results:Compared with control group, the ROS level, the expression levels of oxidative protein carbonyl compounds and nitrotyrosine,the expression level of Aβ1-42 protein,and the apoptotic rate of hippocampal neurons in Iso group were significantly increased (P < 0.05);compared with Iso group,the ROS level,the expression levels of oxidative protein carbonyl compounds and nitrotyrosine,the Aβ1-42 protein expression level, and the apoptotic rate of hippocampal neurons in Tre group were significantly decreased (P < 0.05).Conclusion:Isoflurane can induce the protein damage and aggregation,the apoptotic rate of hippocampal neurons,aggravate oxidative stress reaction,increase the apoptotic rate of brain hippocampal neurons in the APP transgenic mice;trehalose can intervene the neurotoxicity induced by inhaled anesthetics.
5.Porhyromonas endodontalis liopolysaccharide regulates interleukin-23 expression in mouse osteoblasts

Objective:To investigate the effects of Porhyromonas endodontalis(P.e)liopolysaccharide(LPS)on the expression of IL-23mRNA and protein in mouse osteoblasts MC3T3-E1 and the role of phosphatidylinositol 3-Kinases (PI3K)signaling pathway in this process.Methods:MC3T3-E1 cells were treated with different concentrations of P.e LPS for different hours,or pretreated with LY294002,a special PI3K inhibitor.The IL-23 mRNA and protein expression levels were detected by real-time PCR and Western blot respectively.Results:The level of IL-23 mRNA increased in MC3T3-E1 cells with the increase of concentration and the treatment time of P.e LPS(P <0.05).The IL-23 protein expression was increased by P.e LPS in a time dependent manner(P <0.05).The mRNA and protein of IL-23 decreased(P <0.05)after pretreatment with LY294002.Conclusion:P.e LPS can induce the expression of IL-23 mRNA and protein in MC3T3-E1 cells,and the PI3K signaling pathway may play a part in this process.
6.Effects of Low-Molecular-Weight Heparin on Respiratory Syncytial Virus Nephropathy in Rats

0.05).3.Under light microscope, there was no change of the glomeruli detected in all of the LMWH-treated groups compared with the controls,while congestion and swel-ling in part glomeruli of group RSV were observed significantly. 4.Under electron microscope,glomerular structures of the LMWH-treated groups were almost normal compared with the control, while extensive foot process effacement was observed in group RSV under an electron microscope. 5.RSV RNA signal expressed weaker in the LMWH-treated groups than in group RSV.Conclusions Positive charged RSV destroys the glome-rular filtration barrier through its electrostatic interaction with the negative charged heparan sulfate(HS) of GBM. LMWH, as the analog of HS, charged with anion, competes with GBM HS to combine with RSV to keep the glomeruli from being infected and destroyed, and then reduce the proteinuria.
7.Smoking Associated with Aspirin and Clopidogrel Resistance in Patients with Stable Angina after Percutaneous Coronary Intervention

ObjectiveTo explore the association of smoking to the Aspirin and Clopidogrel antiplatelet in patients with stable angina after percutaneous coronary intervention (PCI). Methods241 smoking patients and 252 non-smoking patients underwent PCI for stable coronary artery disease, all patients had taken aspirin 100 mg/d for 7 d or more. The arachidonic acid (AA)- and adenosine diphosphate (ADP)-induced platelet aggregation were tested as they got in hospital. Then, they accepted Clopidogrel 300 mg as loading dose, continued with 75 mg/d for 3 d. The ADP-induced platelet aggregation were re-tested. ResultsThe incidence of aspirin resistance (AR) and aspirin semiresponder (ASR) was 19.1% in all the cases, and was 25.5% in smoking group, 14.3% in non-smoking group (P=0.027). Age (OR=3.79,95%CI: 1.77~8.12) and smoking (OR=1.98,95%CI: 1.18~4.43) were the independent risk factors of AR and ASR. The incidence of Clopidogrel resistance was 19.5% in all the cases, and was 13.2% in smoking group, 24.3% in non-smoking group (P=0.03). Smoking (OR=0.22,95%CI: 0.09~0.54) may reduce the risk of Clopidogrel resistance. ConclusionSmoking increased the risk of AR and ASR, but reduced the risk of Clopidogrel resistance.
8.Selection of incision in the treatment of early osteofascial compartment syndrome

Objective To evaluate the clinical effect of long incision and reticular incision in the treatment of osteofascial compartment syndrome(OCS)in children.Methods The study involved 56 children with OCS who met failure of the conservative treatment.The injury sites included mainly the forearm and the leg.All the children underwent decompression by a small reticular incision procedure from January 2000 to May 2009.The fractures were treated with one stage reduction and fixation or second stage open reduction.Meanwhile,the study involved another 21 children(including 13 earthquake victims)who were treated with long incision for open decompression in the other hospitals before admission.All the wounds were healed by direct suture or dermatoplasty after 1-5 weeks of infection control.Bacterial culture was performed in all the wounds.The fractures were treated with secondary open reduction and fixation.Bacteria culture was done in all wounds.Results The reticular incisional wounds of 56 patients were healed free from dermatoplasty,with no infection or sensory dysfunction.Among 21 patients treated with long incision,the bacteria culture was positive in 16 patients(including 13 earthquake victims)and verve injury found in five patients(including two with radial nerve injury and three with peroneal nerve injury).Four patients with partial or complete cut-off of the verves were improved after repair by secondary operation.Direct suture of incisions was done in nine patients and skin graft performed in 12,with average healing time for three weeks.The patients were followed up for mean 5.6 years,which showed no claw hands,with overall satisfactory rate for about 98% and 95% respectively in two groups according to the probation standard of amputated finger function evaluation from the Hand Surgery Plant of Chinese Medical Association.Conclusions Both long incision and reticular incision procedures can be used early in the treatment of pediatric OCS,while the reticular incision procedure is more convenient and simple,with less complications.
9.The nosocomial infection status in postcardiotomy patients supported with extracorporeal membrane oxygenation:a retrospective observational study

Objective To investigate the nosocomial infection status in adult patients after cardiac surgery , who suffered cardiac shock and were supported with extracorporeal membrane oxygenation(ECMO).Methods The data of this retrospec-tive analysis were collected from adult patients of postoperative cardiac shock supported with veno-arterial( VA) ECMO from January 2011 to December 2013 in the Center for Cardiac Intensive Care, Beijing Anzhen Hospital.Results 125 patients were enrolled and nosocomial infection occured in 44 patients(51 episodes), infection rate was 35.2% or 71.8 episodes per 1 000 ECMO day.Respiratory tract infection occured most(27.2%), and then bloodstream infection(12.8%).The infection increased in the extension of time.The cultivation of the phlegm and blood specimen showed that the major pathogens were gram-negative bacteria.The patients were divided into the infection and the non-infection groups, the incidence of remedially continuous renal replacement therapy were significantly higher in the infection group than that in the non-infection group and the duration of ECMO support, the lenth of postoperative hospital stay, and the lenth of ICU stay were significantly longer in the in-fection group than those in the non-infection group[40.9% vs.22.2%, P<0.05; (7.2 ±4.6) d vs.(4.9 ±2.1) d, P<0.01;(12.5 ±6.2) d vs.(9.1 ±4.9) d, P<0.01和24(13, 36) d vs.15(11,20) d, P<0.01; respectively].The pa-tients in the infection group had higher mortality compared with the patients in the non-infection group(53.5% vs.17.1%, P<0.01).Conclusion Nosocomial infection is one of the common complications of the ECMO treatment and affects the prognosis of patients.The strict aseptic technique and rational application of antibiotics may reduce the mortality .
10.Research progress of ionizable lipid nanoparticles for siRNA delivery

Small interfering RNA (siRNA) is the initiator of RNA interference and inhibits gene expression by targeted degradation of specific messenger RNA. siRNA-mediated gene regulation has high efficiency and specificity and exhibits great significance in the treatment of diseases. However, the naked or unmodified siRNA has poor stability, easy to degrade by nuclease, short half-life, and low intracellular delivery. As an emerging non-viral nucleic acid delivery system, ionizable lipid nanoparticles play an important role in improving the druggability of siRNA. At present, one siRNA drug based on ionizable lipid nanoparticles has been approved for the treatment of rare disease. This review introduces the research progress in ionizable lipid nanoparticles for siRNA delivery, focusing on the effect of each component of lipid nanoparticles on the efficiency of siRNA-mediated gene silencing, which provides new references for the studies on ionizable lipid nanocarriers for siRNA delivery.
