1.Influence of surgical resection extent on prognosis of high-risk gastrointestinal stromal tumors

Objective To investigate the clinical characteristics,surgical resection extent and prognosis influencing factors of pri-mary high-risk gastrointestinal stromal tumors(GIST).Methods The clinicopathological and follow-up data of 48 patients with pri-mary resectable high-risk GIST in this hospital from January 2005 to January 2013 were retrospectively analyzed.The influence of three kinds of different surgical resection methods on the prognosis of GIST was analyzed.Results 34 cases were treated by R0 re-section(70.83%),6 cases by R1 resection(11.50%)and 8 cases by R2 resection(16.67%).All cases were given the postoperative adjuvant therapy of imatinib mesylate.The average medication time was(26.5±13.6)months.The occurrence rate of adverse drug reactions was 100%,and the main symptoms were swelling and leukopenia.The 5-year survival rates in 3 kinds of resection modes were 97.06%,66.67% and 25.00% respectively.The univariate and multivariate prognostic analysis showed that the surgical re-section extent and the tumor rupture were the independent prognostic factors in the high-risk GIST patients.Conclusion The intact operative resection and preventing the tumor rupture are the keys to increase the postoperative survival rate of GIST patients.
2.Clinical analysis of 49 cases with Pseudo-Meigs' Syndrome: a report of two cases and a review of literatures over the last decade

Retrospective analyses were conducted for the clinical data of two cases with pseudoMeigs' syndrome at our hospital.Both had respiratory symptoms,such as cough and dyspnea.Radiological examinations revealed ascites,pleural effusion and pelvic mass.The definite pathological diagnosis was pelvic malignant tumor.After surgical tumor removal,ascites and pleural effusion disappeared without recurrence.Along with reviewed cases from PubMed in the last decade,a total of 49 cases had pseudoMeigs' syndrome.The age range was 11-73 years.Their clinical manifestations include dyspnea (78%),abdominal distension (69%),cough (14%),abdominal pain (12%),fatigue (10%),weight loss (6%) chest pain (4%),fever (4%),abdominal mass (4%),and oliguria (2%).CA125 was commonly elevated.The primary tumors in pelvic cavity accounted for 78%.And ovarian metastasis from colon cancer was one of the most common in metastatic tumor.
3.Association of CA repeats polymorphism in the promoter region of IGF-1 gene with metabolic syndrome

Objective To explore the relationship between CA repeats polymorphism in the promoter region of IGF-1 gene and MS in the Han nationality.Methods 1047 subjects were recruited from general population of Dongcheng District in Beijing.MS was diagnosed based on the criteria for MS in 2005 by IDF.Genomic DNA was extracted by standard methods.PCR,Genescan,Genotyper and direct sequencing were conducted to screen CA repeats polymorphism in the promoter region of the human IGF-1 gene.Levels of plasma glucose,lipids,serum insulin and IGF-1 were determined.BMI and ISI were calculated.Results The prevalence of MS in (CA) 19 homozygote was lower than that in (CA) 19 heterozygote (9.1% vs 18.3%,x2 = 8.55,P < 0.01) and without (CA) 19 (9.1% vs 24.0%,x2 = 18.05,P < 0.01).The level of serum IGF-1 had differences among the three groups [ (114.0 ± 52.6) μg/L vs (136.6 ± 80.5) μg/L vs (129.2±49.1) μg/L,F =3.16,P <0.05],(CA)19 homozygote had lower serum IGF-1 than (CA)19heterozygote and without (CA) 19.BMI,WC,TG,FIns,2hIns and ISI had difference among the three groups (P <0.05).Conclusions (CA)19 repeats polymorphism in the promoter region of IGF-1 gene was significantly associated with MS in Han nationality.
4.Inhibitory effect of arsenic trioxide on human glioma cell growth

AIM: To investigate the inhibitory effect of arsenic trioxide on human glioma cell line U251 growth, change of gene expression and intracellular calcium content. METHODS: MTT method was used to observe the growth inhibition effect. Cell cycle, positive rate of proliferation cell nuclear antigen (PCNA), apoptosis associated protein Fas and Bcl-2, and intracellular calcium ion (IECa~ 2+ ) levels were measured by flow cytometry in U251 cells treated with different doses of As_2O_3. Apoptosis was detected with annexin V-FITC+PI dual parameter. RESULTS: As_2O_3 inhibited the growth of U251 cells dramatically. There were obvious dosage-effect and time-effect correlations (P0.05). The cell cycle was arrested in G_2M phase. Apoptosis occurred in U251 cells treated with As_2O_3 by annexin V-FITC+PI dual parameter detection. CONCLUSION: As_2O_3 inhibits the growth of U251 cells in vitro dramatically and induces apoptosis. The mechanism is probably associated with the improvement of Fas expression and IECa~ 2+ levels, decrease in PCNA protein expression and cell cycle arrest.
5.A novel citrinin derivative from the marine-source fungus Penicillium citrinum.

A novel citrinin derivative, penicitrinol L (1), along with two known analogues, penidicitrinin B (2) and pennicitrinone A (3) were isolated from the marine-source fungus Penicillium citrinum. The structure of the new compound was elucidated by spectroscopic methods including one and two-dimensional NMR as well as high-resolution mass spectrometric analysis. Furthermore, compound 1 showed modest cytotoxic activity against HL-60 cell line and compound 3 showed weak cytotoxic activity against A375 cell line.

6.Establishment of a public-good oriented mechanism for the financing & compensation mechanism of public hospitals

Access of safe, effective, convenient and affordable medical and health care services to the people is set as the goal of public hospitals health services as stated in the "Guidelines for Public Hospitals Pilot Reform". Such a goal, however, can hardly be achieved by mechanism changes within the public hospitals. The input and output of public hospitals rely critically on such factors as the input mechanism, regional pattern of healthcare system, the economic and industrial conditions of the region,payment quota and payment approval procedures of medical insurance. This paper set off from the input compensation mechanism of public hospitals in China and analyzed the input and output characteristics of the hospitals. Furthermore, the paper proposed to establish an input compensation mechanism based on public welfare performance for such hospitals. This provided further reference for improving the management of public hospitals and building a public welfare-oriented operation mechanism of public hospitals.
7.Association of CA repeats polymorphism in the promoter region of the insulin-like growth factor- Ⅰ gene with type 2 diabetes

Objective To investigate the association of CA repeats polymorphism in the promoter region of human IGF-I gene with type 2 diabetes in narthen Han nationality. Methods Gender-age matched subjects with type 2 diabetes and normal glucose tolerance (NGT) were enrolled for this study, 147 subjects in type 2 diabetes group and 159 subjects in NGT group. Genomic DNA was extracted by standard methods. PCR, Genescan,Genotyper, and direct sequencing were conducted to screen CA repeats polymorphism in the promoter region of the human IGF-I gene. Results No significant association was observed between any ( CA), repeat genotype and type 2 diabetes. A lowered serum uric acid was seen in genotypes that included alleles with larger than (CA) 20 repeats [(4.18 ± 1.25 vs 4.63 ± 1.36) mg/dl, P = 0.03]. Conclusion Alleles with larger than (CA) 20 repeats may be a protective factor for type 2 diabetes.
10.99Tcm-MDP bone scan for the diagnosis of SAPHO syndrome

Objective To investigate the clinical value of bone scintigraphy for the diagnosis of synovitis, acne, pustulosis, hyperostosis and osteitis (SAPHO) syndrome. Methods The characteristics of SAPHO syndrome on 99Tcm-MDP imaging of 8 cases were retrospectively analyzed. Results The 99TcmMDP bone scan was positive for all patients. Seven cases were found lesions in the costoclavicular region, 3cases involved with unilateral sacroiliac joint, 2 cases involved with spine and 1 patient with extremity. Four lesions were found without relevant clinical symptoms. Conclusion Bone scintigraphy is useful for the diagnosis of SAPHO syndrome, especially for those lesions with no clinical symptoms.
