Objective To observe neutrophil activation and protease imbalance in airway of children after foreign-body aspiration(FBA).Methods Bronchoalveolar lavage fluid(BALF) was obtained through fiberoptic bronchoscopy.The supernatant was assayed for the concentration of neutrophil elastase(NE) using enzyme-linked immunosorbent assay(ELISA) and elastase inhibition capacity(EIC)/free elastase activity using a colorimetric assay.The cell pellet were smeared for the expression of ?1-antitrypsin(?1-AT) using cellular immunohisto-chemistry and cytologic analysis by Wright-Giemsa stain.Results Percentage of neutrophil,concentration of NE and the expression of ?1-AT in BLAF group,pair group and pneumonia group significantly increased compared with that of control group(Pa0.05).The ratio of Lib NE in FBA,par and peumcria group was 30.3%,27.3% and 22.7%.Conclusions FBA induces similar airway neutrophil activation and protease imbalance to bacterial pneumonia.The contralateral airway showes the same change as the foreign body side.

Objective To use gene engineering technique to express the humanin.Methods Humanin segment was synthesized and cloned into the prokaryocyte expression vector pGEMEX-1 and pet44a. The plasmid was transformed to E. coli BL21(DE3) by screening and identifying to obtain the high expressing positive clone.Results The humanin segment was cloned into vector successfully,and expressed efficiently with the inducement of IPTG. The molecular weight of the expressed recombinant protein of pGEMEX-1-humanin was about 28 kd while it was 63 kd in pet44a-humanin,and the expressed quantity of fusion protein was 40 percentage of the total bacterial protein while it was 20 percent in pet44a-humanin.Conclusion The humanin has been cloned and expressed successfully and the high efficiently expressed recombinant protein might contain the intact important structure domain of the humanin.

OBJECTIVETo investigate clinical effects of arthroscopy in the treatment of medial meniscal cyst.

METHODSFrom June 2011 to January 2013, 7 patients with medial meniscal cyst were treated with arthroscopy. There were 3 males and 4 females,ranging in age from 27 to 63 years old,with a mean age of (43.93±2.10) years old. The cysts have been discovered for 3 to 30 months,with a mean time of (10.6±1.3) months. All the patients complained of knee pain,especially in the medial joint gap. The Pisani sign, Caklin sign and medial McMurray sign were all positive. Preoperative MRI examination confirmed the diagnosis. Lysholm score changes and clinical efficacy were observed through a six-month follow-up.

RESULTSThe postoperative Lysholm scores were all significantly higher than the preoperative scores. According to Sarimo standard, 6 patients got an excellent result, and 1 good.

CONCLUSIONArthroscopic treatment of medial meniscal cyst has replaced the traditional method, which could retain the normal meniscus as much as possible and repair the meniscus injury simultaneously, as well as get a good curative effect and a good recovery of knee function. This method is worthy of clinical application.

Objective To evaluate the plasma level of amino-terminal pro-brain natriuretic peptide(NT-proBNP) before and after treatment in children with dilated cardiomyopathy (DCM) and explore the relationship between NTproBNP and heart functional classes.To evaluate the significance of dynamic change in NT-proBNP level in children with dilated cardiomyopathy.Methods The study comprised 49 children with DCM from Aug.2011 to Sep.2013 in Hunan Children's Hospital.NT-proBNP levels were measured before and after treatment.All the patients were evaluated under the modified Ross Heart Association functional class and echocardiographic determination of left ventricular ejection fraction(LVEF) and fractional shortening(SF).The patients were divided into improvement group(38 cases) and the non-improvement group(11 cases) according to cardiac function after treatment.Results The plasma level of NTproBNP was positively correlated with Ross heart failure classification (r =0.756,P ＜ 0.01),and much better than LVEF and SF.To identify mild heart failure,the cut-off value of 329 ng/L was 93.8％ sensitive and 90.9％ specific.To identify moderate heart failure,the cut-off value of 2 833 ng/L was 95.6％ sensitive and 92.9％ specific.To identify severe heart failure,the cut-off value of 12 125 ng/L was 80.6％ sensitive and 84.8％ specific.There was significant difference in the improvement group before and after treatment(t =2.357,P ＜ 0.05).In non-improvement group there was no significant difference (t =0.132,P =0.778) before and after treatment.Conclusions There is significant correlation between the plasma level of NT-proBNP and heart failure severity.The dynamic changes in NT-proBNP levels before and after treatment can help us to determine the effect of treatment and prognosis.

Objective To report specific clinico-radiological syndromes in neuropsychiatric systemic lupus erythematosus (NPSLE).Methods Ten patients with NPSLE in Peking Union Medical College Hospital from 2005 to 2011 were studied retrospectively with magnetic resonance imaging, computer tomography or positron emission tomography. Results Posterior reversible encephalopathy syndrome was diagnosed in 2 patients with radiological features,headache and tonic-clonic seizure;3 patients with bilateral diffuse leukoencephalopathy,cognitive disorder and acute confusional state; 1 Fahr' s disease patient,with cognitive disorder and psychiatric symptom,movement disorder; 2 Parkinsonism patients with tremor and cogwheel rigidity,and 2 chorea patients. Conclusions The emergence of diffuse brain calcinosis,leukoencephalopathy and edema may happen in lupus encephalopathy particularly.Autoantibody reaction and vascular disease may play an important role in movement disorder including Parkinsonism and chorea.

Objective To study the present situation of legionella pneumophila(Lp) infection of low respiratory tract infection in children less than 5 years old.Methods Three hundred hospitalized patients with the diagnosis of low respiratory tract infection were enrolled in the study,191 males,109 females,aged 27 d to 5 years old,course from 1 d to 6 months.Serum antibody(IgM,IgG) of Lp serogroup 1-7 was detected by enzyme-linked immunosorbent assay(ELISA).Results In the patients,17 cases were positive,and the positive rate was 5.67%.Of the 17 children,the males(n=11) and females(n=6) did not significantly influence the positive rate(5.76% vs 5.50%).Four of them were between 27 days and 1 year old;7 were 1 to 3 years old,6 were 3 to 5 years old.The positive rates were significantly different among the 3 groups(2.53% vs 7.37% vs 12.77%) and tendentially increased following the increase of age(?2=7.8164 P
