1.Clinical observation of gemcitabine in the treatment of elderly patients with advanced non-small cell lung cancer

Objective To investigate the clinical efficacy of gemcitabine in the treatment of elderly patients with advanced non-small cell lung cancer.Methods 50 patients in our hospital from January 2014 to September 2015 were selected as the research object, and were randomly divided into the control group and the experimental group, each group of 25 patients each.The patients in the control group were treated with special therapy, and the patients in the experimental group were treated with gemcitabine.The clinical indexes of the experimental group and the control group were compared and analyzed.Results After the treatment, the experimental group of 25 patients, 12 cases of patients with complete remission, 8 cases partial remission, 3 cases were stable, 2 patients progressed, the effective rate was 75%, clinical benefit rate of 92%.In the control group, 4 patients had complete remission, 2 patients had partial remission, 9 patients were stable, 10 patients progressed, the treatment efficiency was 24%, and the clinical benefit rate was 60%.The effective rate and the clinical benefit rate of the experimental group were significantly higher than that of the control group (P<0.05).1 years later, the survival rate of patients in the experimental group was 22, the survival rate was 88%, and the number of patients in the control group was 14.The survival rate of the experimental group was significantly higher than that of the control group, with statistical difference (P<0.05).Conclusion Gemcitabine monotherapy in the treatment of elderly patients with advanced non-small cell lung cancer can improve the therapeutic effect to a great extent, good safety, low incidence of adverse reactions, high survival rate, with further clinical promotion and application significance.

5.Protective effects of propofol pretreatment against ischemia-reperfusion injury in isolated rat heart

Objective To determine if nitric oxide(NO)is involved in the protective effect of propefolpretreatment(PP)against ischemia-reperfusion injury(I/R).Methods Eighteen male SD rats weighing 250-350 gwere randomly divided into 3 groups(n=6 each):A normal control;B I/R and C PP+I/R.The animals werekilled by a knock on the head.Hearts were immediately removed and passively perfused in a Langendorff apparatusat 37℃ with oxygenated(95% O_2,5% CO_2)Krebs-Hensleit(KH)solution at 90 cm H_20.In group B and C thehearts were subjected to 35 min global ischemia by suspension of perfusion followed by 120 min reperfusion.Ingroup C the hearts were perfused with KH solution containing 50 ?mol?L~(-1) propofol for 10 min followed by 10 minpropefol wash-out before I/R.At the end of reperfusion myocardial specimen was obtained from left ventricle andhomogenized for determination of the activities of total NOS(iNOS+cNOS)aand SOD and expression of iNOS andheme-oxygenase-1(HO-1)and content of NO,The relationship between the NO content and activity and/orexpression of these protein and enzyme were analyzed by correlation analysis.Myocardium was examined with lightand electron microscope.Results The myocardial NO content,tNOS activity and tSOD activity were significantlylower in group B than in group A and C and there was no significant difference in NO content,and activity of tNOSand tSOD between group A and C.The NO content was positively correlated with tNOS and negatively correlatedwith Mn-SOD aetivety and HO-1 expression.Microscopic examination showed severe cell injury or necrosis in groupB but little injury in group C.Conclusion Ischemia and reperfusion decrease activity of tNOS and SOD butincrease HO-1 expression resulting in decrease in NO content.Propofol pretreatment protects the heart from I/Rinjury through increase in NOS and antioxidases(Mn-SOD,HO-1).

7.Clinical Experience on Treatment of Lumbar Intervertebral Disc Protrusion by Traditional Manual Techniques plus Electric Acupuncture

52 cases of the patients with L4-S1 intervertebral disc protrusion were first treated by traditional Tuina manual techniques, including the rolling method, pressing method,oblique-plucking method and shaking method, and then treated by electric acupuncture on Shenshu (BL 23), Yaoyangguan (GV 3), Dachangshu (BL 25), Xiaochangshu (BL 27),Mingmen (GV 4) and Shangliao (BL 31). After 7-28 sessions of the treatments, the results showed cure in 40 cases, remarkable effect in 8 cases, effect in 3 cases, failure in 1 case, and the total effective rate in 98.1%.
8.Application of two placements in infant megacolon radical operation

Objective To compare the effects of two positions in the infant megacolon radical operation.Methods Sixty infants undergoing abdominal perineal megacolon radical operation were randomly divided into the control group and the observation group in equal number.The former took the traditional lithotomy position and the latter modified suspended lithotomy position.Results The operative exposures in the observation group were much more sufficient than that of the control group(P<0.05).While adverse skin reactions in the observation group were more uncommon than those of the control group(P<0.05).Conclusion The suspended lithotomy position may provide a better exposure and result in fewer adverse skin reactions during the abdominal perineal megacolon radical operation.

Objective: To establish a mini invasive way to evaluate endothelium function. Methods: Group 1 consisted of 10 subjects with no risk factors for coronary artery disease, group 2 consisted of 11 subjects with risk factors for coronary artery disease and angiographically normal coronary arteries. Two groups of patients underwent transesophageal echocardiography(TEE) for assessment of LAD diastolic blood flow velocity (V) and time velocity integral (VTI) at rest (B 1), 2 min after immersion of the hand in cold water (CPT), 5 min after rewarming of the extremity (B 2), and 5 min after administered nitroglycerin. 10.Effect of siRNA interfering SLC7a8 on uptaking L-dopa in NRK-52E cells

AIM: To investigate the effect of selective silencing of SLC7a8 on uptaking L-dopa in renal tubular epithelial cells of rat (NRK-52E). METHODS: The three siRNAs targeting SLC7a8 (siRNA-1, siRNA-2, siRNA-3) were designed and synthesized. A siRNA with nonspecific coding sequence (siRNA-con) was used for control. All siRNAs were transfected into NRK-52E cells. The siRNA-con transfected group, blank control group and gene-specific silencing SLC7a8 group were set up. The efficiency of transfection was estimated by flow cytometry. The efficiency of RNA interference was detected and screened by RT-PCR preliminarily, and was followed by Western blotting at protein level. The concentrations of L-dopa uptake into the NRK-52E cells were detected by ultraviolet spectrophotometry at different time points (6, 12, 24, 36, 48, 60, 72 and 120 min). RESULTS: The transfection efficiency was 94% detected by flow cytometry. The initial screening of RT-PCR showed that the efficiencies of RNA interference of siRNA-1 and siRNA-3 were higher, and siRNA-3 was the highest at protein level determined by Western blotting. No distinctive change was found between siRNA-con treated NRK-52E and blank control cells. The L-dopa uptake at different time points (6, 12, 24, 36, 48, 60, 72 and 120min) in siRNA-interference group was lower than that in siRNA-con transfected group and blank control group. No significant difference of L-dopa uptake between siRNA-con group and blank control group was observed. CONCLUSION: RNA interference technology selectively down-regulates SLC7a8 expression in rat renal tubular epithelial cells. The L-dopa uptake is also decreased after specifically silencing the slc7a8 expression.
