1.A Research Of Web-Based Real Time Automatic Fetal Monitoring System.

In this paper, using ultrasonic doppler signal, we composed fetal heart sound / fetal movement diagnosis and web-based database server/client environment. Detecting fetal heart rate and movement at the same time, for the data loss-caused from telemetering-protection and the high speed diagnosis, we made two signals mix and transmit. For the better communication between obstetrician and remote woman/fetus, we proposed database table that could reflect the whole information about remote fetus and its mom. And we supported HL-7 format so that we might be compatible with other vendor product and easy to access web-based hospital computerization system. Applying doctor to web-based high level biosignal analysis algorithm and expert system, we offered remote fetal biosignal and diagnostic assistant data inducing fast diagnosis. We made web-based fetal diagnostic system and improved web system compatibility, fast diagnosis and minimum of the rate of misdiagnosis.

2.Metabolic Challenges in Anticancer CD8 T Cell Functions

Cancer immunotherapies continue to face numerous obstacles in the successful treatment of solid malignancies. While immunotherapy has emerged as an extremely effective treatment option for hematologic malignancies, it is largely ineffective against solid tumors due in part to metabolic challenges present in the tumor microenvironment (TME). Tumorinfiltrating CD8 + T cells face fierce competition with cancer cells for limited nutrients. The strong metabolic suppression in the TME often leads to impaired T-cell recruitment to the tumor site and hyporesponsive effector functions via T-cell exhaustion. Growing evidence suggests that mitochondria play a key role in CD8 + T-cell activation, migration, effector functions, and persistence in tumors. Therefore, targeting the mitochondrial metabolism of adoptively transferred T cells has the potential to greatly improve the effectiveness of cancer immunotherapies in treating solid malignancies.

Sarcomatoid carcinomas of the lung are rare malignant biphasic tumors, which contain both a malignant epithelial component and a sarcomatoid component. The majority of patients are men and the mean age of onset is 60 years at the time of diagnosis. A metastasis to the regional lymph nodes and to distant organs is common. The clinical course of patients with this neoplasm is aggressive, with an overall 5-year survival rate approximating 20%. A sarcomatoid carcinoma of the lung is often observed in the large bronchi and peripheral lung field than in the trachea, and the clinical manifestations are related to their specific location. We report a case of sarcomatoid carcinoma of the lung in a 79-year-old man who presented with dyspnea on exertion.

Endotracheal intubation is the gold standard for airway management in general anesthesia. However, airway patency is not guaranteed by keeping the endotracheal tube (ETT) in place. Sometimes, the ETT itself may become a cause of airway obstruction; there are some reports on airway obstruction related to reinforced tube malfunction. We report a rare case with an obstruction of reinforced endotracheal tubes caused by dissection of the internal wall. Recognition of the possibility of airway obstruction due to a rare cause and monitoring patients vigilantly during anesthesia is very important for patient safety.

8.Part II. What drives Korean adults to seek orthodontic treatment: Factors contributing to orthodontic treatment decisions

Objective:
This study aimed to identify the perceptions of orthodontic treatment among Korean adults and determine the factors that drive them to seek orthodontic treatment.
Methods:
A total of 2,321 adults aged 19–64 years were surveyed using an internet research system from a specialized research company. The participants were divided into the following groups based on their experience of and willingness to undergo orthodontic treatment: experience, acceptance, and non-acceptance groups. The characteristics of the participants were compared using analysis of variance with post-hoc analysis.Multinomial logistic regression analysis was performed in all three models with the non-acceptance group as a reference.
Results:
In terms of demographic characteristics, age, gender, marital status, and education had significant influences on orthodontic treatment decisions in adults in the experience and acceptance groups (p < 0.001). When all the factors were analyzed, age, marital status, past dental treatment experience, regular oral examinations, demand for orthodontic treatment, optimal treatment period, health insurance coverage, information on orthodontic treatments, perceptions regarding orthodontic treatment, and psychosocial impact of dental esthetics significantly influenced orthodontic treatment decisions in adults in the experience and acceptance groups (p < 0.001).
Conclusions
These findings suggest that various factors influence orthodontic treatment decisions in adults. Individuals who seek orthodontic treatment were found to undergo more regular dental treatment and oral examination than those who did not. They also had a better perception of orthodontic treatment and more negative values for the psychosocial impact of dental esthetics.
9.A case of acute kidney injury caused by dapsone-induced methemoglobinemia in a patient with chronic kidney disease

Methemoglobinemia (MetHb), which is characterized by an increased methemoglobin level, is a rare but potentially fatal disorder caused by congenital enzyme deficiencies or exposure to oxidizing agents, including dapsone. Elevation in the methemoglobin level impairs the oxygen-carrying capacity of hemoglobin, produces functional anemia, and induces tissue hypoxia. Such hypoxia results in microcirculation injury and hypoperfusion in the tissue and organs, including the kidney, and is a risk factor for acute kidney injury (AKI). This paper reports a case of AKI caused by dapsone-induced MetHb in a patient with chronic kidney disease, in which the patient ingested approximately 1,500 mg of dapsone in a suicide attempt, which was treated with aggressive management, including methylene blue, ascorbic acid, and transfusion.
10.Polyploidization of Hepatocytes: Insights into the Pathogenesis of Liver Diseases

Polyploidization is a process by which cells are induced to possess more than two sets of chromosomes. Although polyploidization is not frequent in mammals, it is closely associated with development and differentiation of specific tissues and organs. The liver is one of the mammalian organs that displays ploidy dynamics in physiological homeostasis during its development. The ratio of polyploid hepatocytes increases significantly in response to hepatic injury from aging, viral infection, iron overload, surgical resection, or metabolic overload, such as that from non-alcoholic fatty liver diseases (NAFLDs). One of the unique features of NAFLD is the marked heterogeneity of hepatocyte nuclear size, which is strongly associated with an adverse liver-related outcome, such as hepatocellular carcinoma, liver transplantation, and liver-related death. Thus, hepatic polyploidization has been suggested as a potential driver in the progression of NAFLDs that are involved in the control of the multiple pathogenicity of the diseases. However, the importance of polyploidy in diverse pathophysiological contexts remains elusive. Recently, several studies reported successful improvement of symptoms of NAFLDs by reducing pathological polyploidy or by controlling cell cycle progression in animal models, suggesting that better understanding the mechanisms of pathological hepatic polyploidy may provide insights into the treatment of hepatic disorders.
