5.Clinical study of sorafenib for patients with advanced renal cell carcinoma

Objective To evaluate the efficacy and safety of sorafenib in the treatment of advanced renal cell carcinoma.Methods The clinical date of 33 patients with advanced renal cell carcinoma from September 2007 to April 2012 was reviewed retrospectively.26 were males and 7 were females,with an average age of 69 years.Pathological diagnosis showed 30 clear cell RCCs,2 papillary RCCs,and 1 unclassified RCC.These patients were treated by sorafenib 400 mg twice a day until intolerable toxicity or disease progression.The primary end points were objective response rate,clinical benefit rate,median survival time,median progression-free survival and the incidence of adverse reaction.Results All patients were evaluable for response and toxicity,with 8 patients (24%) of partial remission,19 cases (58%) of stable disease,and 6 cases (18%) of disease progression.The disease control rate was 82%,the median progression-free survival was 10.2 months,while the median survival time was 16.5 months.The common adverse reactions included hand-foot skin reaction (61%),diarrhea (46%),hypertension (21%).Most adverse reactions occurred around the second week after drug therapy,with the duration unequal.The majority of adverse reactions could be released by symptomatic treatment,which did not affect the medication.Conclusion Sorafenib has good short term efficacy for patients with advanced renal cell carcinoma,and most adverse reactions were tolerable.
6.The inhibitory effects of peripheral electrical stimulation on chronic central pain after spinal cord injury

Objective To investigate the mechanisms of the inhibitory effects of peripheral electrical stimu- lation(PES)on chronic central pain(CCP)after spinal cord injury(SCI).Methods Twenty-four male Sprague- Dawley rats with CCP following SCI were randomly divided into three groups:a group without stainless steel needles implanted (NSSN group,n=8),a group with a stainless steel needle implanted but no peripheral electrical stimula- tion applied(NPES group,n=8)and a PES group(PES group,n=8).The rats' CCP was evaluated through ob- serving their response to nociceptive stimulation by means of the paw withdrawal pressure threshold(PWPT)and the paw withdrawal latency(PWL).Spontaneous pain behaviors including autophagia and scratching were observed at the same time.PES was applied via stainless steel needles inserted into standard acupoints on the hind limps and the back.The expression of the NMDA receptor 1(NR-1)subunit in the spinal cord horn was measured using immuno- chemical methods.Results Compared with the NSSN and NPES groups,CCP in the PES group was alleviated, PWPT and PWL were dramatically increased(P<0.01)and the expression of NR-1 was obviously decreased (P<0.01).Conclusion Peripheral electrical stimulation may alleviate chronic central pain after spinal cord injury in rats.
7.Clinical study of somatostatin combined with raw vegetable oil in treatment of intestinal obstruction

Objective To evaluate the clinical effects and adverse reaction of somatostatin combined with raw vegetable oil in treatment of intestinal obstruction From Tongchuan City People's Hospital were conducted through clinical medication.Methods From August 2012 to August 2015,A total of 364 cases of intestinal obstruction were randomly divided into observation group (182 cases) and control group (182 cases).Two groups were treated with the non-surgical treatment based on basic therapy,including gastrointestinal decompression,correctional of water,electrolyte disturbance and acid-base imbalance to prevent infection and toxication by sedative and spasmolytic.The non-surgical treatment was invalid or worse,the surgical treatment wasselected.Additionally,on the basis of non-surgical treatment,somatostatin combined with raw vegetable oil was additional treatment for the observation group.The comparison of thecurative effect,surgical treatment rate,gastrointestinal decompression,hospital stays and adverse reaction of two groups was conducted.Results The curative effect of observation group (80.22%)was superior to control group(68.68%).Compared to control group,the cases of adverse reaction in observation group was insignificantly increased (x2 =8.58,P < 0.05).Conversely,the observation group showed lower surgical treatment rate (29.67% vs 40.11%,x2 =4.37,P < 0.05),lower gastrointestinal decompression [(224 ± 171) ml/d vs (543 ± 186) ml/d,t =-21.28,P <0.05)],and less hospital stays [(6.14 ± 2.04) d vs (10.26 ± 3.12) d,t =-4.78,P < 0.05)].Conclusions The basic therapy and somatostatin combined with raw vegetable oil showed high efficiency,and cases of adverse reaction was insignificantly increased.It was worthy of promotion and application.

10.Expression and clinical significance of cathepsin K in intervertebral disk degeneration in humans

BACKGROUND: Several studies have confirmed that activation of intervertebral disc enzymes is closely related to matrix degradation. Matrix metalloproteinase and tissue inhibitor of metalloproteinase have been shown to exert important roles in the process of extracellular matrix degeneration in intervertebrel disk. Besides these two enzyme systems, whether other proteeses that exhibit degrading effects on extracellular matrix are involved in the intervertebral disk degeneration remains poorly understood.OBJECTIVE: To detect the cathepsin K expression in normal and degenerated human intervertebrel disc cells and investigate the correlation between cathepsin K and intervertebral disc degeneration.METHODS: Cathepsin K expression was detected in intervertebral disc tissue from 30 patients with lumbar intervertebral disc protrusion using immunohistochemistry SP method and ELISA. At the same time, the intervertebral disc tissue from 15 healthy adult cadavers and/or spine fracture patients was taken as control, Cathepsin K protein expression in normal and degenerated human intervertebral disc tissues were compared.RESULTS AND CONCLUSION: Cathepsin K expression was observed in normal and degenerated intervertebral disc tissues.The expression level was significantly higher in degenerated tissue than normal tissue (P < 0.05). These findings demonstrate that Cathepsin K possibly participates in the intervertebral disc degeneration.
