1.Long-term survival outcomes and prognostic factors following radical resection of pancreatic body and tail cancer:a retrospective analysis of 992 patients

Objective:To investigate the survival outcomes and prognostic factors in patients undergoing radical resection for pancreatic body and tail cancer.Methods:A retrospective case series study was conducted on 992 patients who underwent radical resection for pancreatic body and tail cancer at the Pancreatic Center of the First Affiliated Hospital of Nanjing Medical University from January 2016 to June 2024. In this study, 577 (58.2%) were male and 415 (41.8%) were female,with an age of (65±9) years (range: 26 to 86 years). Follow-up continued until June 2024. Survival rates were estimated using the Kaplan-Meier method,and prognostic factors were identified using univariate and multivariate Cox proportional hazards models.Results:Among 992 patients,open surgery was the predominant approach (89.1%, 884/992), and radical antegrade modular pancreatosplenectomy (RAMPS) was performed in 317 patients (32.0%). Combined organ resection,venous resection,and arterial resection were performed in 23.5%, 9.3%,and 11.2% of patients,respectively. The rates of R0, R1-1 mm, and R1-direct resections were 49.8% (494/992),41.5% (412/992), and 8.7% (86/992),respectively. Stage ⅡB was the most common TNM stage (32.2%,319/992). A total of 801 patients (80.8%) received adjuvant chemotherapy. The median follow-up period was 32.0(8.8) months(range:3.2 to 105.3 months),during which 508 patients (51.2%) died. The overall median survival (OS) was 26.4 months,with 1-,3-, and 5-year survival rates of 79.0%,40.0%, and 29.0%, respectively. In the recent five years (from 2020 to 2024), the median OS improved significantly to 34.1 months compared to 20.0 months from 2016 to 2019 ( P<0.01). Histological subtype analysis showed that the median OS time was 26.7 months for pancreatic ductal adenocarcinoma (PDAC, n=855),58.9 months for invasive intraductal papillary mucinous carcinoma (IPMC, n=32),and 15.7 months for adenosquamous carcinoma of pancreas (ASCP, n=73) ( P=0.001). Among PDAC patients, adjuvant chemotherapy significantly improved survival (29.1 months vs. 14.4 months, P<0.01);in IPMC patients, adjuvant chemotherapy also extended survival (65.7 months vs. 58.9 months, P=0.047). Although ASCP patients receiving chemotherapy had a longer median OS time than those without (18.8 months vs. 8.9 months),the difference was not statistically significant ( P=0.151). Multivariate Cox regression analysis in PDAC patients indicated that adjuvant chemotherapy, R0 resection, T stage,N stage,and tumor differentiation were independent prognostic factors ( P<0.01). The median OS time by TNM stage was:not reached for stage ⅠA, 51.6 months for ⅠB, 25.5 months for ⅡA, 23.7 months for ⅡB, 23.0 months for Ⅲ, and 14.4 months for Ⅳ. The median OS time for R0,R1-1 mm,and R1-direct resections was 34.1,24.7,and 15.7 months,respectively ( P<0.01). Conclusion:Adjuvant chemotherapy,R0 resection,tumor stage,and differentiation are independent prognostic factors for pancreatic body and tail cancer.
2.DING Ying's Clinical Experience in Treating Systemic Lupus Erythematosus Based on Differentiation of Yin Fire,Latent Fire,and Fire Toxin

This paper summarizes Professor DING Ying's clinical experience in the treatment of systemic lupus erythematosus (SLE) through differentiation of three states, yin fire, latent fire, and fire toxin. It is proposed that fire pathogenic factors constitute a key pathological element running throughout the entire disease course of SLE. The evolution of its pathogenesis centers on these three states, spleen-kidney deficiency with the initial emergence of yin fire as the onset of disease, damage to yin by medicinal toxicity with internal blazing of latent fire as the driver of disease progression, and the interlocking of blood stasis and heat with intense scorching by fire toxin as the critical factor leading to severe and life-threatening conditions. Corresponding to these three stages, targeted prescriptions are formulated, Jiuwei Yishen Formulation (九味益肾方) to tonify the spleen and kidney, raise yang, and disperse fire; Ziyin Xiehuo Decoction (滋阴泄火汤) to nourish yin and fluids while clearing latent fire; and Santeng Changluo Jiedu Decoction (三藤畅络解毒汤) to dispel blood stasis, unblock the collaterals, detoxify, and restrain fire. This staged and integrated therapeutic strategy aims to address both root and branch and to achieve overall regulation, providing valuable guidance for the clinical differentiation and treatment of SLE.

4.Prostate cancer in the world and China in 2022:an epidemiological analysis

Objective To analyze the incidence and mortality of prostate cancer(PCa)in the world and China based on the data of 2022 global cancer statistics published by the World Health Organization(WHO).Methods Based on the data of 2022 global cancer statistics,the number of incident cases,crude incidence,standardized incidence rate(SIR),number of deaths,crude mortality,and standardized mortality rate(SMR)of PCa in the world and China were systematically described,and stratified according to the age and human development index(HDI).Pearson correlation analysis was applied to evaluate the correlations of SIR,SMR,and mortality-to-incidence ratio(M/I)with HDI.Results In 2022,there were 1 467 000 new cases of PCa in the world,with an SIR of 29.4/100 000,ranking the 2nd among all cancer types in the males;the number of deaths in the world was 397 000,with an SMR of 7.3/100 000,ranking the 5th among all cancer types in males.In 2022,the number of new PCa cases in China was 134 000,with an SIR of 9.7/100 000 and an SMR of 3.3/100 000,both ranking the 7th among males.The number of new PCa cases and SIR in the world increased rapidly from 45 years of age,and the number of deaths and SMR showed a rapid growth trend from 50 years of age;the number of new PCa cases and SIR in China increased rapidly from 60 years of age,and the number of deaths and SMR increased rapidly from 65 years of age.The M/I of PCa in China was 0.337,ranking the 30th among countries and territories with high HDI.Pearson correlation analysis showed that SIR was positively correlated with HDI(r=0.440,P=0.008),M/I were negatively correlated with HDI(r=-0.877,P<0.001),while SMR was not correlated with HDI(P>0.05).Conclusion The burden of PCa is heavy in the world.The incidence of PCa in China is gradually increasing,and it is characterized by high mortality.Actively carrying out early screening of PCa,optimizing the diagnosis and treatment mode with multidisciplinary collaboration,and achieving the whole process management can improve the diagnosis,treatment and prognosis of patients.
5.Study on strategies and methods for discovering risk of traditional Chinese medicine-related liver injury based on real-world data: an example of Corydalis Rhizoma.

In recent years, there have been frequent adverse reactions/events associated with traditional Chinese medicine(TCM), especially liver injury related to traditional non-toxic TCM, which requires adequate attention. Liver injury related to traditional non-toxic TCM is characterized by its sporadic and insidious nature and is influenced by various factors, making its detection and identification challenging. There is an urgent need to develop a strategy and method for early detection and recognition of traditional non-toxic TCM-related liver injury. This study was based on national adverse drug reaction monitoring center big data, integrating methodologies such as reporting odds ratio(ROR), network toxicology, and computational chemistry, so as to systematically research the risk signal identification and evaluation methods for TCM-related liver injury. The optimized ROR method was used to discover potential TCM with a risk of liver injury, and network toxicology and computational chemistry were used to identify potentially high-risk TCM. Additionally, typical clinical cases were analyzed for confirmation. An integrated strategy of "discovery via big data, identification via dry/wet method, confirmation via typical cases, and precise risk prevention and control" was developed to identify the risk of TCM-related liver injury. Corydalis Rhizoma was identified as a TCM with high risk, and its toxicity-related substances and potential toxicity mechanisms were analyzed. The results revealed that liver injury is associated with components such as tetrahydropalmatine and tetrahydroberberine, with potential mechanisms related to immune-inflammatory pathways such as the tumor necrosis factor signaling pathway, interleukin-17 signaling pathway, and Th17 cell differentiation. This paper innovatively integrated real-world evidence and computational toxicology methods, offering insights and technical support for establishing a risk discovery and identification strategy for TCM-related liver injury based on real-world big data, providing innovative ideas and strategies for guiding the safe and rational use of medication in clinical practices.

6.Blood sugar management in aged patients with Type Ⅱ diabetes based on theoretical domain framework:a qualitative study

Objective To investigate the requirement of blood sugar management in elderly patients with Type Ⅱ diabetes and to provide a reference for development of a program in management of blood sugar.Methods Using interpretive decriptive phenomenological research,an interview outline based on the theoretical domain framework was developed and face-to-face semi-structured interviews were conducted with 16 aged patients who had Type Ⅱ diabetes in the Department of Endocrinology and Metabolism of a Grade ⅢA hospital in Jiangxi Province.The participants were selected by purposive sampling.Colaizzi 7-step analytical method,in conjunction with Nvivo 11 software,was used to analyse,summarise and extract the themes.Results Seven themes related to the theoretical domain framework were extracted,which included knowledge,skills,environmental context and resources,behavioural adjustment,motivation and goals,emotions and self-efficacy.After refinement,nine sub-themes on major requirement for blood glucose management were extracted,which included the requirements for knowledge and skills in blood sugar management,support from the community and healthcare resources,support from the family,changes in maladaptive behaviours,individualised blood sugar management,motivation for self-management,clarity in goals of blood sugar control,emotional support and psychological adjustment,and support for strengthened beliefs(9 sub-themes).Conclusion Elderly patients with Type Ⅱ diabetes have diverse and individualised needs in management of blood sugar.In the future,healthcare staff should develop appropriate strategies or programs to meet the individual needs of patients by addressing the actual needs of patients,improving the self-management behaviours of patients so as to improve the effects of the blood sugar control.
7.Effect of sustained platelet aggregation inhibition by tirofiban on arterial duct closure in neonatal canines

Objective:To investigate whether repeated tirofiban injections can continuously inhibit platelet aggregation in the arterial duct and affect its closure in neonatal canines.Methods:Four 24-month-old pregnant beagles underwent cesarean sections in two batches (two dogs per batch) 1-2 days before the expected delivery date at the Xuzhou Medical University Animal Experiment Center. The first litter of 21 neonates served as the control group (receiving 10 ml/kg normal saline) and were randomly divided into 1-h ( n=7, injected immediately after birth), 4-h ( n=7, injected at 0 h and 2 h after birth), and 12-h subgroups ( n=7, injected at 0 h, 2 h, 4 h, 6 h, 8 h, and 10 h after birth). The second litter of 18 neonates served as the experimental group (receiving 10 ml/kg tirofiban) with identical subgroup assignments ( n=6 per subgroup). Echocardiography was performed at 1 h, 4 h, and 12 h after birth to measure arterial duct inner diameter, maximum shunt velocity, and left atrial diameter/aortic root diameter (LA/Ao) ratio. Plasma platelet-derived growth factor (PDGF) was detected by enzyme-linked immunosorbent assay, while platelet membrane glycoprotein Ⅱb-Ⅲa in the arterial duct was assessed by Western blot and immunohistochemistry. Data were analyzed using t-tests, one-way ANOVA, Chi square tests, or Fisher's exact test. Results:No significant bleeding tendency occurred in either group. Two control neonates (one each in the 4-h and 12-h subgroups) died. In both control and experimental 1-h subgroups, all arterial ducts remained open, with no significant differences in ductal diameter, shunt velocity, or LA/Ao between groups (all P>0.05). In the 4-h subgroups, all experimental neonates had patent ducts arteriosus, while two controls exhibited closure; the experimental group had larger ductal diameters [(1.05±0.05) vs. (0.55±0.44) mm, t=-2.75, P<0.05)] and higher LA/Ao ratios (1.31±0.09 vs. 1.14±0.03, t=-4.90, P<0.05), but lower maximum shunt velocities [(107.06±17.47) vs. (153.74±12.78) cm/s, t=4.54, P=0.002). In the 12-h subgroups, all the controls had closed arterial ducts, while four of six experimental neonates exhibited closure, though the difference in closure rate was not statistically significant (6/6 vs. 4/6, Fisher's exact test, P=0.455). Plasma PDGF and glycoprotein Ⅱb-Ⅲa levels did not differ between two 1-h subgroups (all P>0.05). However, the 4-h and 12-h experimental subgroups showed lower PDGF levels [(373.5±13.1) vs. (880.3±80.2) pg/ml, t=10.81; (356.7±35.0) vs. (1 111.2±125.3) pg/ml, t=9.74; both P<0.05] and reduced glycoprotein Ⅱb-Ⅲa expression (0.32±0.07 vs. 0.80±0.23, t=3.29; 0.42±0.07 vs. 0.92±0.26, t=3.24; both P<0.05) compared to controls. Conclusion:Repeated tirofiban injections sustainably inhibit platelet aggregation in the arterial duct of neonatal canines and delay ductal closure, suggesting that intraductal platelet aggregation may be one factor influencing this process.

9.Analysis of risk factors for intolerance of valsartan with Sakubatrol in patients with heart failure

AIM:To investigate the risk factors for intolerance to sacubitril/valsartan in the treatment of heart failure.METHODS:A total of 124 heart fail-ure patients hospitalized in our hospital from May 2023 to May 2024 were collected.Logistic regres-sion analysis and nomogram analysis were used to identify the risk factors for drug tolerance.RE-SULTS:22 patients exhibited intolerance,with low blood pressure being the primary cause(P<0.05).Multivariate regression analysis found that patients with a low BMI(OR 95%CI 1.814(1.385-2.525))and those without concomitant right heart failure(OR 95%CI 4.998(1.552-17.301))were more likely to ex-perience drug intolerance.The model had a stable C-index of 0.923(95%CI=0.863-0.975).CONCLU-SION:Low BMI and absence of right heart failure are risk factors affecting the tolerance to sacubitril/valsartan,which has certain value for guiding clini-cal treatment.Large-sample clinical studies are still needed to confirm these findings.
10.Establishment and application of JEV,PRRSV and CSFV TaqMan triple RT-qPCR method

To establish a TaqMan-based multiplex RT-qPCR method for the identification of Japa-nese encephalitis virus(JEV),Porcine reproductive and respiratory syndrome virus(PRRSV),and Classical swine fever virus(CSFV),this study designed and synthesized three pairs of specific primers and probes based on the conserved sequences of JEV E,PRRSV ORF6,and CSFV E2 a-vailable in the NCBI GenBank.By optimizing the reaction system and protocol,a multiplex RT-qPCR method for detecting these three viruses was developed and applied to the detection of clini-cal samples.The results showed that the established TaqMan multiplex RT-qPCR specifically am-plified the gene fragments of JEV,PRRSV,and CSFV,and did not amplify other non-target genes,indicating good specificity of the method.Intra-assay and inter-assay repeatability tests showed that the coefficient of variation(Cv)values were all below 3%,demonstrating that the method has ex-cellent repeatability.Sensitivity tests revealed that the minimum detectable amount for the recom-binant plasmids of the three viruses was 100 copies/pL.Using the established method,a total of 969 samples,including blood,aborted fetuses,semen,and deceased pigs,from 26 pig farms in Guizhou Province were tested.The detection rates were 34.3%(332/969)for JEV,28.3%(274/969)for PRRSV,and 19.8%(192/969)for CSFV.The co-infection rates were 10.1%(98/969)for JEV and PRRSV,12.1%(117/969)for JEV and CSFV,and 14.6%(141/969)for CSFV and PRRSV.Additionally,the triple co-infection rate of JEV,PRRSV,and CSFV was 7.9%(77/969).These results indicate that the TaqMan multiplex RT-qPCR method developed in this study is ef-fective for detecting these three viruses in pig farms,providing technical support for identifying vi-ral causes of reproductive disorders.
