1.The comparison of diagnostic value of FNAB and CNB in the treatment of patients with invasive breast cancer

Objective To investigate and compare the diagnostic value of fine needle aspiration biopsy(FNAB) and core needle biopsy(CNB) in the treatment of patients with invasive breast cancer.Methods Given 100 cases of patients with newly diagnosed breast cancer biopsy in our hospital from November 2015 to November 2016,50 cases of patients with FNAB into the observation group,50 cases of patients with CNB into the control group.Taken the pathologic results as a diagnostic gold standard and compared the diagnostic value of two diagnostic methods in invasive breast cancer.Detected the human epidermal growth factor receptor 2(Her-2),estrogen receptor(ER) and progesterone receptor(PR) preoperatively and compared the adverse reactions of puncture of patients in two groups.Results 30 cases of patients in the observation group showed axillary lymph node metastasis and 20 cases of patients had no axillary lymph node metastasis,21 cases of patients were positive cases of axillary lymph node metastasis accepted FNAB examination,the sensibility was 70.00%.29 cases of patients in the control group showed axillary lymph node metastasis while 21 cases of patients didn′t have axillary lymph node metastasis,23 cases of patients were positive cases of axillary lymph node metastasis accepted FNAB examination,the sensibility was 74.19%.Compared the ER、PR、Her-2 antibody with the testing result of primary tumor after operation.The diagnostic accordance rates in the observation group were 93.55%,91.67%,92.86% respectively.The diagnostic accordance rates in the control group were 94.12%,92.31%,86.67%,the differences in two groups were not statistical significance(P>0.05).The adverse reaction rate was 4.00% in the observation group and 0.00% in the control group after puncture.Conclusion The diagnostic sensibility of FNAB is poorer than CNB in the diagnostic procedure of invasive breast cancer.ER,PR,Her-2 in two diagnostic methods keep a high consistency with the testing result of primary tumor after operation.
2.Clinical observation of coaxial microincision phacoemulsification for beginners

To investigate the methods of learning coaxial microincision phacoemulsification and assess the related complications.METHODS:The clinical data of 1 080 eyes of coaxial microincision phacoemulsification performed by beginners from October 2011 to December 2013 was retrospectively analyzed. ln order of surgery dates, the patients were equally divided into groups A, B, C and D, 270 eyes in each group. Comparisons were made among the four groups in operation time, cumulative dissipated energy (CDE), rate of intraoperative, and postoperative complications at 1d; and best corrected distance visual acuity (BCDVA) at 1mo postoperatively.RESULTS:No significant differences were found in age, sex, lens grading, BCDVA at 1mo postoperatively among the four groups ( P> 0. 05 ). The operation time was significantly longer in group A than in groups B, C and D (25. 15 ± 3. 11min vs 15. 20 ± 3. 40min, 14. 71 ± 3. 02min and 14. 41±2. 91min, P<0. 05). No significant differences were found in the operation time among the other three groups (P>0. 05). The CDE of grade ‖ and Ⅲ was significantly higher in group A than that in groups B, C and D (95. 9%, 97. 8% and 98. 5%) (P<0. 01). The number of continuous curvilinear capsulorhexis ( CCC ) were performed successfully was significantly less in group A ( 80. 7%) than the other three groups (95. 9%, 97. 8% and 98. 5%) (P<0. 01). The incidence of posterior capasular rupture of group A (7. 8%) was higher than that of group B (3. 7%), group C (1. 1%) and group D (0. 4%) (P<0. 05), but there was no significant difference between group C and group D (P>0. 05). There were no significant differences both in the number of corneal edema and BCDVA at 1d between group A and group B. (P=0. 66, P=0. 53).CONCLUSlON: With learning step by step, appropriate training program, careful case selection, and accurate managing complications, coxial microincision phacoemusification can be safe for a beginner.
3.Impact of celecoxib on prognosis of elderly patients with hip fracture

One hundred and six patients with hip fracture admitted in hospital from 2011 to 2014 were randomly divided into two groups: 52 patients received celecoxib and parecoxib ( trial group ) and 54 patients received oxycodone hydrochloride and bucinnazine (control group).On d1 and d7 after the surgery, serum levels of C-reactive protein[(26.2 ±5.4) and (10.4 ±2.8) mg/L], tumor necrosis factor-α[(40.6 ±6.1) and (31.8 ±5.3)ng/L], interleukin-6 [(22.3 ±5.7) and (18.3 ±5.2) ng/L] and prostaglandin E2 [(181.1 ± 19.4) and (153.3 ±25.8) ng/L] in trial group were all significantly lower than those in control group [(46.1 ± 14.0) and (22.1 ±5.2)mg/L, (53.2 ±5.3) and (44.6 ±7.4) ng/L, (43.3 ±11.0) and (29.1 ±8.6)ng/L, (354.3 ±105.9) and (251.0 ±37.3)ng/L, all P<005].The length of hospital stay [(11.0 ±2.2) d], rate of lung infections (6/52) and neurological abnormality (3/52) in trial group were significantly lower than those in control group [(15.5 ±1.8) d, 15/54 and 11/54, all P <0.05].The expression of CD4 +CD45 + T cells [(34.9 ±3.7)% vs.(42.5 ±4.3)%] was reduced and CD4 +Foxp3 +T cells [(19.7 ±3.8)% vs.(6.9 ± 1.5)%] was increased at 24 h after surgery in trial group compared with control group( all P<0.05).The results indicates that calecoxib can reduce the inflammatory reaction by promoting CD4 + to Foxp3 + CD4+T cell differentiation and may improve the prognosis of elderly patients with hip fracture.

7.Simultaneous determination of nine chemical markers of bletillae rhizoma by ultra performance liquid chromatography.

A UPLC method has been developed in the current investigation for simultaneous determination of nine chemical markers of Bletilla striata, 4-hydroxymethylphenyl beta-D-glucoside, blestroside, dactylorhin A, militarine, dihydrophenanthrene 5, gymnoside V, dihydrophenanthrene 1, benzylphenanthrene 3 and gymnosides IX. Separation was performed at 45 degrees C on an ACQUITY UPLC BEH C18 column (2.1 mm x 150 mm, 1.7 microm) with a gradient solvent system of acetonitrile-water as the mobile phase. The flow rate was 0.3 mL x min(-1), the detection wavelength was 280 nm. The results showed that the nine chemical markers could be well resolved and that in the selected linear range, all calibration curves of the nine chemical markers showed good linearity (r > or = 0.999 3). The recoveries (n = 6) were in the range of 98.15% - 102.2% and RSDs were between 2.1% - 3.6%. The data suggested that the developed UPLC-UV method had good reproducibility, robustness, and accuracy, which was suitable for the quality control of Bletilla striata. Applications of the method showed that the nine chemical markers had higher contents in the wild B. striata than in the cultivated ones.

8.In vitro absorption mechanism of strychnine and the transport interaction with liquiritin in Caco-2 cell monolayer model.

To study the effect of liquiritin (Liq) on the transport of strychnine (Str) in Caco-2 cell monolayer model, the transport parameters of Str, such as apparent permeability coefficient (P app (B-->A) and P app (A-->B)) and cumulative transport amount (TRcum), were determined and comparatively analyzed when Str was used solely and co-used with Liq. The effect of drug concentrations, conveying times, P-glycoprotein (P-gp) inhibitor verapamil and conveying liquor pH values on the transport of Str were also investigated. The results indicated that the absorption of Str in Caco-2 cell monolayer model was well and the passive transference was the main intestinal absorption mechanism of Str in the Caco-2 monolayer model, along with the excretion action mediated by P-gp. Liq enhanced the absorption of Str. Meanwhile, conveying liquor pH value had significant influence on the excretion transport of Str.
9.Clinical utility and safety of low dose milrinone in patients suffering from refractory heart failure and renal dysfunction

Objective To observe the effect and safety of low dose milrinone used in patients suffering from refractory heart failure and renal dysfunction. Methods Forty-two patients with refractory heart failure and renal dysfunction were divided into treatment group(21 cases ) and control group(21 cases )by random digits table. All the patients accepted a therapy of cardiac booster, diuretics and vasodilators, and treatment group also accepted the therapy of milrinone [0.375 μ g/( kg· min), 10 mg/d, for 7 days]. And then the symptom, signs, blood pressure, heart rate, heart function and renal function before and after the treatment were observed. Results The total effective rate in treatment group was 85.7%( 18/21 ) ,significantly higher than that in control group [57.1% (12/21)] (P <0.05=. After treatment,the heart rate,systolic blood pressure,diastolic blood pressure,stroke volume,cardiac output and left ventricular ejection fraction in treatment group and control group improved significantly than those before treatment, and these index improved better in treatment group [(79.3 ± 12.4) beats/min vs. (85.4 ± 10.2) beats/min, ( 107.6 ± 15.4)mm Hg ( 1 mm Hg = 0.133 kPa) vs.( 119.1 ± 13.5 ) mm Hg, (60.8 ± 9.4) mm Hg vs. (65.8 ± 8.5 ) mm Hg,(66.3 ± 10.2 ) ml vs. (61.2 ± 9.3 ) ml, (5.3 ± 0.6 ) L/min vs. (4.8 ± 0.9) L/min, (56.6 ± 8.4 )% vs. (48.9 ±7.3)% ,P < 0.05=. In two groups,there were no statistical difference in renal function. Conclusions Low dose of milrinone can improve the heart function of the patients with refractory heart failure and renal dysfunction and has good renal safety.
