1.Locking proximal humerus plate for three- or four-part humeral proximal fractures

Objective To discuss the treatment effect of locking-proximal-humerus-plate (LPHP) for three- or four-part proximal humeral fractures. Methods A retrospective study was done on 13 cases of proximal humeral fractures treated with LPHP from July 2003 to January 2005. There were six males and seven females, with age of 24-73 years (mean 49.8 years). According to Neer classification, five cases were with three-part fractures and eight with four-part fractures including six cases with osteoporosis. Reduction and fixation was done via deltopectoral-groove approach, without cutting the capsule or peeling the soft tissues of the greater and lesser tuberosity. Of all, three cases with bone defects was treated with allografts. Shoulder exercise could be performed passively three days after operation. One week after operation, passive and active shoulder exercise was done, which was gradually intensified two weeks after operation. Results A follow up for 3-20 months (average 11 months) on 12 cases showed that all fractures were healed. According the Neer shoulder score, the results was excellent in six cases, good in four and fair in two, with excellence rate of 83.3%. Conclusions The screw of LPHP can lock the plate so that the plate can firmly fixate the fractures without tightly contacting the bone, as simultaneously reaches fracture healing and function exercise and enhances the treatment effect. LPHP is a specially suitable procedure for osteoporosis and three-or four-part proximal humeral fractures.

AIM To investigate the anti-inflammatory properties of IL-6 on the development of IDDM(insulin dependent diabetes mellitus) in NOD mouse. METHODS IL-6 was administered exogenously in female NOD mice (spontaneous model) and cyclophosphamide-treated NOD mice (accelerated model) respectively. During the study period, the mice were evaluated for development of diabetes once a week. At the end of experiment, 8 euglycosuria mice from each group were sacrificed for histoimmunological analysis, and the production of TNF-? and IFN-? from spleen lymphoid cells were detected. 5.Th1 cellular immune response elicited by recombinant adenovirus transduced dendritic cells for HCV-NS3 in vivo

Objective:The aim of this study was to explore the vaccine potential of adenovirus transduced DC encoding for HCV nonstructural protein NS3 in a murine model.Methods:Dendritic cells were isolated from bone marrow of BALB/c mice and cultivated with cytokine cocktail for 7 days. Recombinant adenovirus was added to DC cultures at day 7,at a titration of 2?10~9 pfu/10~6 DC/well. FACS and Western blot techniques were used to identify the DC mature state and the protein expression, respectively. BALB/c mice were primed/boosted immunized with DC based vaccines at 3?10~5 cells for one mouse at a 10 day interval. 10 days after the second immunization, cytokines secretion and IFN-? generating T cells response were evaluated by ELISA and ELLISPOT analysis.Results:Our current data indicate that AdNS3 can stimulate the maturation of DC and efficiently express authentic HCV nonstructural proteins in infected cells. 6.Intradural metastasis of extramedullary space of medulloblastomas:MRI

Objective:To evaluate MRI manifesattions of intradural metastases of medulloblastomas.Methods:The clinical data and MRI of 6 cases with intradural metastasis of medulloblastomas were reviewed retrospectively.Results:21 masses were found in 6 cases,mostly in lumbar segment.Most of them were oblong or fusiform shagped.The size averaged 1.8 cm?2.3 cm.They had the similar signal intensity with those in brain,but hemorrhage,cyst and calcification were less common.Conclusion:MRI plays an importment role in diagnosis of intradural metastases of medulloblastomas. 7.The value of serum pro-adrenomedullin for assessing prognosis of patients with closed rupture of small intestine

Objective To explore the relationship between preoperative serum pro-adrenomedullin (pro-ADM) in the patients with closed rupture of small intestine,and postoperative secondary abdominal infection,and to study the value of postoperative serum pro-ADM in assessment of severity of postoperative secondary abdominal infection of this patients.Methods Eighty-five patients with closed rupture of small intestine treated in the emergency surgery from June 2014 to May 2015 were selected.According to the presence of postoperative abdominal infection or not,these patients were divided into infection group and non-infection group.The infection group was further divided into SIRS,sepsis,severe sepsis,septic shock subgroups as per the severity of infection.The levels of serum pro-ADM,interleukin-6 (IL-6),C-reactive protein (CRP) were determined and the acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ) score was calculated at admission and the first day、the second day and the third day after operation.Comparisons of these biomarkers were carried out to find out the factors associated with postoperative abdominal infection.In addition,ROC curve was used to verify the factors for predicting the abdominal infection of these patients after operation.The relationship between serum levels of pro-ADM and APACHE Ⅱ score after operation was analyzed.The severity of abdominal infection after operation was assessed with laboratory findings.Results Compared with non-infection group,preoperative and postoperative serum pro-ADM (P =0.03,P < 0.01),IL-6 (P =0.02,P <0.01) levels and APACHE Ⅱ scores (P < 0.01,P < 0.01) were significantly higher in infection group (P < 0.05).In the infection groups,the postoperative levels of serum pro-ADM and APACHE Ⅱ scores were increased with the severity of infection increased (r =0.924),and the difference between the groups was statistically significant (P < 0.05),but there were no significant differences in IL-6 and CRP levels between two groups.Conclusion (1) The serum levels of pro-ADM before operation has value in predicting the genesis of abdominal infection in these patients after operation.(2) The serum levels of pro-ADM after operation has value in severity assessment of abdominal infection in these patients after operation.
9.Effects of KLF2 on ox-LDL induced expression of microRNA-146a and proinflammatory cytokines in human umbilical vein endothelial cells

Objective To investigate the effects of Kr üppel-like factor 2 ( KLF2 ) on oxidized low density lipoprotein (ox-LDL) induced expression of microRNA-146a (miR-146a) and proinflammatory cyto-kines (MCP-1 and IL-6) in human umbilical vein endothelial cells (HUVECs).Methods Human umbili-cal vein endothelial cells (HUVECs) were cultured and then stimulated with 50μg/ml ox-LDL for 24 hours. HUVECs were infected with adenovirus vectors over-expressing human KLF2 at an appropriate multiplicity of infection.KLF2-siRNA duplexes were transfected into HUVECs to silence the gene expression .HUVECs were collected at time points of 0 h, 3 h, 6 h, 12 h, 24 h and 48 h after infection.Real-time quantitative-PCR was performed to measure the expression of miR-146a at mRNA level.Silenced endogenous miR-146a using LNA-anti-miR-146 a was transfected into HUVECs with lipofectamine 2000 .The levels of MCP-1 and IL-6 in supernatants were detected by ELISA .Results KLF2 remarkably inhibited the expression of miR-146 a in unstimulated and ox-LDL-stimulated HUVECs in a time-dependent manner .The ox-LDL induced ex-pression of miR-146a, MCP-1 and IL-6 in HUVECs were significantly decreased by KLF2.Silenced expres-sion of miR-146a downr-egulated the ox -LDL induced expression of MCP-1 and IL-6 in HUVECs.Moreover, silenced miR-146 a could partly reverse the inhibitory effects of KLF 2 on ox-LDL induced expression of MCP-1 and IL-6 in HUVECs.Conclusion KLF2 inhibited ox-LDL induced expression of MCP-1 and IL-6 in HUVECs partly through down-regulating the expression of miR-146a.
