1.Changes of Systolic Time Interval after Neonatal Asphyxia and Its Relationship with Clinical Manifestation and Prognosis

Objective To observe the changes of systolic time interval after neonatal asphyxia and explore its relationship with clinical manifestation and prognosis. Methods Tow-dimensional and Doppler echocardiography were employed to detect tow - dimension parameters and left and right ventricular STI in 27 mild and 18 severe asphyxiated neonates as well as 14 normal controls and its relationship with clinical manifestation and prognosis was analyzed. Results There was no difference in cavity and thickness of heart and great arteries between normal and asphyxiated neonates. In acute stage mild and severe asphyxiated neonates had a shorter right ventricular ejection time (RVET) than normal neonates and a longer right ventricular prejection period (RPEP) was found in severe neonates than that in normal and mild asphyxiated neonates. Left ventricular preejection time (LPEP) was prolonged in comparison with normal neonates in acute stage and convalescence. There were more cases with increased RPEP/RVET in severe asphyxiaed group than those in mild and normal group in acute stage. The incidence of heart failure in acute stage and disability in late period was higher in cases with increased RPEP/RVET than that with normal RPEP/RVET. Conclusion Asphyxia has more severe damage to right ventricle than that to left ventricle. Cases with increased RPEP/RVET are prone to suffering from heart failure in acute stage and more likely to undergo disability in late period.
2.Nasal submicron emulsion of Scutellariae Radix extract preparation technology research based on phase transfer of solute technology.

Based on the demand of nasal drug delivery high drug loadings, using the unique phase transfer of solute, integrating the phospholipid complex preparation and submicron emulsion molding process of Scutellariae Radix extract, the study obtained the preparation of the high drug loadings submicron emulsion of Scutellariae Radix extract. In the study of drug solution dispersion method, the uniformity of drug dispersed as the evaluation index, the traditional mixing method, grinding, homogenate and solute phase transfer technology were investigated, and the solute phase transfer technology was adopted in the last. With the adoption of new technology, the drug loading capacity reached 1.33% (phospholipid complex was 4%). The drug loading capacity was improved significantly. The transfer of solute method and timing were studied as follows,join the oil phase when the volume of phospholipid complex anhydrous ethanol solution remaining 30%, the solute phase transfer was completed with the continued recycling of anhydrous ethanol. After drug dissolved away to oil phase, the preparation technology of colostrum was determined with the evaluation index of emulsion droplet form. The particle size of submicron emulsion, PDI and stability parameters were used as evaluation index, orthogonal methodology were adopted to optimize the submicron emulsion ingredient and main influential factors of high pressure homogenization technology. The optimized preparation technology of Scutellariae Radix extract nasal submicron emulsion is practical and stable.

4.Insulin resistance in first-degree relatives of patients with Graves disease

Objective To observe insulin resistance in first-degree relatives of patients with Graves disease. Methods All subjects in control group and experiment group including first-degree relatives of GD patients underwent oral glucose tolerance tests (OGTT) and insulin releasing tests then the degree of insulin resistance was analyzed. Results Blood glucose at each point of OGTT, insulin level and insulin resistance index 1 (HOMA-IR) of experiment group were higher than those in control group, while insulin activity index (IAI) and HOMA-βwere significantly lower than those in control group. Conclusion Patients insulin resistance could be found among first-degree relatives of GD patients.

Objective To explore the association of Toll-like receptor 4 TLR4 and lipopolysaccharide receptor CD14 gene polymorphisms with Kawasaki disease (KD) susceptibility.Methods Three-color fluorescent staining flow-cytometry was used to detect the expression of TLR4 in peripheral blood white blood cell of 76 KD children and 118 healthy control group.The gene of TLR4 (-896A/G), (-1196C/T) and CD14 (-260C/T) polymorphisms was identified by polymerase chain reaction-restriction fragment length polymorphisms; and the relationship between genotype and KD was analyzed.Results 1.The values of mean fluorescence intensity (MFI) of TLR4 in peripheral blood white blood cell of the KD groups and the healthy control groups were 2.87?0.96, 10.55?4.87, 23.36?8.28 and 3.26?0.65, 7.55?1.21, 25.41?6.97, respectively; There was a gradual increase of these values on lymphocyte, neutrophilic leukocyte and mononuclear cell in both groups.2.(-896A/G), (-1196C/T) polymorphisms of TLR4 gene were not found in both groups.3.The frequency of each genotype of CD14 gene (-260C/T) was 35.5%CC, 30.3%CT, 34.2%TT in KD group and 38.1%CC, 47.5%CT, 14.4%TT in healthy control group.The frequency of each genotype was significantly different in 2 groups(?2=11.62 P

Objective To explore the relationship of dynamic changes between the iodine nutritional condition and growth of children born before and after the implementation of universal iodine.Methods By means of sampling investigation,urinary iodine and goiter,height,weight,intelligence quotient(IQ)were comprehensively evahated in children aged 7~14 years old born before and after the implementation of iodine in 10 counties and 2 cities of Qiannan.Results The median of urinary iodine of children Was 91.88μg/L in 1984,133.33μg/L in 1994,316.00μg/L in 2006;goiter rate was 21.40%(122/570)in 1984,12.86%(107/832)in 1994,5.28%(45/851)in 2006;the average IQ was 89.18±4.10 in 1994,94.26±0.94 in 2006;children in each age group in 2006 has a higher height and a heavier weight compared with those in 1984.Conclusion The physical development and intelligence of children in the areas of iodine deficiency have improved along with the correction of iodine deficiency.
10.Observation of the protective effect of N-NAC on radiation-induced pulmonary injury

Objective To observe the protective effect of N-NAC on radiation-induced lung injury. Methods 86 cases of thoracic neoplasm patients were chosen and randomly divided into two groups,group RT +N(n =43)and group RT(n =43).Two groups were observed by CT after radiotherapy.Acute and chronical toxicities were graded by RTOG.TGF-β1,IL-1,IL-4,TNF were observed before and after the radiotherapy.Results After 3 monthsof radiotherapy,RTOG≥2 was 23.4%(RT +N),while RTOG≥2 was 53.1%(RT).there was significant differencebetween the two groups(P <0.01).At 6,9 and 12 months,fibrosis was present in 28.4%,25.4%,22.4% receivingRT vs 58.4%,54.4%,52.4% receiving RT +N,there was significant difference between the two groups(P <0.05).TGF-β1,IL-1,IL-4,TNF were observed which showed that The RT +N were lower than RT.Conclusion N-NAC can reduce incidence rate of lung injury in radiotherapy,and can reduce the content and the release of TGF-β1,IL-1,IL-4,TNF.
