3.Anatomic study of single-nostril transsphenoidal approach for pituitary adenoma

Objective To provide the anatomical data for single-nostril transsphenoidal microsurgery for pituitary adenoma.Methods Microanatomic structures were observed and measured in 20 cadaveric a- dults heads.Results(1)The diaphragmal opering was 5 mm or greater broad in 90%.The mean thick- ness of the diaphragma sellae was (0.18?0.08)mm.(2) A sphenoid sinus with complete pneumatization was named as all-sellae type,its percentage in all specimens was 85%,85% sphenoid sinus had middle sep- tum of which only 18.8% located in the medial sagital section.(3)The meant thickness of the sellar floor was (0.81?0.34) mm,70% of which were not more than 1 mm.Of the sellar floors,that the shape was flatness reach 15%.(4) The relationship between ICA and sphenoid sinus had two aspects,one was to be mutually apart (55%),the other was to communicate (45%),which formers carinae on the lateral wall of the sphe- noid sinus,espeially in the superior surface of posterior part.The relationship between optic nerve and sphe- noid sinus,one was to be mutually apart (27.5%),the other was to communicate (72.5% ),which formers carinae on the lateral wall of the sphenoid sinus,especially in the superior surface of anterior part.(5) Three main types of the intercavernous sinus were the anterior intercavernous sinus,present in 80%,the inferior in- tercavenous sinus (25%),and the posterior intercavenous sinus (15%).Conclusion The knowledge of the anatomy of the sellar region and sphenoid sinus is helpful to safe operation in the single-nostril transsphe- noidal approach for pituitary,adenoma microsurgery.
4.Evaluation on Efficacy and Safety of Jinying Capsule in Treatment of Pelvic Inflammatory Disease Patients with Accumulated Damp-heat Syndrome.

OBJECTIVETo evaluate the efficacy and safety of Jinying Capsule (JC) in treating pelvic inflammatory disease patients with accumulated damp-heat syndrome (ADHS).
METHODSTotally 328 patients were recruited in a prospective, positive drug parallel controlled, and multi-center clinical trial. Of them 213 patients in the treatment group took JC (0.5 g per capsule), 4 capsules each time, 3 times per day, while 115 patients in the control group took Kangfuyan Capsule (KC, 0.4 g per capsule), 3 capsules each time, twice per day. The course of treatment was 4 weeks for all. Scores of Chinese medical syndromes, visual analogue scale (VAS) of the lower abdominal pain, and European quality of life-five dimension scale (EQ-5D) were observed before treatment and after 4 weeks of treatment.
RESULTSThere were 204 patients in the treatment group and 109 in the control group who completed this trial. The total effective rate of Chinese medical syndrome was 89.71% (183/204 cases) in the treatment group and 76.15% (83/109 cases) in the control group (P < 0.01). Compared with before treatment in the same group, EQ-5D scores increased, and VAS scores of the lower abdominal pain decreased in the two groups after treatment. EQ-5D scores was 0.857 ± 0.157 in the treatment group, obviously higher than that in the control group (0.753 ± 0.126, P < 0.05). VAS scores of the lower abdominal pain was 2.14 ± 1.23 in the treatment group, lower than that in the control group (2.33 ± 1.24), but with no statistical difference between the two groups (P > 0.05). No adverse reaction occurred in the two groups.
CONCLUSIONJC was superior to KC in improving Chinese medical syndrome and quality of life of pelvic inflammatory disease patients with accumulated damp-heat syndrome.

5.Type high-risk human papilloma virus infection status of investigation and analysis in Guangxi region

Objective To discuss the high-risk human papillomavirus (HPV )infection in the Guangxi .Methods A total of 26 796 copies of the test results for female cervical secretions specimens were analyzed retrospectively from what the second-genera-tion hybrid capture technology mixed detect 13 kinds of high-risk HPV viral DNA .Results Infection rate was 17 .94% in Guangxi , and the infection of HPV in north was higher than other regions ,and the differences of regions had statistically significant (P<0 .05) .The peak age of infection in the Guangxi region was less than 20 years old ,50- <60 years old ,and equal or more than 60 years old .The differences among age groups was statistically significant(P<0 .05) .Patients constitute HPV infection Guangxi re-gion had high viral load for 40 .93% ,and high viral load constituent ratio rise with age .Conclusion The existence of high-risk HPV infection is regional different and age different in the Guangxi region ,to high-prone areas ,infection high age should focus on monito-ring ,high viral load in patients with complete cervical cytology and histology examination if necessary .
6.Effects of Different Doses of Dexamethasone on Bone Mineral Density and Body Composition in Rats

Objective To investigate the effects of different doses of dexamethasone (Dex) on bone mineral density (BMD) and body composition in Sprague-Dawley (SD) rats. Methods Forty 3-month-old female SD rats were divided into four groups:control group (Cont, saline), low dose Dex group (LDG, 1 mg/kg), medium dose Dex group (MDG, 2.5 mg/kg) and high dose Dex group (HDG, 5 mg/kg). Ten rats for each group. Dex was injected intramuscularly twice a week. The values of BMD and body composition were measured by DEXA densitometer at the beginning and 4-week of treatment. Results The body weights of different doses of Dex intervention groups were decreased after 1 to 4-week intervention compared with those of Cont group (P<0.01). After 4-week intervention, the total BMD, femur BMD, total bone mineral content (BMC), to-tal fat mass, trunk fat mass and leg fat mass were significantly increased in Cont group (P<0.01), while the total lean mass, trunk lean mass were significantly decreased (P<0.01). The total BMC, femur BMD, leg lean mass, leg fat mass were signifi-cantly lower in LDG group and MDG group than those of Cont group (P<0.05). The femur BMD, leg fat mass were signifi-cantly lower in LDG group and MDG group than those of HDG group (P<0.05). Conclusion The doses of 1 mg/kg and 2.5 mg/kg Dex have greater impact on the femur BMD and the leg composition in SD rats than that of Dex (dose of 5 mg/kg ).
7.Clinical effect of head and neck reconstruction using microsurgical free flap transfer techniques

Objective To analyze our clinical results of head and neck reconstruction using microsur- gical free flap transfer techniques.Methods The free flap donor sites with long vascular pedicle and large diameter of vessel were routinely chosed,and chose receipt vessels with large diameter and proper position, and perform vessel ananstomosis under surgical loups instead of microscope.The un-buried free flap with a mo- nitoring window were harvest,and do double venous anastomoses in some flaps to ensure adequate venous out- flow.Results From May 1999 to March 2005,1066 consecutive free flap transfers were used to reconstruct head and neck defects.The overall success rate of free flap was 98.3%.The vessel thrombosis rate was 3.1%,and the flap salvage rate was 45.5%.Conclusion Head and neck reconstruetion using microsurgi- cal free flap transfer technique is safe and reliable,and good clinical results can be obtained.

In the development of diabetic nephropathy (DN), reactive oxygen specie (ROS) over much in vivo leads to oxidative stress(OS)-related renal injuries, which are characterized by the structural and functional changes in glomerular and renal tubular cells in morphology. The regulative approaches of OS involve the several signaling pathways, in which, both p38 mitogen-activated protein kinase (MAPK) signaling pathway and adenosine monophosphate-activated protein kinase (AMPK) signaling pathway play the important roles as the target of anti-oxidants. The interventional actions of Chinese herbal compound prescriptions and the extracts of single Chinese herbal medicine (CHM) on OS in the kidney in DN include regulating the balance between ROS and antioxidants, reducing the production of AGEs, inhibiting the expression of growth factors and intervening the activity of signaling pathways.

10.Study on identification of "Digeda" raw materials in Mongolian patent medicine by PCR amplification of specific alleles.

To explore a new method for identification of Mongolian patent medicine (MPM) by PCR amplification of specific alleles. Eight kinds of MPM were used to study the identification of "Digeda" raw materials. The total DNA of Lomatogonium rotatum and Corydalis bungeana samples were extracted through modified CTAB method, psbA-trnH sequence was amplified by PCR and sequenced directionally. Specific primer was designed. The DNA of 8 kinds of MPM also was extracted and purified by the commercial DNA purification kits. The rbcL and two pair of specific primers sequences were amplified. The specific amplified products were sequenced in forward directions. All specific sequences were aligned and were analyzed. The results indicated that L rotatum can be identified by specific primers from Digeda-4 Tang, Digeda-8 San, Digeda-4 San, and C. bungeana medicinal materials can be identified by specific primers from Li Dan Ba Wei San, Yi He Ha Ri-12 and A Ga Ri-35. PCR amplification of specific alleles can stably and accurately distinguish raw medicinal materials in MPM.
