1.A Study on microendoscopic discectomy system in the treatment to lumbar disc herniation

Objective To evaluate the second generation micro-endoscopic discectomy (MED) system in the treatment of lumbar disc herniation. Methods Eighty-five patients with single or double level lumbar disc herniation were treated with MED under epidural anesthesia from May 2000 to October 2001.Single disc herniation was in 72 cases, double disc herniation in 13 cases; lateral recess stenosis in 15 cases. Results The average following-up period was 14 months and the successful therapeutic results achieved 92 9%(79/85). the patients were allowed to walk 3 d and discharged 3~5 d after operation. The length of incision was only 1.6 cm. Conclusions MED has short-term satisfactory therapeutic efficacy in the treatment of lumbar disc herniation. The suitable indication and precise technique are the key for the operation.
2.Liver ischemic preconditioning in mice-implications of HSP27 induction to ameliorate ischemiareperfusion injury

ObjectiveTo explore the role of HSP27 in hepatic ischemia-reperfusion (IR) injury and hepatic ischemic preconditioning (IP).MethodsMale C57BL/6 mice were randomly divided into 3 groups:control group (sham operation),IR group (only hepatic IR),rHSP27 + IR group (rHSP27 injection before IR).The extent of liver injury was evaluated among these 3 groups.Murine HSP27 was silenced by short hairpin RNA and mice were subjected to IR with or without IP after interfering succeeded.Serum ALT and AST levels,hepatic histological damages and mRNA expression levels of pro-inflammatory mediators (TNF-α,IL-1β and IL-6) were measured.Results Silencing hepatic HSP27 aggravated IR injury indicated by higher concentration of serum ALT and AST (P<0.05),reinforced by severer histological damage and more expression of pro-inflammatory mediators (P<0.05),while injection of rHSP27 or IP ameliorated hepatic IR injury.Western-blot showed HSP27 expression was significantly higher in IP group than sham group.Silencing HSP27 largely removed the IP-induced hepatoprotection,indicated by restoration of caspase-3 induction.ConclusionHSP27 is an essential endogenous defense of liver against IR injury.Hepatic ischemic preconditioning can improve subsequent IR injury via up-regulation of HSP27,which is related to HSP27-mediated reduction of apoptosis.

Purpose: To evaluate the HPQP as a modifier of radiation-induced pulmonary fibrosis in rats sacrificed 4 months and 6 months separately after a single dose of 0, 15, 20, 25 Gy60Co r rays to the right hemithorax. 6.Methods and techniques for organ procurement from donation after cardiac death

Objective To investigate the methods and techniques for organ procurement from donation after cardiac death (DCD),and to evaluate post-transplant outcomes.Methods In this retrospective study,clinical data of 26 cases of DCD organ procurements were analyzed through either epigastric multivisceral organ harvesting or solitary organ harvesting.Results Twenty livers,44 kidneys and 2 multivisceral grafts were procured,followed by 24 cases of liver transplantation,42 cases of kidney transplantation,and 2 cases of multivisceral transplantation.The operations were successful and all transplanted organs were satisfactorily recovered with no primary nonfunction or other complications observed.Conclusion Our methods and techniques for organ procurement from DCD donors require experienced surgery skills,while can maximatily shorten donor organ ischemic time,guarantee procurement of high quality of organs and ensure a favourable transplant outcomes.
7.Endoscopic retrograde cholangiopancreatography in management of biliary complications after liver transplantation

Objective To evaluate the effect of endoscopic retrograde cholangiopancreatography (ERCP) in treatment of biliary complications after liver transplantation. Methods Data of 39 patients who underwent ERCP between January 2005 and December 2007 because of biliary complications after liver trans-plantation were retrospectively evaluated. Endoscopic sphincterotomy, dilatation, nasal-biliary drainage (ENBD) and stent placement were performed in 25 patients with biliary strictures (14 at anastomosis site and 11 at non-anastomosis site). ENBD and stent placement were applied in 6 patients with biliary leakage, while endoscopic sphincterotomy, ENBD and stone extraction with baskets were performed in 16 patients with biliary stones. Procedures were repeated when necessary. Results ERCP was successfully performed at a rate of 95.9% (94/98) without any severe complications. Strictures at anastomosis site were resolved in all patients (100%, 14/14), while for strictures at non-anastomosis site, only 27.3% (3/11) were cured. Biliary leakage was resolved in 83.3% (5/6) patients. Complete bile duet clearance was achieved in 81.3% (13/16) of the patients with biliary stones. Conclusion ERCP proves to be safe and effective in the treatment of post liver transplantation biliary complications with low incidence of severe complications.

9.Stochastic clinical contrastive study on two reconstructive methods after resection of cardiac carcino- ma

Objective To explore the reconstructive methods of keeping cardiac function after resec- tion of cardiac carcinoma. Methods After anastomosis of esophageal mucosa and sub-mueosa with gastric counterparts, oblique invagination with gastric ehorion and visceral muscle were adopted in experimental group to keep cardiac anatomical and physiological function.Additionaly, tissues around pylorus were suffi- ciently dissociated to keep pylorie function.Contrastively,regular anastomosis with GF-1 26/28 after resection of cardiac carcinoma was adopted to reconstruct cardia,with thread marked 4 intervally used to reinforce anastomotic ostium in control group.The sphincter of pylorus was dilated to prevent pylorospasm and obstruc- tion.Results In experimental group,no anastomotic leakage,anastomotic stricture and gastroesophageal re- flux occurred.In control group,the incidence of anastomotic leakage and anastomotic stricture were 2.8 %(1/ 36)and 50 %(18/36)respectively,and pH≤4 at anastomotic ostium were found in 12 eases,accounting for 33.3 %, and pH≤6 in 29 cases, accounting for 80.6 %(29/36). Conclusion The reconstructive method of keeping cardiac and pyloric function in resection of cardiac carcinoma benefits recovery of post-operation pa- tients and improve their life quality.
10.Protective effect of EA against cerebral ischemia-reperfusion injury

AIMTo study the protective effects of EA against cer ebral ischemia-reperfusion injury. METHODSAcute cerebral ischemia w as produced by the occlusion of bilateral common carotid arteries of mice .The s urvival time in 12 h of the mice was observed, and the mortalities in 2 h, 6 h, 12 h were recorded. Pulsinelli four-vessel occlusion method was used to make ce rebral ischemia-reperfusion model of rats. The EEG and the reappearing time of righting reflex were recorded and the activity of GSH-Px,LDH,NOS in the brain w as tested. Researching the iNOS expression in hippocampiwith immunocytochemistry method and measure the mean optical density. RESULTSEA can decrease the mortalities and prolong the survival t ime in 12 h of the mice. EA can promote the recovery of the EEG and the RR after cerebral ischemia-reperfusion operation in rats and enhance the activities of the GSH-Px LDH, reduce the activity of the NOS. EA also inhibits the expression of iNOS and reduce it's value of mean optical density in hippocampi of the rat s. CONCLUSIONEA has protective effects against cerebral ischemai-r eperfusion injury.
