3.Protective effect of extract from Shuilu Erxiandan on oxidative injury of structure and function of human sperm membrane

Objective To explore the mechanism of Shuilu erxiandan against male′s infertility and asthenoospermia .Methods Human sperm with normal physiological function were collected ,and then sperm suspension were divided into the normal group ,the model group ,Vitamin C group and the groups of large ,medium and small dose of Shuilu erxiandan .The reactive of species was made from hypoxanthine-xanthine oxidase .In the oxygen environment ,Vitamin C(0 .25 g/L) and different content (1 .0 ,0 .5 ,0 .25 g/mL) of extract from Shuilu erxiandan were hatched with sperm .Then the function of sperm membrane in every group was evalu-ated using hypoosmoticswelling .Meanwhile the content of MDA and SOD were detected .Results Expansion of sperm and sperm motility in all the groups of vitamin C and large ,medium and small dose of Shuilu erxiandan′s extract were significantly higher than that in the model group at time point of 1 ,3 and 6 h (P< 0 .05) .Also ,medium dose of Shuilu erxiandan (0 .5 g/mL) displayed more effective on expansion of sperm and sperm motility compared with that in the vitamin group C (P< 0 .05) .All the groups of vitamin C and large ,medium and small dose of Shuilu erxiandan′s extract significantly indicated descreased MDA content and improved SOD activity compared with that in the model group (P< 0 .05) .Also ,this effect of medium dose of Shuilu erxiandan (0 .5 g/mL ) on MDA and SOD activity were superior to that in vitamin C group ,which showed significant difference (P< 0 .05) .Conclusion Ex-tract from Shuilu erxiandan could significantly protect structure and function of sperm membrane ,which is correlated with interve-ning its lipin peroxidation .
4.Application of Spiral CT for Pancreatic Adenocarcinoma: Emphasis on the Value of the Tumor-pancreas Contrast

Objective To evaluation the application value of two-phase enhanced spiral CT in pancreatic adenocarcinoma. Methods Two-phase enhanced spiral CT was performed in 27 cases of pancreatic adenocarcinoma. After plain CT examination, performed arterial phase(25s after infusion)and portal venous phase(60~70s after infusion)with a 150ml bolus injection of contrast material at 3ml/s. CT attenuation values of tumor and normal pancreas in all scanning phase were obtained and the tumor-pancreas contrast was calculated. Results Mean tumor enhancement were 48±8 HU(arterial phase)and 72±6 HU(portal venous phase), normal pancreas enhancement were 125±18 HU、98±11 HU, mean tumor-pancreas contrast was significantly differenced on arterial phase (73±21 HU)and portal venous phase (25±12 HU). Conclusion Excellent tumor-pancreas contrast is obtained with arterial phase scans, as such it is of significance reference value for diagnosis pancreatic adenocarcinoma.
5.Relationship between coronary slow flow and vascular inflammatory factors

Objective:To explore the relationship between coronary slow flow (CSF) and vascular inflammatory fac-tors .Methods :A total of 229 inpatients with normal coronary angiography (CAG) results were selected as research object .According to CAG TIMI flow and corrected TIMI frame count ,patients were divided into CSF group (n=102 ,no obvious lesion in epicardial coronary artery but coronary flow was slow ) and normal coronary group (n=127 ,completely normal epicardial coronary artery and blood flow ) .General clinical data were collected ,vascular inflammatory factors were measured and their relationship with CSF was analyzed .Results:Compared with normal coronary group ,there were significant rise in levels of high sensitive C reactive protein [ (1.41+0.72 ) mg/L vs . (2.82+0.58) mg/L] ,interleukin 1 [ (0.18 ± 0.05)μg/L vs .(0.26 ± 0.06)μg/L] and interleukin 6 [ (0.363 ± 0.016) ng/L vs .(0.465 ± 0.015) ng/L] in CSF group , P<0.05 all .Logistic regression analysis indicated that vas-cular inflammatory factors were risk factor for CSF occurrence (OR= 1.008 , P= 0.015 ) .Conclusion:Coronary flow velocity is closely related to vascular inflammatory factors .
6.Influence of neurtophil elastase on sperm quality in male infertile patients

Objective To explore the relationship between sperm quality and neurtophil elastase (NE) in male infertile patients . Methods the NE concentration was detected in1 30 male infertile patients ,the research subjects were classified into 3 groups based on the neurtophil elastase concentration :neurtophil elastase concentration more than 1 000 ng/mL (group A ) ,250-1 000 ng/mL (group B) less than 250 ng/mL (group C) .Then sperm basic parameters and biochemical markers (phosphatase ,citrate ,zinc and fructose) were detected .Results (1) The sperm density and motility showed statistically significant differences among the group A ,B and C(P<0 .05)moreover neutrophil elastase concentration was negatively correlated with sperm density (r= -0 .43) and sperm motility(r= -0 .37) .Sperm volume and morphology showed no statistically significant differences among three groups (P>0 .05) .(2) There were statistically significant differences in the levels of phosphatase ,citrate and zinc among three groups (P<0 .05) ,moreover the neutrophil elastase concentration had significantly negative correlation with the concentrations of phosphatase , citrate and zinc(r= -0 .19 ,r= -0 .17 ,r= -0 .23) ,while the fructose concentration had no statistical difference among three groups(P>0 .05) .Conclusion The NE level in semen has significant reverse correlation with sperm density ,sperm motility and bi‐ochemical markers (phosphatase ,citrate and zinc) ,implying that semen NE has significantly negative effect on the sperm quality , thus affecting male fertility ability .
7.Translaminar microendoscopic discectomy in the management of special lumbar disc herniation

Objective To assess the clinical application of microendoscopic discectomy (MED) system in the treatment of 36 cases of special lumbar disc herniation (LDH). Methods Laminectomy and decompression, and removal of fractured end plate or calcified herniated discs were performed by MED using self-made bone chisel, osteotome and L-shaped osteotome. Results Intraoperative dural laceration occurred in 2 cases, and no conversions to open surgery were required. Follow-up observation ranged (3~12) months (mean, 9.5 months). According to Macnab criteria, 28 patients had excellent, 5 had good, 2 had fair, and 1 had poor outcomes, with good or excellent results in 91.7% (33/36) of the patients. Conclusions MED may be carried out in patients in side position. Calcified herniated discs and fractured end plate can be excised by MED with self-made tools.
8.Recent advances in curcumin and its derivatives for treatment of liver diseases.

Curcumin is a principal polyphenolic curcuminoid extracted from turmeric rhizome, which has been used for treating inflammation of joints, ulcers, jaundice and other disorders in Asian traditional medicine. In recent years, many studies have indicated that curcumin plays important roles in treatment of liver diseases. Curcumin attenuates liver injury and non-alcoholic fatty liver disease by lowering the release of inflammation cytokines, minimizing oxidative stress, enhancing the sensitivity of insulin and altering lipid metabolism. Curcumin shows potent anti-fibrosis activity, contributing to inhibit the activation of hepatic stellate cells and reduce the deposition of extracellular matrix by its regulation of PPAR-γ, NF-ΚB and TGF-β signaling pathways. Moreover, curcumin exhibits anti-cancer effect by inducing G2/M phase cell cycle arrest and apoptosis in several hepatoma cell lines. However, poor water solubility and low bioavailability of curcumin limit its clinical applications. To overcome its limited systemic bioavailability, many new approaches have been explored to deliver curcumin effectively. This article focuses on advances in the effects of curcumin and its derivatives for treatment of liver injury, non-alcoholic fatty liver disease, liver fibrosis and hepatocarcinoma.

9.Influence of thyroxine treatment on serum lipid levels in patients with subclinical hypothyroidism:a Meta-analysis

Objective:To examine whether the thyroxine treatment can affect the serum lipid levels in patients with subclinical hypothyroidism by Meta analysis.Methods:Pubmed and China National Knowledge Infrastructure(CNKI)were searched for randomized controlled trials reporting the effectiveness of thyroxine treatment on serum lipid levels in subclinical hypothyroidism patients from January 1980 to September 2006;hand-search was also performed.The included trails were analyzed by RevMan4.2 software.Results:Six studies were finally included and five of them found thyroxine to be significantly more effective than placebo in improving the serum lipid disorders;one of them found thyroxine had no obvious effect.The pooled effect sizes(95%CI)of total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C),apoprotein A(ApoA),apoprotein B(ApoB)and lipoprotein a(Lp(a))were -0.39(-0.71, -0.07),-0.02(-0.07,0.04),-0.02(-0.06,0.03),-0.22(-0.31,-0.12),-0.08(-0.11,-0.06),-0.05(-0.08, -0.02),and -0.38(-2.41,1.64),respectively.The analysis results showed that thyroxine treatment was significantly effective on serum levels of TC,LDL-C,ApoA,and ApoB,but not on TG,HDL-C,or Lp(a).Conclusion:This Meta-analysis demonstrates that thyroxine treatment can improve thyroid function and lipid metabolism in patients with subclinical hypothyroidism,reducing the prevalence of artherosclerosis and coronary heart disease.
10.Prevalence and risk factors of pterygium

Pterygium is a common disorder of ocular surface with unknown etiology and pathogenesis. The epidemiological studies around the world have shown that the prevalence rates range from 0.3% to 37.46%. Pterygium is related to geographic setting, sunlight and ultraviolet exposure,age, gender, economic situation, dry eye syndrome and others. The purpose of this review is to present a summary of the more recent literature about the epidemiological study, paying particular attention to prevalence and risk factors of pterygium.
