1.Study on the effect of adiponectin on enzymatic activity of glucokinase and phosphoenolpyruvate carboxykinase in cultured rat's liver cells

Objective:To investigate the effect of adiponectin on enzymatic activity of glucokinase and phosphoenolpyruvate carboxykinase in cultured rat's liver cells.Methods:The BRL was used as cell model.The rat liver cells were incubated with adiponectin of three different concentrations (10?g/ml,30?g/ml and 50?g/ml).Controlled cells were incubated without adiponectin.Enzymatic activity of GK was investigated by using a standard G6P-coupled assay.Enzymatical activity of PEPCK was assessed by using a standard LDH-coupled assay.Results:Compared with the control group,treated group with adiponectin of all concentrations for 24h had no significant effect on enzymatic activity of glucokinase.Enzymatic activity of phosphoenolpyruvate carboxykinase was significantly decreased even in the lowest concentration of adiponectin,and along with increased in adiponectin concentration enzymatic activity of phosphoenolpyruvate carboxykinase decreased more significantly.Conclusion:Adiponectin has no significant effect on ezymatic activity of glucokinase.However adiponectin decreases enzymatic activity of phosphoenolpyruvate carboxykinase significantly.Through this effect,adiponectin decreases liver glucose production and improves insulin resistance.
2.Anesthetic management of patients undergoing off-pump coronary artery bypass grafting

Objective To summarize anesthetic management for 56 patients undergoing off-pump coronary artery bypass grafting (OPCABG). Methods Recorded and analyzed the hemodynamie changes of 56 patients undergoing OPCABG including heart rate (HR),rate-pressure product (RPP), mean arterial pressure(MAP), mean pulmonary artery pressure(MPAP), central venous pressure (CVP), cardiac output (CO) and mixed venous oxygen saturation when left anterior descending (LAD), right coronary artery (RCA), diagonal (DiAG) and left circumflex coronary artery (LCX) was anastmosed. Results The patients undergoing OPCABG had hemodynamic fluctuation during vascular anastomosis, mainly showing MAP and CO decreased, CVP increased. The change was especially significant when the LCX or RCX was anastmosed. The based value, when LAD and RCA anastmosed of MAP were (82.0±11.7) mm Hg(1 mm Hg = 0.133 kPa), (60.1±11.5) mm Hg and (58.4±12.1) mm Hg, CO were (4.2±1.2) L/min, (3.1±0.7) L/min and (3.2±0.6) L/min, CVP were (5.6±2.6) cm H2O (1 cm H2O=0.098 kPa), (9.6±4.4) cm H2O and (9.2±3.6) cm H2O. Conclusion The hemodynamie changes during OPCABG,especially in LCX and RCA anastmesed are transient and well tolerated under suitable anesthetic management.

SO2 is one of the main pollutants in the air. Tradational disposal methods are limited in the industrial application owing to their high cost. Some foreign scholars began to research in microbial treatment of SO2 both in laboratory test and in full-scale test in recent years and good results were achieved. This method can significantly lowering costs and acquire sulfur product and microbial biomass protein of high quality. The microbial method which is simple, effect and of low costs can satisfy needs of industry and give attention to environmental, economical and social benefits, therefore, it will have amplitude and well application prospects.
4.The risk factors study of type A aortic dissection and aneurysm after aortic valve replacement

Objective This retrospective study was perform to assess risk factors of the type A aortic dissection after aortic valve replacement,in order to provide a basis for devising a strategy for future treatment.Methods From 2009 to 2012,23 patients were found type A aortic dissection and 20 patients were found aortic aneurysm after aortic valve replacement in Beijing Anzhen hospital.Analysis the clinical,pathology and surgery procedure characteristics of these patients,and assess the risk factors for type A aortic dissection after aortic valve replacement.Results After aortic valve replacement,There were significant different in age (P =0.012),bicuspid aortic valves (P =0.014),aortic valve regurgitation (P =0.009) and postoperative pathoanatomical characteristics(P =0.001) between type A aortic dissection group and ascending aortic aneurysm group.Multivariate logistic regression analysis indicated that age(P =0.032),aortic valve regurgitation(P =0.046) and pathoanatomical characteristics(P =0.002) were important risk factor for type A aortic dissection after aortic valve replacement,respectively.Conclusion Older age,aortic regurgitation combined and thinned or fragile aortic walls with systemic hypertension in patients with ascending aortic dilatation (≥45 mm diameter) at the time of AVR might be predisposing factors for postsurgical type A aortic dissection.These patients should be considered for concomitant replacement or repair of the ascending aorta unless the patient has a high operative risk.
5.The relationship of gastric acid with intragastric bacterium and pulmonary infection in rats at different ages

Objective To investigate the relationship of gastric pH value with intragastric bacterium and pulmonary infection in rats at different ages.Methods Thirty Wister rats were divided into three groups: young control group, senile group garaged with saline and young omeprazole group garaged with omeprazole (30mg/kg/d). The gastric pH value was measured after two weeks, and bacterial culture of gastric and lung tissues were done and the pathological sections of lung tissues were made.Results The gastric pH (2. 450±1. 344), number of intragastric bacterium (5. 579±4. 316) cfu/g and number of pulmonary bacterium (1. 617±3. 509) cfu/g in senile group had no differences compared with young control group [(2. 010±0. 507), (1. 505±3. 259)cfu/ g, (0. 475±1. 503)cfu/g, respectively, all P>0. 05]. The differences in gastric pH value, number of intragastric bacterium and number of pulmonary bacterium had statistical significances between young omeprazole group and young control group [(5. 560±1. 007)vs. (2. 010±0. 507), (9. 942±1. 663) cfu/g vs. (1. 505±3. 259)cfu/g, (6. 272±3. 830) cfu/g vs. (0. 475±1. 503)cfu/g, all P<0. 01]. The lung tissues were slightly infiltrated with lymphocytes both in young control group and senile group, and 30% of rats in young omeprazole group had pulmonary infection.Conclusions The number of intragastric bacterium and pulmonary bacterium is increased with the increasing of the gastric pH value, and the pulmonary infection becomes heavier in these rats. But the number of intragastric bacterium and pulmonary bacterium and the pulmonary inflammation have no changes with the increasing of age.
6.Changes of tissue structure of gastric mucosa with aging in rats

Objective To observe the changes of tissue structure of gastric mucosa with aging in rats. Methods Twelve healthy Wistar rats were divided into two groups: aged (12 months,n=6) and non-aged rats (3 months,n=6). The thickness of gastric gland and muscular layer of mucosa were observed under the light microscopy. Pepsin activity and the expression of proliferating cell nuclear antigen (PCNA) were detected by biochemical method and immunohistochemical method respectively. Results Intestinal metaplasia, dysplasia and adenoma were not found in gastric mucosa in two groups. In aged versus non-aged rat group, the thickness of gastric gland was decreased [(271.5±47.4) μm vs. (350.9±53.3) μm, t=2.731, P<0.05], while the thickness of muscular layer of mucosa was increased [(98.1±13.1) μm vs. (62.4±10.6)μm, t=5.174, P<0.01]. The content of pepsin in aged rats were lower than in non-aged rats[(3. 011±1. 222) U vs. (4.733±1.304) U, t= 2.361, P<0.05]. Immunohistochemistry results showed that the positive cells of PCNA were located in gastric stem cells in cervical part, and the thickness of the gastric stem cells was decreased in aged rats. Conclusions The incidences of gastric gland atrophy and muscular layer hypertrophy are increased, and the secretion of pepsinogen and the proliferation ability of gastric stem cells are decreased in aged rats.
7.Association of the serum adiponectin concentration with coronary heart disease in Chinese Han population: a meta -analysis

The association of serum adiponectin concentration with coronary heart disease (CHD) in Chinese Han population was evaluated. Present evidence demonstrated that the lowered serum adiponectin concentration was a susceptibility risk factor for CHD, while the precise relationship between serum adiponectin concentration and CHD in Chinese population requires further study.
8.Complications and recurrence following sphincter preserving procedure for rectal cancer: Analysis on causes and prevention thereof

Objective To discuss causes and prevention of complications and recurrence following sphincter preserving procedure(SPP) for rectal cancer.Methods A retrospective study on clinical and follow-up data of 375 cases of rectal cancer treated by SPP from January 2000 to January 2006 was carried out.Factors relating complications and recurrence after the operation were analyzed with the Logistic regression.Results A follow-up was conducted for 1~60 months(mean,37 months).The 5-year survival rate was 75.77%,and 79 cases of those followed died.Postoperative complications included anastomotic leakage in 14 cases(3.73%),anastomotic stenosis in 37 cases(9.87%),and anastomotic bleeding in 13 cases(3.47%).Anastomotic tumor recurrence was seen in 25 cases.Logistic regression analysis showed that the distance between the tumor and the dentate line was the most important factor for postoperative complications(P=0.017),and the distance and patient's age were the most important factors for recurrence(P=0.040,P=0.041). Conclusions The distance of tumor and patient's age are the most important factors for complications and recurrence following sphincter preserving procedure for rectal cancer.Strict selection of patients for surgery,normative procedure performance,and proper peri-operative management are essential for the prevention of complications and recurrence.
9.Analysis of the scores of 456 doctors from 72 township health centers in a written knowledge test

doctors from 72 township health centers in 3 counties of Jiangxi Province were given a written test in basic medical knowledge, clinical knowledge of prevention and healthcare and professional knowledge. The rate of excellence of the scores was 45.4% while the rate of failure was 16.9%. A comparative analysis is made of the differences in the scores of doctors of different sexes, ages, specialties, educational backgrounds, and professional titles. It is suggested that it is imperative to conduct training for all doctors so as to enhance the overall quality, to lay stress on the key points and focus on the tackling of the weak links, and to perfect the mechanism so as to promote the renewal of knowledge.
10.Protective effects of propofol on isolated rat heart against ischemia-reperfusion injury

Objective To assess the effects of propofol on left and right ventricular function of isolated rat heart against ischemia reperfusion injury Methods Sixteen male SD rats were randomly divided into control and experiment groups The isolated rat hearts was connected to Langendorff preparation and perfused as in our previous experiment After being perfused for 25 min, the isolated heart was subjected to 30 min no flow global ischemia followed by 40 min reperfusion The temperature of the isolated heart was maintained at 36℃ 37℃ during global ischemia In experiment group the isolated heart was perfused with propofol 6?g/ml in perfusate for 10 min before global ischemia The heart rate was paced at 348 beats/min The isovolumetric force velocity indexes and coronary flow were monitored continuously with MacLab instruments Results As compared with the isolated hearts in control group, propofol (6?g/ml) perfusion before ischemia significantly improved left and right ventricular diastolic function by decreasing ventricular end diastolic pressure, dp/dt min and T value At the same time, propofol protected left and right ventricular systolic function by elevating developed pressure, dP/dtmax and Vpm during reperfusion At the end of reperfusion, ventricular tissue superoxide dismutase (SOD) activity was significantly well preserved in the hearts pretreated with propofol Conclusions Propofol 6?g/ml perfusion before ischemia protects the isolated rat heart against ischemia reperfusion injury by improving diastolic and contracting function of right and left ventricles and intrinsic contractivity of myocardium Propofol increases coronary blood flow during reperfusion and increases SOD activity of myocardial tissue
