Objective To study the characteristics of serum lipid metabolism in cadres aged 90 and above.Methods A total of 105 in-patient cadres aged 90 and above were assigned to the longevity group,and another 85 in-patient cadres aged 60 to 80 years were used as controls.Medical history of the participants were retrospectively reviewed,and serum levels of total cholesterol(TC),triglyceride(TG),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),apoB,apoA-I,Lp(a)and body mass index(BMI)were tested.Results Compared with the control group,TC,TG,LDL-C,apoB and non-HDL-C were significantly decreased in the longevity group[3.90 +0.89 vs 4.36 ± 1.06,0.89(0.57)vs 1.16(0.69),2.35 ±0.72 vs 2.70 ±0.88,0.68 ±0.19 vs 0.75 ±0.22,2.69 ±0.79 vs 3.18 ± 1.00,respectively].However,HDL-C/TC and HDL-C/LDL-C were significantly increased in the longevity group [0.32 ±0.084 vs 0.28 ±0.082,0.52(0.22)vs 0.43(0.28),respectively].Lp(a)of the longevity group was higher than that of the control group,although no statistically significant difference was found.BMI of the longevity group was significantly lower than that of the control group.In the longevity group,the percentage of individual who were combined with hypertension,hyperglycemia,hyperlipidemia and overweight/fat was lower (24.8％ vs 43.5％).Conclusions Serum lipid level is relatively lower in cadres aged 90 and above,and those who are combined with hypertension,hyperglycemia,hyperlipidemia and overweight/fat are significantly lower in this populations.

Type B ultrasonic scanning was performed routinely for 10 944 pregnant women at their third semester when they visited outpatient department or were admitted to our hospital during January 2004 to December 2008, and their fetal heart rate, amniotic fluid, umbilical cord and neonate conditions were monitored and recorded during delivery. The sensitivity and specificity of type B ultrasonic were 95.74% (3301/3448) and 96. 25 % (7215/7496). Umbilical abnormality was diagnosed in 3448 pregnancies by type B ultrasonic scanning, with prevalence of 31.51%, cesarean section was performed in 44.00 %, fetal distress occurred in 18. 27% of them, 15.46% of them with Apgar score equal to or less than seven, and fetal death occurred in 2. 41% of them, all significantly higher than those in pregnant women without umbilical abnormal (23. 77%, 8. 56%, 7.84% and 1.36%, respectively, all P ＜0. 01 ). Cesarean section was performed in 41.90 % of those with 0. 8 cm or more in the notch depth of nuchal cord entanglement,fetal distress occurred in 29.76% of them, 25.30% of them with Apgar score equal to or less than seven and fetal death occurred in 4. 05% of them, all significantly higher than those in pregnant women with the notch depth less than 0. 8 cm (20. 22%, 8. 60%, 9. 26% and 0. 54%, respectively, all P ＜0. 01 ). Type B ultrasonic scanning plays an important role in clinical diagnosis for umbilical abnormality, guiding selection of delivery method and improving neonatal prognosis.

With the frequent culture communication between china and Japan, YanQi theory of TCM spread to Japan and exerted an influence on Japanese medicine. From the volume of"YunQi" in "The literature investigation of TCM", we can tell YunQi theory occupied a major position in the Japanese medical literatures which spread from china. The primary books of YunQi edited by Japanese scholars reveal Japanese doctors are apt to use this theory to promote their clinical practice. The accordant cognition of Chinese and Japanese scholars is that Japanese doctors accept the value of YunQi theory and beware of it's deficiency in clinic.

Objective To investigate the clinical characteristics,surgical resection extent and prognosis influencing factors of pri-mary high-risk gastrointestinal stromal tumors(GIST).Methods The clinicopathological and follow-up data of 48 patients with pri-mary resectable high-risk GIST in this hospital from January 2005 to January 2013 were retrospectively analyzed.The influence of three kinds of different surgical resection methods on the prognosis of GIST was analyzed.Results 34 cases were treated by R0 re-section(70.83%),6 cases by R1 resection(11.50%)and 8 cases by R2 resection(16.67%).All cases were given the postoperative adjuvant therapy of imatinib mesylate.The average medication time was(26.5±13.6)months.The occurrence rate of adverse drug reactions was 100%,and the main symptoms were swelling and leukopenia.The 5-year survival rates in 3 kinds of resection modes were 97.06%,66.67% and 25.00% respectively.The univariate and multivariate prognostic analysis showed that the surgical re-section extent and the tumor rupture were the independent prognostic factors in the high-risk GIST patients.Conclusion The intact operative resection and preventing the tumor rupture are the keys to increase the postoperative survival rate of GIST patients.

Objective To explore the efficacy and safety of terminating biochemical pregnancy (the stage in which intrauterine or ectopic pregnancy cannot be confirmed) with mifepristone and misoprostol. Methods Mifepristone (150 mg) combined with misoprostol (600 ?g) 3 days later were given to 500 biochemical pregnancies (G_1),500 early clinical pregnancies (G_2) and 500 clinical pregnancies (G_3) which were classified according to amenorrhea days,serum human chorionic gonadotropin-beta subunit (?- hCG) and vaginal B-ultrasonic examinations.All were observed for 6 hours after taking misoprostol and returned for assessment per week.Results Expulsion of conceptus was G_1 123 (24.6%,123/500),G_2 438 (87.6%,438/500) and G_3 467 (93.4%,467/500).Failure rate was G_1 6 (1.2%,6/500),G_2 24 (4.8%,24/500) and G_3 79 (15.8%,79/500) for ongoing pregnancies,hospitalizations for suspected ectopic pregnancies and surgical intervention for heavy or long-time bleeding.Bleeding cases during the administration of mifepristone were G_1 272 (54.4%,272/500),G_2 141 (28.2%,141/500) and G_3 87 (17.4%,87/500);the mean bleeding days were G_1 (5.8?1.5),G_2 (9.0?2.9) and G_3 (14.3?5.9) days.Other side effects including abdominal pain,nausea,vomiting and diarrhea were low and light in each group,increasing with advancing gestational age.Menses recovery was 486 (97.2%,486/500),452 (90.4%,452/500) and 433 (86.6%,433/500) for each group on scheduled time.Satisfaction was 499 (99.8%,499/500),485 (97.0%,485/500) and 369 (73.8%,369/500) respectively.Conclusion Mifepristone and misoprostol in combination is as safe,and effective for termination of biochemical pregnancies as ordinary medical abortion.It does not need to wait till ectopie pregnancy is excluded.

Background and purpose:Osteosarcoma is the most common primary malignant bone tumor in the children and young adults. Until now, there is no prognostic indicator with high speciifcity and high sensitivity. This study investigated the relationship between miR-92a level in palsma and clinical pathological characteristics, as well as the prognosis in osteosarcoma.Methods:The plasma from 30 healthy volunteers who underwent physical examination and from 45 cases of osteosarcoma before and after operation were collected. The miR-92a level was detected by real-time lfuorescent quantitative polymerase chain reaction (RTFQ-PCR) method. According to the relative expression≥5 of miR-92a, the osteosarcoma patients were divided into 2 groups: miR-92a high expression group and low expression group.Results:The miR-92a level in palsma from osteosarcoma is signiifcantly higer than that from healthy volunteers. Before surgery, the miR-92a level was markedly higher than after surgery. There was no correlation between the miR-92a level and the gender, age, tumor location, tumor size and histological type. However, signiifcant correlation was found with Enneking grade, tumor cell necrosis rate and lung metastasis. Compared with the low-expression group, high miR-92a expression was associated with a signiifcantly shorter overall survival using Kaplan-Meier analysis (P=0.035). Conclusion:The results suggest a signiifcant relationship between miR-92a and survival ratio. miR-92a expression level may be a useful prognostic indicator in osteosarcoma.

ObjectiveTo investigate the effect of cardiopulmonary bypass (CPB) on the secretory function of islet cells in rabbits.MethodsTwenty adult New Zealand white rabbits of both sexes,weighing 2.5-3.0kg,were randomly divided into 2 groups ( n =10 each):sham operation group (group S) and CPB group.The rabbits were anesthetized with 3％ pentobarbital sodium 30 mg/kg.Blood samples were collected from the left femoral artery at 5 min after anesthesia (T1),immediately before CPB (T2 ),immediately after aortic clamping (T3 ),and at 5,35 and 75 min after aortic unclamping (T4-6) in the two groups for determination of levels of blood glucose,insulin and glucagons.Insulin resistance index was calculated.ResultsCompared with group S,the blood glucose concentration and levels of insulin and glucagons and insulin resistance index at T3-6 were significantly increased in group CPB ( P ＜ 0.05).ConclusionAlthough increase in blood glucose enhances the secretion of insulin in islet β cells,hyperglycemia cannot be compensated completely by the increased insulin during CPB in rabbits.The increase in blood glucose may be related to islet α cell resistance.
