1.Investigation and analysis on current situation of clinical teaching

Objective To investigate the clinical teaching situation by using developmental inspection of School of Medicine,Shanghai Jiaotong University(SJTU-SM),and to put forward some suggestions. Methods By checking questionnaires and informal discussions,the relevant information was collected and analyzed by using SPSS statistics sofware. Results The clinical teaching quality of SJTU-SM was basically satisfied.The satisfaction from internship of grade 2004 was better than that of grade 2003.However,some problems in clinical teaching must be improved.Conclusion The investigation showed that the clinical teaching quality of SJTU-SM is being improving.However,in order to achieve the international accreditation standards,the quality guarantee system of clinical teaching need to be further perfected.
2.MRI study of acute radiation-induced hepatic injury with super paramagnetic iron oxide enhancement in experimental model

Objective To investigate the MR imaging technique and features of acute radiation- induced hepatic injury with superparamagnetic iron oxide(SPIO)enhancement in a rabbit model.Methods On the 10th day after half-liver single 40 Gy X-ray irradiation,MR imaging before and after SPIO administration and pathologic study of 12 adult rabbits were performed.Results On the 10th post-irradiation day,TSE T_2 WI before SPIO enhancement,the irradiated liver of two rabbits showed relatively higher signal, and 1 showed slightly lower signal.With SPIO enhancement,the irradiated livers of 9 rabbits were found to be abnormal,manifesting as higher or slightly higher areas on multiple MR imaging sequences,especially the TFE T_1 WI sequence.Histological specimen with HE stain under light microscopy revealed occlusive injury of central veins(veno-occlusive disease,VOD)in each irradiated liver to some extent.Electron microscopy investigation of the irradiated liver disclosed intracellular edema,fibrin deposition,and widening of the Disse-space.Conclusion The early pathologic feature of the irradiated liver is occlusive injury of the central vein.MR imaging with SPIO enhancement is effective to valuate the early irradiation-induced liver injury.
3.Research progress on barrel cortex and its plasticity.

Synaptic plasticity of barrel cortex is one of the most widely studied topics in neuroscience in recent years. The primary somatosensory cortex of the rodent has a good topology character,which provides an ideal experimental model for plasticity study. This system displays very strong experience-dependent plasticity both during development and in adulthood. The changes of sensory cortex's neural circuit can induce experience-dependent plasticity. In the synaptic level,thalamocortical synapse is considered to be the main location of plasticity. In the circuit level,both synapses from layer 4 to layer 2/3 and those within layer 2/3 are also the necessary parts of achieving synaptic plasticity in primary somatosensory cortex. The GABAergic inhibitory circuit may be involved in this plasticity of S1, but the exact mechanism remains unknown.

4.Study on the prevalence rate and risk factors of chronic obstructive pulmonary disease in rural community population in Hubei province.

OBJECTIVETo explore the prevalence of chronic obstructive pulmonary disease (COPD) and its risk factors in rural area in the southern part of Hubei province and to provide evidence for prevention and treatment.
METHODSUsing uniform scheme, procedures and questionnaire, a multistage survey for 1883 people in rural area in southern part of Hubei province was performed, and physical examinations and lung function tests were conducted for every participant.
RESULTSIn this survey, 186 COPD cases were identified with a overall prevalence of 9.88%. The prevalence for male was 13.68% while in female it was 6.46%. The prevalence was higher in males than in females (chi2 = 27.48, P< 0.001) and higher with the increase of age (chi2 = 79.22, P<0.001). Factors associated with COPD were identified in one-way variance model as follows: sex, age, smoking, time for cooking, using biomass fuels for in-door cooking, family history, frequent cough before age 14, low body index and having house remodelling in the last five years. Results from multivariate logistic regression analysis showed that sex (OR= 1.3010), age (OR = 1.8506), smoking (OR= 3.0118), cooking time (OR= 1.7651), family history (OR = 1.5278), frequent cough before age 14 (OR = 2.8965) were risk factors.
CONCLUSIONThe prevalence of COPD in the southern part of Hubei province was high, suggesting that comprehensive intervention measures should be taken.

5.Effect of hydrogen sulfide on expressions of ectodermal dysplasia-1 and kidney injury molecule-1 of renal interstitial in rats with unilateral ureteral occlusion

Objective To investigate the effects of hydrogen sulfide (H2S) on the tubular interstitial fibrosis and on the levels of kidney injury molecule-1 (KIM-1) and ectodermal dysplasia-1 (ED-1) in the process of renal interstitial fibrosis in rats with unilateral ureteral occlusion(UUO).Methods Ninety-six Sprague-Dauley (SD) male rats were divided into 4 groups randomly:sham-operated group (n =24),model group (n =24),low-dose therapy group (n =24) and high-dose therapy group(n =24).The rats in the model group and treatment groups were ligated at the left ureter and UUO was induced,meanwhile,the rats in the control group were free from the left ureter ligation.Rats received sodium hydrosulfide(NaHS) in traperitoneatly(1.4 μmol/kg,twice a day),and NaHS(7.0 μmol/kg,twice a day),respectively.Rats in sham-operated group and the model group were injected intraperitoneally with identical voluminal 9 g/L saline.Eight rats in each group were sacrificed randomly at the 7 days,14 days and 21 days,respectively.The concentration of plasma H2S was detected.Renal tubular interstitial damage and interstitial fibrosis were evaluated with routine HE staining and Masson staining under microscope,and both of them were used to evaluate the obstruction of renal histopathological changes.The expressions of ED-1 and KIM-1 were measured with immunohistochemical staining.Results 1.The pathological findings showed that the renal tubular interstitial changes were not obvious in the shamoperated group.The tubular epithelial cells in the model group and treatment groups showed swelling and vacuoles degeneration,with renal interstitial broadening,interstitial cells and extracellular matrix increasing.The renal tubular interstitial pathological injury of model group and treatment group were more serious than those in the sham-operated group at each time point(P < 0.05).The renal tubular interstitial pathological injury of the treatment groups were obviously lower than that in the model group(P <0.05).However,there was no statistically significant difference between highdose therapy group and low-dose therapy group(P > 0.05).2.Immunohistochemical analysis showed that expressions of ED-1 and KIM-1 in renal tubular interstitices in the model group and the treatment groups were higher than those in the sham-operated group at each time point(P < 0.05).The expressions of ED-1 and KIM-1 in renal tubular interstitices in the treatment groups were obviously lower than those in the model group(P < 0.05).There was no statistically significant difference between the high-dose therapy group and the low-dose therapy group(P >0.05).3.Plasma H2S levels in model group and treatment group were lower than those in the sham-operated group at each time point (P < 0.05).The plasma H2S level of the treatment group was obviously higher than that in the model group(P < 0.05).However,there was no statistically significant difference between high-dose therapy group and low-dose therapy group (P >0.05).Conclusions H2S implements renal protection effect partly by reducing the expression of ED-1 and KIM-1 in tubule interstitices to ease tubular interstitial fibrosis.
6.Transposition of orbital fat and orbicularis muscle flap over the orbital rim for correction of lower eyelid pouches complicated with lacrimal groove deformity.

OBJECTIVETo introduce the experience in the treatment of lower eyelid pouches orbital rim.
METHODSAn incision was made along the margin of lower eyelid and dissection was performed under the orbicularis muscle to expose the orbital septum and periosteum of lower orbital rim. The fat released from orbital septum was transposed just below the lower orbital rim and fixed on the periosteum. If lacrimal groove deformity was not corrected completely, the musculocutaneous flap, which may be excised beside the incision, was kept to correct the deformities further with only the muscle portion.
RESULTS72 cases with lower eyelid pouches complicated with lacrimal groove deformities were treated with transposition of orbital fat and orbicularis muscular flaps. Satisfactory results were achieved in all the patients after a follow-up period of 3-6 months.
CONCLUSIONIt is an effective and feasible technique to correct lacrimal groove deformities with transposition of orbital fat and orbicularis muscular flaps.

7.The significance of multi-slice helical CT multiplanar reconstruction in the diagnoses of laryngeal carcinoma

Objective To evaluate the significance of multi-slice helical CT with multiplanar reconstruction in laryngeal carcinoma.Methods Thirty-five patients with laryngeal carcinoma were studied by helical CT,MPR were subsequently done.The lesion extent of the axial image findings,MPR findings and the combined image findings were compared with the pathological results respectively.The data were statistically analyzed.Results In the evaluation of the anterior commissure,the axial image findings,MPR findings and the combined image findings were 82.9%,68.6% and 91.4% in accuracy respectively,the results were statistically different(P0.05).The combined images were superior to the axial images and the MPR images in sensitivity,specificity and accuracy of the lesion extent.Conclusion The axial images could show the shape,size,extension of the tumor and the lymphadenopathy,MPR images displayed the shape,size and extension roundly and directly,they were the supplement for the axial images.Axial images combined with MPR could improve the accuracy in the diagnoses of laryngeal carcinoma.
8.Construction of a red fluorescent shuttle vector controlled by recA operon promoter of Streptococcus mutans.

OBJECTIVETo construct a red fluorescent shuttle vector controlled by recA operon promoter to transform Streptococcus mutans.
METHODSThe promoter of recA was amplified from Streptococcus mutans UA159, and connected to plasmid pDsRed2-N1 to construct pRred with a red fluorescent coding gene, which was then inserted into the shuttle vector pDL276 to construct pLRred.
RESULTSpLRred was successfully constructed, and Escherichia coli transformed with the pLRred plasmid could express reporter gene DsRed.
CONCLUSIONSThe recombination plasmid pLRred can be used in the further research of the expression of cariogenic virulence factor gene by Streptococcus mutans in biofilm.

9.Differential expression of secretagogin and glucose-related protein 78 in colorectal carcinoma: a proteome study.

OBJECTIVETo identify the differentially expressed proteins or peptides and potential biomarkers of tumorigenesis for colorectal cancers.
METHODSImmobilized pH gradient two-dimensional gel electrophoresis (2-DE) was used to separate and obtain the differentially expressed protein spots between colorectal cancers and matched normal mucosa. Liquid chromatography/mass spectrometry (LC-MS/MS) was used to characterize these proteins. Selected candidate proteins were further studied by Western blot, semi-quantitative RT-PCR and immunohistochemical staining.
RESULTSThirty-five protein spots showed marked expression changes (more than 5-fold) in colorectal carcinoma compared to normal mucosa. Fifteen proteins were up regulated and 20 were down regulated. Fourteen of these proteins were identified by tandem mass spectrometry, among which secretagogin (SCGN) was down-regulated and glucose-related protein (GRP) 78 was up-regulated in the tumors. The SCGN down-regulation was further supported by Western blot and RT-PCR analyses. Immunohistochemistry revealed that SCGN was strongly expressed in neuroendocrine cells of the colonic crypts and 53 of 54 (98%) neuroendocrine tumors. At protein level, although GRP78 was up regulated in colorectal carcinoma, there was no difference in the mRNA expression level between the tumor and paired normal mucosa.
CONCLUSIONSThe 2-DE combined with MS is a powerful tool for screening potential tumor biomarkers. The differentially expressed candidate proteins identified by 2-DE may be of significance in understanding the tumorigenesis of the colon cancer. SCGN is a potential biomarker for neuroendocrinal differentiation. GRP78 up-regulation in colorectal carcinomas may be related to its post-translational modification.

10.Clinical analysis of extracranial and intracranial complications of cholesteatoma otitis media.

OBJECTIVETo study the clinical feature of extracranial and intracranial complications caused by cholesteatoma otitis media.
METHODSA retrospective study as made of 78 patients with extracranial and intracranial complications caused by suppurative otitis media between January 1992 and December 2002 was conducted.
RESULTSFifty-six of the 78 patients had one complication and the other 22 patients had two or more complications among them. In this series, 24 cases had facial paralysis, 16 cases with suppurative labyrinthitis, 12 cases with meningitis, 11 cases with periauricular abscess, 9 cases with extradural abscess, 9 case with otogenic meningitis brain abscess, 6 case with lesion of sigmoid sinus and 5 cases with Bezold abscess. Of the 24 patients with facial paralysis, 15 with continued nerves had obtained omatomic preservation of the facial nerves, The nerve function of 11 patients recovered from House-Brackmann grade IV to grade II, the rest 4 patients didn't regain after operation. Four of the 9 patients with their facial nerves broken received anastomosis of the facial nerves, three cases received transplantation of the nervus auricularis magnus, and two received anastomosis to nervus auricularis magnus. The nerve function recovered from House-Brackmann grade V - IV to grade III.
CONCLUSIONSEven though the incidence of the complications has been low down in past years, there are still some serious complications. Temporal bone CT scan plays an important role in the diagnosis of extracranial and intracranial complications of cholesteatoma otitis media. The most reliable method to diagnosis and treat the complications is surgery.
