1.Correlated Factors of Compliance in Patients with Depression in Polyclinic

ObjectiveTo investigate compliance with drug, the factors influencing compliance in patients with depression, main dropout time and the characteristics. MethodsA retrospective study was carried out with a total of 154 patients with depression. The correlated factors were studied and dropout time and characteristics were explored. ResultsThe depression patients with bad compliance were 95 (61.7%). The factors influence the drug compliance was various. Different dropout reasons occur at different time. The different dropout rate of depression were 25.3%,18.2%,7.8%,5.8%,4.5% within the 1st, 1st~3rd, 3rd~6th ,6th~9th,after 9th month respectively. ConclusionBad compliance with antidepressant is popular with various reason. Dropout of antidepression is mainly in the 1st month after treatment. Deficiency of associated knowledge is the main reason to stop treating.
2.Retrospective study on the changes of refractive state and stability after cataract surgery

AIM: To compare and contrast different operation after cataract patients with refractive change rules. To analyze the patients with refractive stability after cataract surgery, and to provide a reference for cataract patients with clinical surgery after visual quality.
●METHODS: Retrospective study. A total of 126 cases (150 eyes) were selected from Jan. 2014 to Dec. 2015 in Changzhou First People′s Hospital of cataract extraction combined with foldable intraocular lens implantation for cataract patients as the research samples. According to the different operation for three groups, the first group of 42 patients ( 50 eyes ) underwent above 3 mm clear corneal incision;52 cases in group 2 (60 eyes) underwent temporal side 3 mm clear corneal incision. The third group, 32 cases (40 eyes) underwent 3 corner above the scleral tunnel incision. All the cases were measured at different time point in patients with naked eyes far visual acuity, best corrected visual acuity, spherical degree, the degree of astigmatism and astigmatic axial, comparative analysis of after cataract surgery in patients with refractive change regularity and stability of refraction.
●RESULTS: The uncorrected distance visual comparison within the group, and each time point after preoperative differences were significant ( P< 0. 01 ), and the early postoperative period after 1, 3mo significantly different (P<0. 05). Three groups of patients after surgery compared with preoperative uncorrected distance visual acuity improved significantly, and were stable after 1mo. Compare the best corrected distance vision within the group, and each time point after preoperative differences were significant (P<0. 01), postoperative 1wk and after 1, 3d significantly different (P<0. 05), after 1wk and after 1, 3mo was not significantly different ( P > 0. 05 ), three groups of patients were compared with the preoperative best corrected distance visual acuity were increased significantly, and were in stable after 1wk; relatively spherical degree within the array, after 1d and 3d was not significantly different (P>0. 05), hyperopia drift, after 1wk and 1, 3d was significantly different (P<0. 05), after 1wk and 1, 3mo was not significantly different ( P>0. 05 ). Three groups of patients′spherical degrees after 1wk were stabilized. Comparative degree of astigmatism within the array, postoperative compared with preoperative corneal astigmatism were increased 1d after surgery. Corneal astigmatism in each group reached the maximum, and then decreases 1wk and 1d after surgery, compared with postoperative 3d was significantly different ( P<0. 05 ). After 1wk and 1, 3mo was not significantly different ( P>0. 05 ) . Three groups of patients were compared with preoperative astigmatism were significantly increased, and in operation after 1wk were stabilized;astigmatic axis were three groups in the preoperative astigmatism against the rule, the first and third group after 1d, three Tianshun rule astigmatism proportional were increased, and then decreased. Group 2 the- rule astigmatism proportion, after 1wk, 1 and 3mo, the first and third group gradually reduced the proportion of cis regulatory astigmatism, and compared with preoperative increased, increasing the-rule astigmatism group 2 ratio, and increased compared with preoperative.
●CONCLUSION: Above 3 mm the transparent corneal incision, temporal clear corneal incision and above the scleral tunnel incision different surgical postoperative visual acuity are good. lt can be used as a routine surgical procedure in treatment of cataract;phacoemulsification in cataract patients with former majority against the rule astigmatism. After cataract surgery, early refractive state is a state of mild hyperopia and stabilized about 1wk, combined with clinical guide glasses.
3.Investigating the correlation between PTPN22 C1858T polymorphism and rheumatoid arthritis in Han nationality in Xiamen area

Objective To assess the correlation between FFPN22 C1858T polymorphism and rheumatoid arthritis(RA)in Han people in Xiamen area by TaqMan-MGB real-time PCR.Methods A casecontrol study was carried out in Xiamen Han population.Their blood samples(100 RAs and 100 controls respectively)were collected and the PTPN22 C1858T polymnrphism was tested by TaqMan-MGB real-time PCR.Results①A technique of TaqMan-MGB teal-time PCR was established to investigate PTPN22 C1858T polymorphism;② Onlv 1858 C allele was presented in all the RAs and controls,and no T allele was detected.Conclusion There is no PTPN22 1858T allele in Han people in Xiamen area.It is suggested that there's no association between PTPN22 C1858T polymorphism and RA.

6.Clinical value of ultrasonography in typing epididymal tuberculosis

Objective To investigate the value of ultrasound in typing of epididymal tuberculosis on the clinical treatment . Methods The sonographic characteristics and treatment outcomes were analyzed in 125 patients with epididymal tuberculosis . Results In 125 patients with epididymal tuberculosis ,5 types were divided:( 1 ) Diffusive type ( 37 cases ) presented images of the epididymal diffuse enlargement , abnormal shape ,not smooth surface and inhomogeneous reduced in echogenicity ,13 cases were treated conservatively and 24 cases underwent surgical treatment after failure of conservative treatment . (2) Mass type (24 cases) showed images of hypoechoic mass lesion of caput epididymidis or cauda epididymidis ,4 cases were treated with surgical operation and 20 cases were treat with conservative therapy . (3) Abscess type ( 33 cases) manifested images of irregular liquid anechoic of caput epididymidis and cauda epididymidis , poor ultrasound penetration ,and unclear border . (4) Ulcer‐fistula type (9 cases) revealed images of liquid hypoechoic caput epididymidis or cauda epididymidis extend to the surface of the scrotum . ( 5) Testicular involvement type (22 cases) detected inhomogeneous hypoechoic mass lesion within the testicle and unclear border . The patients of abscess type ,ulcer‐fistula type and testicular involvement type were all treated surgically . Conclusions Ultrasonic diagnosis of epididymal tuberculosis provided a reliable basis for clinical treatment scheme and it was worth recommending .
7.CT Diagnosis of Thymoma

Objective To study CT features of thymoma,so that to improve the accuracy of CT diagnosis and differential diagnosis.Methods 31 cases of thymomas proved by surgery and pathology were examined with conventional CT scans.CT findings of thymoma were analyzed.Results The lesions in 27 cases(87.1%)were located in the anterior upper and middle mediastinum.There were benign lesion in 11,including mass-cardiovascular interface(MCI) with convex type(8 cases),flatness type(1 cases) and concave type(2 cases).20 cases were malignant lesion,including MCI with cast type(18 cases) and concave type(2 cases).Irregular invasion to adjacent organs was found in 11 cases,others included pericardiac effusion(n=6),pericardial and mediastinal invasion(n=2),pleural effusion(n=4),pneumonia(n=2),lung,bone,mediastinal lymphadens metastasis(n=2) and liver,pancreas metastasis(n=1).Conclusion CT scans is of significant value in diagnosis and differential diagnosis of thymoma.
8.Effects of ulinastatin on intestinal mucosal barrier after occlusion of portal vein in rats

Objective:To study the effect of portal vein occlusion on intestinal mucosal barrier in rats and the protection of ulinastatin to the injury,to present the experimental data for the clinical surgery.Methods:70 Sprague-Dawley rats were randomly divided into controlled group (n=10),operation group (n=30) and operation+medication group (n=30).The portal vein were occlused 40 min in the operation groups and operation+medication groups.2ml blood from portal vein,lymph nodes around appendix,1cm small intestine wall were taken for endotoxin levels,bacterial translocation and pathiology examinations in the all rats 280 mins after operation.The mocusal barrier and microscopic structure of intestine were observed.Results:Compared between the control group and the operation group,endotoxin levels,bacterial translocation rates rise greatly and gut structure change obviously in the latter.Compared between the operation group and operation+medication group,the former changes is also obvious.Conclusion:The occlusion of portal vein can leads the decrease of intestine mocusal barrier and the increase of its permeability.Ulinastatin has a good protective effect on the damages above.

Objective To investigate the mechanisms of inhibitory effect of Erlotinib,an epidermal growth factor(VEGF) receptor inhibitor,on angiogenesis of pancreatic carncer.Methods①In a tube formation assay,Erlotinib(100?mol/L) was applied to the culture media and compared to the serum free media.The expression of vascular endothelial growth factor(VEGF) in BxPC-3 cells treated with Erlo- tinib at different concentrations(5,50,100,200?mol/L) was determined by RT-PCR.②The xeno grafts derived from BxPC 3 cancer cells were inoculated into the BALB/C nude mice.The mice were treated with either Erlotinib(100 mg/kg of Erlotinib oral lavage daily) or saline for four weeks.The vol- ume of the xenografts was measured and the tumor growth rate was calculated.The microvessel density (MVD) of tumor tissue was determined by immunohistochemistry with an antibody against factorⅧ. Results There were less endothelium cells and close hollow tubular structures in grlotinib treated group compared to the control group in the tube formation assay.The mean weight of xenografts in Erlotinib treated group[(0.397?0.550)g] was significantly lower than that in the control group[(1.570?1.060)g] with a inhibitary rate of 74.5%.The expression of VEGF mRNA in Ertotinib treated groups (=50?mol/L) were decreased comparing to the control group.The VEGF expression in xeno- grafts tumor tissues was also markedly down-regulated.The MVI) was significantly decreased in Erlotinib treated group( 1.86?0.43)than that in the control group (5.98?1.27,P
10.Apparent Diffusion Coefficient Histogram Analysis:Differentiation of Genetic Subtypes of Diffuse Lower-grade Gliomas.

To explore the utility of apparent diffusion coefficient(ADC)histogram analysis for differentiating genetic subtypes of diffuse lower-grade gliomas. A total of 55 patients with WHO grade Ⅱ/Ⅲ diffuse lower-grade gliomas who underwent preoperative routine brain magnetic resonance imaging and diffusion weighted imaging in our center were retrospectively evaluated.Among whom there were 14 patients with isocitrate dehydrogenase(IDH)wild-type gliomas(IDH group),19 patients with IDH-mutant 1p19q intact gliomas(IDH 1p19q group),and 22 patients with IDH-mutant 1p19q co-deleted gliomas(IDH 1p19q group).The whole-lesion ADC values derived from histogram analysis(including ADC,ADC,ADC5%,ADC10%,ADC25%,ADC50%,ADC75%,ADC90%,ADC95%,ADC,mode,range,skewness,kurtosis,standard deviation,inhomogeneity,and entrophy)were measured for each patient.All parameters between the different genetic subtypes were compared by using the Student's test or Mann-Whitney test.Receiver operating curve(ROC)analysis was used to assess the diagnostic performance of ADC histogram in distinguishing the different genetic subtypes. Compared with IDH group,the ADC75%(=0.021),ADC90%(=0.015),ADC95%(=0.014),ADC (=0.035),range(=0.009),standard deviation(=0.001)and inhomogeneity(=0.001)were significantly lower in IDH group;in contrast,the ADC (=0.031)and kurtosis(=0.020)of IDH group were significantly higher than those in IDH group.The ADC(=0.010),ADC5%(=0.016),ADC10%(=0.012),ADC25%(=0.007),ADC50%(=0.005),ADC75%(=0.015),and mode(=0.002)were significantly higher in IDH 1p19q group than in IDH 1p19q group.Inhomogeneity achieved the highest area under ROC(AUC)(0.811)in differentiating IDH gliomas and IDH gliomas,with a cutoff value of 0.229;the sensitivity and specificity were 85.7% and 73.2%.The mode achieved the highest AUC(0.744)in differentiating IDH 1p19q gliomas and IDH 1p19q gliomas,with a cutoff value was 1448.75×10 mm /s;the sensitivity and specificity were 57.9% and 90.9%. ADC histograms analysis may be helpful to differentiate genetic subtypes in lower-grade gliomas.
