PURPOSE: The aim of this study was to develop a baptist electronic Nutritional Screening Program (B-eNSP) that could be easily used to identify patients with nutrition at risk or malnutrition on their admission to hospital using an integrated medical information system and to evaluate validity, reliability and efficiency of B-eNSP. METHODS: From June 1 2009 to June 21 2009, we enrolled 496 patients for prospective research. We analyzed age, sex, weight, height, weight loss in the last 3 months, serum albumin, alteration of dietary intake, gastrointestinal symptom, functional capacity, diagnosis and its relationship to nutritional requirements, physical examination and Subjective Global Assessment (SGA). B-eNSP included body mass index (BMI), weight loss in the last 3 months, serum albumin. Each component was scored. Sensitivity and specificity were calculated to evaluate the validity of the B-eNSP. The receiver operating characteristic (ROC) curve was drawn by using B-eNSP to choose a cut-off value that maximizes sensitivity and specificity and Yoden Index. Comparison with SGA and the reliability of the B-eNSP was done using kappa statistics. RESULTS: The maximum Yoden Index was 0.866 and the cut-off value of the ROC curve was 2. A B-eNSP score higher than 2 was defined as nutrition at risk or malnutrition. Reliability of the B-eNSP was in accordance with SGA by kappa 0.845. CONCLUSION: The B-eNSP can be used efficiently to identify patients with nutrition at risk or malnutrition by simply using an integrated medical information system.

We report a case of MALT lymphoma in a 49-year-old woman. Her disease occurred simultaneously in the mucosa of her right upper eyelid conjunctiva and in her left nostril as ulcerating tumors associated with itchy ichthyosiform skin lesions on the trunk, hyperkeratotic palms and soles, and dystrophic nails. Histopathological examinations revealed consistent findings of MALT lymphoma with dissemination; i.e., diffuse infiltrates of lymphoplasmacytoid cells with a few Russel body-like structures, eosinophils, some shoddy granulomas under the irregularly hyperplastic epidermis, and diffuse infiltrates of CCL ( centrocyte-like ) cells and small lymphocytes inside and outside many lymphoid follicle-like structures in the subcutaneous tissue forming florid lymphoepithelial lesions. She died after 27 months duration of her disease with worsening of ichthyosiform skin lesions and dystrophic nails in spite of total excision of the tumors. We discuss the clinical and histopathologic features of MALT lymphoma with dissemination and the various similar diseases to differentiate.

9 Cases of femoral head fractures treated in Seoul National University Hospital were reviewed. They were classified according to Pipkin, and assessed according to Ebstein's criteria. The longest follow-up was 3 years and the shortest 6 months, the average being 1 year and 6 months. l. 8 cases were injured due to auto accident and there was no one who had worn seatbelt. 2. 1 case of intestine rupture and 2 cases of homothorax were accompanied. 3. There were 1 excellent, 1 good, and 1 poor results in 3 cases of Pipkin type 1, 1 fair result in 1 case of Pipkin type 2, 1 good and 1 fair results in 2 cases of Pipkin type 3, 1 good and 2 fair results in 3 cases of Pipkin type 4. 4. The following complications were encountered, 2 cases of sciatic nerve palsy, 1 case of avascular necrosis of femoral head, 1 case of traumatic arthritis.

BACKGROUND: Mild cognitive impairment represents a cognitive state between the subject memory impairment and early dementia. Proper cognitive interventions at this period can delay its development into dementia. However, it is yet uncertain whether cognitive interventions for people affected with leprosy and mild cognitive impairment have beneficial effects. OBJECTIVES: The purpose of this study was to investigate the efficacy of group-based cognitive intervention in people affected with leprosy and mild cognitive impairment. METHODS: We consecutively recruited the PALs who came to the neurology clinic in Sorokdo National Hospital form 2018 January to September complaining with cognitive decline. Of 25 patients who visited, 10 patients were diagnosed with MCI. 5 patients agreed to participate in structured group-based cognitive intervention (CogMCI) which was validated through nation-wide randomized clinical trial. For 12 weeks, subjects received CogMCI twice per week by a trained instructor. They were assessed with Seoul Neuropsychological Screening Battery-2 at baseline and postintervention. We performed Wilcoxon signed rank test to validate efficacy of the CogMCI program. RESULTS: In comparison to the baseline score, the subjects receiving CogMCI showed significant improvements in cognition at postintervention. Memory domain and Frontal/Executive function scores were significantly improved. K-Mini-Mental-Status-Examination was increased from 24.2 to 26.4 though not significant. K-Instrumental-Activity of Daily Living score was decreased from 0.32 to 0.18 and Short-term Geriatric Depression Scale was also decreased from 7.6 to 5.6. CONCLUSIONS CogMCI, the group-based cognitive intervention program, resulted in cognitive improvements, especially in memory and frontal/executive function domains in PALs with MCI. Further study is needed to assess the persistent effect of cognitive intervention program after it is discontinued.

Cortical dysplasia is a cause of intractable epilepsy and a candidate for surgical resection to control epileptic attacks. The neuronal cytomegaly and balloon cell change are the diagnostic hallmarks of cortical dysplasia. Little research has been performed about the normal-sized dysplastic neuron which has complex arborizing dendrites and lacks in its polarity. The aim of this study was to define the histopathologic characteristics of the neurons in cortical dysplasia. Twelve cases of cortical dysplasia who underwent partial lobectomy for intractable seizures were selected and immunohistochemical staining for NF-M/H, MAP2, tau, and ubiquitin was performed. The perikarya and dendrite of dysplastic neurons were more intensely labeled with antibodies for the high and medium molecular weight neurofilament proteins (NF-M/H) than normal neurons. Immunoreactivity with the MAP2 antibody expressed mainly within the somatodendritic regions was present in the dysplastic or normal neurons without any significant difference in intensity. The complex arborizing dendrites of dysplastic neurons were easily identified due to pronounced immunoreactivity within the somatodendritic regions. Immunoreactivity with the primary antibody against tau and ubiquitin was present in the normal-looking neurons as well as the dysplastic neurons. This study suggests that the dysplastic neurons in cortical dysplasia are accompanied by changes of cytoskeletal neurofilaments, and the immunohistochemical stains for NF-M/H, MAP2, tau, and ubiquigin are useful to detect them.
