1.Traditional Chinese medicine to prevent environmental pollutant-induced epigenetic changes of sperm DNA.

In recent years, the incidences of male infertility and recurrent abortion are increasing. The causes of the conditions are varied and complex, but the affected stability of the genetic material in the sperm is an important etiological factor. Environmental pollutants may invade the body through respiration, diet and other channels, resulting in infertility and abortion or even affecting the reproductive system development of the offspring by changing the epigenetics of sperm DNA. In this paper, we propose the idea of using traditional Chinese medicine to prevent environmental pollutant-induced epigenetic changes of sperm DNA, hoping to provide a new insight into the protection against genetic material damage, male infertility, and recurrent abortion caused by environmental pollutants.

2.Protective effect of Ambroxol on lung injury induced by ischemia reperfusion

Objective To investigate the protective effect of Ambroxol against experimental lung ischemia reperfusion (I/R) injury in a situ hilar clamp model.Method Left lung of rat was rendered and ischemic for 90 minutes,and reperfused for up to 2 hours,as the model.Twenty-four SD rats were randomly divided into 3 groups with 8 rats each group:control group,I/R group,I/R and Ambroxol treatment group (AMB group).Rats of AMB group received Ambroxol (25 mg/kg) Intraabdominally 30 minutes before ischemia and intravenously 5 minutes before reperfusion.After 2 hours of reperfusion,blood-gas analysis,the serum level of IL-1?,IL-8 and TNF-?from carotid artery were delected.The wet/dry ratio of lung,the activity of erythrocuprein (SOD),the content of malonaldehyde (MDA) and the activity of myeloperoxidase (MPO) were determined and pathematology changes were observed in the left lung tissue.Differences within the groups were analysed using two-sample t-test. Results After 2 hours of repeffusion,there were no significant changes of artery partial pressure of oxygen (PO_2) and partial pressure of carbon dioxide (PCO_2) among three groups.The wet/dry ratio of lung,the activity of MPO (U/g) and the content of MDA (nmol/mgprot) of I/R group were (5.3?0.5),(1.30?0.26) and (0.66?0.16),significantly higher than those of the control group (P
3.Clinical research on intravitreal injection of triamcinolone acetonide for treating macular edema caused by pre - retinal membrane of the macular

AIM: To evaluate the efficacy and complications of intravitreal injection of triamcinolone acetonide ( TA ) for the treatment of macular edema caused by pre-retinal membrane of the macular.
METHODS: Totally 23 patients ( 24 eyes ) with macular edema caused by pre-retinal membrane of the macular were treated withintravitreal injection of 4mg TA. Best-corrected visual acuity ( BCVA ) , intraocular pressure ( IOP ) , slit - lamp examination, fundus fluorescein angiography ( FFA ) and optical coherence tomography (OCT) were performed before and after treatment. The SPSS 12. 0 software was used for statistical analysis.
RESULTS: After 10, 30, 90d of treatment of TA, as compared with before treatment, visual acuity improved significantly ( P<0. 05 ), and central macular thickness (CMT) was significantly thinner (P<0. 01). The average central macular thickness decreased from 522 ± 126μm before treatment to 264±115μm, 245±128μm, 286±131μm at 10, 30, 90d after treatment. Macular edema reduced. IOP increased in 7 eyes ( 29%) , one cataract case, no other complications associated with vitreous injection.
CONCLUSION: Intravitreal injection of TA in the treatment of macular edema caused by pre - retinal membrane of the macular is simple, safe and easy to operate. It can quickly reduce macular edema, and improve the visual acuity in the short term. Part of patients may recur after injection in the first half of the year.
4.Analysis of postoperative complications after ventriculo-peritoneal shunt in children

Objective To analyze the curative effects and complications of ventriculo-peritoneal shunt in treating hydrocephalus caused by different diseases in children. Methods The data of 289 children with hydrocephalus who were performed ventriculo-peritoneal shunt were examined and the incidence rate of complications was analyzed. Results Thirty-seven incidences of complications occurred in 24 patients with the total incidence rate of 12.46%.There was obstruction of proximate tube in five cases,distal end in two cases,with the obstruction rate of 2.42%.Postoperative infection happened in nine cases(3.11%).Bacterial culture was positive in three cases,two of which were Staphylococcus epidermidis and Bacillus coli in the other one.Negative results were found in six patients for the bacterial culture.Subdural hematoma occured in one case,and tube break after injury in two cases.Slit ventricle syndrome broke out seven times in three cases,leakage of cerebrospinal fluid in two,exposure of pumps in three,and subcutaneous hydrops in two.Four cases(1.38%)were dead from infection. Conclusion Ventriculo-peritoneal shunt is preferred for hydrocephalus in children if the complications are well prevented and controlled.
5.Characteristics and significance of IGF-1 and IGFBP2 gene expressions in ankle cartilage of patients with Kashin-Beck disease

Objective:To investigate the characteristics and significance of insulin-like growth factor 1(IGF- 1) and insulin-like growth factor binding protein 2 (IGFBP2) expressions in ankle cartilage of patients with Kashin-Beck disease (KBD).Methods:In this case-control study, 10 KBD patients who were hospitalized in the Department of Orthopedics of Shaanxi Provincial People's Hospital from January 2010 to December 2016 were selected as KBD group, and 10 patients with ankle fracture caused by trauma but without talus injury during the same period were selected as control group, the cartilage tissues of the two groups were collected. IGF-1, IGFBP2 positive cells, the mRNA and protein expressions of IGF-1, IGFBP2 in the cartilage tissues were detected by immunohistochemistry, real-time fluorescent quantitative PCR and Western blotting, respectively. According to the expressions of IGF-1 and IGFBP2 in ankle cartilage of KBD patients, a patient with amputation caused by trauma was selected in Shaanxi Provincial People's Hospital, and ankle joint cartilage was taken to prepare chondrocytes for in vitro cell verification experiments. The chondrocyte were divided into control group (0 ng/ml T-2 toxin), T-2 treatment group (20 ng/ml T-2 toxin) and T-2+ IGFBP2 silenced group (20 ng/ml T-2 toxin+ 50 nmol/L IGFBP2 siRNA), the MTT method and dimethyl methylene blue staining were used to detect the activity of chondrocyte and the secretion of sulfated glycosaminoglycan (sGAG). Results:In the control group and the KBD group, the number of IGF-1[(47.26 ± 8.97), (68.15 ± 7.42) cells] and IGFBP2 positive cells [(27.56 ± 5.40), (71.85 ± 7.62) cells] in the cartilage tissues were significantly different ( t = 4.487, 9.402, P < 0.01). Compared with the control group, the IGF-1, IGFBP2 mRNA and protein expression levels in KBD group were significantly higher, the differences were significantly different ( t = 3.340, 20.700, 4.684, 8.699, P < 0.05 or < 0.01). In cell experiment, the chondrocyte activitives and sGAG contents of the control group, T-2 treatment group, and T-2+ IGFBP2 silenced group were significantly different ( F = 226.70, 80.66, P < 0.01); among them, the cell activitives and sGAG contents of the T-2 treatment group and T-2+ IGFBP2 silenced group were lower than those of control group ( P < 0.05), and the T-2+ IGFBP2 silenced group were higher than those of the T-2 treatment group ( P < 0.05). Conclusions:The expressions of IGF-1 and IGFBP2 in the ankle cartilage of KBD patients are significantly higher. Silencing IGFBP2 gene can reduce the inhibitory effect of T-2 toxin on chondrocyte activity and the secretion of sGAG.
6.Comparison of curative effect between two mesh suspensions in treating female stress urinary incontinence

ObjectiveTo evaluate the curative effect difference between Tong's anterior mesh suspension and modified tension-free vaginal tape-obturator (TVT-O). Methods75 SUI cases were random divided into two groups: Group A (35 cases) were accepted Tong's anterior mesh suspension and group B (40cases) were accepted modified tension-free vaginal tape- obturator. Curative effects, operation safety,and complications and so on, were recorded and compared. ResultsThere had no statistical differences between these two groups on operation time, bleeding lose, operation injury, immediate postoperative urinary retention andcure rates, but the operation expense of group A was cheaper than group B [ (980. 74 ±212.45)yuan vs (2879.06 ±467. 13)yuan , P <0.05). ConclusionThe curative effect between two methods were similar. It's a little complicated and fit to be popularized in large hospital to modified tensionfree vaginal tape-obturator. Tong's anterior mesh suspension was performed under completely direct vision,not through pelvic cavity, and it was an economic, convenient and easy way, which not only fit to be popularized in basic hospital, but also a surgical remedial measure to the failures who accepted these operations,such as TVT, SPARC, IVS, MONARC, TOT, TVT-O, etc.

8.Contrasted study on pharmacokinetics of Tibetan medicine Renqing Mangjue compatible with Zuota.

To provide insights into the mechanism for the attenuate-synergistic effect of Zuota to Tibetan medicine Renqing Mangjue, a contrasted study was carried out on the pharmacokinetics of brucine and strychnine in mice plasm, which are active and toxicant ingredient in the Tibetan medicine Renqing Mangjue. LC-MS/MS was used to detect simultaneously the concentrations of brucine and strychnine in mice plasm at-different time intervals after administration parallelly and randomly, and the pharmacokinetic software Kinetica 5. 0 was selected to non-compartmental analysis (NCA) for data, and statistical analysis software SPSS 19. 0 was used for significance test on the pharmacokinetic parameters. A reliable LC-MS/MS method was established for the determination of brucine and strychnine in blood plasma, which are consistent with the requirements of the preclinical pharmacokinetic study confirmed by the methodology. The linear concentration ranges of brucine and strychnine were 0.301-104.4 µg · L(-1) (r = 0.999 5) and 0.305-106 µg · L(-1) (r = 0.999 7), respectively; The intra-day and inter-day variable coefficients were both less than 10.0% with good precision; The average extraction recoveries of brucine and strychnine were 116.23% and 112.82%, and RSD were 3.2% and 2.3% separately;The average matrix effects of brucine and strychnine were 122.48% and 116.36%, and RSD were 7.7% and 4.4%, respectively. The pharmacokinetic results showed that AUCtot of brucine and strychnine in Zuota group were both increased remarkably (P < 0.05), and the Cmax of brucine in Zuota group was about 5.25-fold higher than that of brucine in non-Zuota group (P < 0.05). The Tmax of brucine and strychnine reduced to one-eighth and one-quarter respectively compared with those in Non-Zuota group. In addition, the eliminations of brucine and strychnine in vivo were accelerated after the compatibility of Zuota. A significant difference (P < 0.05) occurred at the MRT0-t, of brucine, while the MRT0-∞ and Lz of strychnine were statistically significant upon the inspection level α = 0.1. It was found that the absorption degree of brucine and strychnine in Zuota group increased in the range of the safe dose (or concentration), while their elimination rates were accelerated, which may be one of the mechanisms for attenuate-synergistic effect of Zuota to Tibetan medicine Renqing Mangjue.

9.Diagnosis and surgical treatment of 51 cases of lung mycosis

Objective To summarize the clinical characteristics, diagnosis and surgical management experiences of 51 cases of pulmonary fungal infections. Methods The clinical data of 51pulmonary fungal infection patients hospitalized in department of thoracic surgery of PLA General Hospital from 1981 to 2008 were retrospectively analyzed. Results The recruited cases included three cases of Candida albicans, 22 of Aspergillus, and 26 of Cryptococcus. The clinical symptoms included cough, hemoptysis, fever, chest tightness, chest pain and asthma. Twenty cases were identified through routine healthy examination. Of chest X-ray and computed tomograply scan, 29 cases presented with solid nodules in the lung, some of which had burrs and spikes; 13 presented with pulmonary cavity with even wall thickness and smooth inner wall including 7 with aspergillar glomera.Forty-eight cases underwent standard thoracotomy operations and 3 cases underwent thoracoscope or thoracoscopy-assisted minithoracotomy. Seventeen underwent pulmonary lobectomies, and 34 wedge resections. There was no operative mortality in the 51 patients. All diagnoses were confirmed by postoperative pathology. There was no relapse during 3- 10 years of follow-up. Conclusions Primary pulmonary fungal infections lack characteristic presentations in clinical manifestations and imaging examinations. Pathology is the evidence for definite diagnosis. Surgical intervention is an effective tool for diagnose and treatment of this disease.
10.The effects of arsenic trioxide on epidermal growth factor-induced proliferation and migration of retinal pigment epithelial cell

Background Growth factor-induced proliferation and migration of retinal pigment epithelium (RPE) cells are the major pathological changes of proliferative vitreoretinopathy (PVR).Arsenic trioxide ( As2O3 ) is an active ingredient of Chinese traditional medicines,which has an inhibition on proliferation and migration of tumor cells.However,it is not clear whether As2O3 could inhibit growth factor-induced proliferation and migration of RPE cells. Objective This study was to explore the effects of As2O3 on epidermal growth factor (EGF)-induced proliferation and migration of ARPE-19 cells. Methods RPE cell line (ARPE-19 cells) were cultured.Different concentrations of As2O3(0,0.5,1.0,2.0,5.0,10.0,20.0 μmol/L) were added in the culture plate to treat ARPE-19 cells with or without 10 mg/L EGF in serum-free group for 24 and 48 hours,respectively.The MTT colorimetric assay was used to check the cell viability and evaluate the drug toxicity.The effects of As2O3 on EGF-induced proliferation of ARPE-19 cells were analyzed to get an effective and avirulent concentrations of As2O3.The effects of As2O3 on EGF-induced migration of ARPE-19 cells were observed by scratch-wound assay and the Boyden chamber assay.Results MTT assay showed that the A values were gradually declined with the increase of As2O3 concentrations after As2O3 treatment without EGF for 24 hours and 48 hours ( Fgroup =38.269,P =0.000 ; Ftime =0.874,P =0.358 ).Compared with the control group,no significant differences were seen in the A values of ARPE-19 cells in 0.5-5.0 μmol/L groups (all P>0.05).Meantime,As2O3 reduced the A values of ARPE-19 cell with 10 mg/L EGF in dose- and time-dependent manner ( Fgroup =152.155,P =0.000 ; Ftime =51.649,P =0.000 ).There were not significant differences in 10 mg/L EGF-induced cell growth after 0.5,1.0,2.0 μmol/L As2O3 was added for 24 and 48 hours ( Fgroup =2.215,P =0.126 ;Ftime =2.230,P =0.155).However,when 5.0-20.0 μmol/L As2O3 added,the A values of 10 mg/L EGF-induced ARPE-19 cells lowed,showing a significant difference in comparison with the control groups ( all P<0.05),with the cellular inhibiting rate 12%,32%,37% in 24 hours and 39%,44% and 53% in 48 hours.Scratch-wound assay showed that EGF-induced horizontal migration of ARPE-19 cells was slow after 0.5-2.0 μmol/L As2O3 treated,and the same results also appeared in cell lognitudinal migration by Boyden chamber assay,with the inhibitory rates 22%,33% and 46% respectively. Conclusions As2O3 is avirulent on ARPE-19 cells within definite concentration range.At ≤ 2.0 μmol/L concentrations,As2O3 dose not affect EGF-induced proliferation of ARPE-19 cells,but it suppresses EGF-induced cell migration.At ≥ 5.0 μmol/L concentrations,As2O3 plays an inhibitory role to EGF-induced proliferation of ARPE-19 cells.
