1.The relationship research between β2-microglobulin and D-Dimer concentration and kidney injury in pregnancy-induced hypertension patients

Objective To explore the relationship between β2-microglobulin and D-Dimer concentration in the pregnancy-induced hypertension patients with kidney injury.Methods The 120 cases of kidney injury pregnancy-induced hypertension patients were enrolled in our hospital from Nov 2010 to Nov 2015 (served as observation group).The levels of BUN,Scr,UA,β2-microglobulin and D-Dimer were detected.The 100 cases of healthy subjects and 120 cases of normal pregnancy women were taken as controls,respectively.Results Compared with healthy control and normal pregnancy women,the levels of BUN,Scr,UA were increased significantly in the observation group (P<0.05).The results also showed that the expression of NGAL,NLRP3,NADPH oxidase subunits,TGFβ,Smad2,Smad3,β2-microglobulin and D-Dimer were sharply increased as compared with the other two group (P<0.05).Conclusion β2-microglobulin and D-Dimer are significantly related with pregnancy-induced hypertension patients with kidney injury and they would be taken as clinical indicators for the diagnosis of this disease.

3.Effect of Various Kind of Dyslipidaemia on Insulin Resistance and Pancreatic Islet Function in Newly Diagnosed Type 2 Diabetic Patients

Objective To evaluate the effect of various kinds of dyslipidaemia on insulin resistance and pancreatic islet ? cell function in newly diagnosed type 2 diabetic patients. Methods 263 patients with newly diagnosed type 2 diabetes were divided into normal lipidemia group and dyslipidaemia group according to ATPⅢ standard. The dyslipidaemia group included 3 subgroups as the following: high TG group, high TC group, and mix group with high TG and TC. Homeostasis model assessment (HOMA) was applied to estimate insulin resistance (IR) and pancreatic islet ? cell function. 4.Effects of Chlamydia and Mycoplasma Infections on the Healing of Wounds after Surgery for Haemorrhoids,Fistula and Anoschisis

Objective To explore the effects of chlamydia and mycoplasma infections on the healing of wounds after surgical operation for haemorrhoids,fistula and anoschisis.Methods Sixty-two patients with haemorrhoids,fistula and anoschisis undergoing the surgical operation during 2000.1 to 2008.10 were collected,and wounds did not heal 40 days after operation and the wound's surface infection occurred after postoperative anti-inflammatory therapy and dressing change.All patients were positive at least for laboratory Ct or Uu,and those with infections caused by other fungi,bacteria,viruses and the other systemic diseases were excluded.The correlation between the infections in 62 cases and sex,surgical types was analyzed.Results In 62 patients,there were 30 cases (48.4%) positive for both Ct and Uu,22 cases for single Ct (35.5%),and 10 cases for single Uu (16.1%),respectively.The infection rate in females was higher than in males (P<0.01).The surgical types included:surgery for 8 cases of haemorrhoids (12.9%),for 18 cases of fistula(29.0%),and for 36 cases of anoschisis(58.1%).respectively.Conclusion There is the possibility of chlamydia and mycoplasma infections in the patients with delayed healing of wounds following the surgical treatment on the anal region.Early diagnosis,early treatment,avoidance of ineffective medicines and repeatedly management of the wounds can shorten the healing period.
5.Effect of positive airway pressure on catheterization of right internal jugular vein in anesthetized patients

Objective To investigate the effect of positive airway pressure on catheterization of right internal jugular vein (IJV) in anesthetized patients. Methods One hundred and twenty-five ASA Ⅰ or Ⅱ patients aged 19-64 yr weighing 44-85 kg undergoing right IJV catheterization were randomly divided into 2 groups: control group (group C, n = 60) and positive airway pressure group (group P, n = 65). Anesthesia was induced with midazolam, sufentanil and etomidate. Tracheal intubation was facilitated with vecuronium. The patients were mechanically ventilated. Right IJV was catheterized under the guidance of ultrasound and was punctured at the plane of cricoid cartilage. IJV catheterization was performed when mechanical ventilation was suspended in group C and when airway pressure was maintained at 20 cm H2O in group P. The cross section area of right IJV, CVP, the shortest dislance between right IJV and the skin and the depth of needle insertion were measured. The number of attempts to puncture the vein was calculated and the easiness of withdrawing blood from the right IJV after being pierced was assessed. MAP and HR were measured before and after positive airway pressure. Results Positive airway pressure (20 cm H2O) significantly increased the cross section area of IJV, CVP, success rates of puncturing IJV at first attempt and within 30 seconds and easiness of withdrawing blood from IJV after being pierced and deceased the depth of needle insertion as compared with control group. Positive airway pressure (20 cm H2O) also significantly increased the incidence of bradycardia and hypotension. Conclusion Positive airway pressure is helpful for right IJV catheterization guided by ultrasound under general anesthesia.
6.Evaluation of efficacy and safety of percutaneous radiofrequency ablation for hepatic cavernous hemangioma

3.0 cm in diameter were given multiple overlapping ablations that encompassed the entire lesions as well as an approximately 0.5 cm rim of normal liver tissue. The operation was performed under local anesthesia in association with systemic analgesia. Results The procedure was successfully accomplished in all the 26 patients. Adverse reactions happened at 2~5 postoperative days, including local pain and discomfort, pyrexia, and elevations of ALT levels. No bile leakage or hemorrhage occurred. Follow-up observations in 23 patients for 1~5 years (mean, 2 years and 8 months) showed a complete response (CR) in 22 patients and a partial response (PR) in 1 patient by one mission of PRFA, the effective rate (CR+PR) being 100%. The hepatic cavernous hemangiomas decreased by a mean of 68.8% (range, 41.2%~81.5%) in diameter. Conclusions Minimally invasive PRFA is a safe and effective therapy for HCHs in properly selected patients by experienced hands.
7.Establishment of a rat model of inhalation lung injury induced by powder combustion

Objective To establish a model in rats for studying the lung injury induced by powder combustion. Methods The explosion box and trauma chamber were constructed based on the fire simulation-platform. The smoke composition was analyzed on-line. The animal clinical manifestation was observed, and the lung water ratio, arterial blood gases, gross appearance and macropathology were analyzed to evaluate the injury level. Results The smoke contents included CO, SO 2, NO and NO 2. When the injury occurred, the CO concentration was 505 23 ppm, and SO 2 concentration was 67?4.8ppm. The time to cause injury was 4 minutes. The tachypnea immediately occurred for most of the rats, then bradypnea and dyspnea. The metabolic acidosis happened at the early stage of injury and the combined effects of metabolic acidosis and respiratory acidosis appeared at the late stage. Water ratio in the lung gradually increased and reached the peak at 6 hours later, and the normality was not recovered after 24 hours. The gross appearance and macropathology proved the lung injury. The obvious bleeding appeared at 4 hours, and the edema reached the most grievous level at 6 hours, and the normality was not recovered after 24 hours. Conclusion The model has been successfully established to study powder combustion-induced acute aspiration lung injury in rats .This model can keep the injury conditions stably and has the benefits of easy-making and repeatable.

9.Relationship study on internet overuse and personality of college students: mediating effect of emotion regulation self-efficacy

Objective To explore the relationship among regulatory emotional self-efficacy,Internet overuse and personality,in order to provide the basis for reducing the incidence of Internet overuse and formulating relevant intervention.Methods Bv cluster sampling method,a questionnaire survey was conducted among 1750 students in a university.Results There was a statistically significant difference in gender(t=5.47,P<0.05) and grades(F=6.11,P< 0.05) of Internet overuse.The rate of college students' Internet overuse was 19.92%.The scores of personality of intelligence,stability,perseverance,boldness,sophistication and self-discipline in Internet overuse group were lower than the normal use group(t=1.73-6.01,P<0.05),but the scores of dimensions of aggressiveness,suspicion,anxiety and tension were higher(t=-7.74--2.61,P<0.05).The total scores of regulatory emotional self-efficacy were lower in Internet overuse group(t=5.538,P<0.05).There was a significant negative correlation between the scores of Internet overuse and the scores of regulatory emotional self-efficacy(P<0.05).The scores of Internet overuse were significant positive correlation with suspicion,anxiety and tension,but significant negative correlation with stability,excitability,perseverance,boldness and self-discipline (P<0.05).There was a significant positive correlation between the scores of regulatory emotional self-efficacy and intelligence,stability,dimensions of aggressiveness,excitement,perseverance,boldness and self-discipline of personality,but significant negative correlation with suspicion,anxiety and tension(P<0.05).Regulatory emotional self-efficacy played a partial mediating role in Internet overuse and personalitv traits,the rate respectively was 30.8%,16.7%,14.5%,37.3%.Conclusion There were a closely relationship among regulatory emotional self-efficacy,Internet overuse and personality.Regulatory emotional self-efficacy played a partial neliating role in Intemet overuse and personality traits.
10.PBL curriculum in the University of Manchester in the UK and some relevant thoughts about its current practice

University of Manchester who first introduced Problem based learning(PBL)to the UK medical education,and adopted this new curriculum in 1994.Notably,Cambridge and Oxford have always had a high proportion of student-centred and self-directed learning,and have therefore not introduced PBL.Other medical schools have followed the example of Manchester,now PBL has become the mainstream curriculum in British medical edacation.First,this article is going to introduce how PBL curriculum is structured at Manchester medical school,and then reflect and give some thoughts on its current practice.
