1.Analysis on the Liabilities of Urban Public Hospitals in Jiangxi

Objective:To understand the liability situation of urban public hospitals in Jiangxi.Methods:Qualitative and quantitative analysis,descriptive analysis and comparative analysis were used to analyze the causes and changes of urban public hospitals' debt increase.Results:To meet the increasing demand for health diagnosis and services,debt infrastructure and the purchase of equipment coupled with inadequate financial investment were performed in public hospitals,which resulted in the sustainably increasing of liabilities level in urban public hospitals.Conclusion:Governments at all levels should increase the capital expenditure of hospital construction and equipment investment,strictly control the public hospital debt behavior,the implementation of the reform of public hospitals canceled drug addition,establishing the characteristics of the medical industry personnel salary system and other measures to resolve the synchronous speed of public hospital debt and prompt public hospital debt risk control in the reasonable scope.
2.Transplantation of fresh amnion and preserved amnion for corneal alkali burn

AIM:Recently,amnion transplantation has been one of effective methods for curing corneal alkali burn,but its biomechanism is unclear.This study was to discuss the efficiency of different kinds of amnion on rabbit corneal alkali burn and to find the bioactive functions of the amnion.METHODS:The experiment was carried out in the Central Laboratory of Experimental Animals at Chongqing University of Medical Sciences between January and July in 2004.All the disposals were in accordance with the ethical standard of animals.①Placenta from healthy puerperants underwent uterine-incision delivery were offered by Department of Obstetrics in the First Affiliated Hospital of Chongqing University of Medical Sciences.A total of 45 New Zealand rabbits were used for corneal alkali burn models and divided into fresh amniotic membrane(FAM),glycerin preserved amniotic membrane(GPAM),vacuum xeransis amniotic membrane(VXAM),alcolhol preserved amniotic membrane(APAM) and control groups,every group had 9 rabbits.②The FAM was prepared within 24 hours,and the preserved amnion were transplanted after 1 month.③The corneal opacity and corneal neovascularization were observed everyday during days 2-21 after operation and once every three days after days 21;corneal fluorescein staining was conducted to observe the repair of corneal epithelium on days 14,30,60 after operation;2 weeks,1 month and 2 months after operation,we did histopathological examination and scanning electron microscope observation.RESULTS:Forty-three rabbits were involved in the result analysis.①There were no significant differences in the corneal opacity and repair velocity of corneal epithelium among FAM,GPAM and VXAM,which were remarkably superior to APAM and controls.APAM group and control group had not distinct difference.②FAM had better effects on inhibiting corneal neovascularization than GPAM and VXAM.CONCLUSION:After transplantation,VXAM,GPAM and FAM can all accelerate corneal epithelial healing and lighten inflammation.FAM containing more bioactive materials,plays best.
3.Experimental study on the change of osteoinductive potential of bone in ovariectomized rat

Objective To exoplore the causes in healing difficulties of osteoporotic fracture, and to study the feasible approaches and methods for clinical treatment of osteoporotic fracture. Methods Forty female Sprague-Dawley rats, used as donor animals, were divided randomly into ovariectomized (OVX) group and control (CON) group, with each group of 20 animals. OVX group was subjected to bilateral ovariectomy and CON group was subjected to sham operation. Four months after surgery, the rats were sacrificed, and long bones of the extremities were harvested respectively for preparation of bone matrix gelatin (BMG) in terms of Urist's method. BMG implants obtained from OVX and CON rats were implanted into left and right femoral muscle pouches respectively of 17 female SD rats. The host rats were killed after 14-day feeding, and tissue block of BMG implants were removed for histologic study, alkaline phosphatase (ALP) activity, and calcium content (CA) determination, respectively. 4.Outcomes of different-method-preserved amniotic membrane graft after transplanted to rabbit occular surface

Objective To compare the effects of fresh amniotic membrane (FAM), glycerol-preserved amniotic membrane (GPAM), alcohol-preserved amniotic membrane (APAM) and amniotic membrane preserved under vacuum drying condition (VXAM) on rabbit corneal alkali burn and to observe the outcomes of the grafted amniotic membrane. Methods Thirty-six New Zealand rabbits were used to establish corneal alkali burn models and then divided into 4 groups according different preserved amniotic membrane they received (FAM, GPAM, VXAM and APAM), with 9 rabbits in each group. After transplantation, we observed the changes of amniotic membrane graft, corneal opacity, corneal neovascularization (CNV) with slit lamp microscopy. The amniotic membrane graft was examined by HE staining 10 d, 14 d and 1 month after transplantation. Results One month later, FAM graft, GPAM graft and VXAM graft showed better curative effect than APAM graft. Ten days after transplantation, the amniotic membrane graft began to dissolve and desquamate, and disappeared completely till 14 d after transplantation. Amniotic membrane tissues were not found under light microscope in any rabbit. FAM graft dissolved most quickly, and APAM graft was the most slowly. The dissolution of GPAM graft and VXAM graft had no significant difference. Conclusion After amniotic membrane transplantation, it dissolves and sheds instead of fusing with recipient tissues. Corneal epithelium grows under amniotic membrane graft. Amniotic membrane graft transplantation should be called as amniotic membrane patching.
6.Analysis of Clinical Characteristic Pathology and Prognosis in Infant with Intra-Abdominal Solid Tumor

ObjectiveTo analyze the relationship between clinical characteristic pathology and prognosis in infant with intra-abdominal solid tumor.MethodsFifty-two infants(less than 1 year old) with abdominal solid tumor from Apr.1998 to Feb.2007 in Shanghai Children's Medical Center and from Jan.2001 to Feb.2007 in Shanghai Xinhua Hospital were reviewed.The history of these children were reviewed.Features and clinical pathology of these children with their prognosis were analyzed and followed up by telephones and children return visit records from 5 months to 8 years.ResultsLess than 1 month,7 cases;1 month to 1 year old,45 cases.Teratoma 23 cases(44.23%),neuroblastoma 9 cases(17.31%),nephroblastoma 6 cases(11.54%),hepatoblastoma 5 cases(9.62%),epithelioid hemangioendothelioma of the liver 3 cases(5.77%),congenital mesoblastic nephroma 3 cases(5.77%),fusiform cell epithelioid hemangioendothelioma of pancreas 1 case(1.92%),hamartoma of the liver 1 case(1.92%),retroperitoneal small cell malignant tumor 1 case(1.92%).Benign:malignant=1:1.Among the benign tumor,male:female=1:1.Among the malignant tumor,male:female=2.33:1.0.All children were treated with tumor resection,and combined with chemotherapy for those whose tumors were malignant.ConclusionsAmong infant abdominal solid tumors,teratoma and neuroblastoma are much more than other tumors.The cases of benign tumors are almost as much as the malignant tumors.The benign tumors did not have sex differences,and had good prognosis after surgical resection.However,in malignant tumors,the incidence rate of male is obviously higher than female.Completely resection of those malignant tumors with chemotherapy would get little incidence of recrudescence and low case fatality rate.Early diagnosis and early treatment play an important role in prognosis.
7.Expression and cytotoxicity of a human interleukin-6 tumor necrosis factor derivative fusion protein (5'IL6-TNF△) in vitro

Objective :To develop an agent that is more active against receptor-bearing target cells without increasing the toxic effect on non-target cells. Methods :By the use of molecular biology techniques,we designed and constructed a fusion protein 5'IL6-TNF△ by connecting the human interleukin-6 (hIL-6) gene and a human tumor necrosis factor α derivative (TNF△) gene througha synthetic linker sequence followed by subsequent expression in E. Coli. Results: In cytotoxicity assay with myeloma cell line U266, the normal type of 5' IL6-TNF△ showed an antitumor activity 3 times higher than that of TNF△;and the antitumor activity of 5'IL6-TNF△ blocked by IL-6Rwas only 1/30 of that of normal type of 5' IL6-TNF△. Meanwhile,the 5'IL6-TNF△ blocked by an ti-TNF antibody did not show any cytotoxicity to U266 cells. In activity assay with L929 cells ,the toxic effect of the fusion protein was found 1/22 of that of TNF△. Conclusion: The 5'IL6-TNF△fusion protein might be a useful cytotoxic agent in cancer treatment.

Objective To study the mechanism of Bushen Granule (G ranule for rein forcing the kidney) for the treatment of chronic aplastic anemia (CAA). Methods The CAA patients were divided into the herbal group (60 cases) treated mainly by Bu shen Granule with syndrome differentiation and the Cyclosporin A group (16 cases ) tr eated mainly by Cyclosporin A for 6 months. The healthy control group consisting of 20 healthy subjects was set up. The expressions of CD3 +, CD3+CD28+, and CD3+CD95+ of the healthy control groups were compared with the herbal group and the Cyclosporin A group. 9.Protective effect of Niaoduqing on the fibrosis in adriamycin nephropathy rats

Objective To investigate the renoprective effect and its possible mechanism of Niaoduqing on the adriamycin(ADR)-induced nephropathy rats. Methods Forty eight male Sprague-Dawley rats were randomly divided into normal control(n=12), ADR-induced nephropathy(model, n = 12), Benazepril-treated ADR nephropathy(Benazepril, n=12)and Niaoduqing-treated ADR nephropathy(Niaoduqing, n =12)groups. The rat nephropathy model was established by adriamycin injection and unilateral nephrectomy. The rats were sacrifficed per batch at the 4th and 8th weekend.The pathological change of nephridial tissue, the 24-hour urinary protein excretion and renal function were examined. Immunohistochemistry was used to meassure the expression of fibronection(FN), collagenⅣ(COLⅣ), osteopontin(OPN). Results The 24-hour urinary protein excretion, BUN, Scr, triglyeride(TG), cholesterol(Cho)in model group were significantly higher than those in normal group(P<0.01), as well as more server glomerulosclerosis in kidney were observed in model group than those in control group(P<0.01), while the albumin(Alb)was lower (P<0.01). Compared with model group, the 24-hour urinary protein excretion, BUN, Scr, TG, Cho were significantly reduced and renal glomerulosclerosis was improved in Niaoduqing group(P< 0.01), while the Alb was higher(P<0.01). Conclusion Niaoduqing palys an important role in the prevention and treatment for nephropathy.
10.Relationship between ABCD3-I score and outcome in patients with acute minor ischemic stroke

Objective To investigate the value of ABCD3-I score in predicting the outcome of acute minor ischemic stroke. Methods Totally 255 patients were valued by ABCD, ABCD2, ABCD3, ABCD3-I and ESSEN score then the clinical characters, outcome and early progression of these patients were investigated. Results Forty-eight patients had poor outcome after 90 days. Univariate logistic regression indicated that the differences of hypertension, diabetes, cardiovascular disease, stenosis of criminal artery, abnormal signal in diffusion weighted imaging (DWI) and other clinical symptoms between poor outcome group and good outcome group were statistically significant (P<0.05). The AUC of ABCD, ABCD2, ABCD3, ABCD3-I and ESSEN score in predicting outcome of acute minor stroke was 0.791 0, 0.798 3, 0.827 9, 0.930 0 and 0.735 9 respectively. There was difference among patients with different ABCD3-I scores both in outcome and early progression. Conclusion ABCD3-I score can predict the outcome of acute minor stroke and supply a new method for personalized treatment.
