1.Immunohistochemical Study on the Ha-ras p21 Expression in the Gastric Carcinoma.

We have investigated an immunohistochemical expression of the Human-Ha-ras oncogene product p21 in tumor cells of the primary mass and metastatic lymph nodes with different histological features of gastric cancer by using avidinbiotin complex immunoperoxidase method in formalin-fixed tissue sections from 73 cases of primary tumor mass and 23 cases of metastatic lymph node. Histologic type of the gastric cancer was classification. The results obtained were as follows: 1) Expression of Ha-ras p21 was consistantly increased in the well differentiated tubular adenocarcinoma as compared with poorly differentiated tubular adenocarcinoma (p<0.01), and was substantially decreased in mucinous carcinoma and signet ring cell carcinoma. 2) Signet ring cell carcinoma showed that positive immunoperoxidase reaction for Ha-ras p21 exhibited in the majority of immature signet ring cell with scant cytoplasm rather than in the mature signet ring cells which have abundant cytoplasm filled with mucin. This findings indicate that mucin production from the tumor cell was not correlated with activation of ras gene in the tumor tissue of gastric carcinoma. 3) In general Ha-ras p21 expression was enhanced in the metastatic tumor cells of the regional lymph node compared with primary tumor, especially it was consistantly increase in the well differentiated tubular adenocarcinoma.

3.Long-term Follow-up of Severe Blepharoptosis.

In case of severe blepharoptosis, frontalis muscle suspension with fascia or frontalis transfer has been popular for decades, but these static procedures have some disadvantages such as lagophthalmos, lid lag and remnant ptosis. Twenty-six patients with severe blepharoptosis who underwent frontalis suspension, frontalis transfer, or levator resection at Yonsei University Severance Hospital from 1980 to 1988 were studied. The follow-up period of patients ranged from 10 to 18 years with a mean of 12 years. Surveys and clinical results were obtained, In our review of postoperative complications, lagophthalmos persisted in sleep and even in forced eye closure in most patients 2 to 6 months after operation. But no recurrence was noted. In conclusion, undercorrection of ptosis is more desirable than overcorrection. As well levator resection can be considered in selected cases to prevent complications.

4.Mechanisms of Increase in Renal Blood Flow During Partial Ureteral Obstruction in Dogs.

Although tubuloglomerular feedback (TGF) is involved in ureteral obstruction-induced increase in renal blood flow (RBF), its contribution to RBF is not well established due to the concommitant increases in prostaglandin (PG) and renal interstitial fluid pressure (Pisf), both of which affect RBF one way or the other. Since Pisf and TGF are closely affected by renal hemodynamics, RBF will respond differently to increases in ureteral pressure depending on renal hemodynamic conditions. Therefore, the purpose of the present study was to investigate how the changes in renal hemodynamics affect the response of RBF to increases in ureteral pressure. The effect of PG on RBF was assessed by comparing the effects obtained before and after indomethacin, a cyclooxygenase inhibitor. Six anesthetized dogs were prepared with flow probes and inflatable silastic occluder around the renal artery, the ureteral catheter with its free end attached to a water reservoir, and the arterial and venous catheters. RBFs were obtained at ureteral pressures of 0, 15, and 40cmH2O during the maintenance of the renal artery pressure (RAP) at the level of systemic arterial pressure, 10mmHg above and below the lower autoregulatory limit of RBF (65+/-4 mmHg) both before and after indomethacin administration (10mg/kg). In response to the ureteral pressure of 40cmH2O, RBF increased from 172+/-6 to 185+/-10ml/min when RAP's were equal to systemic arterial pressure and decreased from 162+/-10 to 120+/-9 ml/min when RAP's were 55+/-4mmHg. Indomethacin pretreatment, depending on the level of RAP either prevented an increase or augmented a decrease in RBF in response to ureteral pressure elevation. This suggests that RAP-dependent changes in susceptibility of the renal venous system to compression by increased Pisf is the main mechanism by which the changes in renal perfusion pressure modulate the response of RBF to ureteral pressure elevation.

5.Longitudinal Supraspinatus Tear Associated with Antegrade Humeral Intramedullary Nailing: A Case Report and Literature Review with Focus Placed on Nail Entry Point.

Iatrogenic damage of the rotator cuff followed by postoperative shoulder function loss is a potential complication after antegrade intramedullary nailing (AIN) for a humeral fracture. The authors present a case of arthroscopic rotator cuff repair and subacromial decompression of a non-healed rotator cuff tendon (mainly supraspinatus) and secondary impingement syndrome caused either by the tear or a proud nail after AIN for a mid-shaft humeral fracture. At presentation, the patient complained of right shoulder pain and 'snapping', especially during forward elevation and abduction of the shoulder, of 4 years duration. Right shoulder pain started sometime after pain due to the humeral shaft fracture, operation had subsided, and persisted after nail removal. Arthroscopic findings showed a longitudinal rotator cuff tear at the nail entry point that had not healed and severe fibrous hypertrophy on the acromion underspace, which is a unique finding since most longitudinal splits of tendon fibers are more likely to heal than conventional rotator cuff tears detached from bone. The torn rotator cuff was repaired after debridement and placing side-to-side sutures. At his 34-month follow-up after rotator cuff repair, the patient showed complete recovery and had excellent clinical scores.

6.The Effect of theophylline on eotaxin mRNA expression in pulmonary epithelial cell line A549.

BACKGROUND: Eotaxin, a CC chemokine specific for eosinophils, is implicated in the pathogenesis of asthma recruiting eosinophils into the airways. Theophylline has been used for the treatment of asthma and rece was proposed to have an anti-inflammatory action. The aim of this study is to examine whether theophylline may inhibit the eosinophilic airway inflammation by reducing the expression of eotaxin. METHODS: The expression of eotaxin mRNA was assessed by Northern analysis in A549 cells 4 h after stimulation with TNF-α or IL-1β And then, theophylline was added to A549 cells stimulated with 0.1 ng/ml IL-1β. RESULTS: Eotaxin mRNA expression rates induced by 0.1, 1, 10 ng/mL TNF-α as compared with β-action were 7%, 22%, 28%, respectively. Eotaxin mRNA expression rates induced by 0.01, 0.1, 1, 10 ng/ml IL-1β as compared with β-action, were 10%, 42%, 63%, 72%, respectively. Eotaxin mRNA expression rates after addition of 0, 0.001, 0.01, 0.1 µM dexamethasone induced by 10 ng/mL TNF-α, as compared with β-action were 27%, 18%, 8%, respectively. Eotaxin mRNA expression rates after the addition of 0.001, 0.01, 0.1 mM dexamethasone induced by 0.1 ng/mL IL-1β, as compared with β -action, were 43%, 47%, 12%, 8%, respectively. Eotaxin mRNA expression rates after the addition of 0, 0.001, 0.01, 0.1, 1, 10 mM theophylline induced by 0.1 ng/mL IL-1β, as compared with β-action, were 48%, 40%, 33%, 22%, 16%, 14%, respectively. CONCLUSION: These results suggest that theophylline may reduce eosinophil infiltration of the airway at least in part by reducing the expression of eotaxin under the conditions of these experiments.

8.The in Vitro Proliferative Properties of Fibroblasts Originating from Upper and Lower Dermis of Psoriatic Skin Lesions.

To investigate the heterogeneity of fibroblast proliferation rate depending on the depth of dermis in the psoriatic skin lesions, fibroblasts were obtained from the upper and lower dermis of the forearm skin lesions in 9 psoriatir, patients and of the comparable sites in 7 healthy persons respectively, and were cultured. The in vitro proliferation rates of the fibroblasts were calculated by measuring the efficiency af cell attachment, cell numbers on varying days of culture and the population doubling time during the 3rd passage of subculture. The results were as follows: 1, The efficiencies of cell attachment at 24 hours after seeding were not statistically different between the upper derrnal fibroblasts and the lower ones in both psoriatic skin lesions and normal skins. 2. By rneasuring the cell numbers and the population doubling times, the prolifeatior rate of the upper der mal fibroblasts was greater than that of the lower ones in both psoriatic skin lesions and normal skins and that the fibroblastic proliferation rate was significantly inereased in the psoriatic skin lesion compared to the normal skin.
