1.SCREENING OF PROBIOTIC LACTOBACILLUS AND THEIR CHARACTERISTIC

According to the requirements of ptobiotics, we have separated 13 strains of Lactobacillus from the content in gut of lactating piglet. Among them 3strains are L. acidophilus, one is L. casei, one is L. atenaforme and 8 strains are L. fermentum. They can resist 1.0% bile salt and the acidity of pH3.0, can inhibit pathogenic Escherichia coli and Staphylococcus aureus. They are safe and non-toxic to mice.
2.Parapharygeal space neoplasms clinical analysis of 85 cases

Objective To improve the diagnosis and management of parapharyngeal space(PPS)neoplasms. Methods Records of 85 patients with PPS neoplasms operated in 11 years were reviewed with regard to presenting signs and symptoms,histological characteristics,imaging technique,surgical approach and postoperative complica- tions.Results Benign rumors were found in 77 cases and malignant tumors were founded in 8 cases.Forty-one tu- mors originated from salivary glands;neurogenic tumors were found in 36 patients and 8 patients with tumors in oth- er organs.All were treated by surgical operation:the trans-eervical approach was used in 76 cases,tans-oral in 6 cases and the combined transoral-cervical approach in 3 cases.The complications after operation were facial nerve palsy(4 cases),hypoglossal nerve paralysis(2 cases)Horner syndrome(2 cases),vocal cord paralysis(2 cases),thyroid gland fistula(1 case),infects(1 case)and difficulty in breathing(1 case).Conclusion Tumors originated from salivary glands were the most common neoplasms in the primary parapharyngeal space neoplasms,and neurogenic tumors fol- lowed.High-resolution computed tomography or magnetic resonance imaging was essential for diagnosis and presurgi- cal planning.The transcervical approach was the preferred procedure for PPS tumors.

Objective To investigate the diagnostic value of ADC histogram obtained from MR DW imaging for stage ⅠB cervical cancer. Methods Seventy three patients diagnosed by cervical smear screening as cervical cancer without priortreatment were included prospectively in the patient group, and staged according to the international federation of gynecology and obstetrics (FIGO) staging system. Forty three patients with uterine leiomyoma detected by gynecologic examination, ultrasonography or CT and with negative result of cervical smear screening who were scheduled for hysterectomy were included prospectively in the control group. The patients of both groups underwent routine pelvic MR sequences, dynamic contrast enhanced imaging and DWI before hysterectomy. ADC histograms of the entire tumor and cervix volume were generated by post-processing software. Features of ADC histogram for the 2 groups were observed. Histogram parameters such as mean ADC (ADCmean), median ADC (ADCmedian), the 25th percentile of ADC (ADC_25th), the 75th percentile of ADC (ADC_75th), skewness and kurtosis were recorded. Student's t test or Mann-Whitney U test depending on homogeneity of variance was employed for the comparison of
those parameters. ROC analysis was employed for assessing the diagnostic performance of ADC histogram in distinguishing the 2 groups. Results Thirty five patients in the patient group were staged as FIGO IB. Five patients in the control group ended up with pathologic findings of cervical intraepithelial neoplasia grade 3. Therefore 38 patients in the control group were investigated. ADC histograms of the patient group were mostly skewed positively, while the curves were largely skewed negatively. ADCmean, ADCmedian, ADC_25th, ADC_75th, skewness and kurtosis for the IB stage patient group were (1.10±0.21)×10-3mm2/s, (1.05±0.21)× 10-3 mm2/s, (0.90 ± 0.19) × 10-3mm2/s, (1.26 ± 0.23) × 10-3mm2/s, 0.83 (median) and 1.25 (median) respectively. ADCmean, ADCmedian, ADC_25th, ADC_75th, skewness and kurtosis for the control group were (1.62 ± 0.25)×10-3mm2/s, (1.64±0.24)×10-3mm2/s, (1.42±0.24)×10-3mm2/s, (1.84±0.27)×10-3mm2/s,-0.11(median) and 0.29 (median) respectively. All parameters showed statistically different (t values were -9.693,- 11.117, -10.255, and -9.988 for ADCmean, ADCmedian, ADC_25th and ADC_75th respectively;Z values were -6.360 and -4.445 for skewness and kurtosis respectively; P< 0.01). ROC analysis indicated that ADCmedian had the highest diagnostic accuracy for differentiating the 2 groups, with the area under the curve being 0.97, a cutoff value of 1.21×10-3mm2/s, and a sensitivity of 95.6%and a specificity of 89.3%. Conclusion ADC histogram of DWI may be valuable for diagnosing stage IB cervical cancer by distinguishing stage IB cervical cancer from normal cervix or cervical benign lesions.
7.Clinical observation of severe blepharoptosis correction with modified frontalis muscle suspension

Objective To explore the clinical effect of modified frontalis muscle suspension for severe blepharoptosis correction. Design Retrospective case series. Participants Fifty-six cases (101 eyes) with severe blepharoptosis. Methods Modified frontalis mus- cle suspension was adopted. The technique included single blepharoplasty-type incision, dissecting the posterior gaps of frontalis muscu- lar fasciae ahead,then euthyphoria isolating anterior gaps of rontalis muscular fasciae, using frontalis muscle transfer without vertical incision. Main Outcome Measure The positon chang of the upper eyelid in the primary position gaze. Results The follow-up period ranged from 8 to 20 months (mean, 13.6 months). All the patients were deemed to have a good surgical outcome. Complications such as ectropion and corneal exposure were avoided. But ten eyes required reoperation for undercorrection, six eyes for overcorrection and two eyes for entropion. Conclusion This surgical technique is a useful procedure that results in substantial cosmetic and functional im- provement with few complications.
8.Total en bloc spondylectomy for 9 patients with primary tumor of the lumbar spine

Objective To evaluate the possibility and curative effect of total en bloc spondylectomy (TES) for the treatment of primary tumors of the lumbar spine. MethodsBetween June 2005 and July 2009,nine cases of primary tumor of the lumbar spine were treated with total en bloc spondylectomy through a single posterior approach performed in seven cases(L1 giant cell tumor, L1 osteosarcoma, L2 giant cell tumor,L2 solitary plasmacytoma, L2 chondrosarcoma, L2 chordoma, and L3 histiocytic sarcoma), and two cases through a single stage anterior and posterior combined approach (L4 giant cell tumor with paraspinal mass and L4 fibrosarcoma). They were performed a single stage reconstruction after removing tumors of vertebra. Neurofunction was evaluated perioperatively and recurrence of tumor was tracked for all cases.ResultsThe follow up was obtained for 3-49 months (averaging 19 months). All patients attained significant clinical improvement after surgery without any severe complications. There was no local recurrence and distal metastasis in shortterm follow-up. ConclusionThe treatment by TES to remove tumors and reconstruction of spine are reliable for primary tumors of the lumbar spine. The curative effect of short-term follow-up is satisfactory. However, the long-term curative effect remain to be further follow-up.
9.The expressive vector of RNA interference can effect the expressions of PADI-4 gene in HL-60 cells

Objective:To prove the effect of PADI-4 gene in the development of rheumatoid arthritis.Methods:Four SiRNA sequences were designed for PADI-4 gene,and the SiRNAs were cloned into blank pSiRNA-hH1neo G2 vectors.The vectors were transformed into GT116 E.coli competent cells.By white-blue selection system,the right vectors were gotten.After transfection into HL-60 cells,the cells were collected on 3,5,7,10 and 14 day,the levels of PADI-4 mRNA were detected by Real-time PCR.Results:Digestion by Acc 65Ⅰand Hind Ⅲ,the recombinant expressive vector of RNA interference was obtained successfully.The PADI-4 mRNA generated by the cells transfected with the vector of SiRNAs were reduced,and the level was not change in normal cells.Conclusion:The recombinant expressive vector of RNA interference is obtained successfully and the recombinant expressive vector can affect expression of PADI-4 gene in HL-60 cells.
10.Effect comparison of levosimendan and milrinone on treatment of severe heart valve disease patients with postoperative low cardiac output syndrome

Objective To compare the effect of levosimendan and milrinone on treatment of severe heart valve disease patients with postoperative low cardiac output syndrome. Methods Fifty-six severe heart valve disease patients with postoperative low cardiac output syndrome were selected, and the patients were divided into levosimendan group and milrinone group according to treatment method with 28 cases each. Both groups received symptom-relieved therapy, including cardiotonic, diuresis and other drugs. The patients in levosimendan group were combined with 24 h of continuous intravenous injection of levosimendan 0.05-0.20 μg/(kg·min) for 1 week, and the patients in milrinone group were combined with 24 h of continuous intravenous injection of milrinone 0.25-1.00 μg/(kg·min) for 1 week, in order to maintain mean arterial pressure ≥ 65 mmHg (1 mmHg=0.133 kPa). The cardiac output, cardiac index, left ventricular ejection fraction (LVEF), and the serum levels of lactic acid, creatinine, N-terminal pro brain natriuretic peptide (NT-proBNP) were compared between 2 groups. Results There were no statistical differences in cardiac output, cardiac index, LVEF, and the serum levels of creatinine, lactic acid, NT-proBNP before treatment between 2 groups (P>0.05). The cardiac output, cardiac index, LVEF, and the serum levels of creatinine, lactic acid and NT-proBNP after treatment in 2 groups were significantly better than those before treatment, and there were statistical differences ( P<0.05). There were no statistical differences in cardiac output, cardiac index and LVEF after treatment between 2 groups (P>0.05). The serum levels of creatinine, lactic acid and NT-proBNP after treatment in levosimendan group were significantly lower than those in milrinone group: (102.82 ± 21.31) μmol/L vs. (115.64 ± 58.73) μmol/L, (1.7 ± 1.4) mmol/L vs. (2.2 ± 1.0) mmol/L and (1 149 ± 515) ng/L vs. (1 321 ± 472) ng/L, and there were statistical differences (P<0.05). Conclusions Both the two drugs can significantly improve cardiac function in severe heart valve diseases patients with postoperative low cardiac output syndrome, while the levosimendan has more advantages in lowering serum creatinine, lactic acid value and NT-proBNP.
