Objective To evaluate the effect of procaine on prevention of skin slap necrosis after modified radical mastectomy in breast cancer.Methods 106 breast cancer patients were randomly divided into treatment and control groups.In the treatment group,procaine(1% 100ml diluted in 300ml mormal saline,42℃)were compressed under the skin flap in operation,the control group received same amount of normal salin instead.The rates and sizes of skin flap necrosis were obeserved and compared.Results There were 13 cases suffured from skin flap necrosis among 106 breast cancer cases who received surgery operation.The rate of the flap necrosis in treatment group was5.76%(3/52),and the control group was 18.51%(10/54),there was significant difference(P

50 years).The values of ADC,FA,?_1,?_2 and ?_3 in putamen and globus pallidus were calculated after diffusion tensor scanning,and then analyzed the differences between the two tissues.The differences of ADC,FA,?_1,?_2 and ?_3 values among age groups were analyzed by using analysis of variance(ANOVA),and their relationship with age were accessed by using Pearson correlation.Results Between the correspondent age groups,the ADC value was higher in the putamen[(6.68?0.40)?10~(-4),(6.47?0.36)?10~(-4),(6.44?0.34)?10~(-4)mm~2/s]than that in the golbus pallidus[(6.13?0.50)?10~(-4),(6.05?0.33?10~(-4),(6.05?0.52)?10~(-4)mm~2/s],and the value of FA in the putamen(0.20?0.03,0.23?0.03,0.25?0.03)was lower than that in the globus pallidus(0.35?0.03,0.36?0.03,0.37?0.04).ANOVA analysis of the FA value among age groups revealed there were significant differences in the putamen(F=10.082,P=0.000),and the same analysis of eigenvalue ?_3 in the putamen also showed significant differences(F=5.675,P=0.005).The FA value in the putamen was significantly elevated with aging(r=0.555,P

Objective To observe the relationship of awake bispectral index (BISawake) and the postoperative cognitive dysfunction (POCD). Methods 100 patients aged 60~75 years with ASA status Ⅰ~Ⅱ, underwent elective lower abdominal surgery with intravenous anesthesia (propofol, remifentanyl and rocuronium). The BIS was adjusted in 40~60 during the surgery. Their cognitive function was assessed with the neuropsychological test battery commended by International study of postoperative cognitive dysfunction (ISPOCD). Results 8 patients withdrew for unwilling or serious complications. The BISawake was more than 65 in 68 patients (group A), and less than 65 in 24 patients (group B). The difference of the verbal learning immediate and delayed of Hopkins Verbal Learning Test-Revised was more in the group B than in the group A. Conclusion Lower BISawake may be associated with the decline of postoperative cognitive function.

OBJECTIVE: To analyze the case characteristics of poisoning by exhaust gas of the imperfect combustion of natural gas and provide references for forensic identification and prevention of such accidents. METHODS: Twenty-two cases of poisoning by exhaust gas of the imperfect combustion of natural gas in Minhang District during 2004 to 2013 were collected. Some aspects such as general conditions of deaths, incidence time, weather, field investigation, and autopsy were retrospectively analyzed. RESULTS: In the 22 cases, there were 15 males and 16 females. The age range was between 2 and 82 years old. The major occurring time was in January or February (8 cases in each) and the cases almost occurred in small area room (21 cases). There was wide crack next to the exhaust port when the gas water heater was been used in all cases. CONCLUSION There are more prone to occurrence of exhaust gas poisoning of imperfect combustion of natural gas in small area room with a ventilation window near the exhaust port of gas water heated. It shows that the scene of combustion exhaust gas poisoning should be more concerned in the cold season.

ObjectiveTo observe the effect of megace (MA) on the survival condition of cancer patients in chemotherapy (CT) periods.Methods92 patients with cancer were divided into the MA group (treated with MA+CT) and control group (treated only with CT). The changing of the appetite, normal food amount, weight, gastrointestinal reaction and whole body conditions of two groups were evaluated.ResultsIn the MA group, 52.2% patients had appetite improvement, 47.8% had more food amount, 45.7% gained more weight, 50% had no obvious gastrointestinal reaction such as vomiting and nausea, and 50% had improve the survival condition according to the Karnofsky performance status (KPS) scores (increment >10). In the control group, only 6.5% had appetite improvement, 4.3% had more food amount, 13% gained more weight, 28.3% had no vomiting and nausea, and 15.2% had improved the survival condition. There was a significant difference between two groups (P<0.01).ConclusionMegace is able to reduce nausea and vomiting caused by CT, improve appetite, increase patients weight, protect bone marrow from the inhibition of CT, improve the life quality of cancer patients, and has no evident side-effects.

Postoperative pancreatic leakage remains a persistent problem after pancreaticoduodenectomy.For patients with a soft and nonfibrotic pancreas,double binding continuous hemstitch suture is an optimal method for anastomosis.From January 2011 to June 2012,92 cases of periampullary carcinoma with a soft pancreas underwent pancreaticoduodenectomy,and then a modified technique of pancreaticogastrostomy was performed with 2 continuous hemstitch sutures placed in the mucosal and seromuscular layers of the posterior gastric wall,respectively.The median time for pancreaticogastrostomy was 12 minutes,and only 15 patients had postoperative complications.Two patients developed pancreatic leakage(1 grade A and 1 grade B)postoperatively.Pancreaticogastrostomy with double binding continuous hemstitch sutures is a simple and safe reconstruction procedure for patients with a soft and fragile pancreas who received pancreaticoduodenectomy.

Objective To explore the significance of 1.5-Tesla MRI in the early diagnosis of prostate cancer,and analyze the imaging features in T2WI which are helpful for the diagnosis on early prostate cancer.Methods The clinical data of patients with PSA ＜ 20 μg/L who received prostate biopsy between Feb.2006 and Jan.2011 were reviewed including pelvic MRI and biopsy results.We retrospectively analyzed T2WI image features and evaluated the signal of peripheral zone,seminal vesicles,prostate capsule and the relationship with their surrounding tissues.The results of the reading were analyzed with reference to histopathologic evaluation of prostate biopies.Results Among 359 patients,137 patients were diagnosed prostate cancer and 332 cases were found with low signal lesions on peripheral zone of T2WI.Univariate logistic regression analysis showed that age and PSA are closely related to the diagnosis of prostate cancer (P ＜ 0.05).Low signal lesions on peripheral zone of T2WI are more helpful for the diagnosis of prostate cancer on the image.Number,shape,margin of lesions,the contour of the prostatic capsule,the boundary of peripheral zone and transitional zone are significantly associated with the diagnosis of prostate cancer.Through multivariate logistic regression analysis,it was found that age (P ＜ 0.05),PSA (P ＜ 0.05),prostate volume (P ＜ 0.05),lesion shape (flaky vs nodular,P ＜ 0.05 ; diffuse vs nodular,P ＜ 0.05) and the contour of the prostatic capsule (disappearing vs completely clear,P ＜ 0.05) are risk factors of prostate cancer.The areas under receiver operating characteristic curve are 0.833 (P ＜ 0.05).Conclusions In T2WI,lesion shape and the contour of the prostatic capsule are risk factors of prostate cancer.Age,PSA,prostate volume are also significantly associated with the diagnosis of prostate cancer.

Objective To explore the clinical value of image gradings of T2 weighted imaging (T2WI) and diffusion weighted imaging (DWI) for prostate cancer diagnosis in patients with prostate specific antigen (PSA) less than 20 μg/L.Methods A total of 133 patients with PSA＜20 μg/L who received prostate biopsies in our hospital from December 2011 to January 2013 were recruited.The prostate was divided into six regions in MRI images.T2WI and DWI image gradings on each region in peripheral zone were evaluated with reference to histopathological evaluation of prostate biopsies.The significance of grading system for the diagnosis of prostate cancer in the two scanning methods alone or in combination were compared.Results 60 patients were diagnosed as prostate cancer.The positive biopsy rates were 2.8％,13.5％,20.4％,34.6％,52.6％ and 88.9％,respectively in grade 0-5 based on T2WI imaging grading.The positive biopsy rates were 11.1％,16.1％,48.5％,71.0％ and 86.1％,respectively in grade 1-5 based on T2WI+ DWI imaging grading.The positive biopsy rate was significantly increased with the increments of pre-established T2WI and T2WI+DWI image gradings (Z=-9.8552,-13.4148,both P＜0.0001).Receiver operating characteristic analysis showed that the diagnostic capability was better in T2WI combined with DWI than in T2W1 alone in prostate cancer detection (AUC:0.743 vs.0.715,P=0.0072).Conclusions T2WI image grading can predict the risk of prostate cancer in patients with PSA＜20 μg/L,and DWI can enhance the ability to predict prostate cancer.

AIM: To investigate the changes of the L-type calcium channel subunit expression and calcium currents (I_~Ca -L) in cultured rat ventricular myocytes infected by coxsackie virus B_3 (CVB_3). METHODS: Primary cultured neonatal rat ventricular myocytes were infected with CVB_3. The changes of L-type calcium channel subunits mRNA in normal and infected myocytes were measured by semi-quantitative reverse transcription-polymerase chain reaction. I_~Ca -L was recorded in two groups respectively using whole cell patch-clamp techniques. RESULTS: The expression of ?_1 and ? subunits of L-type calcium channel mRNA increased in the infected group compared with the normal one (4.00?0.07 vs 2.21?0.41, P0.05).
