Recently, the rates of various traffic accidents and industrial injuries accordlng to industrial progression were increased very much. In those injuries, open tibial shaft fractures were one of the most common ones of all long bones of the body. The authors have reviewed and analyzed 30 cases of the open tibial shaft fractures which were treated by pin and plaster method at our hospital from April 1976 to August 1979. The results were as follows: 1. Most of the fractures occurred in 20 and 30 age levels, average age was 34.4. Male 5 > female 1 in accident frequency. 2. The causes were traffic accident (67%), direct blow (17%), falling down from a height (13%), and mechanical trauma (3%) in order. 3. In location of fractures, Lt. 16 > Rt. 11 > both. 3; lower (47%), middle (33%), upper (20%), one thirds and comminuted (60%) > transvers (20%) > oblique (13%) > spiral (4%) > segmental fractures in frequency. 4. The associated injuries were fractures of fibulas (67%) > multiple skin damages (40%) > knee joint patella injuries... etc. Total of those injuries were 68.; One patient gained 2.26 kinds of those injuries simultaneously. 5. By classification of Ellis, minor severity (33%) > moderate one (40%) > major one fractures (27%). And according to results of pin and plaster treatment, bony union took roughly 11, 15, above 23 weeks respectively. 6. The summed contaminatlon and infection rate was below 20%, and cultured bacterias were E. Coli (50%) > staphylocci (33%) > pseudomonas (17%) and susceptabilities to penicillin were 60, 90, 25% respectively. 7. By classification of time interval from injury to apply pin and plaster method, early applied group (17 cases) showed bony union rate (7.6.4%), delayed one (11.7%), nonunion (5.8%), late group showed 50, 12.5, 6.3%, far late group showed 40, 40, 20% respectively. 8. Duration of the cast immobilization was 4–7 weeks and then it was changed by P. T. B. cast until about 4 monthes from injury, after it, vigorous P. T. was carried out. Good results were come in 80% (24 cases). 9. Pin and plaster method will be indicated in minor & moderate severity-fractures, but in major ones, it will be used as a preliminary stage operation.

Aneurysmal bone cyst is an uncommon tumor-like lesion of the bone containing distended vascular channels or cvities filled with blood. These cases, twenty-nine old female, eighteen years old male and female patients complained of pain and swelling in Rt. Medial aspect of proximal foot, Lt. proximal tibial portion and Rt. entire talar aspect of foot, respectively. Roentgenograms showed ballooned-out distensions and eccentric buldging of those lesions and soapbubble like multiple radiolucent cystic changes within interiors of the bones at talar neck, Proximal tibia and entire talus. Those cases were treated with radical curettage and autogenous iliac bone grafts and snugly fitted long leg casts in functional positions, respectively. On 2nd postoperative day, Cast windows were made and daily sterile dressings were carried out until complete wound healing. Pathological Examinations grossly and Microscopically revealed typical Aneruysmal bone cysts in all 3 cases. All-Patients had good therapeutic recovery within about seven weeks post-operatively.

Background: The aim of this study was to assess whether the anteroposterior coverage of the acromion reflecting acromial morphology affects the rotator cuff tear (RCT) and tear size, in addition to the lateral coverage. Methods: Medical records of 356 patients with RCTs, concentric osteoarthritis, and calcific tendinitis identified using threedimensional computed tomography between January 2016 and December 2017 were retrospectively analyzed. The patients were divided into group A (those with RCTs) and group B (those with concentric osteoarthritis or calcific tendinitis). Subsequently, group A was subdivided into three categories according to the size of RCTs: small-to-medium, large, and massive. The lateral coverage was measured through the lateral acromial angle (LAA) and critical shoulder angle (CSA), whereas the anteroposterior coverage was measured via the acromial tilt (AT), acromiohumeral interval (AHI) in the sagittal view, and anteroposterior coverage index (APCI) as a new radiologic parameter. Results: Between groups A and B, CSA (34.5° ± 3.4° and 30.8° ± 3.4°, respectively), APCI (0.83 ± 0.10 and 0.75 ± 0.08, respectively), and AHI (6.3 ± 2.0 mm and 7.8 ± 1.8 mm, respectively) were significantly different (all p < 0.001), whereas LAA and AT did not show a significant difference between the groups (p = 0.089 and p = 0.665, respectively). The independent predictive radiologic parameters of the RCT were the CSA, APCI, and AHI (p < 0.001, p < 0.001, and p = 0.043, respectively); among these, the APCI showed the highest regression coefficient (odds ratio = 2.82). The parameters associated with the size of RCTs were CSA (p = 0.022) and AHI, of which AHI, in particular, had the most significant effect on both small-to-medium and large tears (all p < 0.001). Conclusions Large CSA, high APCI, and low AHI were predictors of RCTs, with the APCI showing the strongest correlation. In addition to the large CSA, low AHI also correlated with the size of RCTs and affected the entire size groups. We suggest that both the lateral coverage and anteroposterior coverage of the acromion should be considered essential factors for predicting the presence of RCTs and tear size.

Spontaneous spinal epidural hematoma is a rare cause of spinal cord compression. Recently, at our hospital, the author et al had the opportunity of observing case with spontaneous cervical epidural hematoma in a 28 years old woman who had suddenly felt a strong pain in interscapular area. Rapidly after she noticed weakness in her legs and one day later she was completely paraplegia. The MRI of spine was done. There show tubular like low signal intensity at posterior epidural space of C7 level and ghigh signal intensity of cord at C7 level due to cord compression effect. In this article, we are adding one more rare case of spontaneous cervical spinal epidural hematoma confirmed by MRI.

Purpose: A survey was conducted to investigate the role, priority, and awareness of physical therapists in the community care system for the elderly. Methods: Nine hundred ninety-three physical therapists were invited to an online survey from 1st to 30th June 2020. Results: While the awareness of physical therapists for the community care system for the elderly was found to be low, the role priority and performance ability were found to be high. Fall training scored the highest for the job duties of physical therapists, and functional training scored the highest for the performance ability of physical therapists. For the role priority by the education status regarding the community care system, the participants who received the education program showed higher scores in the performance ability than those who did not. For the role priority and performance ability according to the education levels, higher education levels resulted in higher scores in both outcomes. Regarding the clinical experience, low scores were found in the participants with less than five years of experience. Conclusion Physical therapy establishes itself as an essential area of expertise in the community care system for the elderly. Therefore, these results can be utilized in developing a model for Korean community care for the elderly.

We have treated total 37 cases of femoral neck fracture in children who were under the 16 years old in age at the Catholic University Medical College since 1985, followed them for average 21 months to analyse their types of fracture and treatment, complications and prevention of complications, and obtained the following results. 1. According to the Delbet's classification, type I was 1 case, II were 18(49%), most common, III were 10 and IV were 8 cases. Their ages were average 8.2 years and boys were predominant in 27 cases. 2. Thirty-six cases were treated with open reduction and internal fixation within 1-13 days after fracture, but only one type IV was treated with closed reduction, followed by hip spica cast for 6 to 9 weeks in all. 3. The fixation devises were removed at average 8.7 months after operatio. 4. fifteen complications were observed in 11 cases(37%), that were AVN(3 cases), early physeal closure(8 cases), coxa vara(3 cases), coxa valga and limb shortening(each 1 case), and their causes were severe fracture displacement(2 cases), penetration of fixation devices to physis(8 cases), incomplete reduction and loss of initial reductio (1 case) etc, but were not correlated with their method of fracture treatment. Conclusively, the femur neck fracture in children could favorably be treated with open reduction and internal fixation to reduce their complications. Physeal early closures due to penetration of screws and pin were most common cause of complications, and their long-term follow-up should be necessary.

OBJECTIVE: To ananlyze the direct effect of nitric oxide (NO), generated from sodium prusside (SNP) on the embryo developments in reproductive process. DESIGN: ova from mouse were treated to allow fertilization in in vitro culture. And the samples of fertilized ova were alloted into five alliqutos. Each alliquot was cultured in media treated with either concentration at 0 (n=92), 25microM (n=84), 50microM (n=80), 100microM (n=77), 500microM (n=54) of SNP. Main Outcome MEASURE: Rates of embryonal cell cleavages, viability and cell morphology were assessed during in vitro fertilization and culture. RESULTS: As analyse the cell cleavage at 24 hours after in vitro culture of fertilised egg in variuos NO concentration, all of egg cells of each alliquot were developed to 2~4 cell stage. But the alliquot of egg cells treated with 500microM, which were totally degenerated. And also all embryonal cells of each alliquot were developed to 8 cell stage and morula stage on culture continuosly. And the embryonal cells of each alliquot were analysed at 24 and 48 hours following the in vitro culture. The rates of cell fragmentation and fusion were 4.2+/-3.4% in control group which is not treated with NO, while experimental groups was high, as rated 23.4+/-6.2% in 25microM, 28.2+/-5.7% in 50microM and 32.1+/-6.4% in 100microM concentration of NO. Accordingly the rate of abnormal morphology of embryonal cell in control was lower significantly than that in each alliquot of experimental groups (p<0.05). And the degenerated rates of embryonal cells were 0% in control, 17.8+/-6.7% in 25microM, 23.6+/-4.7% in 50microM and 26.8+/-11.2% in 100microM at 8 cells and morula on culture of 48 and 72 hours. On the examination of embryonal cells developed to blastocyst through in vitro culture, the rates of degenerated cells were 16.8+/-7.2% in control, 37.5+/-6.2% in 25microM, 73.4+/-4.6% in 50microM, 100% in 100microM. CONCLUSION: This results suggeted that the No in any concentrations is harmful on embryos in view of morphology as well as viability of cell, and the toxicity of No on embryo is stronger at condition in higher concentration of NO.

The cell cycle is composed of a series of steps which can be negatively or positively regulated by various factors. p53 gene aberrations are common in human malignancies, and recent studies suggest that in cervical carcinoma p53 function is inactivated either by complex formation wilh human papilloma virus (HPV) E6 product or by gene mutation. To study the expression of p53 gene in the cervical cancer and cervical intraepithebal neoplasia, immunohistochemistry for the p53 protein was done in the 47 cases of squamous cell carcinoma, 6 cases of adenocarcinoma and 32 cases of cervical intraepithelial neoplasia. I. The p53 protein was detected in the 31% of cervical intraepithelial neoplasia (10/32 cases). 2. The p53 protein was detected in the 55% of invasive cervical cancer (29/53 cases). 3. By the histologic type of cervieal cancer, the p53 protein was detected in the 57% of squamous cell carcinoma (27/47 cases) and 33% of(2/6 cases) adenocarcinoma. The p53 protein wes more frequently detected in the squamous cell carcinoma than in the adenocarcinoma. 4. By the staging in cervical cancer, the p53 protein was detected in the 31% of stage 0, 50% of Stage Ia, 50% of stage I b, 75% of IIa and 50% of stage II b.

OBJECTIVE: We retrospectively analyzed survival, local control rate, and incidence of radiation toxicities after radiosurgery for recurrent metastatic brain lesions whose initial metastases were treated with whole-brain radiotherapy. Various radiotherapeutical indices were examined to suggest predictors of radiation-related neurological dysfunction. METHODS: In 46 patients, total 100 of recurrent metastases (mean 2.2, ranged 1-10) were treated by CyberKnife radiosurgery at average dose of 23.1 Gy in 1 to 3 fractions. The median prior radiation dose was 32.7 Gy, the median time since radiation was 5.0 months, and the mean tumor volume was 12.4 cm3. Side effects were expressed in terms of radiation therapy oncology group (RTOG) neurotoxicity criteria. RESULTS: Mass reduction was observed in 30 patients (65%) on MRI. After the salvage treatment, one-year progression-free survival rate was 57% and median survival was 10 months. Age (<60 years) and tumor volume affected survival rate (p=0.03, each). Acute (< or =1 month) toxicity was observed in 22% of patients, subacute and chronic (>6 months) toxicity occurred in 21%, respectively. Less acute toxicity was observed with small tumors (<10 cm3, p=0.03), and less chronic toxicity occurred at lower cumulative doses (<100 Gy, p=0.004). "Radiation toxicity factor" (cumulative dose times tumor volume of <1,000 Gyxcm3) was a significant predictor of both acute and chronic CNS toxicities. CONCLUSION: Salvage CyberKnife radiosurgery is effective for recurrent brain metastases in previously irradiated patients, but careful evaluation is advised in patients with large tumors and high cumulative radiation doses to avoid toxicity.
