2.Cost-Effectiveness Analysis of Three Therapeutic Schemes for Knee Osteoarthritis

OBJECTIVE: To evaluate the economic effects of different therapeutic schemes for knee osteoarthritis.METHODS: The clinical data of 116 patients with knee osteoarthritis were divided into three groups,namely,Group A(taking Celebrex 200 mg q.d for 28 days);Group B(Celebrex 200 mg daily orally plus topically application of Diclofenac diethylammonium);and Group C(Celebrex 200 mg daily orally plus topically application of 4 boxes of Capsacin ointment).The improvement of knee osteoarthritis was evaluated by Lysholm rating system.And a cost-effectiveness analysis was conducted.RESULTS: The costs of Group A,B,and C were 206.97 yuan,252.77 yuan and 298.53 yuan respectively;the effective rates were 61.54%,71.05% and 84.62%,respectively;the cost-effectiveness ratios were 336.32,355.76 and 352.79,respectively.The incremental cost-effectiveness ratios of Group B and C as against Group A were 481.60 and 396.71,respectively.CONCLUSION: In Group C compared Group A,the effective rate was significantly higher;and as compared with Group B,the cost for gaining each unit of effectiveness in Group C was significantly lower.Group C is the optimal one among the three schemes.

Brain metastases from lung cancer is a significant cause of morbidity and mortality,whichis lack of effective treatment.Because chemotherapeutic drugs are thought to not cross the blood-brain barrier(BBB), the role of chemotherapy in the treatment of brain metastases remains controversial. For yearsradiotherapy has been the standard treatment for brain metastases.However, recent studies have confirmedthat the BBB in patients with brain metastases tumors are broken down.At the same time,researchers havefound some new methods for chemotherapeutic drugs to reach the brain metastases.Chemotherapy will play amore and more active role in the treatment of brain metastases tumors.
4.Comparison of the hemostatic efficacy of pituitrin and oxytocin applying in muscle intramural laparoscopic u-terine myomectomy

Objective To compare and analyze the clinical curative effect of pituitrin and oxytocin applying in laparoscopic uterine myomectomy intraoperation .Methods According to surgical hemostasis method ,68 patients with uterine fibroids were divided into pituitrin group (group A) and oxytocin group(group B).The surgery time,intr-aoperative blood loss ,the change of postoperative hemoglobin levels and the change of blood pressure before and after treatment were observed.Results In A group,the operative time was (69.8 ±18.0)min,blood loss was (68.3 ± 19.1)mL,postoperative discharge time was (15.2 ±7.8)h,preoperative and postoperative hemoglobin difference val-ue was (10.4 ±2.6) g/L,which were significantly better than those in group B [(82.8 ±17.6) min,(88.5 ± 25.6)mL,(19.4 ±5.9)h,(13.5 ±2.9)g/L],the differences were statistically significant (all P<0.05).Postopera-tive morbidity rate in group A was 5 cases,that in group B was 9 cases,the difference was not statistically significant (P>0.05).In A group,the systolic blood pressure was (152.0 ±8.5)mmHg after treatment for 15min,which was significantly higher than (132.5 ±10.2)mmHg in group B,the difference was statistically significant (P<0.05). There were no statistically significant differences in the systolic blood pressure after treatment for 30min and 45min, and the diastolic blood pressure before and after treatment (all P>0.05).Conclusion Application of pituitrin in laparoscopic surgical hemostasis for patients with uterine fibroids can effectively reduce the surgery time ,decrease the blood loss ,its surgical hemostasis effect is better than oxytocin ,which is worth further clinical application .

It has been shown that two pathways of signal transduction are activated under the stimulation of cytokines , such as IL-1?TNF-? ?EGF?TGF-? ?PDGF?IGF-1, secreted by fibroblast-like synoviocytes with rheumatoid arthritis. They are receptor tyrosine kinases-Ras-MAPK pathway and non- receptor tyrosine kinases-JAK-STAT pathway. The former activation plays an important role in chronic synovitis of rheumatoid arthritis. More researches are expected to focus on G-protein-AC-cAMP signal transduction pathway and the internal relations of all kinds of pathways.

Objective To investigate IL-1?mRNA expression and protein secretion after ultraviolet irradiation in human keratinocyte(KC)originated SCC12F cells.Methods Expression of IL-1?mRNA was detected by Northern blot and the secretion level of IL-1?protein was analyzed by ELISA.Results There was a spontaneous expression of IL-1?mRNA in SCC12F cells in culture system of normal KC,which was increased along with the culture time,and reached the highest expression level in120hours.The secretion level of IL-1?protein was increased in a time-and dose-dependent manner in SCC12F cells after ultraviolet B(UVB)irradiation.Conclusions SCC12F cells may express IL-1?mRNA spontaneously under regular KC media.The secretion level of IL-1?protein shows both time-and dose-effect after UVB irradiation.

9.Protective effect of gynostemma on myocardium in diabetes associated with hyperlipemia and its mechanism

Objective To investigate the protective effect of gynostemma on myocardium and its mechanism in patients with diabetes associated with hyperlipemia.Methods 178 patients with type 2 diabetes mellitus with hyperlipidemia from the department of endocrinology in our hospital were selected and divided into two groups, 89 cases in the control group treated with basic treatment of diabetes+atorvastatin, 89 cases in the experiment group received gynostemma on the basis of the control group, myocardial injury index, blood glucose level, blood lipid level, serum cytokine levels and serum oxidative stress response indexes were detected and compared after the treatment.Results Compared with the control group, serum levels of aspartate aminotransferase(AST), creatine kinase(CK), CK-MB, lactate dehydrogenase(LDH)were lower in the experiment group after treatment, fasting plasma glucose(FPG), 2h postprandial blood glucose(2hPG), HbA1c level were lower after treatment, serum levels of total cholesterol(TC), triglycerides( TG ) , low-density lipoprotein cholesterol ( LDL-C ) were lower, high-density lipoprotein cholesterol ( HDL-C ) level was higher after treatment, serum level of adiponectin(ADPN) was higher, tumor necrosis factor-alpha(TNF-α), interleukin-6(IL-6), interleukin-18(IL-18) levels were lower after treatment, serum levels of malondialdehyde(MDA), reactive oxygen species(ROS) were lower, glutathione peroxidase(GSH-Px), superoxide dismutase(SOD) levels were higher after treatment, with significant difference (P<0.05).Conclusion Gynostemma can significantly reduce the cardiac muscle damage of patients with diabetes associated with hyperlipemia , plays a role in myocardial protection, the mechanism may be to reduce blood glucose and blood lipid levels, inhibit the inflammatory reaction caused by inflammatory cytokines, improve the antioxidant capacity of the body and reduce oxidative stress damage.
10.Clinical significance of ultrasonic scanning in detecting umbilical abnormality and measuring notch depth of nuchal cord entanglement

Type B ultrasonic scanning was performed routinely for 10 944 pregnant women at their third semester when they visited outpatient department or were admitted to our hospital during January 2004 to December 2008, and their fetal heart rate, amniotic fluid, umbilical cord and neonate conditions were monitored and recorded during delivery. The sensitivity and specificity of type B ultrasonic were 95.74% (3301/3448) and 96. 25 % (7215/7496). Umbilical abnormality was diagnosed in 3448 pregnancies by type B ultrasonic scanning, with prevalence of 31.51%, cesarean section was performed in 44.00 %, fetal distress occurred in 18. 27% of them, 15.46% of them with Apgar score equal to or less than seven, and fetal death occurred in 2. 41% of them, all significantly higher than those in pregnant women without umbilical abnormal (23. 77%, 8. 56%, 7.84% and 1.36%, respectively, all P <0. 01 ). Cesarean section was performed in 41.90 % of those with 0. 8 cm or more in the notch depth of nuchal cord entanglement,fetal distress occurred in 29.76% of them, 25.30% of them with Apgar score equal to or less than seven and fetal death occurred in 4. 05% of them, all significantly higher than those in pregnant women with the notch depth less than 0. 8 cm (20. 22%, 8. 60%, 9. 26% and 0. 54%, respectively, all P <0. 01 ). Type B ultrasonic scanning plays an important role in clinical diagnosis for umbilical abnormality, guiding selection of delivery method and improving neonatal prognosis.
