1.Subluxation of a Lateral Meniscus That Was Intact on Arthroscopic Probing

Several mechanisms for subluxation of the lateral meniscus, such as a bucket-handle tear, popliteomeniscal fascicle tear or meniscocapsular junction separation, have been recognized. The cause can be identified through a magnetic resonance imaging (MRI) examination and the arthroscopic findings, and treatment can proceed accordingly. The authors report a case of a 28-year-old male in whom the lateral meniscus was subluxated and reduced without any abnormal findings on arthroscopic probing contrary to preoperative MRI findings.
2.The Impact of Personality Traits on Ratings of Obsessive-Compulsive Symptoms.

OBJECTIVE: The goal of this study was to evaluate consistencies and discrepancies between clinician-administered and self-report versions of the Yale-Brown Obsessive Compulsive Scale (Y-BOCS) and to examine relationships between these scales and personality traits. METHODS: A total of 106 patients with obsessive-compulsive disorder (OCD) participated in this study. All participants were assessed with both clinician-administered and self-report versions of the Y-BOCS. The Structured Clinical Interview for DSM-IV Axis II Disorders Personality Questionnaire (SCID-II-PQ) was used to evaluate relationships between personality traits and scores on the Y-BOCS. RESULTS: Scores on the clinician-administered Y-BOCS and its obsession subscale were significantly higher than were those on the self-report version. However, we found no significant differences in compulsion subscale scores. We also found that the discrepancies in the scores on the two versions of the Y-BOCS and its compulsion subscale were significantly positively correlated with scores for narcissistic personality traits on the SCID-II-PQ. Additionally, narcissistic personality traits had a significant effect on the discrepancy in the scores on the two versions of the Y-BOCS and its compulsion subscale in the multiple linear regression analysis. CONCLUSION: This is the first study to elucidate relationships between personality traits and discrepancies between scores on the two versions of the Y-BOCS. Although clinicians tend to rate obsessive symptoms as being more severe than do patients, clinicians may underestimate the degree to which individuals with narcissistic personality traits suffer more from subjective discomfort due to compulsive symptoms. Therefore, the effect of personality traits on symptom severity should be considered in the treatment of OCD.

3.Nonleukemic Granulocytic Sarcoma in the Bile Duct: A Case Report.

Granulocytic sarcoma (GS) is an extramedullary tumor composed of immature myeloid cells, typically occurring during the course of acute myelogenous leukemia. Non-leukemic GS, that is, GS with no evidence of overt leukemia and no previous history of leukemia, is very rare, and even more unusual is nonleukemic GS of the bile duct. We report a case of nonleukemic GS of the bile duct. The patient was initially misdiagnosed as a bile duct carcinoma arising in the hilum of the liver (so-called Klatskin tumor), and received a right lobectomy of the liver. Histological examination of the tumor yielded the diagnosis of GS, and the bone marrow biopsy did not show any evidence of leukemia. Considering the risk of subsequent development of overt leukemia, the patient was treated with two cycles of combination chemotherapy as used in the cases of acute myelogenous leukemia. To date, he has remained free of disease 15 months after treatment.

4.A Case of Retransplantation in A Patient with Graft Loss Caused by Polyoma Virus Nephropathy.

Polyoma virus (PV) nephropathy is a known cause of graft loss after renal transplantation. In a renal transplant patient suspected of graft rejection, it is important to discriminate between PV induced interstitial nephritis and acute cellular rejection, because of similar pathologic findings. After the loss of the first allograft secondary to PV nephropathy, transplant graft nephroureterectomy before retransplantaton may have an influence in the recurrence of PV nephropathy. However, this question has not been completely resolved. Case: A 23-year-old male underwent first renal transplantation from his HLA haploidentical 25 year-old-sister. His renal function had been good with cyclosporine, steroid and azathioprine until 9 months after transplantation, when his serum creatinine level rose to 2.2 mg/dL. A renal biopsy revealed features of tubulitis and we confirmed PV nephropathy through a positive PV monoclonal antibody reaction to inclusion body. After gradual loss of graft function, he underwent hemodialysis. After 48 months of hemodialysis, the patient underwent cadaveric renal retransplantation without transplant graft nephroureterectomy. Thrombocytopenia and suspected delayed graft function occurred after 2 days of transplantation. A graft biopsy revealed thrombotic microangiopathy. Improved graft function was attained after a temporary stop of tacrolimus and ATGAM(R) bridging therapy. The patient is maintaining satisfactory graft function 33 months after retransplantation without clinical and serological evidence of recurrent PV infection.

5.Pharmacokinetic Modeling of Phagocytic Activity of the Liver Using Superparamagnetic Iron Oxide Nanoparticles in Dynamic MR Imaging.

The purpose of this study was to determine whether phagocytic activity is measurable by dynamic superparamagnetic iron oxide-enhanced MR imaging. For these experiments on New Zealand White rabbits, which were randomly allocated to normal and silica treated groups, we performed a dynamic MR study and radioisotope study with Tc99m-phytate. In this dynamic MR study, the ratio (Rv) of the distribution volumes of iron oxide (Vm/Ve) could be obtained by applying three- compartment model to the data obtained from the kidney and liver simultaneously. Changes in Rv caused by silica injection and by dosing superparamagnetic iron oxide, AMI-25, were evaluated. In the dynamic MR study using a Beagle dog model the input function could be calculated from data obtained from the hepatic artery and portal vein. Rv's reached maximum values at around 80 minutes after the AMI-25 injection. The Rv of the normal group was 5.06 +/- 1.53 whereas the Rv of the silica treated group was 2.13 +/- 1.20. The results were similar to tissue count data obtained by radioisope study. The Rv value was not dependent on the injected dose of AMI-25. The rate of transport constants (k1, k2, k3) could not be estimated using the 3 compartment model regardless of obtaining the input function. We conclude that Rv may be an quantitative index of decreased phagocytic activity in the liver as determined by dynamic superparamagnetic iron oxide- enhanced MRI.

6.A study of the panoramic radiographic images of the buccolingual dilaceration.

PURPOSE: We want to identify the appearance of the buccolingual root dilaceration teeth in the panoramic views and specify the characteristics of these teeth. MATERIALS AND METHODS: One thousand-six patients were examined on the basis of both panoramic and CT image criteria. We diagnosed and excluded certain teeth from the samples; both prosthodontic or pathologic lesion appearing teeth and mesiodistally dilacerated ones. We meticulously discerned buccolingually dilacerated teeth in the CT images and total 48 samples were selected. The degree of severity in dilaceration was standardized by 2 types of criteria. The samples were differentiated into 3 groups and again categorized into six types showing from the panoramic views: irregular view on the root apex area, clear blunt on the root tip, stepping on root tip, double lamina dura or double tip, arrow-target shaped root, bull's eye, normal view. RESULTS: The types of teeth selected from total 48 buccolingual root dilaceration samples were mandibular first and second molar, premolars, canines, and lateral incisors. The direction of dilaceration was an even percentage to each buccal and lingual side for most selected teeth, however, that of both canines and lateral incisors were directed in almost a buccal side. In the panoramic views, the root types of the buccolingually dilacerated teeth were irregular view on the root apex area, clear blunt on the root tip, stepping on root tip and normal types were almost always normal view. The more severity in dilareated degree, the more chances of observation in the panoramic views were clear blunt on the root tip and stepping on root tip. CONCLUSION: As observed in the shape of stepping on root tip or double lamina dura in the panoramic views, there can be much more probability to diagnose as a buccolingually dilacerated root.

7.Diagnostic imaging analysis of the impacted mesiodens.

PURPOSE: The research was performed to predict the three dimensional relationship between the impacted mesiodens and the maxillary central incisors and the proximity with the anatomic structures by comparing their panoramic images with the CT images. MATERIALS AND METHODS: Among the patients visiting Seoul National University Dental Hospital from April 2003 to July 2007, those with mesiodens were selected (154 mesiodens of 120 patients). The numbers, shapes, orientation and positional relationship of mesiodens with maxillary central incisors were investigated in the panoramic images. The proximity with the anatomical structures and complications were investigated in the CT images as well. RESULTS: The sex ratio (M : F) was 2.28 : 1 and the mean number of mesiodens per one patient was 1.28. Conical shape was 84.4% and inverted orientation was 51.9%. There were more cases of anatomical structures encroachment, especially on the nasal floor and nasopalatine duct, when the mesiodens was not superimposed with the central incisor. There were, however, many cases of the nasopalatine duct encroachment when the mesiodens was superimpoised with the apical 1/3 of central incisor (52.6%). Delayed eruption (55.6%), crown rotation (66.7%) and crown resorption (100%) were observed when the mesiodens was superimposed with the crown of the central incisor. CONCLUSION: It is possible to predict three dimensional relationship between the impacted mesiodens and the maxillary central incisors in the panoramic images, but more details should be confirmed by the CT images when necessary.

8.Relationship of serum lipid profile and the degree of depressive symptoms in Korean adults.

BACKGROUND: An inverse relationship between cholesterol level and the degree of depressive symptoms was found in several studies targeted to Westerners. But, other studies failed to replicate this finding, giving diverse opinion on this matter. The objective of this study was to investigate the association between the degree of depressive symptomps and lipid level, total calorie intake and fat intake respectively in adults screened in a health promotion center in Korea. METHOD: The study group consisted of 352 men and women who visited the Health Promotion Center of Seoul National University Hospital from June to August in 1998. Their depressive symptoms were measured by Korean version of the Center for Epidemiologic Studies Depression Scale(CES-D). Lipid level was measured by enzymatic methods with fasting blood specimen. Total calorie and fat intake were calculated from a self-reported questionnaire on the types and amount of food. RESULTS: Depressive symptoms were not significantly related to lipid levels, total calorie intake, fat intake, age, exercise, BMI, smoking or alcohol consumption(p>0.05). But sex(P<0.05), marital status(P<0.05), educational level(P<0.05) and average income(P<0.05) were significantly related to depressive symptoms in the whole study group(n=352). Educational level(P<0.05), menopausal status(P<0.05) and LDL cholesterol(P<0.05) were significantly related to depressive symptoms only in women(n=185). However in multivariate models which adjusted for confounding factors these associations were not found(p>0.05). CONCLUSION: In contrast to the results of previous studies, no significant association between the degree of depressive symptoms and lipid levels, total calorie intake and fat intake respectively were found in Korean adults after adjusting for several factors related to health.

9.Role of nitric oxide in penile erection.

The present study was undertaken to investigate the role of nitric oxide (NO) in erectile physiology by correlating its action with the existence and activity of nitric oxide synthase (NOS), which produces NO. We applied Western blot analysis in both human and rat penile tissue. In the rat, reduced nicotinamide adenine dinucleotide phosphate (NADPH) diaphorase staining and spectrophotometric assay were also performed, in addition to in vivo electroerection study with pharmacological manipulation. Western blot analysis identified a protein of 155 KDa identical to the neural form of NOS in the human and rat penis. The NOS blot densities in the two species were similar, and both were lower than that in the rat cerebellum. Histochemical staining localized NOS to neurons innervating the corpora cavernosa, including the pelvic plexus, the cavernosal nerves and their terminal fibers within the corporeal erectile tissue, and dorsal penile nerves. NOS activity was also found in the cerebellum, urethra, penis, and urinary bladder, in decreasing order of intensity. Intracavernous injections of NOS inhibitor (L-NOARG or L-NAME in concentrations from 10(-6) M to 10(-3) M suppressed electrostimulation-induced erection in a concentration-dependent manner. Subsequent intracavernous injection of L-Arginine (10(-2) M) partially restored the erection. The neural form of constitutive NOS in the corpora cavernosa synthesizes NO, which mediates penile erection. Determination of cavernosal NOS expression or activity may permit characterization of certain pathological conditions that cause impotence.

10.Prediction of Histologic Grade and Multidrug Resistance of Brain Tumors Using Brain SPECT.

OBJECTIVE: Computerized tomography(CT) and magnetic resonance imaging(MRI) are very useful in detection of structural change in the brain including tumors. However, they can not inform functional and biological behavior of such lesions. 99m Tc-Methoxyisobutyl isonitrile(MIBI) is considered as a substrate for MDR1 gene-encoded permeability glycoprotein(P-gp) and it has been used in the evaluation of multidrug resistance(MDR) in various tumors. The purpose of the study is to demonstrate the presence of MDR in brain tumor and brain tumor grading by an external imaging with 201Thallium and 99m Tc-MIBI SPECT. METHODS: 201Thallium and 99m Tc-MIBI SPECT were performed in 18 patients with malignant tumors and in six patients with benign tumors. Immunohistochemical staining(IHC) of the tumor specimen for P-gp was also performed. The histologic grading of the tumors and immunohistochemical staining for P-gp were compared to the dual brain SPECT findings of 201Thallium and 99m Tc-MIBI SPECT studies. Brain tumor with positive 201Thallium SPECT and negative 99m Tc-MIBI SPECT is considered to be multidrug resistance. An uptake index obtained from brain SPECT was used for tumor grading. MDR1 gene-encoded P-gp was assessed by immunohistochemical staining using a monoclonal antibody for P-gp. RESULTS: The malignant group showed significantly higher uptake indices in the 201Thallium and 99m Tc-MIBI SPECT than benign group. The uptake index of 99m Tc-MIBI SPECT was inversely correlated with P-gp immunohistochemical staining without statistical significance. CONCLUSION: 201Thallium and 99m Tc-MIBI SPECT are useful for predicting histologic grade of brain tumors, and 99m Tc-MIBI SPECT might be useful for predicting the presence of MDR protein.
