1.The Effects of Korean Medical Drug on Lymphocyte Activity in Allergic Contact Dermatitis.

The purpose of this study was to elucidate the effects of Hwangyun, Hwangyunhaedogtang, and Kumeunhwa on the lymphocyte activity in allergic contact dermatitis induced by contact allergen 1-chloro-2,4-dinitrobenzene (DNCB) of Balb/c mouse. For examination of a contact hypersensitivity, MEST (mouse ear swelling test) and lymphocyte proliferation assay measured by [(3)H]-methylthymidine incorporation were done. Cytokine mRNA in the draining lymph node cells were examined by RT-PCR. Contact hypersensitivity was more effectively induced by 0.25% DNCB treatment than 1% DNCB treatment. Local lymph node cell proliferation from DNCB-sensitized mouse was most highly elicitated with 100 ug/ml DNBS stimulation in vitro. The cytokine profiles of lymph node cells from DNCB-sensitized and DNBS-stimulated mouse were strong expression of IL-2 and IFN-r, weak expression of LT and IL-4, and no expression of IL-6 and IL-10. This lymphocyte proliferation was significantly inhibited in mice administered Korean medical drug for 10 days and sensitized with DNCB at day 5 (88.22%, 65.14%, and 52.29% in Hwangyun, Hwangyunhaedogtang, and Kumeunhwa, respectively). But Kumeunhwa was not effective in the inhibition of lymphocyte proliferation when administered after DNCB-sensitization. The cytokine expressions were also inhibited especially IL-2 and IFN-r in Hwangyun administered- mice. These inhibitions of lymphocyte activity by Korean medical drugs were also observed when stimulated with ConA (1 ug/ml). Conclusively, immune responses of contact hypersensitivity induced by DNCB are involved in local lymph node T cells mainly Th1 type cells, and Korean medical drugs especially Hwangyun suppressed allergic contact dermatitis via inhibition of the activity of these cells.

3.Correlation Analysis between SF36 and Polysomnographic Measures in Patients with OSAS.

BACKGROUND AND OBJECTIVES: Despite the trend in the medical field to understand the effect of diseases on the quality of life, such a study regarding sleep apnea has not been prevailing in Korea. This study analyzed the result of a Medical Outcome Study Short Form-36 (SF-36), which is widely used to evaluate the effect of sleep apnea on patient's medical condition. SUBJECTS AND METHOD: This study included 131 patients of over 18 years of age and who participated in the SF-36 survey and finished the polysomnography study. The relationship between parameters was conducted with Spearman, and the comparison with the independent variables was done by running a paired t-test. RESULTS: AHI was on average 30.9+/-23.5 and BMI was on average 26+/-3.9 kg/m. Except body pain, all values which derived from the statistics of eight categories of SF-36 from the patients was significantly lower than those of the healthy control. In the comparable analysis of polysomnography and SF-36, AHI, the total sleep time, and the sleep efficacy (%) had significant correlation, but correlation coefficient was low (r<3.0). CONCLUSION: It is recommended that the quality of life of OSAS patients should be evaluated beside PSG in order to find out the degree of seriousness of OSAS and result of treatment.

4.Prevalence of antibodies to Bartonella henselae and Bartonella quintana in Korean Patients with Lymphadenopathy.

BACKGROUND: Cat scratch disease (CSD) manifests as a self-limiting, regional lymphadenopathy, fever and constitutional symptoms following a cat scratch or close contact with cats. Bartonella henselae is a causative agent of CSD that is globally distributed. In Korea there are no case report of CSD and no reports on the seroprevalence of Bartonella henselae in patients with suspected CSD. In this study the prevalence of antibodies to Bartonella in patients with lymphadenopathies was evaluated. METHODS: We evaluated serological responses of 31 patients (8 male, 23 female) with regional lymphadenopathies. The age of the patients ranged from 6 to 76 years old (mean+/-S.D., 30.6+/-14.3 years). Final diagnoses on the lymphadenopathies were tuberculous lymphadenopathy, Kikuchi's disease, Kawasaki's disease, and viral infection, but considerable cases remained undiagnosed. All sera were examined using MRL Diagnostics Bartonella IgG indirect immunofluorescent antibody (IFA) test kits (MRL Diagnostics, Cypress, California, USA) according to the manufac-turer's instructions. RESULTS: Of the 31 patients, 21 (67.7%) patients showed positive titers of immunoglobulin G (IgG) to B. henselae (1 : 64 or higher). Twelve (38.7%) had IgG titers of 1 : 128 and four (12.9%) had titers of 1 : 256. Among these patients, three patients had a history of close contact with a cat or cat scratch. Twenty patients (64.5%) showed positive reactions against B. quintana. CONCLUSION: This study revealed possible patients with cat scratch disease, but, unlike other countries, Bartonella seropositivity was not closely associated with cat contact. The further studies should be performed in a large number of patients including a control group and be accompanied with culture or polymerase chain reaction of extirpated lymph node.

5.Prospective audit of post-chemotherapy febrile neutropenia in patients with solid cancer and lymphoma in two Singaporean cancer centres.

INTRODUCTIONFebrile neutropenia (FN) is a significant cause of mortality and morbidity in oncology and haematology units worldwide. The overall mortality in hospital surveys in Singapore surveys on post-chemotherapy FN has ranged between 3.0% and 8.8%. However, recent evidence indicates that outpatient management of patients with low-risk FN is safe and cost-effective.
MATERIALS AND METHODSWe conducted a prospective audit on a cohort of adult patients with post-chemotherapy FN seen at 2 local public sector cancer centres over a 1-year period in order to define their epidemiological characteristics and outcomes, and also to assess the uptake of early discharge/outpatient management strategies for these patients.
RESULTSWe reviewed 306 FN episodes from 248 patients. Patient characteristics and outcomes were similar between both institutions. Eleven (3.7%) FN episodes were managed as outpatient and none developed complications. Overall 30-day mortality was 6.6%, while the median length of stay (LOS) was 7 days (IQR: 4 to 11 days). The only independent risk factor for mortality was severe sepsis (OR:13.19; 95% CI: 1.98 to 87.7; P = 0.008). Factors independently associated with a longer LOS were vancomycin prescription (coefficient: 0.25; 95% CI: 0.08 to 0.41; P = 0.003), longer duration of intravenous antibiotics (coefficient: 0.08; 95% CI: 0.06 to 0.10; P <0.001), and prior review by an infectious diseases physician (coefficient: 0.16; 95% CI: 0.01 to 0.31; P = 0.034).
CONCLUSIONThis audit demonstrated that mortality from FN in our 2 cancer centres is low and comparable to international institutions. It also demonstrates that outpatient management of FN is safe in selected patients, and can be further expanded for right-siting of resources.

6.Effect of Bimodal Hearing in Speech Perception Under Noisy Environment According to Residual Hearing.

BACKGROUND AND OBJECTIVES: In this study, we evaluated the effect of bimodal hearing in the speech perception test and the increasing level of bimodal hearing over cochlear implantation in speech perception score regarding residual hearing. SUBJECTS AND METHOD: Nineteen prelingually deaf patients, who had used bimodal hearing over a period of 8 months, were divided in two groups in accordance to their low frequency residual hearing. The children were tested in open-set speech perception under +10 SNR and +20 SNR. The scores of bimodal hearing and unilateral cochlear implantation were compared by the Wilcoxon signed-rank test. Also, the increased level of speech perception scores of bimodal hearing over cochlear implantation alone were calculated using the formula, [bimodal score (%)-unilateral cochlear implantation (%)], and was compared between groups. RESULTS: In pure tone audiometry under silent condition, the result of bimodal hearing was similar to unilateral cochlear implantation in magnitude in both groups. Under the noisy condition, both groups had better result in bimodal hearing in terms of speech perception. Also, the increased level was higher in bimodal hearing over unilateral cochlear implantation in the better residual hearing group. However, since the sample size was small enough, it is considered that there was no statistical significance. CONCLUSION: Cochlear implanted patients with residual hearing are expected to get better speech perception in noisy environment with bimodal hearing regardless of the level of residual hearing.

7.Cytogenetic and molecular aberrations of multiple myeloma patients: a single-center study in Singapore.

BACKGROUNDMuch is known about the cytogenetic lesions that characterize multiple myeloma (MM) patients from the USA, Europe, and East Asia. However, little has been published about the disease among Southeast Asians. The aim of this study was to determine the chromosomal abnormalities of MM patients in our Singapore population.
METHODSForty-five newly-diagnosed, morphologically confirmed patients comprising 18 males and 27 females, aged 46 - 84 years (median 65 years) were investigated by karyotyping and fluorescence in situ hybridization (FISH). FISH employing standard panel probes and 1p36/1q21 and 6q21/15q22 probes was performed on diagnostic bone marrow samples.
RESULTSThirty-four cases (75.6%) had karyotypic abnormalities. Including FISH, a total detection rate of 91.1% was attained. Numerical and complex structural aberrations were common to both hyperdiploid and non-hyperdiploid patients. Numerical gains of several recurring chromosomes were frequent among hyperdiploid patients while structural rearrangements of several chromosomes including 8q24.1 and 14q32 characterized non-hyperdiploid patients. With FISH, immunoglobulin heavy chain (IGH) gene rearrangements, especially fibroblast growth factor receptor 3 (FGFR3)/IGH and RB1 deletion/monosomy 13 were the most common abnormalities (43.4%). Amplification 1q21 was 10 times more frequent (42.5%) than del(1p36) and del(6q21).
CONCLUSIONSWe have successfully reported the comprehensive cytogenetic profiling of a cohort of newly-diagnosed myeloma patients in our population. This study indicates that the genetic and cytogenetic abnormalities, and their frequencies, in our study group are generally similar to other populations.

Background:
Pyomyositis (PM) is a serious soft tissue infection and despite its clinical importance, previous studies have not been able to fully determine the clinical characteristics and microbial epidemiology of PM in Korea, which we therefore aimed to investigate.
Materials and Methods:
We retrospectively identified 140 adult patients diagnosed with PM from 13 general hospitals between January 2012 and December 2015. We analyzed the clinical and microbial characteristics of community-onset PM and compared them with communityacquired (CA) and healthcare-associated (HCA) PM.
Results:
One hundred eleven organisms were isolated from 96 (68.6%) patients with PM.Staphylococcus aureus (38 patients) was the most common pathogen, followed by streptococci (24 patients), and enteric Gram-negative organisms (27 patients). Methicillin-resistant S.aureus (MRSA) was identified in four (2.9%) patients and in-hospital mortality reached 8.6% (12/140). Enterococci isolates were identified in the HCA PM subgroup only The proportion of MRSA isolates was not comparable between CA and HCA PM subgroups. In the 83 patients with PM infected by monomicrobial pathogens, isolates of Gram-negative organisms were more commonly found in HCA PM subgroup than in CA PM subgroup (47.6% [10/21] of patients with HCA PM vs. 20.7% [12/58] of patients with CA PM; P = 0.01).
Conclusion
Gram-positive cocci such as S. aureus and streptococci were dominant etiologies in community-onset PM, whereas MRSA appears to an uncommon causative organism of PM in Korea. Enteric Gram-negative organisms should also be considered as major etiologies, especially in HCA PM patient population in Korea.

10.Influence of storage duration and processing on chromatic attributes and flavonoid content of moxa floss.

OBJECTIVEMoxibustion is an important traditional Chinese medicine therapy using heat from ignited moxa floss for disease treatment. The purpose of the present study is to establish a reproducible method to assess the color of moxa floss, discriminate the samples based on chromatic coordinates and explore the relationship between chromatic coordinates and total flavonoid content (TFC).
METHODSMoxa floss samples of different storage years and production ratios were obtained from a moxa production factory in Henan Province, China. Chromatic coordinates (L*, a* and b*) were analyzed with an ultraviolet-visible spectrophotometer and the chroma (C*) and hue angle (h°) values were calculated. TFC was determined by a colorimetric method. Data were analyzed with correlation, principal component analysis (PCA).
RESULTSSignificant differences in the chromatic values and TFC were observed among samples of different storage years and production ratios. Samples of higher production ratio displayed higher chromatic characteristics and lower TFC. Samples of longer storage years contained higher TFC. Preliminary separation of moxa floss production ratio was obtained by means of color feature maps developed using L*-a* or L*-b* as coordinates. PCA allowed the separation of the samples from their storage years and production ratios based on their chromatic characteristics and TFC.
CONCLUSIONThe use of a colorimetric technique and CIELAB coordinates coupled with chemometrics can be practical and objective for discriminating moxa floss of different storage years and production ratios. The development of color feature maps could be used as a model for classifying the color grading of moxa floss.
