1.Dietary Habits and Taste Preferences of the Korean Americans According to Self Body Image Perception and Length of Residence in US.

BACKGROUND: Weight overestimation of Korean Americans may be associated with dietary habits and preferences for foods and tastes. METHODS: The subjects were 100 Korean Americans in New York City and LA and 100 Koreans in Seoul, who were 20-29 years old. Self-reported questionnaire consisted of age, height, weight, length of residence in US, preferences for 10 foods and 5 tastes, dietary habits and perception of self body image. Korean Americans were further divided into 3 groups according to length of residence in US. RESULTS: More Korean American women underestimated (27.3% vs. 5.8%), and less overestimated their body weights, than Korean women (31.8% vs. 53.8%, P=0.008). Korean Americans who overestimated their own body weights had significantly higher preference for salty taste and significantly lower dietary habit score regarding regular meals. CONCLUSIONS: Overestimation of self body size, rather than longer residence in US may be associated with the undesirable dietary habit and taste preference of Korean Americans, highlighting the importance of correct body image.

2.Clinical Evaluation of Atracurium for Endotracheal Intubation .

The usefulness of nondepolarizing muscle relaxants for intubation is limited by a relatively slow onset of neuromuscular block compared to that achived with succinylcholine. But, authors have reported that larger doses of nondepolarizing muscle relaxants produce a more rapid onset of maximal neuromuscular block and conditions conductive to endotracheal intubation and recent reports support the use of the "priming principle in the clinical practice of anesthesiology. This phenomenon may apply to all nondepolarizing relaxants: it is reported to facilitated neuromuscular blockade and tracheal intubation when used with atracurium, vecurouium, alcuronium or pancuronium. In this study, administering a small subclinical dose of atracurium (75ug/kg) Smin. prior to the remainder of an intubating doae of atracurium (250ug/kg) had no different on grade of block for intubation compared to single IV bolus of atracurium (500ug/kg).

4.Magnetic resonance angiographic screening of aneurysms in migraine

Objective: The purpose of the present study was to screen the prevalence of aneurysms in migraineurs; to differentiate presenting features in migraineurs with and without aneurysm; and also to correlate the locations of aneurysm to the clinical features of migraine. Methods: A total of 4,416 subjects were interviewed and completed self-reported questionnaires on headache. Of these, 1,773 subjects diagnosed to have migraines based on the International Classification of Headache Disorders II (ICHD-II) criteria were screened for aneurysm by magnetic resonance angiography (MRA). When aneurysm was suspected, further investigation with trans femoral cerebral angiography (TFCA) or three dimensional computerized tomography (CT) angiography was performed. Based upon MRA findings, subjects were grouped into unruptured aneurysm migraine patients (UAMP) and no aneurysm migraine patients (NAMP). Results: The prevalence of aneurysm was 3.6% (63 of 1,773) with the mean age of 56.0 years, which were not different from those of general population. There was no difference in migraine subtypes between UAMP and NAMP. Aggravation of headache by estrogen replacement therapy during menopause (p=.039), history of migraine in young age (p= .021), diplopia (p=.026), and retroauricular pain (p=.025) were significantly associated with presence of aneurysm. Although aneurysms were detected more in anterior circulation, there was no correlation between aneurysm site and headache location. The average size of aneurysm was 3.5 ± 2.1 mm and none were ruptured. Interventional therapy of aneurysm did not alter the feature of migraine. Conclusions: The incidence of aneurysm was not different in migraine patients as compared to the general population. Some features which significantly differentiate whether migrainuers have aneurysm or not warrant further study to have a predictive and localizing value.
5.Factors associated with Smartphone Overdependency in Preschool Children.

PURPOSE: The purpose of this study was to identify factors associated with smartphone overdependency in preschool children based on an analysis of mother, child, and their mutual dyadic relationships. METHODS: Data were collected from 171 mothers of preschool children enrolled at daycare centers and kindergartens from January to March 2018. The data were analyzed by descriptive statistics, the t-test, analysis of variance with Scheffé post hoc test, Pearson correlation coefficient, and hierarchical regression analysis using SPSS version 24. RESULTS: The self regulation of preschool children (β=−.358, p < .001) was the factor most closely associated with smartphone overdependency. The second most closely associated factor was children's use of a smartphone 0.5–1 hour daily (β=−.249, p=.005). Additional associated factors were mothers' use of a smartphone for 3–4 hours daily (β=.217, p=.002), children's use of a smartphone for less than 0.5 hour daily (β=−.212, p=.006), and children's use of a smartphone for 1–2 days per week (β=−.205, p=.026). CONCLUSION: Based on the results of this study, it is necessary to try to develop guidelines and programs to prevent smart phone overdependency in preschool children.

6.Fasciola Hepatica Induced Hepatic Abscess Treated with Triclabendazole

Fascioliasis is a zoonotic disease caused by Fasciola Hepatica that infects mainly cattle, sheep, and goats. Humans can be infected by water or aquatic plants contaminated with metacercariae. The authors encountered two cases of F. hepatica infection. One patient reported abdominal discomfort with marked eosinophilia. The other patient had chest discomfort with marked eosinophilia. The abdominal CT images revealed hypodense lesions in the liver. The ultrasonography-guided liver biopsy findings in both patients were indicative of parasitic infections. Serological tests confirmed the definite diagnoses. Both patients were treated with a single dose of triclabendazole, which is the treatment of choice for fascioliasis. These findings suggest that a diagnosis of fascioliasis, particularly in the acute phase, should be considered in patients with abdominal pain, marked eosinophilia, and hypodense hepatic lesions on CT.
7.Clinical Improvement of Atopic Dermatitis by a Topical Cream Containing a Protease-Activated Receptor-2 (PAR-2) Inhibitor 'Pal-KTTKS Peptide'.

BACKGROUND: Serine protease promotes desquamatation of the stratum corneum and this is controlled by serine protease inhibitors (SPI). After disruption of the skin barrier, signals for barrier recovery are started with the activation of cytokines and a migration of calcium ions. On the other hand, the protease-activated receptor-2 (PAR-2) pathway is initiated as a negative signal. As the pH of the stratum corneum become neutral, activated serine protease and PAR-2 inhibit the secretion of lamellar bodies and the formation of the lamellar structure. OBJECTIVE: We wanted to screen noble synthetic peptides and identify the efficacy of a selected peptide, Palmitic acid-Lysine Threonine Threonine Lysine Serine (Pal-KTTKS), on PAR-2 in vitro and in vivo, and a clinical study was performed. METHODS: in vitro:Changes of the intracellular calcium ion concentration were measured in cultured HaCaT cells by fluorescence imaging according to treatment with sample peptides and trypsin. in vivo animal study:The efficacy of 2% Pal-KTTKS cream as a selected noble peptide was evaluated in an oxazolone-induced atopic dermatitis animal model. Clinical study:A total of twenty three atopic dermatitis patients applied 2.5% Pal-KTTKS peptide-containing cream on the one side of their extremities and pseudo-ceramide containing moisturizer on the other side of the extremities as a control twice a day for 4 weeks. Clinical improvement was evaluated by the Eczema Area Severity Index (EASI) score, a subject questionnaire and comparison of photographs. RESULTS: Suppression of the intracellular calcium concentration via PAR-2 inhibition was noted in the Pal-KTTKS peptide treated cultured HaCaT cells. In the oxazolone-induced atopic dermatitis hairless mice model, 2% Pal-KTTKS peptide containing lotion was more effective than vehicle lotion only. In the atopic dermatitis patients, the sites treated with 2.5% Pal-KTTKS peptide-containing cream showed better improvement for the EASI score, the subject questionnaire and the clinical photographs as compared to that of the control sites. There were no remarkable side effects related to the treatment. CONCLUSION: A PAR-2 inhibitor-containing topical agent would be an effective and safe modality for treating atopic dermatitis.

8.The Association between Psychosocial Work Environment and Depressive Symptoms among Korean Teachers.

PURPOSE: This study aimed to identify the association between psychosocial work environment and depressive symptoms among Korean teachers. METHODS: Data on 235 elementary school teachers and 341 middle and high school teachers was obtained from the 2014 Korean Working Condition Survey. The effect of psychosocial work environment on depressive symptoms was determined by multiple logistic regression analyses. RESULTS: The prevalence of depressive symptoms was 18 % in elementary school teachers and 24 % in middle and high school teachers. When adjusted for demographics and other socio-psychological and environmental factors, ‘social community at work’ served as a protection factor for Korean teachers' depressive symptoms. In addition, ‘cognitive demands’ in elementary school teachers and ‘social support from supervisors’ in middle and high school teachers were found to be predictors of depressive symptoms. CONCLUSION: Interventions to increase ‘social community at work’ and ‘social support from supervisors’, and to decrease ‘cognitive demands’ may be helpful for Korean teachers at risk of depression.

9.Death After Accidental Injection of Tranexamic Acid During Spinal Anesthesia.

Medication errors such as administration of wrong drugs, wrong dosage and erroneous route of administration are not rare among medical misadventures. We present an autopsy case of accidental injection of tranexamic acid instead of bupivacaine during spinal anesthesia, accompanying the quantitative result of the tranexamic acid in the blood, cerebrospinal fluid and each internal organs. We think that warning signs on syringes and ampoules, simple and unified guideline for drug administration, separative documentation of drug administration and interpersonal communication on drug information should be done to prevent this type of medical errors.

Ehlers-Danlos syndrome type IV (EDS IV) is a hereditary disorder of the connective tissue, characterized by easy bruising, thin skin with visible veins, and spontaneous rupture of the large arteries, uterus, or bowel. EDS IV is caused by mutations of the gene for type III procollagen (COL3A1), resulting in insufficient collagen production or a defect in the structure of collagen. EDS IV can have fatal complications such as the rupture of great vessels or organs, which can cause hemorrhaging and sudden unexpected death. Here, we report a case of a 43-year-old female who collapsed after a struggle with a neighbor. In this patient, the bifurcation of the bilateral common iliac artery ruptured, with no evidence of trauma, inflammation, or atherosclerosis. Genetic analysis of COL3A1 showed the presence of a c.2771G>A (p.Gly924Arg) mutation, which may be associated with EDS IV. The forensic pathologist should consider the possibility that the spontaneous visceral or arterial rupture was caused by EDS IV. Genetic analysis is not currently a routine procedure during autopsy. However, in this case, we suggest that the patient possibly had an underlying EDS IV condition, and we recommended family members of the deceased to seek genetic analysis and counseling.
