2.Status and progress of diagnosis and treatment of polypoidal choroidal vasculopathy

Polypoidal choroidal vasculopathy(PCV) is a common disease of the retina,the disease is more found in colored race.In China,the mainly nosogenic population is the female about 60 years old.The pathogenesis of PCV is unknown and there is no specific medicine at present.The funduscopic present is orangered lesions,bleed and exudation.Indocyanine green angiography(ICGA) could make a definite diagnosis of PCV.In recent,the main treatment of PCV includes photodynamic therapy,intravitreal anti-VEGF therapy,surgery for treatment of complications and combined therapy.We review the treatment and diagnosis of PCV,and propose the further research direction.
3.A multivariate analysis of cadaver renal transplantation for long-term survival

Objective To review the kidney transplantation performed in our center and analyze the risk factors affecting long-term allografts survival. Methods Thirty two relative variables were analyzed by SAS statistical software. Using Log-Rank method, the influence of these variables on short and long-term grafts survival was investigated. A Kaplan-Meier analysis was used to estimate the 1-, 3-, 5-, 10-year graft survival rate and its half-life. Proportional hazards regression analysis (Cox model) was used to assess and rank the relative risk of potential variables.Results The 1-year, 3-year, 5-year, 10-year grafts survival rate was 82.5 %, 75.2 %, 65.5 % and 48.4 % respectively. When excluding the patients died with functioning grafts, the 1-year, 3-year, 5-year, 10-year grafts survival rate was increased to 91.0 %, 82.0 %, 75.9 %, 68.5 %, respectively. The mean half-life was 8.78? 0.14 and 14.09? 0.20 years, respectively. By Log-Rank analysis, factors affecting short- and long-term grafts survival were identified as: renal function and duration of which become normal, cold-ischemia time, acute rejection, delayed grafts function, immunosuppressive regimen, complication, infection, the method of anti-rejection therapy. A Cox model multivariate analysis showed that there were 18 factors affecting graft survival.Conclusion New immunosuppressive agent not only has significantly increased short-term graft survival, but also prolonged the long-term survival. Making assurance to get high quality donor organ and minimizing the death with graft function may be the most feasible way to prolong a graft survival at present.
4.Different active ingredients of medicinal plant based on function differentiation of homologous gene.

In the research field of quality control in Chinese medicinal materials, variation in active ingredients of medicinal plant is always the key and hot issues. With the development of high-throughput sequencing technologies and reducing cost, a large numbers of genes from medicinal plant were cloning and provide a solid foundation for further research of gene structure and its biological function, and also provides conditions for explore active ingredient variation and its quality control from the perspective of molecular pharmacognosy. This paper introduces the concept of homologous gene, gene duplication and classification. We prospect the function of duplicated genes in the role of molecular mechanism research about variation in active ingredients, aiming at providing a new way for medicinal materials quality control.

5.Discussion on the value of cystic decompression operation on autosomal dominant polycystic kidney disease

Objective: To evaluate the therapeutic effect of cystic de compression (CD) operation autosomal dominant polycystic kidney disease(APKD) based on clinical material, experience and related theory. Methods: Thirty-nine APKD received CD operation(unilateral 31 cases, bilateral 8 cas e s) in our hospital from 1985 to 1995. Four main parameters, cystic renal enlargi ng rate(CRER),lumbar pain recurring rate(LPRR),blood pressure elevating rate(B PER) and renal function abnormal rate(RFAR), were observed 3, 6, 12, 36 and 60 months after CD operation. And the changes were analyzed based on related theory . Results: The changes of 4 main parameters on 5 different time points post operation in unilateral 31 cases were:(1)CRER 19.4%,38.7%,61.3%,1 0 0% and 100%; (2)LPRR 12.9%,48.4%,71.0%,100% and 100%; (3)BPER 6.5%,22.6%,4 1.9%,71.0% and 96.8%;(4) RFAR 3.2%, 12.9%,22.6%,74.2% and 96.8% respectively. Conclusion: During a short period, CD operation can relieve th e lumbar pain, but it is not certain for improving CRER,BPER and RFAR. in the lo ng run, the therapeutic effect is not sure.
6.Current situation survey and analysis of a district general hospital in Shanghai

Objective To analyse the changes of a district general hospital before and after management by Shanghai Renji Medical Group through current situation survey,and explore the conditions for the development of district general hospitals.Methods The Zhujiajiao People's Hospital of Qingpu District in Shanghai was selected,and patients(n=50)and hospital staff(n=100) were conducted current situation survey with questionnaires which included the knowledge and evaluation of the hospital's current situation as well as the changes before and after management by Shanghai Renji Medical Group.Results Half year after management by Shanghai Renji Medical Group,great progress in medical work was achieved,with a favourable development trend.The rate of satisfaction on medical service evaluated by inpatients reached 89.60%,and 81.91% hospital staff had confidence in the sustainable development of the hospital. Conclusion Management by Shanghai Renji Medical Group has brought prospects for the Zhujiajiao People's Hospital.Under current environment,only certain conditions are met can a district general hospital achieve sustainable development.
7.Effects of age on the lenticular nuclei detected by MR diffusion tensor imaging

50 years).The values of ADC,FA,?_1,?_2 and ?_3 in putamen and globus pallidus were calculated after diffusion tensor scanning,and then analyzed the differences between the two tissues.The differences of ADC,FA,?_1,?_2 and ?_3 values among age groups were analyzed by using analysis of variance(ANOVA),and their relationship with age were accessed by using Pearson correlation.Results Between the correspondent age groups,the ADC value was higher in the putamen[(6.68?0.40)?10~(-4),(6.47?0.36)?10~(-4),(6.44?0.34)?10~(-4)mm~2/s]than that in the golbus pallidus[(6.13?0.50)?10~(-4),(6.05?0.33?10~(-4),(6.05?0.52)?10~(-4)mm~2/s],and the value of FA in the putamen(0.20?0.03,0.23?0.03,0.25?0.03)was lower than that in the globus pallidus(0.35?0.03,0.36?0.03,0.37?0.04).ANOVA analysis of the FA value among age groups revealed there were significant differences in the putamen(F=10.082,P=0.000),and the same analysis of eigenvalue ?_3 in the putamen also showed significant differences(F=5.675,P=0.005).The FA value in the putamen was significantly elevated with aging(r=0.555,P
8.Influence of recipient cytokine gene polymorphisms on incidence of infection after renal transplantation

Objective To explore the influence of renal allograft recipient cytokine and cytokine receptor gene polymorphisms on the infection after renal transplantation. Methods (1) 126 cases of cadaveric renal allograft recipients were divided into two groups according to the presence or Absence of infection in 6 months after renal transplantation: infection group and un infection group. The distribution of 22 polymorphisms in 13 cytokines and cytokine receptors gene were compared between two groups as well as latent factors affecting the onset of infection. The production types of IL 10, TNF ?, TGF ?1 and IL 6 were analyzed; (2) All recipients were stratified into two conditions: presence of rejection or Absence of rejection. Previous positive gene polymorphisms were compared between infection group and un infection group under two conditions. Results (1) The incidence of acute allograft rejection was significantly higher in infection group than in un infection group. In infection group, the genotype frequency of IL 1? 889 C/C, IL 1? 511 C/C, IL 1?+3962 C/T or T/T, and the higher production type frequency of TGF ?1 were significantly higher than those in un infection group. (2) In the recipients without acute rejection, the genotype frequency of IL 1? 511C/C was significantly higher in infection group than in un infection group; whereas in those with acute rejection, the genotype frequency of TGF ?1 (codon10 codon 25) CG/TG and frequency of TGF ?1 with high production were significantly higher than those in no infection group. Conclusion This study identified recipient genotype of IL 1? 889 C/C, IL 1? 511 C/C, TGF ?1(codon10 codon 25) CG/TG and higher production type of TGF ?1 gene as major risk factors for the onset of infection after renal transplantation as well as presence of acute allograft rejection, which might affect the action of these gene polymorphisms.
9.Effect of Surgical Treatment on Primary Gastrointestinal Non-Hodgking Lymphoma in Children

Objective To explore the effect of surgical treatment on primary gastrointestinal non-Hodgking lymphoma(NHL) in children.Methods Nine cases of clinical and follow-up data of primary gastrointestinal NHL were studied retrospectively to evaluate the effect of surgical treatment on primary gastrointestinal NHL in children.Results Seven cases were male and 2 cases were female.The mean age was(5.59?3.27)years old.The clinical manifestation included abdominal mass (7 cases),abdominal pain (5 cases),fever (2 cases),haematemesis and melena (2 cases),constipation (1 case) and paroxysmal abdominal pain with vomiting (1 case).Nine cases were diagnosed as primary gastrointestinal NHL,including 1 case of intussusception,1 case of acute appendicitis,2 cases of gastrointestinal obstruction,2 cases of gastrointestinal bleeding and 3 cases of abdominal mass.One case received the operation of intussusception reduction,tumor resection and intestinal anastomosis.One case received appendectomy.One case received the operation of tumor biopsy and transverse colon colostomy.Six cases received laparotomy.Six cases were diagnosed as Burkitt lymphoma.One case was anaplastic large cell lymphoma and 2 cases were diffuse large B-cell lymphoma.One case was at stage Ⅰ,1 case was at stage ⅠE,2 cases were at stage Ⅱ,3 cases were at stage ⅡE and 2 cases were at stage Ⅲ.Nine patients had received operation.One case died after operation and 8 cases had received combined chemotherapy.The 1 and 3 years survival rates were 75.0% and 37.5%,respectively.Conclusions Acute abdomen is often the first symptom of primary gastrointestinal NHL in children and comprehensive surgical treatment is an effective procedure for it.
10.Effect of advanced glycosylation end products on oxidative stress and MCP-1 in human renal mesangial cells.

OBJECTIVETo investigate the effects of advanced glycosylation end products (AGEs) modified bovine serum albumin (AGE-BSA) on the expression of reactive oxygen species (ROS) and monocyte chemoattractant protein-1 (MCP-1) in human renal mesangial cells (HRMCs).
METHODSHRMCs were cultured in vitro with medium containing different doses of AGE-BSA or BSA (50,100, 200, 400 mg/L) for 48 hours, or with AGE-BSA (200 mg/L) for different times (12, 24, 48, 72 h). Immunocytochemistry assay was used to estimate the protein level of RAGE. The ROS in cells were measured by flow cytometry and the mRNA expression of MCP-1 were analyzed by semi-quantiative reverse transcription-polymerase chain reaction (RT-PCR) after treatment with AGE-BSA or BSA.
RESULTSThe protein level of RAGE was upregulated in the HRMCs with AGE-BSA. The expression of ROS and MCP-1 significantly enhanced by incubation of AGE-BSA in a time- and dose-dependent manner. The effects of AGE-BSA-induced up-regulation of ROS and MCP-1 level was significantly blocked by neutralizing antibodies to RAGE, while the expression of ROS and MCP-1 stood nearly unchanged after cultured with huamn IgG.
CONCLUSIONThe expression of ROS and MCP-1 in HRMCs is induced by AGE-BSA through RAGE, which may have potential effects in the pathgenic mechanism of diabetic nephropathy.
