1.The correlation of serum proinflammatory cytokines levels with the severity of acute pancreatitis

Objective To explore the value of serum level of interleukin 6 (IL-6),tumor necrosis factor alpha (TNF-α) and high mobility group box-1 protein( HMGB1) in early assessment the severity and prognosis of acute pancreatitis (AP). Methods Thirty-three severe acute pancreatitis (SAP) patients and 38 mild acute pancreatitis (MAP) patients were selected as study objects;and 51healthy individuals were set as control group. Serum IL-6,TNF-α and HMGB1 concentrations were determined by enzyme-linked immunosorbent assay (ELISA),the association of them and the scores of Ranson,APACHE Ⅱ . Balthazar CT,serum biochemical parameters and prognosis was analyzed.Results The serum IL-6 levels of SAP group,MAP group and healthy control group were (553. 72±175.76) pg/ml,(265. 73±179. 95) pg/ml and (16. 43±3. 32) pg/ml;and there were statistical significance of these three groups (all P<0. 01). There was no significant difference of TNF-α in the three groups (all P>0. 05). The serum HMGB1 levels of SAP group,MAP group and healthy control group were (11. 48±6. 94)μg/L,(6. 13±5. 80)μg/L and (1. 82±0. 64)μg/L respectively,and there were statistical significant of these three groups (all P<0. 05). The correlation coefficient of serum HMGB1 with IL-6 and TNF-α were 0. 896 and 0. 724 (P<0. 01) respectively. The IL-6 level was positively correlated with the scores of Ranson,APACHE Ⅱ and Balthazar CT. The TNF-αconcentration was positively correlated with APACHE Ⅱ score,and the HMGB1 concentration positively correlated with scores of Ranson and Balthazar CT. The levels of IL-6,TNF-α and HMGB1were all positively correlated with the serum creatinine concentration. The IL-6 levels of patients with local and/or systemic complications were significantly higher than those without complications.Conclusion The serum levels of IL-6,TNF-α and HMGB1 are significantly correlated with the severity of pancreatitis,all of them take part in the development of acute renal insufficiency. The high level of serum IL-6 significantly correlated with complications.

4.Role of HMGB1 in organ injury during acute necrotizing pancreatitis and protective effect of HMGB1 monoclonal antibody

Objective To investigate the role of HMGB1 in the pathogenesis of organ injury of acute necrotizing pancreatitis (ANP).Methods Male ICR mice were randomly allocated into control group,ANP group and HMGB1 monoclonal antibody group.ANP model was induced by intraperitoneal injection of 20% L-arginine.Mice in HMGB1 monoclonal antibody group were given intraperitoneal injection of 200 μg of HMGB1 monoclonal antibody immediately after the induction of the ANP model.All the mice were sacrificed at 12,24,and 48 h after ANP induction.Serum level of amylase and liver,renal function were determined,level of serum HMGB1 was measured by using enzyme-linked immunosorbent assay,and then the pathologic changes of pancreas and liver were routinely observed and scored.The HMGB1 mRNA levels in the liver and pancreas were studied by real time fluorescence quantitative PCR.Results The serum levels of HMGB1 at 12 h in control group,ANP group and HMGB 1 monoclonal antibody group were (9.09 ± 1.03),(25.04 ± 4.30),(16.84 ± 4.27) μg/L; and pathological scores of pancreatic tissue were (1.50 ± 0.55),(4.33 ± 0.52),(3.03 ± 0.32) points ; and HMGB1 mRNA expressions in pancreas were 0.48 ± 0.18,7.53 ± 2.71,3.26 ±2.33 ; HMGB1 mRNA expressions in liver were-1.23 ± 0.37,0.15 ± 0.65,- 1.27 ± 0.72.The corresponding values in ANP group were significantly higher than those in control group (P < 0.05).While the corresponding values in HMGB1 monoclonal antibody group were significantly lower than those in ANP group (P <0.05).There was a positive linear relationship between serum HMGB1 level and pancreatic pathological scores 24 h after ANP induction (r =0.768,P < 0.05).In addition,the serum levels of AMY,AST,ALT,LDH,BUN,Cr showed a similar trend as that of serum level of HMGB1,and the serum level of HMGB1 was positively associated with serum levels of Cr,BUN and ALT (r =0.824,0.719,0.590,P<0.05).Conclusions HMGB1 may be a key factor of inflammatory response and organ dysfunction of ANP in mice,and extrinsic supply of its monoclonal antibody may decrease the injuries of pancreas and other organs during ANP.
5.Effect of aging on olfaction center function: a magnetic resonance imaging study

Objective To investigate the localization,zone and activation intensity of olfactory center in young versus elderly healthy volunteers by functional magnetic resonance imaging (BOLD-fMRI),so as to elucidate the effect of age on olfactory center in healthy population.Methods Thirteen right-handed healthy adult volunteers were recruited and divided into two groups:young group (5 males and 3 females,mean aged 23 years) and elderly group (2 males and 3 females,mean aged 69.2 years).The olfactory stimulus was r-undecalactone,and it was given according to a block design.The fMRI detection was performed on Philips Achieva 3.0 T MR scanner,and data of BOLE-fMRI was processed and analyzed to get cerebration image by using SPM2.Results In groupaveraged maps,both young and elderly group showed significant olfactory activation in right parahippocampal gyrus,left hippocampal sulcus,right and left superior temporal gyrus,etc,subcortical activation in right thalamus,dorsal pons,and cerebellum activation in cerebellar vermis.Activations in right inferior frontal gyrus,right middle frontal gyrus,right medial occipito-temporal gyrus and right fimbria of hippocampus were observed only in young group,while activation in bilateral middle temporal gyrus was observed only in elderly group.Activation area was apparently smaller and activation degree was lower in elderly group than in young group.Activation intensity in right superior parietal lobule and bilateral superior temporal gyri was higher in male group than in female group (t=13.7,6.08,5.36,respectively,all P<0.001).Conclusions The intensity of activation in olfactory center is lower in the elderly than in the young,and absence of part of the active regions is found in the elderly,which demonstrates the regression of olfactory center in the elderly.The olfactory center shows right-predominant activation,and olfactory activation intensity in some cortical regions is higher in males than in females.

7.Biofeedback for anxiety: A Meta-analysis

Objective To evaluate the effectiveness of biofeedback for anxiety patients by carrying out a Meta-analysis.Methods We searched CNKI,VIP,CBM(1989 to 2013) to identify relevant randomized-control trials applying biofeedback for anxiety patients.We used Review Management (RevMan) 5.0 software (provided by the Cochrane collaboration) to conduct Meta-analysis.Results We included 11 studies.Meta-analysis for the biofeedback group vs.the control group showed that biofeedback was significantly superior to control (SMD=-3.00,95%CI:-4.40~-1.61); Meta-analysis for the biofeedback plus drug group vs.drug group showed that biofeedback plus drug was significantly superior to drug (SMD=-1.33,95%CI:-1.82~-0.85).Conclusions We found evidence that biofeedback (applying alone or combined with drugs) is effective in improving anxiety symptom.But all the studies had methodological limitations,our conclusion needs further studies with strict design,high quality and consistent assessment tools to be expounded and proven.
8.Reverse Effect of Baicalin/Baicalein on Antibiotic Resistance of Methicillin-resistant Staphylococcus Aureus

OBJECTIVE:To study the reverse effect of Baicalin/Baicalein on antibiotic resistance of methicillin-resistant staphylococcus aureus(MRSA) and its mechanism.METHODS:The synergetic antimicrobial effect of oxacillin and Baicalin/Baicalein against MRSA were detected by plating method,and the synergetic antimicrobial effect of oxacillin and Baicalin/Baicalein on growth density inhibition of MRSA was detected by spectrophotometry.The inhibitory effect of Baicalin/Baicalein on PBP2a synthesis was detected by PBP2a latex agglutination assay kit.RESULTS:The combination of Baicalin/Baicalein with oxacillin showed remarkable synergetic antimicrobial effect on clinical isolated MRSA.16 ?g?mL-1 of Baicalein obviously reversed the strong resistance of MRSA to oxacillin.Baicalin/Baicalein showed remarkable inhibitory effect on PBP2a production in MRSA.CONCLUSION:Baicalin/Baicalein can reverse MRSA's strong resistance to oxacillin by inhibiting PBP2a secretion in MRSA.Theoretically,this phenomenon offers a new approach for the treatment of infections caused by MRSA.
10.Randomized control trail of eye movement desensitization and reprocessing versus cognitive behavior therapy for treating posttraumatic stress disorder

Objective To investigate the effects of eye movement desensitization and reprocessing (EMDR) versus cognitive behavior therapy (CBT) for treating adult posttraumatic stress disorder (PTSD) .Methods A total of 81 patients with PTSD con‐forming to the including standard were randomly allocated to the EMDR group ,CBT group ,and control group ,27 cases per group . The PTSD symptoms ,anxiety and depression moods in 3 groups were assessed before and after treatment by adopting the Clinician‐administered PTSD Scale (CAPS) ,Hamilton Anxiety Scale (HAMA) and Hamilton Depression Scale (HAMD) .Results The drop‐out rates were 29 .63% for the EMDR group ,7 .41% for the CBT group and 7 .41% for the control group respectively ;the re‐experience symptoms score of CAPS in the EMDR group was lower than that in the CBT group with statistical difference (P=0 . 036) .Conclusion Both EMDR and CBT are the effective psychological therapeutic method ,EMDR has more effective than CBT in the reproving the re‐experience symptoms of PTSD .The future studies should pay more attention to the application of stabilization technologies for reducing the dropout rate of EMDR .
