1.Progress on bone marrow mesenchymal stem cells transplantation for spinal cord injury.

Bone marrow mesenchymal cells (BMSCs) are regarded as donor cells in cell transplantation therapies for spinal cord injury (SCI) for they have the ability of favourable proliferation and multi-directional differentiation, and are easily isolated and culturd and have less immunological reaction. It has been confirmed that subarachnoid space injection is the most ideal delivery technique of BMSCs. Bone marrow mesenchymal stem cells transplantation is safe and its reconditioning role is certain for SCI in early clinical application. The mechanism of BMSCs promoting functional recovery after SCI is probably concerned with vicarious function, nerve trophism, immunosuppression and promoting axonal regeneration by BMSCs.

2.Repeatability of TLICS scoring system used in the thoracolumbar spine injury

Objective To evaluate the thoracolumbar injury severity score [ thoracolumbar injury classifica-tion and severity score(TLICS)]and repeatability,and analyze its clinical significance in the treatment of injury of thoracolumbar.Methods The clinical and imaging data of our hospital (thoracolumbar X ray,CT examination,MRI examination) of intact thoracolumbar fractures in 90 patients were retrospectively analyzed ,respectively,TLICS score were made for their site of spinal injury morphology ,neural function and the posterior ligament complex three .Three months after reviewed ,TLICS was analyzed by using Cohen weighted kappa coefficient score .Its repeatability was ana-lyzed.Results According to the TLICS system ,spinal injury body ,nerve function and the integrity of the posterior ligamentous complex were evaluated , respectively, the repeatability coefficient of Kappa , the calculated total to 0.47 non operation group,operation group,the total score was 0.46,with a moderate consistency.The neural function evaluation of the highest high consistency ,consistency .No significant differences between the two groups of repetitive Kappa coefficient(statistical values =0.674 5,P>0.05).According to the TLICS system of statistical accuracy ,the diagnostic sensitivity,specificity ratios was 95.8%,86.4%,97.6%,respectively.Conclusion The reliability of TLICS system for thoracolumbar treatment and recovery is higher ,relative comprehensive evaluation ,which can effec-tively guide the clinical treatment .
3.Surgical management of thoracic aortic aneurysm combined with coronary artery disease

Objective: To review the experience of one-stage surgical management of thoracic aortic aneurysm associated with coronary artery disease. Methods: From June 1999 to July 2002, fifteen patients (all the male, average age 57.21 year-old, ranged 40-66 year-old) underwent thoracic aortic aneurysm repair and concomitant coronary revascularization. The diagnosis are aortic root aneurysm in 5 cases,ascending aortic aneurysm in 2,arch aortic aneurysm in 3,and aortic dissecting in 5. 14 patients received operation through midsternotomy,including ascending aorta replacement in 2, Bentall technique in 4, David technique in 1 and total arch replacements in 3 patients. Left thoracotomy and off-pump CABG was done in 1 case. Results: 1 patient died in 30 days after operation. Followed up for a mean duration of 17 months,there were no late death and no reoperation and no related events occurred. Conclusion: Thoracic aortic replacement and CABG (coronary artery bypass grafting) could be done at one-stage. Concomitant coronary revascularization may aggressively obviate CAD events.
4.The study of cooling control system of the CT bulb tube intelligent closed loop

Objective:To improve the CT bulb tube cooling rate and the testing method of the CT bulb tube temperature, and provide more stable platform for better bulb tube protection and longer life-span. Methods: The designed specified cold air machine could transfer the dry cold wind into machine rack, which can lower the ambient temperature of the machine rack and increase the air flow inside the machine rack and promote the efficiency of the heat exchange of the bulb tube oil cooling circuit. Results:the design of this system’s joint is light and handy, which could collect the instantaneous temperature change of the bulb tube, and carry out simple man-machine interaction monitoring. Conclusion:According to the bulb tube oil-way temperature and ambient temperature and relative humidity, the computer can dynamically adjust the refrigerating capacity and operation/cease of the air cooler. Then it can constitute the closed control loop, and accelerate the efficiency of the bulb tube’s heat dissipation, and promote the heat capacity of the bulb tube equivalently.
5.Nodular regenerative hyperplasia of the liver:a clinical study of 12 cases

Objective To review the clinical,histological and diagnostic aspects of 12 documented cases of nodular regenerative hyperplasia of the liver(NRHL),to make this condition be understood and dealt with better. Method Twelve NRHL cases were diagnosed based on liver biopsy from 300 portal hypertension patients who had been underwent splenectomy.Imaging studies were performed as part of the diagnostic evaluation.Clinical manifestation and biochemical tests were recorded at the time of diagnosis.Management and prognosis were also reviewed.Results Most patients were complicated with autoimmune disease,6 cases were diagnosed systemic lupus erythematosus and 1 was Crohn's disease and 1 suspected ulcerative colitis.Six cases were treated by prednisone and 3 cases by immunosuppressant.Eleven cases had suffered from portal hypertension.All cases had no history of viral hepatitis.Biochemical tests showed mild increase of liver enzyme and relative normal synthetic liver function.The histological finding was nodular in the hepatic parenehyma,with mild periportal fibrosis,intraportal lymphocytic infiltration,narrow and obstruction of branch of portal vein,and lack of hepatocyte necrosis.All cases were diagnosed liver cirrhosis and portal hypertension before operation.Management was directed to portal hypertension and varices bleeding with satisfactory results.Most of them keep a stable condition during the follow-up. Conclusion The NRHL was uncommon and its cause and pathogenesis was unclear,may be related with immune and hepatic blood circulation disorder.It should be considered in patients with unexplained portal hypertension and distinguished it from liver cirrhosis.Liver biopsy confirms the diagnosis.Management directed to portal hypertension may improve clinical condition.

8.Clinical effect of bicyclol combined with polyene phosphatidyl choline in elderly non-alcoholic fatty liver disease

Objective To observe the clinical effect of bicyclol combined with polyene phosphatidyl choline in elderly non-alcoholic fatty liver disease (NAFLD).Methods One hundred and twenty elderly patients with NAFLD were divided into 3 groups by block randomization method,40 cases in each group.Therapeutic group was treated by bicyclol combined with polyene phosphatidyl choline; bicyclol group was only treated by bicyclol; and polyene phosphatidyl choline group was only treated by polyene phosphatidyl choline.The blood biochemical indexes,liver ultrasound score and clinical curative effect of 3 groups were compared after treated for 24 weeks.Results The total cholesterol (TC),triglyceride (TG),alanine aminotransferase (ALT),aspartate aminotransferase (AST) and gamma glutamine transferase (GGT) in 3 groups after treatment were lower than those before treatment,and the difference was statistically significant (P < 0.05) ; TC,TG and ALT levels in therapeutic group after treatment were significantly lower than those in bicyclol group and polyene phosphatidyl choline group [(1.36 ± 0.84) mmol/L vs.(2.77 ± 1.27),(2.84 ±1.35) mmol/L; (1.32 ±0.71) mmol/L vs.(1.89 ±0.87),(1.92 ±0.90) mmol/L; (38.26 ± 12.75) U/L vs.(57.83 ± 16.67),(62.07 ± 18.16) U/L],and the difference was statistically significant (P < 0.05).The liver ultrasound score in 3 groups after treatment was significantly decreased compared with that before treatment,and the difference was statistically significant (P < 0.05).Liver ultrasound scores in therapeutic group after treatment were significantly lower than those in bicyclol group and polyene phosphatidyl choline group [(2.08 ± 0.93) scores vs.(3.17 ± 1.14),(3.34 ± 1.07) scores],and the difference was statistically significant (P < 0.05).The total effective rate in therapeutic group was significantly higher than that in bicyclol group and polyene phosphatidyl choline group [85.0% (34/40) vs.67.5% (27/40),65.0% (26/40)],and the difference was statistically significant (P <0.05).Conclusions Bicyclol combined with polyene phosphatidyl choline has better clinical effect in elderly patients with NAFLD.It is better than single bicyclol and polyene phosphatidyl choline and worth clinical promotion.
9.Cloning, Sequence Analysis and Proeukaryotic Expression of Peroxisomal Membrane Protein Allergen Gene from Chaetomium globosum

The full length cDNA encoding peroxisomal membrane protein from Chaetomium globosum was cloned using RACE technology and the sequence in cDNA library of C. globosum in GenBank ( Accn: BP099709). The 747bp full length cDNA encoding peroxisomal membrane protein allergen (pero) gene was assembled with 412bp 3'and 508bp 5'RACE products. The open reading frame was 501 bp encoding 166 amino acids. The molecular weight of the protein was 17. 5kDa and its theoretical isoelectric point was 5.75. The pero gene was amplified using specific primers of cDNA 5'and 3'untranslated region, sequence analysis indicated that the gene have 3 exons and 2 introns. ClustalX analysis revealed that amino acids sequence of pero gene from C. globosum and Neurospora crassa shared 83% high similarity. To construct pET28a-pero expressive plasmid, pero gene was inserted into pET28a expressive vector. Escherichia coli BL21 transformed by pET28a-pero plasmid was induced with IPTG. The protein expression was analyzed with SDS-PAGE. A 21kDa pero fusion protein representing the pero gene was expressed in recombinant E. coli BL21. The sequences of cDNA,DNA and deduced amino acid of the pero gene from C. globosum were submitted to GenBank (Accn: AY555771, AY584753,AAS66898).
10.Protective effect of isoflurane preconditioning on myocardium against ischemia-reperfusion injury in isolated rat hearts

Objective To investigate the protective effect of isoflurane preconditioning on myocardium against isehemia-reperfusion(I/R)injury and the possible mechanism.Methods Fifty male SD rats weighing 250- 300 g were randomly divided into 5 groups(n=10 each):Ⅰ control group(C);Ⅱ I/R group and 3 isoflurane preconditioning groups with 0.5%(Ⅲ),or 1.0%(Ⅳ)or 2.0% isoflurane(Ⅴ).The animals were anesthetized with intraperitoneal pentobarbital 40 mg?kg~(-1).The hearts were immediately excised and placed in cold K-H solution.The aorta was canuulated and heart retrogradely perfused with K-H solution aerated with 95% O_2 and 5% CO_2 at 37℃ and 10 kPa in a langendorff apparatus.Left ventricular end-diastollc pressure(LVEDP)and left ventrieular systolic pressure(LVSP)were measured from a fluid-filled latex balloon in the left ventricle.The isolated hearts were made globally ischemic for 30 min followed by 60 rain reperfusion in group Ⅱ-Ⅴ.In the 3 isoflurane preconditioning groups the hearts were perfused with K-H solution saturated with 0.5% or 1.0% or 2.0% isoflurane for 15 rain followed by 15 rain washout before ischemia.The cardiac function variables including LVEDP,LVSP dp/dt_(min),dp/dt_(max) and HR were measured after epuilibrium(baseline values),immediately before ischemia,at the end of 30 min ischemia and 60 min reperfusion.The infarct size and cytochrome C level in cytoplasm and mitochondria of myocytes were measured.Results I/R significantly increased LVEDP and decreased LVSP,dp/dt_(min),dp/dt_(max) as compare with control group.Sevoflurane preconditioning significantly attenuated the depression of cardiac function caused by I/R.Only LVEDP was significantly higher during reperfusion period in the 3 sevoflurane preconditioning group than in the control group but there was no significant difference in LVSP,dp/ dt_(min),dp/dt_(max) between control group and the 3 preconditioning groups.The infarct size was significantly smaller in the 3 preconditioning groups than in I/R group.Cytochrome C level was significantly increased in cytoplasm but decreased in mitochondria in I/R group as compared with control group.Sevoflurane preconditioning significantly ameliorated the release of cytochrome C from mitochondria to cytoplasm in the 3 sevoflurane preconditioning group.Conclusion Isoflurane preconditioning can protect the heart against I/R injury by attenuation of the release of cytochrone C from mitochondria to cytoplasm.
