1.Bacteriological change in buccal mucosa during long-term voyage

Objective To study the bacteriological change in buccal mucosa of sailors during long-term voyage and provide basis for the prevention and treatment of buccal diseases .Methods Bacterial cultivation ,isolation of 164 buccal mucosa specimens were used and analyzed by both handwork and matrix-assisted laser desorption/ionization time of flight mass spectrometry (MALDI-TOF-MS) identification .Results 18 species and 64 strains of pathogenic bacteria were isolated from the whole samples .The num-ber of pathogenic bacteria increased while the number of dwelling bacteria decreased significantly later in the voyage .Comparing with the medical group ,the number of pathogenic bacteria detected in mechanical and electrical group was significantly high (P<0 .05) ,during and after the long-term voyage .As for dwelling bacteria ,no significant difference was observed in between the two groups(P>0 .05) .Conclusion Sailors to fatigue during long-term voyage indicates by decline of immunological function and buccal dysbacteriosis ,which easily lead to buccal and other system diseases .
2.Effect of arthrolysis on scapulohumeral periarthritis

Objective To investigate the value of arthrolysis in treatment of scapulohumeral periarthritis.Methods 83 cases of scapulohumeral periarthritis were divided into arthrolysis group(45 cases), which were treated with arthrolysis combined with electrotherapy, and control group (38 cases),which were treated with electrotherapy only.Results The effect of arthrolysis group was better than that of control group (P<0.05).Conclusion Arthrolysis can improve the effect on scapulohumeral periarthritis.
3.Clinicopathological features and treatment of bilateral asynchronous breast cancer: report of 13 cases

Objective To analyze the clinicaopathological features and treatment of bilateral asynchronous breast cancer.Methods The clinical and pathological data of 13 patients with bilateral asynchronous breast cancer admitted in our hospital from 2010 to 2016 ere retrospectively reviewed.Results The bilateral asynchronous breast cancer accounted for 1.7% of all breast cancer patients admitted n the same period (13/782).The median age of patients at first onset was 47.The modified radical operation was the main surgical lity (12/13 for the first cancer;8/13 for the second cancer).The clinical stage Ⅱ was predominant in all patients (10/13 of the first r;9/13 of the second cancer).Invasive ductal cancer was the most common pathologic type (in 11/13 of the first cancer;11/13 of the cond cancer).Hormone receptors were highly expressed in most of the cases (9/13 of the first cancer;7/13 of the second cancer).The disease-free interval was more than 10 years in most cases (8/13).Among 13 cases local recurrence was detected in 1 case, distant metastases in 2 cases, and 1 case died from distant metastasis.Conclusion The bilateral asynchronous breast cancer has a low incidence;however, a close follow up is necessary for early detection and treatment of the second carcinoma.
4.Combination of epristeride and terazosin for treatment of benign prostatic hyperplasia

Objective To evaluate the security and efficacy of combination of epristeride and terazosin for treatment of benign prostatic hyperplasia (BPH). Method One hundred and eighty-four patients with BPH were treated by epristeride for 6 months and terazosin for 1 month, the efficacy and complication were observed. Results After 6 months treated, compared with before treated, the common symptoms improved, the residual urine decreased 20.74ml, maximum flow rate increased 3.76 ml/s, prostate volume grown downwards 6.70cm3 and the quality of life raised apparently(Pall<0.05 ). Condusion Combinationof epristeride and terazosin for treatment of BPH is safe and effective.
5.Interferin-γ and interleukin-10 levels in expressed prostatic secretion of chronic prostatitis/chronic pelvic pain syndrome

Objective To investigate the levels of pro-inflammatory cytokine interferon (IFN)-γand anti-inflammatory cytokine interlenkin (IL)-10 in expressed prostatic secretions (EPS) of chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) and their clinical significance. Methods The levels of IFN-γ and IL-10 in EPS of 20 patients with CP (type Ⅲ A, group A ), 20 patients with CPPS (type Ⅲ B,group B) and 10 healthy men (control group) were measured using enzyme-linked immunosorbent assay (ELISA). The results were analyzed comparatively with NIH-chronic prostatitis symptom index (NIH-CPSI).Results The levels of IFN-γ in EPS of group A and group B were ( 14.92 ± 7.85 ), ( 13.74 ± 5.96) ng/L respectively, which were higher than that in control group [(7.47 ± 1.40) ng/L] (P < 0.05 ). And the levels of IL-10 in EPS of group A and group B were (216.40 ± 33.65 ), (223.70 ± 45.21 ) ng/L respectively, which were higher than that in control group [( 162.10 ± 43.81 ) ng/L] ( P < 0.05 ). There was no statistically significant difference between group A and group B. No correlation between NIH-CPSI and IFN- γ,IL-10 levels in EPS(P > 0.05 ). Conclusion IFN-γ and IL-10 have a very important role in etiology of CP/CPPS and they can be the objective parameters in the diagnosis of CP/CPPS.
6.Double Endobutto reconstituting coracoclavicular ligament combined with repairing acromioclavicular ligament at stage I for the treatment of acromioclavicular dislocation with Rockwood type III - V.

OBJECTIVETo explore clinical efficacy of double Endobutto reconstituting coracoclavicular ligament combined with repairing acromioclavicular ligament in stage I in treating acromioclavicular dislocation with Rockwood type III - V .
METHODSFrom January 2010 to September 2013, 56 patients with Rockwood type III - V acromioclavicular dislocation were treated by operation, including 20 males and 36 femlaes, aged from 32 to 52 years old with an average of 38.5 years old. Twenty-five patients were on the left side and 31 cases on the right side. The time from injury to operation was from 3 to 14 days, averaged 7 days. All patients were diagnosed as acromioclavicular dislocation with Rockwood type III - V, and double Endobutto were used to reconstituting coracoclavicular ligament, line metal anchors were applied for repairing acromioclavicular ligament. Postoperative complications were observed, Karlsson and Constant-Murley evaluation standard were used to evaluate clinical effects.
RESULTSAll patients were followed up from 8 to 24 months with average of 11 months. According to Karlsson evaluation standard at 6 months after operation,42 cases were grade A, 13 were grade B and 1 was grade C. Constant-Murley score were improved from (42.80±5.43) before operation to (91.75±4.27) at 6 months after operation. All items at 6 months after operation were better than that of preoperative items. Forty-eight patients got excellent results, 7 were moderate and only 1 with bad result. No shoulder joint adhesion, screw loosening or breakage were occurred during following up.
CONCLUSIONDouble Endobutto reconstituting coracoclavicular ligament combined with repairing acromioclavicular ligament in stage I for the treatment of acromioclavicular dislocation with Rockwood type III - V could obtain early staisfied clinical effects, and benefit for early recovery of shoulder joint function.

7.Study on Molecular Characterization of Microbial Communities in Tibetan kefir Grain

The diversity in Tibetan kefir grains and dynamics of the microbial community during the fermentation of Tibetan kefir by PCR-DGGE fingerprinting technique were studied.The results showed that bacterial community of Tibetan kefir grains was more complex than that of yeast.The bacteria communities between different Tibetan kefir grains showed 78%~84% similarity, and yeast 80%~92%.Bands B, E and N of DGGE profiles of the microbial community during fermentation of Tibetan kefir were present throughout the fermentation.Analysis of sequence date showed the majority of the DGGE bands of bacteria community corresponded to LAB, and the most intense band (band E) was completely homology to Lactococcus lactis.
8.The Correlation between Blood Uric Acid and Goldstein Grading in Hepatolenticular Degeneration Patients with Different Chinese Medical Syndrome Types.

OBJECTIVETo observe blood uric acid levels and Goldstein grading, as well as their correlation in Wilson's disease (WD) patients with different Chinese medical syndrome types.
METHODSTotally 906 WD patients in line with inclusive criteria were assigned to 6 groups, i.e., the heart spirit confused by phlegm group (HSCP, 26 cases), the phlegm-fire disturbing heart group (PFDH, 90 cases), the retention of damp-heat group (RDH, 113 cases), deficiency of qi and blood group (DQB, 168 cases), the deficiency of Gan-yin and Shen-yin group (DGYSY, 327 cases), the deficiency of Gan and Shen group (DGS, 182 cases) due to different Chinese medical syndrome types. Recruited were another 160 healthy subjects having similar ages and diet structures, who came for medical examinations, as the healthy control group. Venous blood was collected from the medial cubital vein of each-patient on an empty stomach in early mornings to detect blood uric acid levels. Results Blood uric acid levels were lower in each syndrome type group than in the healthy control group (146.08 +/- 67.24 micromol/L in the HSCP group; 157.08 +/- 69.77 micromol/L in the PFDH group; 162.58 +/- 97.72 micromol/L in the RDH group; 156.20 +/- 62.63 micromol/L in the DQB group; 161.83 +/- 111.23 micromol/L in the DGYSY group; 194.41 +/- 90.01 micromol/L in the DGS group; 242.39 +/- 87.55 micromol/L in the healthy control group, P < 0.01). Blood uric acid levels were higher in the DGYSY group than in the other 5 syndrome groups (P < 0.01). Correlation analyses between Goldstein grading and blood uric acid showed that, along with increased Goldstein grade (that was aggravating disease conditions), WD patients' blood uric acid levels decreased (P < 0.01).
CONCLUSIONSWD patient's blood uric acid levels decreased more. Blood uric acid levels and Goldstein grading were different in various Chinese medical syndrome types. Blood uric acid levels had certain value in assessing the severity of WD.

9.Influence of smoking on carotid intima-media thickness in patients with hypertension

Objective To evaluate the influence of smoking on carotid intima-media thickness (IMT) in patients with hypertension.Methods Two hundred and three patients with hypertension were consecutively enrolled with smoking or without smoking.They were divided into two groups:smoking group (84 patients) and no smoking group (119 patients).Carotid IMT was measured.Results Carotid in smoking group was (0.1118 ± 0.0234) cm,which was thickened compared with that in no smoking group [(0.098 ± 0.0249) cm] (P < 0.01).Conclusions Smoking has significant influence on carotid IMT in patients with hypertension.
10.The current situation of traditional risk factors in acute myocardial infarction patients with different age

Objective To analyze the current situation of traditional risk factors in acute myocardial infarction (AMI) patients with different age,and to explore the impact of different risk factors on AMI.Methods The personal clinical characteristics of 682 hospitalized AMI patients in CCU were recorded from October 2003 to October 2010 with a retrospective cohort study.The effect of smoking,hyperlipidemia,diabetes and hypertension on AMI was observed.Results Six hundred and thirteen patients were enrolled in the end,43 cases in young group,216 cases in middle age group,234 cases in older age group,120cases in old-old age group.(1) Compared with that in middle age group,older age group and old-old age group,the ratio of smoking and hyperlipidemia in young group was significantly higher(P < 0.0125 or < 0.001).The ratio of smoking in middle age group was significantly higher than that in older age group(P < 0.001),and the ratio of hyperlipidemia between two groups had no significant difference (P > 0.0125).The ratio of smoking and hyperlipidemia was significantly higher in middle age group compared with that in old-old age group(P<0.001 or < 0.0125).(2) The average age in smoking patients with AMI was (56.84 ± 13.00) years,which was significantly younger than that in non-smoking patients[(74.29 ± 10.57) years] (P < 0.01).The average age in patients with hyperlipidemia suffering from AMI was significantly younger than that in patients with normal lipid level patients [(62.92 ± 15.04) years vs.(68.63 ± 13.93) years] (P < 0.01).Conclusions Smoking and hyperlipidemia,especially smoking plays an important role in inducing AMI in young and middle age.Smoking cessation and lipid-lowering therapy is vital for reducing the occurrence of AMI in young and middle age.
