1.Effects of hyperbaric oxygen on the systemic inflammation response in patients with severe traumatic brain injury

Objective To investigate the effects of hyperbaric oxygen (HBO) on the systemic inflammation response in patients with severe traumatic brain injury (TBI), and to explore HBO therapy mechanisms. Methods Seventy patients with severe TBI were randomly divided into a routine treatment group ( RT group, n = 35 ) and an HBO group (n=35). All patients received conventional treatment, but the HBO group received additional early HBO therapy. Twenty age-and sex-matched normal subjects were recruited and served as normal controls. Serum interleukin-6 (IL-6) was measured by ELISA, and C reactive protein (CRP) was also measured on days 1, 7, 14 and 21 after injury. Sequential organ failure assessments (SOFAs) and Glasgow coma scale (GCS) scores were evaluated at the same time points. Systemic inflammatory response syndrome (SIRS) was assessed daily. Results The concentrations of serum IL-6 and CRP increased obviously following TBI, but patients in the HBO group exhibited significantly lower levels at each time point than those in the RT group. In the HBO group fewer cases of SIRS developed,and they had a significantly shorter average duration than those in the RT group. The average SOFA score in the HBO group was significantly lower than that of the RT group at days 14 and 21 after injury, and the GCS scores had improved significantly more by day 21. Compared with the patients who were free of SIRS, the patients with SIRS showed higher levels of IL-6 and CRP, higher SOFA scores as well as lower GCS scores ( all differences statistically significant). Conclusions HBO therapy can attenuate systemic inflammation after TBI, protect the functions of important organs and improve clinical outcomes. Decreasing the level of IL-6 may contribute to the effectiveness of HBO.
2.Effects of early enteral nutrition combined with probiotics on nutritional status of patients with severe head injury

Objective To investigate the effect of early enteral nutrition combined with probiotics on the nutritional status of patients with severe head injury. Methods Thirty-five patients with severe head injury were randomized into early enteral nutrition group ( control group, n = 18 ) and early enteral nutrition combined with probiotics group (observational group, n = 17). All the patients received enteral nutrition via nasogastric tube within 24-48 hours following admission. Hemoglobin, lymphocyte count,plasma concentrations of total protein, albumin, prealbumin, cholinesterase, and fasting blood glucose were determined before enteral nutrition and at days 8, 15, 21 after enteral nutrition. Arm circumference (AC), triceps skinfold thickness (TSF) and ann muscle circumference ( AMC ) were measured at the same time points. Gastrointestinal reactions as well as the time to first defecation were recorded. Glasgow coma scale (GCS) was assessed. Results There was no difference in terms of hemoglobin, lymphocyte count, plasma concentrations of total protein, prealbumin, and GCS score at each time point between the two groups ( P > 0. 05 ). At day 8 after enteral nutrition, the patients in the observational group exhibited significantly lower level of fasting blood glucose than those in the control group (P < 0.05 ). At day 15,the concentration of plasma cholinesterase in the observational group was significantly higher than that in the control group ( P < 0. 05 ). At day 21, the observational group showed a tendency in increase of plasma albumin level in comparison with the control group( P = 0. 054), and the decrease of AC and AMC was obviously lower than that in the control group ( P <0. 05 ) compared with pre-enteral nutrition. The observational group showed less gastro-oesophageal reflux and constipation as well as earlier time to first defecation compared with the control group ( P < 0. 05,0. 01 ). Conclusion Compared with the ordinary enteral nutrition, the enteral nutrition combined with probiotics can attenuate metabolic disorders and improve the nutritional status of the patients with severe head injury.
3.Analysis of monitoring results of urinary iodine of children aged 8 - 10 of Zhangjiakou city in Hebei province, 2009

Objective To analyze the monitoring results of urinary iodine of students aged 8 - 10 in Zhangjiakou city,problems in monitoring results,and to provide basic information for working out control strategies of iodine deficiency disorders.Methods A township(town,street) in each country of each city(district) in Zhangjiakou was selected according to 5 positions of the east,the west,the south,the north and center,and 1 village elementary school was sampled in each chosen township,twenty students(half male and female) aged 8 - 10 were selected to collect their urine samples in each school.Urinary iodine concentration was determined by arseniccerium method.Results The median of urinary iodine of the 1700 children aged 8 - 10 was 291.5 μg/L,with < 50 μg/L accounted for 0.8%(13/1700),50 ~ 99 μg/L about 4.9%(83/1700),100 - 199 μg/L about 20.5% (349/1700),200 - 299 μg/L about 29.7%(504/1700),and ≥300 μg/L about 44.9%(764/1700).Conclusions Urinary iodine has reached the elimination standard of iodine deficiency disorders in Zhangjiakou city.But the situation of more than adequate amount of urinary iodine and iodine excess is relatively serious and it is necessary to lower iodine concentration.
4.Analysis of iodized salt monitoring results in Zhangjiakou city Hebei province in 2010

ObjectiveTo master iodized salt monitoring results in Zhangjiakou city Hebei province,search problems in these monitoring results,and provide a basis for the development of control measures to iodine deficiency disorders.MethodsBy 2010,in Zhangjiakou city,nine salt samples were collected to detect the iodine level in each processing,packing and wholesale enterprise according to their orientation of east,west,north,south and center positions.In each county(district) with nine townships(towns,street offices) and more,nine townships (towns,street offices) were selected according to their east,west,south,north and center positions,in each township (town,street office ) selected,four villages (neighborhoods) were selected,eight residents per household in each village (neighborhood) chosen were selected,and an edible salt was collected in each household to test iodine level; in a county(district) with nine or less townships(towns,street offices),five townships(towns,district offices) were selected according to their east,west,south,north and center positions,four villages (neighborhoods) were selected,and 15 residents per household in each village(neighborhood) were selected to test the iodine level in an edible salt samples;after population-weighted calculation,indicators of iodized salt monitoring were calculated.ResultsA batch quality pass rate of processing,packing and wholesale enterprise was 100%(192/192); the rate of weighted non-iodized salt in a household was 0.04%(2/4932),iodized salt coverage rate was 99.96% (4930/4932),iodized salt passing rate was 99.55% (4908/4930),and qualified iodized salt coverage rate was 99.51% (4908/4932).ConclusionsMonitoring indicators of iodized salt in Zhangjiakou city have reached the standand to eliminate iodine deficiency disorders.However,there still have unqualified iodized salt and non-iodized salt,and the monitoring and iodized salt market management should be strengthened.

6.Survey on excessive iodine in drinking water and its geographical distribution in Hebei Province

Objective To investigate the geographical distribution of excessive iodine in drinking water and to demarc aI|e the excessive iodine regions in Hebei Province.Methods In 173 counties of Hebei Province, town was surveyed as an elementary unit.Five villages were sampled according to the direction of east,west, south,north,center in every town.If the water sources were more than 5 in a village,water was sampled according to the direction ofeast,west, south,north, center, respectively;If the water sources were less than 5 in a village, all were sampled:If the village used water of cental supply,only one sample wag collected.Arsenic-cerium catalysis was used to determine the iodine concentration of drinking water.Results Two thousands and forty-nine towns were investigated and 19 352 water samples were determined in Hebei Province.One hundred and seventy- two towns were confirmed to be excessive iodine areas,where the range of water iodine Wag 0.1~2840.4μg/L The water 8amDle with 18 358 had a water iodine median less than 150.0μg/L The areas with water iodine median less than 150.0μg/L,in between 150.0μg/L and less than 300.0μg/L,and equal to or more than 300.0 μg/L accounted for 94.86%,2.92%and 2.22%,respectively.There were 110 towns with water iodine median being between 150.0μg/L and iess than 300.0 μg/L and 62 towns with water iodine median equal to or more than 300 gμ/L.Exeessive iodine towns distributed in 33 counties of 6 cities involving 5 854 960 residents in'Hebei Provmce. MOBt of the water sources with excessive iodine Cangzhou were deep wells and there was a positive correlation between we depth and water iodine concentration (r=0.430,P<0.01), while they were shallow wells in Handan, Xingtai.Hengshui,and no correlation was found between well depth and water iodine concentration(r=-0.060,-0.119.-0.121,P>0.05).Conclusions Six cities have excessive iodine water resotlrees in Hebei Province, mostly in Handan.xingtai,Hengshui and Cangzhou Cities,which all are low-lying land.The water with excessive iodine is hypogene,and the towns with excessive iodine water distributein patchy or spot pattern.
7.Efficacy controlled observation on acupuncture and western medicine for gastrointestinal dysmotility in liver cirrhosis.

OBJECTIVETo observe the therapeutic effect of acupuncture on gastrointestinal dysmotility in liver cirrhosis.
METHODSForty cases of gastrointestinal dysmotility in liver cirrhosis were randomized into an acupuncture group and a motilium group, 20 cases in each one. In the acupuncture group, on the basis of the conventional treatment, electroacupuncture was applied at Zusanli (ST 36), Sanyinjiao (SP 6) and Taichong (LR 3) for 30 min, once a day. In the motilium group, on the basis of the conventional treatment, motilium was taken orally 30 min before meals, 10 mg each time, three times a day. The treatment was required for 2 weeks in both groups. The changes in the digestive tract symptom score and liver function Child-Pugh score were observed and the efficacy was assessed.
RESULTSThe total effective rate of digestive tract symptoms was 85.0% (17/20) in the acupuncture group and 70.0% (14/20) in the motilium group. The score improvements in abdominal distention, belching and vomiting in the acupuncture group were superior to those in the motilium group (all P < 0.05). In the acupuncture group, the liver function Child-Pugh total score was 8.40 +/- 0.22 before treatment and reduced to 5.36 +/- 0.17 after treatment, in which the scores for ascites, serum bilirubin and albumin were all reduced (all P < 0.05) and the reducing range was increased in tendency with the improvements in digestive tract symptoms. In the motilium group, Child-Pugh score was not changed obviously as compared with that before treatment. CONCLUSION Acupuncture effectively alleviates digestive tract symptoms and improves liver function for the patients of liver cirrhosis, its efficacy on gastrointestinal dysmotility in liver cirrhosis is superior to motilium.

8.Expression and clinical significance of costimulatory molecule B7-H3 mRNA and B7-H3 protein in gastric carcinoma.

OBJECTIVETo study the expression of B7-H3 mRNA and B7-H3 protein in gastric carcinoma and their clinical significance.
METHODSThe expression of B7-H3 mRNA and B7-H3 protein in gastric carcinoma and the nearby normal tissue of 38 patients was detected by real-time RT-PCR and immunohistochemical assay respectively.
RESULTSB7-H3 mRNA was expressed both in gastric carcinoma and nearby normal tissue, but the expression level in gastric carcinoma was much lower than that in nearby normal tissue. There were no significant differences of B7-H3 mRNA expression among gender, age, histological type, tumor size, lymph node metastasis and invasive depth (all P >0.05). The positive rate of B7-H3 protein expressed in gastric carcinoma was 39.5%. There were no significant differences of B7-H3 protein expression among gender, age, histological type, tumor size, lymph node metastasis and invasive depth (all P >0.05), but there were significant differences among groups of clinical stage (P=0.022) and pathological grade (P=0.039). Kaplan-Meier analysis revealed that disease-free survival or overall survival of the patients with positive B7-H3 expression were significantly longer than those with negative B7-H3 expression (P=0.009 and P=0.010 respectively).
CONCLUSIONDetection of B7-H3 expression in gastric carcinoma will be beneficial to the judgment of the prognosis of gastric carcinoma and the choice of individualized treatment.

9.Synthesis of opiate receptor radioligand 11C-carfentanil and its biodistribution in rats

Objective To establish an automatic synthesis method for 11C-carfentanil (CFN) as an novel opiate receptor radioligand and study its biodistribution in rats. Methods 11C-Triflate-CH3 was bubbled into 0.5 mg precursor desmethyl-CFN (which was dissolved in 0.15 ml DMSO) to generate 11C-CFN in a V-tube at room temperature. Sep-Pak C2 column was used for purification of 11C-CFN, which was eluted by 3ml binary system aqueous solution, 10 ml water thrice, and then I ml ethanol. The biodistribution (% ID/g) of 11C-CFN in SD rats was studied. SPSS 13.0 was used for statistical analysis. Non-normal distribution data were analyzed using nonparametric test. Results The synthesis time for 11C-CFN was 20 min (end of bombardment, EOB). The synthesis yield was (35.5 ± 2.2) % on average (n = 12, uncorrected)with the radiochemical purity over 98%. Biodistribution study in rats showed that the tracer had a high brain uptake, rapid blood clearance, and a metabolic pathway via liver and kidney. The highest tracer uptake was in thalamus (4.26 ± 0.89) % ID/g and striatum (4.05 ± 1.08) % ID/g at 5 min after injection, followed by cerebral cortex (2.63±0.89) %ID/g, pons (2.26 ±0.57) % ID/g, hippocampus (2. 17 ±0.55) %ID/g and cerebellum (2. 15 ±0.39) %ID/g. Conclusions The automatic synthesis of 11C-CFN is fast and reliable, and this radioligand can be used for opiate receptor imaging.
10.Clinical observation on warming needle moxibustion plus exercise for treatment of temporomandibular joint dysfunction syndrome.

OBJECTIVETo search for a better therapy for temporomandibular joint dysfunction syndrome.
METHODSTwo hundred and ten cases of temporomandibular joint dysfunction syndrome were randomly divided into a treatment group, a control group I and a control group II, 70 cases in each group. The treatment group were treated with warming needle moxibustion plus exercise, with Hegu (LI 4), Xiaguan (ST 7), Yifeng (TE 17) and so on selected, in combination with opening and closing the mouth; the control group I were treated with simple filiform needle needling, and the control group II with local blocking therapy. The therapeutic effects of the 3 groups were compared.
RESULTSThe total effective rate was 94.3% in the treatment group, 87.1% in the control group I and 85.7% in the control group II with a significant difference among the 3 groups, the treatment group being better than the two control groups (P < 0.05).
CONCLUSIONWarming needle moxibustion plus exercise has a good therapeutic effect on temporomandibular joint dysfunction syndrome.
