2. Noise-induced hearing loss and cochlea neurotransmitters: Recent progress

Being the primary neurons in the auditory system, spiral ganglion neurons form the afferent synapse with hair cells and play a critical role in the analysis and integration of hearing. Many kinds of neurotransmitters can be found in the cochlear and they have different effects under both physiological and pathological conditions with complicated mechanism. This article reviews the characters of some transmitters in cochlear, such as glutamate, ATP, dopamine and GABA, and their roles in noise-induced hearing loss, hoping to provide more clues for research and therapy of inner ear diseases.
3.Analysis on macular choroidal thickness of patients with myopic choroidal neovascularization

? AlM: To examine choroidal thickness and its relationship with axial length and spherical equivalent by spectral domain optical coherence tomography ( SD -OCT ) in patients with myopic choroidal neovascularization ( mCNV) .
?METHODS: We prospectively recruited 30 cases of mCNV in Beijing Anzhen Hospital from October 2010 to December 2014. SD-OCT was used to measure choroidal thickness subfoveally and at 1. 5mm superiorly, inferiorly, nasally and temporally ( SFCT, SCT1. 5mm , lCT1. 5mm , NCT1. 5mm , TCT1. 5mm ) . Paired t- test was conducted to assess the difference of spherical equivalent, axial length and choroidal thickness between mCNV group and controls. Logistic regression analysis was performed to evaluate the correlation between spherical equivalent, axial length, choroidal thickness and mCNV.
?RESULTS: The difference of spherical equivalent and axial length between mCNV group and controls was not significant (P>0. 05). lnferior was decreased in mCNV group compared with controls (140. 85±33. 46μm vs 168. 95±45. 36μm). The difference was significant (P=0. 008). Logistic regression demonstrated that decreased choroidal thickness ( lCT1. 5mm ) at inferior location was associated with mCNV (OR=2. 12;95%CI 1. 35~3. 28;P=0. 02).
?CONCLUSlON: The decrease of lCT1. 5MM may be one of the risk factors of mCNV.

MicroRNA are endogenous non-coding small RNA composed of approximately 18 - 23 nucleotides, mainly participating in post-transcription gene regulation. More and more data show that miR-21 is over-expressed in many human cancer cells and it was involved in cell differentiation, proliferation and apoptosis, closely related to tumorigenesis and the development of carcinoma as oncogene. Here, we will make a review about miR-21' s target gene, biosynthesis and regulation and clinical significance in order to provide a valued data for the future study and clinical application.
6.Effect of Various Kind of Dyslipidaemia on Insulin Resistance and Pancreatic Islet Function in Newly Diagnosed Type 2 Diabetic Patients

Objective To evaluate the effect of various kinds of dyslipidaemia on insulin resistance and pancreatic islet ? cell function in newly diagnosed type 2 diabetic patients. Methods 263 patients with newly diagnosed type 2 diabetes were divided into normal lipidemia group and dyslipidaemia group according to ATPⅢ standard. The dyslipidaemia group included 3 subgroups as the following: high TG group, high TC group, and mix group with high TG and TC. Homeostasis model assessment (HOMA) was applied to estimate insulin resistance (IR) and pancreatic islet ? cell function. Early insulin secretion index (EISI) and area under curve of insulin (AUCI) at all OGTT time points were calculated. Results Incidence of dyslipidaemia in the type 2 diabetic patients was 63%. Since Fujita first described uvulopalatopharyngoplasty(UPPP)in 1981,UPPP and its modified procedures have been widely used to treat obstructive sleep apnea and hyponea syndrome(OSAHS).However,despite of its wide application,the success rates was uncertain,ranging from 20% to 80%,with patients of varing Friedman stages.It is well known that the principle of UPPP is to remove the redundant tissue of palate,elongated uvula and hypertrophic tonsils in order to widen the anteroposterior space at the level of palate.But recently,surgeons have found that not only the collapse of soft palate but also the collapse of lateral wall at the palate level can contribute to the obstruction of upper airway at the level of palate.As a result,many surgeries which can widen the lateral velopharyneal space have sprung up in these years.This review focuses on the development of techniques that emphasize the enlargement of lateral velopharyneal space in patients with OSAHS.

8.How does transmembrane electrochemical potential drive the rotation of Fo motor in an ATP synthase?

While the field of ATP synthase research has a long history filled with landmark discoveries, recent structural works provide us with important insights into the mechanisms that links the proton movement with the rotation of the Fo motor. Here, we propose a mechanism of unidirectional rotation of the Fo complex, which is in agreement with these new structural insights as well as our more general ΔΨ-driving hypothesis of membrane proteins: A proton path in the rotor-stator interface is formed dynamically in concert with the rotation of the Fo rotor. The trajectory of the proton viewed in the reference system of the rotor (R-path) must lag behind that of the stator (S-path). The proton moves from a higher energy site to a lower site following both trajectories simultaneously. The two trajectories meet each other at the transient proton-binding site, resulting in a relative rotation between the rotor and stator. The kinetic energy of protons gained from ΔΨ is transferred to the c-ring as the protons are captured sequentially by the binding sites along the proton path, thus driving the unidirectional rotation of the c-ring. Our ΔΨ-driving hypothesis on Fo motor is an attempt to unveil the robust mechanism of energy conversion in the highly conserved, ubiquitously expressed rotary ATP synthases.

The academic connotation of "pathogenic factors" is discussed in this article from three aspects of etiology,diagnosis and treatment.It is considered that endogenous or exogenous pathogenic factors oppose to and destruct healthy energy.TCM pathogenic factors,obtained from "examining syndromes for causes of diseases",are diverged from physical,chemical and biological those in modern medicine.What is meant by "therapy of expelling pathogenic factors" is not a single method of dispelling toxin and killing germs,but the resort of regulating the integrated abilities of the human body fighting against pathogenic factors.
10.Evaluation of the change of anterior chamber parameters before and after laser peripheral iridectomy in primary angle -closure suspetive with Pentacam anterior segment analysis system

?AlM:To investigate the sensitive parameters of the anterior chamber changes with Pentacam anterior segment analysis system before and after laser peripheral iridectomy (LPl) in primary angle-closure suspetive (PACS).
? METHODS: Sixty eyes of 33 PACS patients were enrolled in this study. Pentacam examination was performed before and 1d after LPl to measure the central anterior chamber depth ( CACD ) , the peripheral anterior chamber depth ( PACD ) , the anterior chamber volume ( ACV) and the peripheral anterior chamber angle ( ACA) . Statistical analysis used paired t test.
?RESULTS: There was no statistical significance on the changes of ACD. PACD and ACV increased significantly between before and 1d after LPl. ACA was widened from (22. 26o±5. 18o) to (26. 42o±5. 20o), which were increased significantly between before and 1d after LPl.
?CONCLUSlON: LPl can deepen the PACD and increase the ACV in PACS. PACD and ACV are the sensitive parameters of the anterior chamber changes with Pentacam anterior segment analysis system.
