1.Effect Evaluation of Comprehensive Intervention with Low-salt Diet in Elderly Patients with High Blood Pressure

Objective To investigate the role of the extended-care in improving the compliance with low-salt diet in elderly patients with hypertension.Methods We selected 100 cases of elderly patients with hypertension in hospital.The 100 cases were divided into control group (50 cases) and observation group (50 cases) randomly.Observation group was given comprehensive intervention in low-salt diet education and control group was given traditional low-salt diet education.Tow groups were divided into three stages (on the day of admission,seven days after admission,one month after discharge).We analyzed low-salt cognitive scores,low-salt behavioral scores and salt-intake/d comparably.Results On the day of admission,tow groups had no statistically significant difference (P>0.05) in low-salt cognitive scores,low-salt behavioral scores and salt-intake/d;seven days after admission,tow groups had statistically significant difforence (P<0.05) in low-salt cognitive scores,low-salt behavioral scores and salt-intake/d;one month after discharge,tow groups had statistically significant difference (P<0.05) in low-salt cognitive scores,low-salt behavioral scores and salt-intake/d.Conclusion We use comprehensive intervention method to elderly patients with hypertension in low-salt diet education,through personalized low-salt diet education following evidence-based nursing,we can increase the compliance with low-salt diet in elderly patients with hypertension after discharge and improve blood pressure control effectively and to promote the image of the hospital and achieve the higher satisfaction from patients and families.

3.Investigation of Magnetic Resonance Imaging Features and Treatment of Sacrococcygeal Teratoma in Children

Objective To analyze the magnetic resonance imaging (MRI) features of sacrococcygeal teratoma in children,and explore the diagnostic value of MRI and the critical suggestion for treatment.Methods The MRI features,clinical features,pathological data and operation complication of sacrococcygeal teratoma in 26 children were retrospectively analyzed. Phenotype teratoma in 9 cases grew from apex of coccyx to fundament.They presented spherical or ellipsoidal,distinct boundary and septatus in cystic MRI.All pathological data presented benign tumors.Mixed teratoma in 16 cases growed from fundament to cavitas pelvis.They presented dumbbell,distinct boundary,mixed signal of cystiform and solid density in MRI.There were 12 cases in benign tumors and 4 cases in malignant in pathology,the invisible teratoma in 1 case which lay between rectum and sacrum,extended to cavitas pelvis.Its boundary was not intact and segmented,and the pathological data presented malignant tumors.Results All of the 26 cases were completely excised only once and 21 of them received sacrococcygeal incision,5 cases had abdominosacral combined incision.Twenty-two cases were followed up (benign 20 cases,malignant 2 cases),and 2 cases of them died. 8 cases had complications,of the 8 cases,2 cases with necrosis of flap,1 case with chronic sinus tract and 2 cases with recidivism occurrd.Conclusions MRI may detect discovering the location and appearance of sacrococcygeal teratoma in children in diagnosis and in differential diagnosis of tumor nature.It is a very significant to reconstruct cavities pelvis and eradicase focal dead space in prevention and treatment of post-operation complication in childrens sacrococcygeal teratoma.
4.Clinical study on treatment of pulmonary interstitial fibrosis with ginkgo extract.

OBJECTIVETo observe the effect of ginkgo extract on pulmonary interstitial fibrosis.
METHODSForty-five patients with pulmonary interstitial fibrosis were randomly divided into two groups, the treated group (n = 30) received ginkgo biloba extract 1 g, three times a day; the control group received prednisone 30 mg, once a day, the therapeutic course for both groups was 3 months. Changes of clinical symptoms, pulmonary function, arterial partial pressure of oxygen, computerized tomography (CT), interleukin-6 (IL-6), interleukin-8 (IL-8) and tumor necrosis factor-alpha(TNF-alpha) in the two groups were observed before and after treatment.
RESULTSThe efficacy of treatment in the two groups showed insignificant difference, clinical symptoms, pulmonary function, arterial partial pressure of oxygen were improved after treatment (P < 0.05), and the levels of IL-6, IL-8 and TNF-alpha significantly decreased after treatment as compared with those before treatment in the two groups. The occurrence of pulmonary infection was less in the treated group than that in the control group (P <0.05).
CONCLUSIONGinkgo is effective in treating pulmonary interstitial fibrosis.

5.Determination of ellagic acid, flavonoids and goshonoside-F5 in Rubi Fructus by HPLC.

High-performance liquid chromatographic coupled with variable wavelength detection (HPLC-VWD) has been developed for simultaneous determination of 5 analytes including ellagic acid, quercetin, kaempferol-3-O-beta-D-rutinoside, tiliroside and kaempferol, and high-performance liquid chromatographic with an evaporative light scattering detector (HPLC-ELSD) has been established to determine goshonoside-F5 in extract of Rubi Fructus. Chromatographic separations were carried out on an Agilent ZORBAX SB-C18 column (4.6 mm x 250 mm, 5.0 microm). All calibration curves of reference standards revealed good linearity (R2 > 0.999 5) within the concentration ranges tested. The method limits of detection ranged 0.297-90.144 ng and the method limits ofquantitation ranged 0.990-300.480 ng, respectively. Recoveries of 6 analytes were from 97.11% to 101.7%, with RSD less than 2.1%. The result shows that amounts of the 6 analytes in the samples from 16 localities were found to be different. The higher latitude of growing environment, the more ellagic acid in herb. The content of total flavonoids in sample from east localities were higher than that in middle and west localities, and the content of goshonoside-F5 in Bozhou, Anhui province was higher than others. This method was found to be simple, accurate, sensitive with good repeatability. Those results might serve as a sound foundation for further study, quality control and application of Rubi Fructus.

6.The Techniques of Isolation and Determination of Ergosterol as the Indicator of Fungal Biomass

Ergosterol is the important component of the fungal membrane, and having stable structure. This makes it a suitable indicator for growth of fungi. In the paper, isolation and determination techniques of ergosterol as the indicator of the fungal biomass were reviewed. The methods of extracting ergosterol include traditional saponification and refluxing, rapid physical disruption, rapid ultrasonication, supercritical fluid extraction and so on. The ergosterol determination methods are high performance liquid chromatography, gas chromatography-mass spectrometry, liquid chromatography-mass spectrometry, and thin-layer chromatography, et al. The application of these techniques was also introduced. Finally, the paper prospected the feasibility of applying the ergosterol as the indicator of fungal biomass in composting.
7.Effects of scutellaria stem-leaf total flavonoids on cardiocyte apoptosis induced by hypoxia/reoxygenation.

OBJECTIVETo study the effect of Scutellaria baicalensis stem-leaf total flavonoids (SSTF) on cardiocyte apoptosis of neonatal rats induced by hypoxia/reoxygenation and its action of mechanism. METHODS Sixty one to two days old rats, male or female, were selected. Hypoxia/reoxygenation injured model was established in cultured cardiocytes of neonate rats. The cultured neonatal rat cardiocytes were divided into 5 groups, i.e., the normal control group, the hypoxia/reoxygenation injury group (as the model group, cultured cardiocytes were exposed to hypoxia 2 h and subsequently reoxygenated for 4 h), the hypoxia/reoxygenation injury plus 50 mg/L SSTF group (as the low dose SSTF group), the hypoxia/reoxygenation plus 100 mg/L SSTF group (as the middle dose SSTF group), and the hypoxia/reoxygenation plus 200 mg/L SSTF group (as the high dose SSTF group). The cell viability was detected by methyl thiazolyl tetrazolium (MTT) colorimetry. The apoptosis of cardiocytes was determined by terminal deoxynucleotidyl transferase-mediated dUTP nick end labeling (TUNEL) and the apoptosis rate calculated. The Bcl-2 and Bax protein expressions were determined by immunohistochemistry.
RESULTSCompared with the normal control group, the cell viability, Bcl-2 protein contents and Bcl-2/Bax decreased, the apoptosis rate and Bax protein contents increased in the model group (all P<0.01). Compared with the model group, the cell viability, Bcl-2 protein contents and Bcl-2/Bax increased, while the apoptosis rate and Bax protein contents decreased in each SSTF treated group (P<0.05, P<0.01). Compared with the low dose SSTF group, significant difference existed in each index of the high dose SSTF group (all P<0.05).
CONCLUSIONSSSTF had protection on hypoxia/reoxygenation induced cardiocyte apoptosis. Its protective mechanism might be correlated with its up-regulation of the expression of Bcl-2 protein ahd down-regulation of the expression of Bax protein.

8.In vitro balanced sustained-release of Panax notoginseng saponins controlled with various matrix materials.

To explore the influence of matrix materials in complicate ingredients on traditional Chinese medicine and investigate the excipients selection model based on balanced release characteristics of multicomponents, the influence of HPMC (K4M, K15M, K100M) and Carbomer (934P, 971P, 974P) was illustrated by testing in vitro release of ginsenoside-Rg1, ginsenoside-Rb1 and notoginsenoside-R1 in Panax notoginseng saponins (model drug, PNS). According to in vitro release results of PNS matrix tablets in water and artificial intestinal juice, the release curves were analyzed with Peppas equation and simulating factor (f). Significant differences in k value and n value among ginsenoside-Rg1, ginsenoside-Rb1 and notoginsenoside-R1 existed in various formulations. The release behaviors from various excipients could be described with Non-Fickian transport or super Case II transport pattern. The f2 values for ginsenoside-Rg1, ginsenoside-Rb1 and notoginsenoside-R1 in 971P matrix tablet containing 30% Carbomer 971P were 74.91, 53.45, 57.89 in water and 79.35, 55.51, 51.89 in artificial intestinal juice, respectively. The release profiles fit for the regulation of FDA. The result revealed that the balanced release rates of Rg1, Rb1 and R1 in 971P matrix tablet were obtained.

9.Application of a digital three-dimensional reconstruction technique in the diagnosis and treatment of hepatic alveolar echinococcosis

ObjectiveTo investigate the value of a digital three-dimensional reconstruction technique in the treatment of hepatic alveolar echinococcosis (HAE).MethodsThe computed tomography scan data for 13 patients with HAE who were admitted to the First Affiliated Hospital of Xinjiang Medical University from February 2011 to October 2011 were reconstructed and analyzed by a three-dimensional reconstruction system to assess resectability,and to facilitate surgical planning and individualized virtual surgery.The results of preoperative analysis were compared with the results of actual operations.ResultsThe three-dimensional models of the liver were reconstructed successfully,and intrahepatie lesions and vessels were clearly displayed.One patient received an autologous liver transplantation,10 underwent hepatectomy,and 2 received percutaneous transhepatic cholangial drainage.Virtual operation planning was carried out for 11 patients using the three-dimensional reconstruction system.The mean volume of the liver to be resected was predicted to be 920 ml (range,339-2678 ml),and the mean percentage of liver to be resected to the total liver volume was predicted to be 45% ( range,23% -68% ).The mean volume o[ the actual liver resection was 834 ml (range,315-2250 m[),and the mean percentage of actual liver resected to the total liver volume was 42% (range,22% -70% ),which was consistent with the results of preoperative three-dimensional reconstruction.All patients were followed up for 2-8 months,and no severe complications such as liver failure,hemorrhage and bile leakage were detected.ConclusionDigital three-dimen-sional reconstruction is helpful in the diagnosis and treatment of HAE and effectively reduces surgical risks.
10.Expression of the keratin 10 related genes in buccal mucous membrane exfoliated cells and changes of liver function in coal-burning arsenism

Objective To observe expression of the keratin 10 (K10), c-myc and cox2 in buccal mucous membrane exfoliated cells, and changes of liver function of coal-burning arsenism patients, in order to explorate coal-burning arsenic poison circumference biology symbol. Methods The buccal mucous membrane exfoliated cells were collected from both arsenism patients and normal controls. Real-time PCR was employed to detect the changes of K10, c-myc, cox2 genes expression in these cells. Simultaneously, circumference venous blood was extracted and examinated liver function. Results K10 was found obviously overexpressed in arsenism patients(3.60±0.94) compared to control(1.82±0.68), the difference had statistics significance(t=2.15, P<0.05), c-mye and cox2 did not found obviously changed(c-myc: 3.50±2.77,3.39±2.07; cox2:5.90±1.40,4.73±1.91; t=1.26,1.65, P> 0.05). The serum ALT of patients(25.83±2A5) obviously increased than control(36.86±1735, t=2.55, P<0.05). Conclusions Expression of K10 gene in buccal mucous membrane cells may be regarded as sensitive molecular markers for skin pathologic changes in arsenic patients. The liver is a sensitive target organ of inorganic arsenic.
