1.Application of intravenous flurbiprofen axetil combined with small dose of morphine for postoperative and preemptive analgesia

Objective To compare analgesic efficacy of intravenous postoperative and preemptive analgesia with flurbiprofen axetil combined with small dose of morphine and with morphine alone. Methods One hundred and twenty patients were randomly divided into four groups: group A,morphine 30 mg +0.9% NaCl for postoperative analgesia,n=30;group B,morphine 20 mg +0.9% NaCl for postoperative analgesia,n=30;group C,flurbiprofen axetil 100 mg + morphine 20 mg +0.9% NaCl for postoperative analgesia,n=30;group D,flurbiprofen axetil 100 mg before operation + morphine 20 mg +0.9% NaCl for postoperative analgesia,n=30.The drugs in each group were diluted to 100 mL and infused by a pump at a rate of 2 mL/h with a patient-controlled analgesia(PCA)bolus of 2 mL after a loading dose of 5 mL.The visual analogue scale(VAS),demanding times for PCA and incidence of side effects were recorded during the period of postoperative 24 h. Polypoidal choroidal vasculopathy(PCV) is a common disease of the retina,the disease is more found in colored race.In China,the mainly nosogenic population is the female about 60 years old.The pathogenesis of PCV is unknown and there is no specific medicine at present.The funduscopic present is orangered lesions,bleed and exudation.Indocyanine green angiography(ICGA) could make a definite diagnosis of PCV.In recent,the main treatment of PCV includes photodynamic therapy,intravitreal anti-VEGF therapy,surgery for treatment of complications and combined therapy.We review the treatment and diagnosis of PCV,and propose the further research direction.
3.Model of Multidisciplinary Team Treatment on Hepatoblastoma in Children

Objective To study the diagnosis and treatment of the model of multidisciplinary team treatment on hepatoblastoma in children.Methods Retrospective analysis of treatment and long term follow-up of 16 patients with hepatoblastoma(8 cases were boys,8 cases were girls;aged 3 months to 11 years old,average age was 2 years old) during Aug.1998 to Jan.2006 in Shanghai Children's Medical Center.Special in multidisciplinary team treatment,preoperative evaluate and operation.Results The morbility locum of 8 cases were in right liver lobe,8 cases were in left liver lobe.Fifteen cases of the clinical presentation were abdominal mass,1 case was peritonitis.According to International Society of Pediatric Oncology(SIOP) Pretext staging system,43.7%(7/16)cases were stage Ⅱ,43.7%(7/16)cases were stage Ⅲ,12.5%(2/16)cases were stage Ⅳ.In 7 cases of stage Ⅱ,6 cases were accepted tumor total resection,1 case was accepted emergency surgery because tumor was fracture.Patients were given ICE protocal.In 7 cases of stage Ⅲ,6 cases were able to undergo delayed tumor resection following open biopsy and subsequent treatment with chemotherapy 2-3 courses.One case gave up treatment because its had poor response to chemotherapy.In 2 cases of stage Ⅳ,1 case was received liver transplantation due to poor respond to chemotherapy.One case was not received any treatment.In 7 cases of stage Ⅱ,6 cases were alive,1 case died,3-year free survival rate was 100%(4/4).In 7 cases of stage Ⅲ,5 cased were alive,3-year free survival rate was 75%(3/4).Two cases of stage Ⅳ all died.Conclusions Surgery play a very important role in the treatment of hepatoblastoma in children.Chemotherapy has improved the resectability of the tumor.Multidisciplinary team treatment is an effective model for diagnosis and treatment on childhood hepatoblastoma.
4.Therapy of Rhabdomyosarcoma in Children

Objective To explore the management therapy of pediatric rhabdomyosarcoma.Methods Retrospective analysis of treatment of 29 cases with rhabdomyoscroma from Oct.1998 to Jun.2006.Primary sites included 5 cases of neck,1 chest wall,2 abdominal wall,12 retroperitoneal pelvic cavity and 9 extremity.Pathologic types included embryonal type 19(65.5%),alveolar 8(27.6%),pleomorphic 2(6.9%).According to IRS staging system they were Ⅰstage(n=2),Ⅱstage(n=6),Ⅲstage(n=9)and Ⅳstage(n=12).Most tumors were large than 5 cm(n=26).Results Twenty-seven cases were followed up but 2 of the cases were lost,19 cases got complete remission(4 cases relapsed or died after stopping treatment for 3 months to 3 years and 1 case relapsed after giving up treatment for 3 months),4 cases got partial remission(3 cases relapsed after 6 months diagnosed),4 cases got no response(3 of which deteriorated or relapsed and 1 case surved with tumor).Fourteen cases had constant remission between 5-77 months,averaging 22.9 months with average guitting time of treatment 16.9 months.Conclusion Total tumor resection,chemotherapy and radiotherapy play an important role in management of pediatric rhabdomyosarcoma.
5.Evaluation of soft tissue profile in 20 adults with unoperated isolated cleft palate

Objective To evaluate the soft tissue profile in adults with unoperated isolated cleft palate. Methods Twenty patients older than 16 years(median age,18.4) with unoperated isolated cleft palate were enrolled.Soft tissue lateral cephalograms were traced and compared with those of cleft operated patients and class-Ⅰocclusion individuals.The results of various measurements were analyzed with ANOVA by SPSS11.5. Results In adults with unoperated isolated cleft palate,there was restriction at the top of the upper lip,and the antelabrum of upper lip and the lower lip were found normal. Conclusion The soft tissue profile in adults with unoperated isolated cleft palate exhibits almost the same characteristics as the corresponding hard tissue,and there is covering over effect of the soft tissue to some extent.
6.Imaging of three-dimensional fetal echocardiography with spatio-temporal image correlation

Objective To discuss the optimal imaging conditions of spatio-temporal image correlatin (STIC). Methods Conventional 2D fetal echocardiography and 3D STIC volume collection were performed in 130 fetuses. The images obtained using two methods were scored and compared. Results Complete 3D volume collection with the initial view of four chamber view were abtained in 121 fetuses, the successful rate was 93.13%. There was no significant differences in total score between the two methods. Significant difference was found between the score of different condition groups.The highest scores mostly appeared in lateral four chamber heart in both method groups. The highest scores mostly appeared in 25-29 gestational weeks with 2D ultrasound, while 20-24 gestational weeks with 3D ultrasound, scores in long axis views of vessels were statistically different. Conclusion STIC has advantages over conventional ultrasonography, but needs conditions control and analysis skills.

Objective To study Chinese medicine and Western medicine preventing the anaphylaxis of ionic-type radiography agent,to solve the anaphylaxis in brain enhancment CT.Methods Eight hundred and forty cases have been diagnosed in brain enhancement CT at our hospital,from Fed.1995 to May 2000.Results Chinese medicine,ginger,pinellia and hormone medicine were all useful in preventing the anaphylaxis of ionic-type radiography agent in brain enhancement CT.Conclusion Chinese medicine:ginger and pinellia could prevent the anaphylaxis of ionic-type radiography agent.
8.Experimental study on the change of osteoinductive potential of bone in ovariectomized rat

Objective To exoplore the causes in healing difficulties of osteoporotic fracture, and to study the feasible approaches and methods for clinical treatment of osteoporotic fracture. Methods Forty female Sprague-Dawley rats, used as donor animals, were divided randomly into ovariectomized (OVX) group and control (CON) group, with each group of 20 animals. OVX group was subjected to bilateral ovariectomy and CON group was subjected to sham operation. Four months after surgery, the rats were sacrificed, and long bones of the extremities were harvested respectively for preparation of bone matrix gelatin (BMG) in terms of Urist's method. BMG implants obtained from OVX and CON rats were implanted into left and right femoral muscle pouches respectively of 17 female SD rats. The host rats were killed after 14-day feeding, and tissue block of BMG implants were removed for histologic study, alkaline phosphatase (ALP) activity, and calcium content (CA) determination, respectively. 10.Correlation study between the reason of heterotopic ossification after cervical artificial disc replacement and the degeneration of cervical facet joints

Objective To investigate the correlation between the reason of heterotopic ossification after cervical artificial disc replacement and the degeneration of cervical facet joints.Methods From May 2009 to May 2012,133 patients who had undergone cervical artificial disc replacement were included into this study.There were 74 males and 59 females with an average age of 42.63±4.15 years old (range,23-56 years old).There were 109 patients who had undergone single level implant,and 24patients double level implant.Cervical spine A-P and the flexion-extension X-rays were taken to assess the range of motion (ROM)of the surgical level.Degeneration degree of patients' cervical facet joints were evaluated by Park grading standards with the preoperative cervical CT scan images on GE-PACS system.Heterotopic ossification (HO) situation were evaluated by McAfee classification through cervical lateral X-ray film.Patients were divided into two groups (HO group and non-HO group) according to whether heterotopic ossification appeared during the follow-up period.The data were collected before surgery to the latest follow-up.Results The average follow-up time was 2.9 years (range,2.0-4.8 years).HO was detected in 25 patients (18.80%,25/133) at latest follow-up.The ROM of surgical level of patients in HO group was significantly lower than non-HO group (6.8°±3.9° vs 9.1 °±2.4°).In addition,the degeneration degree of cervical facet joints of patients in HO group was significantly higher than that in non-HO group.There was a significant correlation between preoperative cervical facet joint degeneration and the occurrence of HO after CDR (r=0.683,P=0.033).Conclusion There was a significant correlation between preoperative cervical facet joint degeneration and the occurrence of HO after CDR.Patients present HO after surgery have significantly higher degeneration degree of cervical facet joints than patients without HO.Patients with higher degree of HO may have a higher level of cervical facet joints degeneration.
